[Hemodynamics and myocardial contractility in experimental tourniquet-shock].

A reproducible tourniquet-shock has been produced in hind limbs of dogs by unilateral and bilateral extremity ischemia. The following parameters have been measured for analysing the function of the cardiovascular system: mean aortic pressure, heart rate, cardiac output, intraventricular pressure and left ventricular pressure. From these data the stroke volume, stroke work, total peripheral resistance and the parameters of heart contractility dp/dtmax, dp/dtmax:IP and t-dp/dtmax were derived. During the ischemic period all circulatory parameters did not change in comparison to the controls. A tourniquet-shock developed upon recirculation of the ischemically stressed extremity which was more pronounced after bilateral than after unilateral hind leg ischemia. After release of the tourniquet all animals with unilateral tourniquet survived an observation period of 5 hours duration, whereas 6 out of 8 dogs with bilateral tourniquet died of heart failure. Upon release of the tourniquet, the cardiac output raised up to 140% of the normal value: the abruptly decreasing aortic pressure was fully compensated by a tachycardia from 100 to 190 (beats/minute). The parameters dp/dtmax:IP and t-dp/dtmax indicated a distinct increase of the left ventricular contractility in the early tourniquet-syndrom. Already after 30 minutes an increasing circulatory depression developed indicative of the decrease in aortic pressure, and enddiastolic pressure. At the same time an increase of heart rate and total peripheral resistance occurred. The parameters of left ventricular contractility did not change markedly during the course of shock except for the final stage.

[Treatment of congenital tibial pseudarthrosis using a free vascularized fibula transfer and plate osteosynthesis. Case report].

Congenital pseudarthrosis of the tibia is a rare disorder of unknown etiology. The free vascularized fibula-pro-tibia-transplant permits correction of the defect and the leg length-discrepancy during one surgical procedure. A 5 year old girl with congenital pseudarthrosis underwent a free vascularized fibular-pro-tibia-transplant. At a follow-up examination 5 years post surgery she had no subjective complaints, she walked without external orthopaedic support, there was no leg length-discrepancy. As stated in the literature the rates of bony unions are much higher than after conventional methods.

[Osteomyocutaneous flap].

A method for the reconstruction of the bony chin contour with an iliac crest free flap is described. The skin pad may be used to replace mucosa or skin or both. Excessive bulk can be a problem, particularly where a relatively small bone and oral mucosal defect requires replacement. Attempts at minimizing this bulk risk the loss of blood supply to the overlying skin. Another procedure, such as the internal oblique-iliac crest osseomyocutaneous flap or inclusion of the thoracodorsal pedicle with the scapular osteocutaneous flap, should then be considered.

[Ligation with elastic ligature: an alternative in the treatment of hemorrhoidal diseases].

Rubber band ligation, first described by Blaisdell, is an effective way of treating internal haemorrhoids. Treatment is ambulatory, thus saving time and costs. Experiences with 72 patients show this method to be superior to injection therapy and haemorrhoidectomy because of the simplicity of technique, the short period of treatment and the minimal discomfort for the patients. In our opinion rubber band ligation is the treatment of choice in managing internal haemorrhoids.

Microvascular anastomosis by invagination: an experimental study.

A technique of end anastomosis of microvessels of equal or unequal diameter, requiring only two sutures is described. In the present investigation we used the common carotid artery and the femoral vein of 37 white rats. The adventitia of the proximal part of the anastomosed artery was subsequently prepared and removed. However, in case of vein anastomosis, the adventitia of the distal part was removed. Following this washing with saline solution was done. 10-0 Nylon monofil sutures, were used which were placed at 180 degrees. The anastomosis was completed by ligation in such a way that one end was invaginated into the other and was fixed firmly. Five months later the anastomoses were examined after careful preparation under the surgical microscope and photographed. A SEM examination was then done in half of them and photos were taken at 24X, 80X, 1,200x magnification. The remaining tissue was examined histologically. The findings after surgical microscope SEM and histological examination are described.
