Induction of apoptotic cell death in peripheral blood mononuclear and polymorphonuclear cells by an oral bacterium, Fusobacterium nucleatum.

It is largely unknown why a variety of bacteria present in the oral cavity are capable of establishing themselves in the periodontal pockets of nonimmunocompromised individuals in the presence of competent immune effector cells. In this paper we present evidence for the immunosuppressive role of Fusobacterium nucleatum, a gram-negative oral bacterium which plays an important role in the generation of periodontal disease. Our studies indicate that the immunosuppressive role of F. nucleatum is largely due to the ability of this organism to induce apoptotic cell death in peripheral blood mononuclear cells (PBMCs) and in polymorphonuclear cells (PMNs). F. nucleatum treatment induced apoptosis of PBMCs and PMNs as assessed by an increase in subdiploid DNA content determined by DNA fragmentation and terminal deoxynucleotidyltransferase-mediated dUTP-biotin nick end-labeling assays. The ability of F. nucleatum to induce apoptosis was abolished by either heat treatment or proteinase digestion but was retained after formaldehyde treatment, suggesting that a heat-labile surface protein component is responsible for bacterium-mediated cell apoptosis. The data also indicated that F. nucleatum-induced cell apoptosis requires activation of caspases and is protected by NF-kappaB. Possible mechanisms of F. nucleatum's role in the pathogenesis of periodontal disease are discussed.

Interactions between periodontal bacteria and human oral epithelial cells: Fusobacterium nucleatum adheres to and invades epithelial cells.

Bacteria are causative agents of periodontal diseases. Interactions between oral bacteria and gingival epithelial cells are essential aspects of periodontal infections. Using an in vitro tissue culture model, a selected group of gram-negative anaerobic bacteria frequently associated with periodontal diseases, including Bacteroides forsythus, Campylobacter curvus, Eikenella corrodens, Fusobacterium nucleatum, Porphyromonas gingivalis, and Prevotella intermedia, were examined for their ability to adhere to and invade primary cultures of human gingival epithelial cells (HGEC). The effects of these bacteria on the production of interleukin-8 (IL-8), a proinflammatory chemokine, were also measured. These studies provided an initial demonstration that F. nucleatum adhered to and invaded HGEC and that this was accompanied by high levels of IL-8 secretion from the epithelial cells. The attachment and invasion characteristics of F. nucleatum were also tested using KB cells, an oral epithelial cell line. The invasion was verified by transmission electron microscopy and with metabolic inhibitors. Invasion appeared to occur via a "zipping" mechanism and required the involvement of actins, microtubules, signal transduction, protein synthesis, and energy metabolism of the epithelial cell, as well as protein synthesis by F. nucleatum. A spontaneous mutant, lam, of F. nucleatum, isolated as defective in autoagglutination, was unable to attach to or invade HGEC or KB cells, further indicating the requirement of bacterial components in these processes. Sugar inhibition assays indicated that lectin-like interactions were involved in the attachment of F. nucleatum to KB cells. Investigation of these new virulence phenotypes should improve our understanding of the role of F. nucleatum in periodontal infections.

Myxococcus xanthus dif genes are required for biogenesis of cell surface fibrils essential for social gliding motility.

Myxococcus xanthus social (S) gliding motility has been previously reported by us to require the chemotaxis homologues encoded by the dif genes. In addition, two cell surface structures, type IV pili and extracellular matrix fibrils, are also critical to M. xanthus S motility. We have demonstrated here that M. xanthus dif genes are required for the biogenesis of fibrils but not for that of type IV pili. Furthermore, the developmental defects of dif mutants can be partially rescued by the addition of isolated fibril materials. Along with the chemotaxis genes of various swarming bacteria and the pilGHIJ genes of the twitching bacterium Pseudomonas aeruginosa, the M. xanthus dif genes belong to a unique class of bacterial chemotaxis genes or homologues implicated in the biogenesis of structures required for bacterial surface locomotion. Genetic studies indicate that the dif genes are linked to the M. xanthus dsp region, a locus known to be crucial for M. xanthus fibril biogenesis and S gliding.

Adnexal masses in women with breast cancer: US findings with clinical and histopathologic correlation.

