Disturbances of selected plasma proteins in hyperdynamic septic shock.

This study was performed on patients (n = 18) suffering from strictly defined hyperdynamic septic shock. Plasma factors (C-reactive protein, acid alpha 1-glycoprotein, fibrinogen, fibrinopeptide A, fibrinogen-fibrin split products, factor XIII, antithrombin III, complement factors C3 and C4, inter-alpha-trypsin-inhibitor and alpha 2-macroglobulin) measured during hyperdynamic septic shock were highly abnormal. The activation and consumption of clotting, fibrinolytic and complement factors due to system-specific proteinases (such as thrombokinase or plasminogen activators) seemed to be intensified by the nonspecific proteolytic activity of granulocytic proteinases probably released by the action of endotoxins. Possible therapeutic measures to maintain the endogeneous defence mechanism against enhanced proteolysis during septic shock are discussed.

Alpha-Globulins↗

Computer averaging of single molecules of alpha 2-macroglobulin and the alpha 2-macroglobulin/trypsin complex.

From electron micrographs single molecules of alpha 2-macroglobulin in the "closed" form, the "open" form and as the trypsin complex have been computer averaged. The molecular images are discussed. Molecules of the electrophoretically fast migrating "F-form" have the "closed" form. In the case of the alpha 2-macroglobulin/trypsin complex the two attached trypsin molecules are located very near to each other and in the central part of the alpha 2-macroglobulin molecule.

An application of gas chromatography mass spectrometry to the quantitation of substances and their labelled variants.

A method is proposed for the quantitative estimation of a substance and its labelled analogue using the selected ion monitoring technique of gas chromatography mass spectrometry, in which estimation is achieved without a third labelled analogue as internal standard. In a first measurement the ratio of natural to labelled substance is established. Then a known amount of either labelled or natural compound is added, the isotope ratio is determined a second time and from these two measurements the mass amounts can be calculated. When applied to the heptafluorobutyrates of androstenedione and progesterone about 10 pg of 14C labeled compounds can be determined in the presence of 1 ng of the natural substances.

Simultaneous quantitative estimates of several isotopically labelled substances in the picogram range with selected ion monitoring.

The use of a model developed by the authors for the simultaneous quantitative determination of several isotopically labelled substances of the same chemical structure is described in detail for a two substance system. A computational flow chart is included. An example using natural estradiol and 4-[14C] labelled estradiol with manifold deuterated estradiol as an internal standard is presented. The basis of the model is discussed.

[A standardized reproducible technique to induce traumatic edema in the rat paw].

A standardized reproducible technique is reported for inducing traumatic edema in the rat paw showing considerable swelling volume and low tissue damage. The edema is measured volumetrically subsequent to squashing the rat paw under standardized conditions in a wringing-machine with two wooden rollers. The technique is compared with other methods.

[Hyperdynamic septic shock in man: concentration course of selected coagulation factors and plasma proteins].

Coagulation factors and plasma proteins are significantly decreased in patients with hyperdynamic septic shock. Besides the activation of the coagulation system, the endotoxin-induced release of granulocyte proteases is responsible for a septic disseminated intravascular coagulation. In this situation special emphasis should be placed on the levels of antithrombin III regarding application of heparin.

[The pathogenesis of postoperative abdominal wound dehiscence. Part I: Clinical investigations to determine patient groups at risk (author's transl)].

Between 1955 and 1974 a total of 27 404 patients were laparotomied at the University Surgical Clinic in Munich. Abdominal wound dehiscence occurred during the postoperative phase in 324 patients. In comparison with the clinical findings of undisturbed healing in the total number of patients, some predisposing factors could be established which were statistically confirmed for the first time: age over 60, males, malignant primary disease, long operation time, low preoperative hemoglobin concentration, high blood loss and bigger blood transfusions. With the help of these risk criteria, endangered patients can be detected pre-or intra-operatively.

[The hemophiliac as a surgical patient (author's transl)].

