Simple computer-assisted diagnosis of acute myocardial infarction in patients with acute thoracic pain.

In order to minimize the initial diagnostic uncertainty in patients suspected of having acute myocardial infarction, we prospectively extracted predictive variables from previous history, ECG, and clinical chemical parameters of 87 patients, who were admitted for acute thoracic pain. The variables thus extracted were: Thoracic pain in previous history, duration of pain, white blood cell count, blood glucose, creatine-kinase, and S-T elevation in the ECG. These parameters were used for formulating a mathematical model based upon univariate and multivariate statistical methods. The sensitivity of the model in the study population was 95% and the specificity 77%. Correct classification was achieved in 89% of cases. In a second phase, the prognostic index was prospectively evaluated in a second set of 122 consecutive patients. In this test population, the sensitivity was 89% and the specificity 86%. 87% of patients were classified correctly.

Infradian rhythms of alanine aminopeptidase excretion during gentamicin therapy.

Urinary excretion of alanine aminopeptidase (EC 3.4.11.2) was examined in 30 patients (22 women, 8 men, range 38-81 years; mean age 55.4) receiving gentamicin in a therapeutic dosage, which was based on the monitored blood creatinine concentration. In general, therapy lasted 10 days. The excretion of 24 individuals displayed significant infradian rhythms with periods between 2.2 and 8.1 days. In 10 of these 24 patients (42%) a circaseptan period was detected. The high portion of circaseptan rhythms might have been induced by the detrimental effects of gentamicin on the proximal tubule and the resulting processes of reconstitution.

[Macro-creatine kinase; circulating immunoglobulin enzyme complexes as cause of elevated CK-activities (author's transl)].

We report a case of a woman (58 years old) with persistent and elevated CK activities and without corresponding clinical signs. For seven months CK-B activity was determined above normal by an inhibition test. By exclusion chromatography, ion-exchange chromatography, electrophoresis of CK isoenzymes and immunological methods we were able to explain this phenomenon as Macro CK consisting of immunoglobulin G bound CK-BB.

Serum creatine kinase BB linked to immunoglobulin G.

We describe an atypical form of creatine kinase in the serum of a woman after myocardial infarction. Electrophoresis on agarose gel showed a single fraction between CK-MM and CK-MB isoenzymes. After ion-exchange chromatography on DEAE-Sephadex A-50 we isolated the atypical CK and CK-BB as detected by electrophoresis of the eluted fractions. Results of our immunological investigations and exclusion chromatography clearly demonstrate that the atypical CK consists of complexes with high molecular masses formed by CK-BB isoenzyme and immunoglobulin G. The clinical significance, if any, of this Macro CK has yet to be determined.

[Evaluation of the aca lipase (DuPont) determination and of the lipase determination according to Myrick (Calbiochem-Behring) (author's transl)].

Two lipase methods, the method for the aca (DuPont) and the method according to Myrick (1976) Thesis, Birmingham Al.) (Calbiochem-Behring), were compared with the lipase method of Rick (1969) Z. Klin. Chem. Klin. Biochem. 7, 530--539) and with the amylase determination. The diagnostic value of the aca method, the amylase and the lipase method of Rick were in good agreement, whereas the method of Myrick gave conflicting results.

[Properties and significance of macro-creatine kinases (author's transl)].

Sera from seven-patients with an unusual high ratio of creatine kinase-MB versus total creatine kinase, as found in the immunoinhibition test, were investigated. In every case, the electrophoretic separation of the isoenzymes showed a slow moving band, and exclusion chromatography showed the presence of macro-creatine kinase. Further chromatographic, electrophoretic and immunological investigations indicated that the macro-creatine kinases in six cases are complexes of creatine kinase-BB linked to immunoglobulin G.

[Comparison between cytochemical and gelelectrophoretic demonstration of acid phosphatases from leukocytes (author's transl)].

The cytochemical and gelelectrophoretic demonstration of acid phosphatases with substrates alpha-naphthyl phosphate and naphthol AS-BI phosphate and with couplers fast garnet GBC and hexazotized pararosanilin is described. Cytochemically best results are achieved wit naphthol AS-BI phosphate and pararosanilin, electrophoretically however with alpha-naphthyl phosphate and pararosanilin. This discrepancy ist explained by a different substrate affinity and membrane penetration of substrates.

