[Running time of ultrasound in the distal portion of the optic nerve (author's transl)].

In 160 cross sectional ultrasonograms of normal optic nerves the double running time of ultrasound in 4.5-5 musec. The standard deviation is up to 1 musec. The conditions of measurement and the typical ultrasonographic patterns are discussed. These results are of value in the differential diagnostic of bilateral optic nerve lesions. Increased running time of ultrasound could be found in papilloedema due to neuritis, raised intracranial and intraorbital pressure, and in trauma and tumor. It can be decreased in optic atrophy.

[Early neuro-ophthalmological symptoms of periarteritis nodosa. A case report (author's transl)].

A report is given on a 47-year-old woman with periarteritis nodosa. A unilateral retinopathy with disseminated cotton-wool-spots surrounded by retinal haemorrhages and edema was observed. Fluoresceinangiography showed a multifocal constriction of the retinal capillaries. This lesion of the retinal vessels can be distinguished from those caused by hypertension. An acute immunological reaction has been suggested as the pathogenesis. Some authors have described this retinal lesion in collagen diseases as a "toxic" retinopathy. We prefer the term acute immunological microangiopathy (AIM).

[The use of A-scan echography in the clinical diagnoses of persistent hyperplastic primary vitreous (PHPV) (AUTHOR'S TRANSL)].

In 15 eyes with PHPV (3 suspected cases) patterns of rudimentary hyaloid vessels were found by A-scan echography in 9 cases all under 7 years of age; although these vessels were seen ophthalmoscopically in only 1 case. This ultrasonographic finding appears to add an important contribution to the clinical diagnosis of PHPV, when the monolateral cataract is so dense as to observe other signs (elongated ciliary processes, retrolental fibrovascular tissue). It could also be demonstrated that the axial diameter of the lens remains smaller than that of the partner eye even when the cataract is intumescent with fattening of the anterior chamber. Contrary to our expectations, the axial length of the bulbus was increased in one third of the cases; while a relative microcornea was absent only in 2 eyes!

[A-scan-ultrasonography in unilateral optic nerve lesions (author's transl)].

With A-scan-ultrasonography it is possible to determine the time of transmission of ultrasound within the cross section of the distal optic nerve. In this way information regarding its thickness can be obtained. 156 patients with various unilateral optic nerve lesions were examined. Thickening of the affected nerve was found in the acute phase of neuritis nervi optici, trauma, optic nerve tumors and in papilloedema due to raised orbital or intracranial pressure. This investigation has proved a valuable aid in diagnosis in the following unilateral conditions: 1. Acute visual loss in the absence of significant fundus findings--an enlarged optic nerve ultrasonogram is an objective sign of a retrobulbar neuritis, or after injury, a haematoma. 2. Papilloedema--the optic nerve ultrasonogram gives information about the nerve. 3. Optic atrophy--an enlarged optic nerve ultrasonogram indicates an optic nerve or sheath tumor.

[Oxygen pressure values in the ischemic muscle tissue of patients with occlusive arterial diseases (author's transl)].

The local oxygen pressure of muscle tissue was measured in 10 patients with occlusive atherosclerosis and in 9 healthy volunteers using a micro-Pt-electrode. In 3 cases, the muscular pO2 values of an ischemic leg were compared to the non-ischemic leg of the same patient. In both groups, there was a statistically significant difference in the oxygen pressure of the muscle tissue in normal legs of volunteers as compared to the ischemic legs. Measurements of the local oxygen pressure in the muscle enables us to have a quantitative measure for the ischemic situtation. It is a useful diagnostic method in patients suffering from ischemic claudication and seems to be an objective criterium of therapeutical methods.

Blood flow and tissue -pO2 in the trained and untrained gastrocnemius muscle of the anesthetized guinea pig.

In 10 guinea pigs the gastrocnemius muscles on one side were tenotomised. By the tenotomy the daily work load of the gastorcnemius muscle was lowered in the operated leg ("untrained muscles") and increased in the control leg ("trained muscles"). Before and several weeks after the operation blood flow was measured in the lower legs (by segmental plethysmography) and oxygen pressure was measured in the gastrocnemius muscles (by micro-Pt-electrodes) of the anesthetized animals. 4 to 6 weeks after the operation statistically significant differences between the two extermities were noted: In the operated leg the mean pO2-value was 33%lower (P is less than 0.001) and the mean blood flow value 46% higher (P is less than 0.001). These differences could be explained by a reduced number of perfused capillaries in the untrained muscles (= non uniform blood flow distribution).

[Results of surgical treatment in dislocated lenses. II. Intracapsular lens extraction in adults (author's transl)].

40 subluxated and dislocated lenses were extracted intracapsularly between 1970 and 1974. In 6 cases there was a displacement into the vitreous; in 4 an acute secondary angle-closure glaucoma was present (3 times due to dislocation of the lens into the anterior chamber). Surgical extraction with a corneal incision was undertaken under general anaesthesia after attachment of a flieringa ring to the globe. In 16 eyes a transpupillary vitrectomy was necessary. The morphological and functional results were satisfactory: decrease in visual acuity due to surgical intervention did not occur, and in 32 eyes vision was considerably improved.