PURPOSE: To correlate ultrasonographic (US), clinical, and histopathologic findings in patients with breast cancer who underwent surgery for adnexal masses evident at US. MATERIALS AND METHODS: A database search yielded 54 patients with breast cancer and with adnexal masses at US and histopathologic examinations. Clinical, US, and histopathologic findings were correlated. RESULTS: Forty (74%) patients had benign adnexal masses, and 14 (26%) had malignant masses; three patients had both benign and malignant ovarian masses. Seven patients had primary ovarian cancer, and seven had breast metastases to the ovary. All breast metastases to the ovary were bilateral solid masses at histopathologic examination and occurred in women with stage IV breast carcinoma at the time of US. Eleven ovaries with breast metastases were solid at US. The remaining three ovaries with breast metastases had cystic components at US because of hemorrhage or coexistent benign ovarian cysts. Four of seven patients with primary ovarian carcinoma had bilateral ovarian tumors, and seven of 11 ovarian carcinomas were predominantly cystic at US. No patient with primary ovarian carcinoma had stage IV breast cancer. CONCLUSION: In this small series, half the ovarian malignancies in patients with breast cancer were primary ovarian carcinomas and half were breast metastases to the ovary. Breast metastases to the ovary most frequently are bilateral solid masses at US and are associated with stage IV disease at the time of US.

Altered reactivity of pulmonary vessels in postobstructive pulmonary vasculopathy.

Chronic ligation of one pulmonary artery results in pulmonary vascular remodeling and bronchial angiogenesis, collectively known as postobstructive pulmonary vasculopathy (POPV). To investigate pulmonary vascular reactivity in POPV, we ligated the left main pulmonary artery of guinea pigs and, after 1-10 mo, prepared explants by inflating lungs with agarose and sectioning them into approximately 1-mm-thick slices; we measured areas of pulmonary vessels and determined contractile responses to histamine and serotonin (5-HT) and relaxant responses to ACh and sodium nitroprusside. We found maximal contractions of arteries to 5-HT (24. 4 +/- 2.6%) and of veins to histamine (53.9 +/- 4.7%) were significantly increased in POPV of 3-mo duration compared with those of controls (16.8 +/- 1.5 and 40.8 +/- 5.0%, respectively). Relaxation of arteries with ACh was enhanced at 10 mo but not at 1 mo after ligation. Relaxation with sodium nitroprusside was increased in veins at 1 mo after ligation but was not altered in arteries. Morphometry revealed reduced diameters of arteries and veins without increased medial thickness. Our data suggest that the enhanced contractile responses of pulmonary vessels to histamine and 5-HT in POPV were not a result of endothelial dysfunction or of structural alterations but might be caused by as-yet-undiscovered mechanisms.

Effect of macrophage-derived apolipoprotein E on established atherosclerosis in apolipoprotein E-deficient mice.

Apolipoprotein E-deficient (apoE(-/-)) mice have hyperlipidemia and develop spontaneous atherosclerosis in a time-dependent manner. Although macrophage-derived apoE has been shown to prevent the development of atherosclerosis in apoE(-/-) mice, whether it would induce regression of established atherosclerosis is unknown. To determine this, 8-week-old apoE(-/-) mice were transplanted with apoE(+/+) bone marrow. Four weeks after transplantation, when plasma cholesterol levels had reached normal levels, a group of mice (n=12) were killed and their aortic lesions were measured and used as a baseline to judge regression. Twelve and 20 weeks after transplantation, aortic lesion areas of the mice were 9340+/-2184 micrometer(2) (mean+/-SEM, n=8) and 12 211+/-1433 micrometer(2) (n=9), respectively, values not significantly different from the lesion areas of the baseline mice (12 347+/-2487 micrometer(2); n=12, P>0.05). In contrast, apoE(-/-) mice reconstituted with apoE(-/-) bone marrow developed severe atherosclerotic lesions (453 036+/-29 767 micrometer(2), n=7) 20 weeks after transplantation. These data suggest that macrophage-derived apoE was insufficient to induce significant regression of established atherosclerotic lesions in apoE(-/-) mice, although it was sufficient to eliminate hypercholesterolemia and prevent progression of aortic lesions.

Surgical management of renal tumors 4 cm. or less in a contemporary cohort.

