[Endogenous candida endophthalmitis. Early diagnosis and early vitrectomy improve the prognosis].

Our records on endogenous Candida endophthalmitis in 13 eyes in 10 patients are presented. The diagnosis was confirmed by culture from vitreous aspirate or serologic examination. The records show that cases with immediate diagnosis and treatment by antimycotics and vitrectomy have a better prognosis than late diagnosis or treatment without vitrectomy. Only the 4 cases that were vitrectomized within 4 weeks after onset of the complaints resulted in good visual acuity (greater than 2/5). The other cases were often complicated by traction, retinal scars or choroidal proliferation. Unclear uveitis with a high risk for fungal infection in the patient's history should be evaluated for Candida infection. Every patient with candidemia needs periodic ophthalmologic examinations for endophthalmitis. Diagnosis, treatment and possible prevention of endogenous Candida endophthalmitis are discussed.

[Pseudophakic retinal detachment is frequently a sequela of surgical complications].

In a retrospective study we analyzed the records of 18 patients with pseudophakic retinal detachment. Cataract operation had been performed by extracapsular (n = 14) and intracapsular procedures (n = 6). In 14 of 18 cases, intra- or postoperative complications preceded the retinal detachment: capsular rupture with vitreous loss and anterior vitrectomy during cataract operation (n = 5), vitreous wick (n = 5), and explantation of the intraocular lens because of chronic intraocular inflammation (n = 3). At the time of admittance for retinal surgery, 14 patients (78%) had a detached macula. In 8 cases (44%), proliferative vitreoretinopathy had developed. In 17 of 18 cases (94%), the retina could be reattached by buckling procedures alone or combined with vitrectomy and SF6 or silicone oil tamponade. The risk of retinal detachment in pseudophakic eyes should be reduced by careful anterior vitrectomy in cases of vitreous prolapse and vitreous wick or by avoiding lens implantation in complicated cataract operations.

Effects of monocular strobe rearing on kitten striate cortex.

Monocular deprivation in kittens does not lead to an ocular dominance shift in striate cortex if the visual stimuli do not contain contours. In the present study we sought to find out whether an ocular dominance shift is produced if the visual environment does contain contours but is devoid of motion. Six kittens were reared with one eye occluded in a visual environment that was lit only by the light of a stroboscope (2 flashes per sec). Exposure was started at 5-6 weeks of age after dark-rearing from birth and extended until 8-12 weeks of age for 8 h per day. The rest of the time was spent in total darkness. Thus, the animals were completely deprived of vision in one eye, while the other eye experienced only stationary flashing contours. Single units in area 17 of these animals were studied and compared to normally reared cats. In all six animals ocular dominance was clearly shifted towards the eye with strobe experience. The ocular dominance shift showed, however, the following interdependencies with other parameters: neurones that responded to stationary flashing test stimuli were nearly always dominated by the strobe eye; neurones that responded only to moving bars or edges remained binocular. In the normal control animals the ocular dominance distribution was similar for both groups of cells. Track analysis according to cortical lamination revealed that neurones in infragranular layers consistently showed a weaker OD shift towards the strobe eye than neurones in supragranular layers (including layer 4).(ABSTRACT TRUNCATED AT 250 WORDS)