In the Surgical Clinic of the University of Munich, 23 surgical interventions were performed on 20 hemophiliac patients between 1967 and 1976. There were 13 emergency and 10 elective operations. The spectrum ranged from removal of atheroma to subtotal gastric resection. For adequate substitution of factors a comprehensive preoperative "hemostasis profile", selection of a suitable preparation and postoperative supervision with coagulation analysis are necessary. Despite a normalization of the hemostasis, atraumatic surgical technique is particularly important. Risks of substitution therapy are primarily hepatitis, allergic reactions and the appearance of inhibitors. Whereas, especially using low-fibrogenic substrates, the operative risk is not essentially increased, in our experience the patients seem to be endangered particularly by infection and septic complications.

[The pathogenesis of postoperative abdominal wound dehiscence. Part II: Hemostasiological checks during the course in selected patient groups (author's transl)].

By reference to clinically established risk factors, patients with an increased tendency to disturbances of wound healing were selected and investigated hemostasiologically in a prospective study. Abdominal wound dehiscence occurred postoperatively in 12 of the 28 patients selected clinically as being endangered. In all collectives the Factor XIII concentration fell postoperatively and remained low in the patients with a wound rupture, whereas it rose significantly in patients with undisturbed wound healing. Connections between hemostasis and wound healing are discussed.

[Radioimmunoassay of fibrinopeptide A: double antibody method].

A radioimmunoassay kit was used for measurement of human fibrinopeptide A in plasma. The "charcoal technique" was replaced by the more precise double-antibody method. The possibility of automation allows a shortening of the time for performing the assay and a more general clinical use. Normal values and preliminary results of patients with septic disorders and of a hemophilic patient with antibodies to factor VIII which is substituted by pro-coagulation factors (FEIBA) are reported.

[On the growth characteristics of human osseous sarcoma metastases: mathematical calculations and clinical consequences (author's transl)].

Metastases of osteosarcomas do not grow according to a simple exponential function, but rather according to a type of Gompertz' function where flattening with a tendency toward plateau formation sets in after a certain time. This deviation from an exponential growth type corresponds to a substantial increase in the initial tumor size--doubling time. The metastasis doubles in the period after its transfer faster than when it first becomes visible in an x-ray. Another important conclusion resulting from the use of the Gompertz model is the assumption of a tumor-specific maximum volume which cannot be exceeded over a period of infinite growth. For lung metastases of osteosarcoma this volume amounts to approximately 120 cm3. The critical volume which kills the host is, at 70 to 80 cm3, relatively close to this theoretical growth limit (only approximately one cell division below this limit). If a metastasis develops from a single cell, the number of divisions up to this point is approximately 46. Of these, 38 lie within the growth zone which is not visible via x-ray. Since cell-cycle specific agents (for example Vincristin and Methotrexate) have the greatest effect against rapidly proliferating tumors, these drugs (for example alkylantic drugs) are especially effective in the case of slowly proliferating neoplasms. Therefore, use of these drugs should be favored when the metastasis is visible in the x-ray. Since occasionally, particular when the primary tumor is still relatively small, metastasization may not necessarily have already taken place, radical operation of the primary tumor should be carried out as soon as possible. A preliminary irradiation of the primary tumor cannot prevent metastasization with certainty. Therefore delayed amputation should be avoided.

[Hemostaseopathies in chronic, azotemic renal insufficiency. Pathogenesis, diagnosis, treatment (author's transl)].

A brief survey on the current concepts of diagnosis and pathogenesis of platelet dysfunction in chronic renal insufficiency is given on the basis of clinical hemostaseological investigations. Phenolic compounds, guanidine succinate and fibrin decomposition products are prominent in the discussion of uremia toxins possibly involved in disorders of thrombocytic function. Clinically practicable methods of analysis are briefly presented and the still imperfect status of therapy is emphasized.

Blood Coagulation Disorders↗

[Meniscopathy as occupational disease (author's transl)].

Meniscopathy develops through internal causes alone without any external influence. Work such as that of a miner underground, which is associated with particular strain on the knee joint may aggravate this disorder. If such work has been carried out for at least 3 years, an occupational disease is to be assumed. If the complaint only makes its appearance 5 years after ending injurious work, the causal connection must be assessed critically, taking into account age, duration of work and the results of microscopic investigations.