PURPOSE: We evaluated a patient cohort with renal tumors 4 cm. or less treated with partial or radical nephrectomy. We compared patient and tumor characteristics, and survival in these 2 groups. MATERIALS AND METHODS: We retrospectively analyzed the records of 670 patients with a median age of 63 years treated surgically for renal cell carcinoma between July 31, 1989 and July 31, 1997. Renal tumors 4.0 cm. or less were noted in 252 patients (38%) who underwent a total of 262 procedures, including 183 radical (70%) and 79 partial (30%) nephrectomies. Ten patients required 2 operations each because of bilateral renal cell carcinoma. Median followup was 40 months. We compared clinicopathological parameters in the partial and radical nephrectomy groups using chi-square or Wilcoxon analysis as appropriate. Survival analysis was determined by the log rank test and Cox regression model. RESULTS: The partial and radical nephrectomy groups were comparable with respect to gender ratio, tumor presentation, histological classification, pathological stage and complication rate. Median tumor size was 2.5 and 3.0 cm. in the partial and radical nephrectomy groups, respectively (p = 0.0001). Resection was incomplete in 1 patient (1.3%) in the partial and none in the radical nephrectomy group. There was no local recurrence after either procedure, and no significant difference in disease specific, disease-free and overall survival (p = 0.98, 0.23 and 0.20, respectively). CONCLUSIONS: Patients with a small renal tumor have similar perioperative morbidity, pathological stage and outcome regardless of treatment with partial or radical nephrectomy. Therefore, partial nephrectomy remains a safe alternative for tumors of this size.

[Research on renaturation of recombinant human pro-urokinase expressed from Escherichia coli].

Recombinant human pro-urokinase forms insoluble inclusion body when overexpressed in Escherichia coli, and it must be denatured and renatured in vitro before it acquires activity. This study aimed to increase the renaturation yield of denatured pro-urokinase. We have evaluated the basic renaturation conditions of pro-urokinase through qualitative and quantitative analysis of pH, temperature, denaturant concentration, protein concentration, the ratio of reduced and oxidized thiol reagents. The effects of nonspecific additives, step-wise dilution and urea gradient dialysis have been also compared. The optimal conditions of pro-urokinase renaturation with the yield about 20%-30% have been obtained.

Chemotaxis in pathogenic spirochetes: directed movement toward targeting tissues?

Chemotaxis is an important feature of motile organisms that allows navigation through various environments. It enables them to detect nutrients and to avoid unfavorable or dangerous conditions. Motility and chemotaxis are widely acknowledged as important virulence factors for pathogenic bacteria. In this review, we try to explore the role of chemotaxis in the pathogenesis of spirochetes. Chemotaxis might be involved in tissue identification and penetration, and represents a possible mechanism for evasion of the host's immune defense. The recent development of genetic tools for pathogenic spirochetes and "tracking" techniques, employing fluorescent in situ hybridization (FISH), could revolutionize our understanding of the importance of chemotaxis for infection and persistence of these bacteria in their host.

[Effect of effective fraction of Radix Salviae Miltiorrhizae on procollagen gene expression in fracture healing].

OBJECTIVE: To investigate effect of effective fraction of Radix Salviae Miltiorrhizae (RS9403) on procollagens and transforming growth factor beta 1(TGF beta 1) gene expression in fracture callus, and to explore the mechanism of RS9403 in enhancing fracture healing. METHODS: Standardized radial fractures were performed in 24 Wistar rats, which were randomized into three groups: the RS9403 group, the Sanhua Jiegu powder (SHJGP) group and the blank control model group. They were fed orally with RS9403, SHJGP and normal saline respectively, and were sacrificed on the 3rd day, 1st week, 2nd week and 4th week after fracture in batches. In situ localization of procollagens and TGF beta 1 gene expression were examined on the cryosection of rat fracture callus. RESULTS: On the 3rd day after fracture, the TGF beta 1 gene expression at the site of fracture in the RS9403 group and the SHJGP group was significantly higher than that in the blank control group, and type-III procollagen gene expression was observed in the two groups. At the end of 1st week after fracture, the pro alpha 1(III) expression in fibroblast and chondrocyte-like cells was dominant. The local type II and I procollagen gene expressions in the RS9403 and the SHJGP group were enhanced simultaneously with TGF beta 1 gene expression compared with those in the blank control group. At the end of 2nd week the RS9403 and the SHJGP group were characterized by a marked increase in the mRNA levels of type I procollagen, the numbers of hypertrophic chondrocytes was larger than that in the control group, and the type II procollagen expression declined markedly. The shared phenotype expression was confirmed at this stage, especially in the RS9403 and the SHJGP group. At the end of 4th week, the cartilagi nous callus was almost all replaced by the bone tissue. In the RS9403 and the SHJGP group, the replacement was nearly complete, few type I procollagen mRNA positive osteoblasts and hypertrophic chondrocytes were found scattering in the woven bone and remnants of cartilaginous callus. CONCLUSION: RS9403 could affect the procollagens expression to enhance the healing of bone fracture as SHJGP do, maybe, by modulating the TGF beta 1 expression.