Evaluation of a micromethod for the determination of glucose in skin-puncture blood for the DuPont aca.

A micromethod is described for the determination of glucose in 20 microliter of skin-puncture blood for the DuPont aca. For the determination of glucose we prefer whole blood which is deproteinised with uranyl acetate. Some modification of test conditions are programmed into the computer II. The emergency analysis of supernatant on the aca then gives the same values as the Technicon Autoanalyzer II, using glucose dehydrogenase for routine analyses. The aca readout is linearly related to glucose concentrations up to 60 mmol/l.

[Microdetermination of glucose using glucose dehydrogenase, with independent sample preparation in the routine laboratory (author's transl)].

A micromethod for the determination of glucose in 20 microliter of capillary blood using glucose dehydrogenase is described. After deproteinisation with uranyl acetate, the samples are analysed by an Autoanalyzer II method or by a manual procedure. Precision and accuracy are well correlated with the hexokinase-glucose-6-phosphate dehydrogenase method. Eleven months experience have shown the practicability and economic advantages of this method.

[Isoenzymes of acid phosphatase in blood cells of normal subjects and patients with leukemia (author's transl)].

The activities of acid phosphatases (AP) were measured in leukocytes from patients with chronic myelocytic leukemia (CML), macrophages, granulocytes, in the fractionated mononuclear cells of patients with CML and with hairy-cell-leukemia (HCL) and in the cells from patients with acute leukemia (AL). The lowest activities were found in lymphocytes of normal subjects and of patients with chronic lymphatic leukemia (CLL) and in thrombocytes. Isoenzyme (IsE) 1 was characteristic for thymocytes, IsE 2 for granulocytes, IsE 3 for pathologic blast-cells, lymphocytes and thrombocytes, IsE 4 for macrophages, IsE 5 with components a and b for the mononuclear fraction of patients with HCL. In addition IsE 5 was detected in lymphocytes, macrophages and CLL-cells. In 4 patients with HCL the relative percentage of IsE-5-fraction was slightly greater than the percentage of tartrate resistant cells. In two patients with questionable HCL well marked IsE-5-fractions were recognized but no tartrate resistant cells. In one patient with HCL a relatively high percentage of tartrate resistant hairy-cells and in comparison an inadaquate low IsE-5-fraction was found. These different relations were explained with the more sensitive method of gelelectrophoresis and different affinity of substrates to AP.

Acid Phosphatase

Acral keratoderma.

Three siblings displayed an unusual form of keratoderma characterized by diffuse and striate hyperkeratosis of the palms and soles, hyperkeratotic plaques over the dorsum of the hands and toes, and linear hyperkeratotic lesions over the Achilles tendon area, ankles, elbows, and knees. The predominant location of these lesions led to the term acral keratoderma for this disorder. Histologically, besides thickening of all epidermal layers with that of the stratum corneum being most notable, various dyskeratotic changes were evident in the epidermis. Pedigree analysis of the family suggested an autosomal recessive inheritance pattern. There were similarities and differences between the type of keratoderma displayed by these three patients and that of patients described previously with the disorder known as keratoma hereditarium mutilans.

[Sense and applicability of means. Various critical considerations].

The range of application and account of the arithmetic and geometric mean, the mode and the median set different standards to the frequency distribution of the original data. The quite frequently inadequate use of the geometric mean in serological dilution series will be demonstrated in a practical example. The interview of 66 physicians and 31 graduate and postgraduate engineers was made to give an impression of the participants' knowledge of the above mentioned statistical mean values and their practical application. The arithmetic mean proved to be the best known and most popular method in use, closely followed by the geometric mean and far of by the median and the mode. Other questions were to find out, why the arithmetic mean was the favourite. The result was as astonishing as it was simple: The arithmetic mean seemed to be used most because it was known best at school and university. Therefore in a great number of cases statistical means were chosen from a formalistic point of view and not, as it would be much more appreciated, according to the specific nature of the data or to pertinent accounts.