[A report on investigation and clinical application of corneal storage media].

OBJECTIVE: To develop intermediate term storage media suitable for Chinese eye banks. METHODS: Corneal buttons of rabbits were stored in DX solution freshly prepared. After storage for several days, the survival rate of the corneal endothelial cells was examined, enzymohistochemical staining and ultrastructural examinations were carried out for the buttons. Thirty-seven human corneas stored in DX solution for 3 - 11 days were used for corneal transplantation, clinically. RESULTS: There was no significant difference between rabbit corneal buttons stored in DX sol. and in Optisol in any indices studied. 37 grafts were all transparent in 1 week after transplantation, and the mean endothelial cell density was (2,204.56 +/- 689.56) cells/mm(2). CONCLUSION: As a safe and efficient intermediate term (about 1 week) storage medium, DX sol. is appropriate for Chinese eye banks.

[Experimental study on herpes simplex virus-1 functional gene latency in cornea].

OBJECTIVE: To investigate the possible existence of herpes simplex virus-1 (HSV-1) functional gene latency in cornea. METHODS: A herpes simplex keratitis (HSK) model was established in New Zealand White (NZW) rabbit corneas, latently infected corneas were transplanted to one eye of each naive NZW rabbit. One week after the operation, the corneal buttons were removed and divided into three portions. One portion was processed for HSV-1 antigen detection, another was for HVS-1 latency associated transcript (LAT), thymidine kinase (TK) and DNA polymerase gene detections by polymerase chain reaction (PCR), and the last portion after tissue culture for 3 weeks, it was co-cultured with rabbit kidney cells (RK) for one week, then the expression of HSV-1 antigen in the RK cells and this portion of corneal button was detected. RESULT: In the 15 corneal samples negative for HSV-1 antigen, LAT gene was detected in 8 samples, TK gene in 10 and DNA polymerase gene in 6 samples. HSV-1 antigen was detected in 3 samples of RK cells and 6 cultured corneal samples. CONCLUSION: HSV-1 functional genes are retained in latently infected cornea.

[Secondary glaucoma after penetrating keratoplasty].

OBJECTIVE: To investigate the etiology and management of secondary glaucoma after penetrating keratoplasty (PKP). METHODS: Eighty cases (80 eyes) with secondary glaucoma occurring in 1,450 cases (1,500 eyes) of PKP, its causes, time of onset and the relationship with the primary disease were analyzed. According to different causes, they were treated with different therapies. RESULTS: The intraocular pressure (IOP) of 63 cases (63 eyes) became normal after drug treatment for 2 - 3 weeks. In 17 cases (17 eyes) who had received antiglaucomatous operations, 13 cases' (13 eyes) IOPs were normal and 4 cases' (4 eyes) still high. CONCLUSIONS: Accurate perioperative management can decrease the incidence of secondary glaucoma after PKP. The majority of secondary glaucoma can be controlled by different therapies according to different causes.

[A preliminary report of ocular surface disease treated by limbal transplantation].

OBJECTIVE: To evaluate the effect of ocular surface disease treated by limbal transplantation. METHODS: In a prospective study, 5l eyes of 50 patients were treated with three kinds of surgical techniques: first, conjunctival limbal autograft (CLAU) transplantation; second, corneal lamellar allograft combined with limbal autograft transplantation and third, corneal lamellar allograft combined with limbal allograft transplantation. Their effects were evaluated. RESULTS: All of the 18 patients of CLAU group were cured. The second surgical technique resulted in restoration of a stable ocular surface in 19 of 20 cases. 10 patients' visual acuities were improved two or more than two lines. 13 eyes of 12 patients were treated with the third surgical technique, resulting in graft ulceration in 4 patients and graft rejection in 4 patients with bad prognosis. CONCLUSIONS: The effect of unilateral ocular surface disease treated with limbal autograft transplantation is satisfactory. For bilateral ocular surface disease, the method of limbal allograft transplantation should be improved to resolve some problems such as graft rejection.

[Influence of oxygen content on photoluminescence from a-SiOxNy thin films].

Amorphous SiOxNy thin films were deposited by PECVD technique. Strong photoluminescence bands centered at about 340 and 600 nm and peaks centered at 365, 375, 380, 735 and 745 nm from as prepared samples were observed at room temperature. Influence of partial pressure of H2 during deposition was investigated. The results show that strength of PL and oxygen content increase with H2 partial pressure decrease, the position of PL peaks does not change, and the central position of PL bands shifts. The possible origin of PL and mechanism of the variations are discussed according to experimental results.

Hepatic arterial infusion of chemotherapy after resection of hepatic metastases from colorectal cancer.

BACKGROUND: Two years after undergoing resection of liver metastases from colorectal cancer, about 65 percent of patients are alive and 25 percent are free of detectable disease. We tried to improve these outcomes by treating patients with hepatic arterial infusion of floxuridine plus systemic fluorouracil after liver resection. METHODS: We randomly assigned 156 patients at the time of resection of hepatic metastases from colorectal cancer to receive six cycles of hepatic arterial infusion with floxuridine and dexamethasone plus intravenous fluorouracil, with or without leucovorin, or six weeks of similar systemic therapy alone. Patients were stratified according to previous treatment and the number of liver metastases identified at operation. The study end points were overall survival, survival without recurrence of hepatic metastases, and survival without any metastases at two years. RESULTS: The actuarial rate of overall survival at two years was 86 percent in the group treated with local plus systemic chemotherapy and 72 percent in the group given systemic therapy alone (P=0.03). The median survival was 72.2 months in the combined-therapy group and 59.3 months in the monotherapy group, with a median follow-up of 62.7 months. After two years, the rates of survival free of hepatic recurrence were 90 percent in the monotherapy group and 60 percent in the monotherapy group (P<0.001), and the respective rates of progression-free survival were 57 percent and 42 percent (P=0.07). At two years, the risk ratio for death was 2.34 among patients treated with systemic therapy alone, as compared with patients who received combined therapy (95 percent confidence interval, 1.10 to 4.98; P=0.027), after adjustment for important variables. The rates of adverse effects of at least moderate severity were similar in the two groups, except for a higher frequency of diarrhea and hepatic effects in the combined-therapy group. CONCLUSIONS: For patients who undergo resection of liver metastases from colorectal cancer, postoperative treatment with a combination of hepatic arterial infusion of floxuridine and intravenous fluorouracil improves the outcome at two years.

Effect of cellular filamentation on adventurous and social gliding motility of Myxococcus xanthus.

Filamentous bacterial cells often provide biological information that is not readily evident in normal-size cells. In this study, the effect of cellular filamentation on gliding motility of Myxococcus xanthus, a Gram-negative social bacterium, was investigated. Elongation of the cell body had different effects on adventurous and social motility of M. xanthus. The rate of A-motility was insensitive to cell-body elongation whereas the rate of S-motility was reduced dramatically as the cell body got longer, indicating that these two motility systems work in different ways. The study also showed that filamentous wild-type cells glide smoothly with relatively straight, long cell bodies. However, filamentous cells of certain social motility mutants showed zigzag, tangled cell bodies on a solid surface, apparently a result of a lack of coordination between different fragments within the filaments. Further genetic and biochemical analyses indicated that the uncoordinated movements of these mutant filaments were correlated with the absence of cell surface fibril materials, indicating a possible new function for fibrils.

TCR-Mediated internalization of peptide-MHC complexes acquired by T cells.

Peptide-major histocompatibility complex protein complexes (pMHCs) on antigen-presenting cells (APCs) are central to T cell activation. Within minutes of peptide-specific T cells interacting with APCs, pMHCs on APCs formed clusters at the site of T cell contact. Thereafter, these clusters were acquired by T cells and internalized through T cell receptor-mediated endocytosis. During this process, T cells became sensitive to peptide-specific lysis by neighboring T cells (fratricide). This form of immunoregulation could explain the "exhaustion" of T cell responses that is induced by high viral loads and may serve to down-regulate immune responses.
