Quantitative investigations into the elimination of in vitro-obtained spores of the non-pathogenic Clostridium butyricum strain CNRZ 528, and their persistence in organs of different species following intravenous spore administration.

Before the Clostridium tumour assay can be applied to the diagnosis of cancer, we sought to investigate--within the framework of a biopharmaceutical safety test--the organ persistence of test spores of Clostridium butyricum CNRZ 528. We found that non-pathogenic spores obtained in vitro, like pathogenic native spores, escape phagocytosis in various organs up until about 2 years, as tested by anaerobic cultures. The elimination of spores depended on the species of animal, the spore dose and the organs investigated. In rabbits, one week after injection, we recovered clostridial spores from blood and spleen cultures more rarely than from liver and lung. The half-life of blood clearance in patients was one day or, at half the spore dose, two days. That deep tissues of healthy animals are not normally sterile became evident in rabbits after sporadic isolation and characterization of non-administered saccharolytic and proteolytic clostridial species. During a 10-year observation period, the rate of obtainment of viable spores by in vitro cultures lessened; however, for administration of the spores in clinical phase I and phase II studies, the spore quality was acceptable.

A sophisticated lander for scientific exploration of Mars: scientific objectives and implementation of the Mars-96 Small Station.

A mission to Mars including two Small Stations, two Penetrators and an Orbiter was launched at Baikonur, Kazakhstan, on 16 November 1996. This was called the Mars-96 mission. The Small Stations were expected to land in September 1997 (Ls approximately 178 degrees), nominally to Amazonis-Arcadia region on locations (33 N, 169.4 W) and (37.6 N, 161.9 W). The fourth stage of the Mars-96 launcher malfunctioned and hence the mission was lost. However, the state of the art concept of the Small Station can be applied to future Martian lander missions. Also, from the manufacturing and performance point of view, the Mars-96 Small Station could be built as such at low cost, and be fairly easily accommodated on almost any forthcoming Martian mission. This is primarily due to the very simple interface between the Small Station and the spacecraft. The Small Station is a sophisticated piece of equipment. With the total available power of approximately 400 mW the Station successfully supports an ambitious scientific program. The Station accommodates a panoramic camera, an alpha-proton-x-ray spectrometer, a seismometer, a magnetometer, an oxidant instrument, equipment for meteorological observations, and sensors for atmospheric measurement during the descent phase, including images taken by a descent phase camera. The total mass of the Small Station with payload on the Martian surface, including the airbags, is only 32 kg. Lander observations on the surface of Mars combined with data from Orbiter instruments will shed light on the contemporary Mars and its evolution. As in the Mars-96 mission, specific science goals could be exploration of the interior and surface of Mars, investigation of the structure and dynamics of the atmosphere, the role of water and other materials containing volatiles and in situ studies of the atmospheric boundary layer processes. To achieve the scientific goals of the mission the lander should carry a versatile set of instruments. The Small Station accommodates devices for atmospheric measurements, geophysical and geochemical studies of the Martian surface and interior, and cameras for descent phase and panoramic views. These instruments would be able to contribute remarkably to the process of solving some of the scientific puzzles of Mars.

Exposure of an antigen of chromaffin granules on cell surface during exocytosis.

The synthesis rate of the membrane proteins of the catecholamine-storing vesicles (chromaffin granules) of the adrenal medulla is lower than that of the secretory proteins of the contents. Based on these results we proposed that after exocytosis the membranes of chromaffin granules are retrieved and are re-used for several secretion cycles (see also ref. 4). This concept of re-use of granule membranes has been further strengthened by the finding that exogenous markers which are taken up by secretory cells during stimulation can be traced to the Golgi region and to immature secretory organelles. However, one basic question remains: are the membranes of secretory organelles specifically and completely removed from the plasma membrane and if so, how fast is this process? By using an antiserum against a membrane glycoprotein of chromaffin granules we have now obtained quantitative data which demonstrate that during exocytosis this antigen becomes exposed on the cell surface and disappears again to a large degree within 30 min.

[Effect of hypertensive pregnancy complications on neonatal outcome of growth retarded fetuses].

OBJECTIVE: To determine the effect of hypertensive disorders in pregnancy on the neonatal outcome of growth restricted fetuses. There is conflicting data on the effect of hypertension during pregnancy on the incidence of neonatal respiratory distress syndrome (RDS) and intraventricular hemorrhage. Some studies report a lower incidence of RDS and intraventricular hemorrhage in infants of hypertensive mothers, whereas other studies report a similar or higher incidence in infants born to hypertensive mothers. STUDY DESIGN: We performed a retrospective analysis of 220 growth restricted fetuses born between January 1, 1996 to July 1, 1997 at the Department of Obstetrics and Gynecology of the University-Hospital at Homburg/Saar. Data were obtained by review of the medical records. Growth restricted infants born to preeclamptic women or women with HELLP syndrome were compared to growth restricted fetuses born to mothers without hypertensive disorders. RESULTS: Growth restricted fetuses born to hypertensive mothers had a significant lower birth weight (p < 0.05). The incidence of RDS in children born to hypertensive mothers was significantly higher (p < 0.05, p < 0.01) and they stayed significantly longer in the neonatal intensive care unit (p < 0.01). In contrast to infants born to mothers with HELLP syndrome (n = 7) there was no difference in the incidence of intraventricular hemorrhage, infection, sepsis, necrotizing enterocolitis or cardiac complications (arrhythmia, insufficiency) in case of preeclampsia (n = 68). The perinatal mortality of infants born to hypertensive mothers was significantly higher (p < 0.05, p < 0.01). CONCLUSION: This study does not support the contention that hypertensive disorders in pregnancy have a beneficial effect on the postnatal course of IUGR infants.

[Intrauterine growth retardation--perinatal mortality and postnatal morbidity in a perinatal center].

OBJECTIVES: The intrauterine growth retardation (IUGR) contributes specially to perinatal mortality and morbidity. An effective treatment is not yet available. The purpose of this study is to analyse the perinatal data of a cohort with IUGR-infants including mortality and morbidity and to describe prematurity and IUGR as a combination of high risk. STUDY DESIGN: We have analysed the perinatal data of a cohort of 220 patients with IUGR-infants including mortality and morbidity. Regularly we examined fetal ultrasonographic growth, doppler measurements of fetal and maternal vessels and CTG, as well as indicated amniocentesis, placentesis and cordocentesis. Entry criteria were: normal menstrual period before pregnancy, clear gestational age, complete history of pregnancy. RESULTS: At the Department of Gynaecology & Obstetrics, University Hospital, Homburg/Saar, the incidence of IUGR (< 10th Percentile) in premature babies and newborns was 13.1% over five years (1993-1997). The percentage of premature babies with IUGR was 18.6%. The overall mortality was 4.1%, the perinatal mortality was 3.6% and the neonatal mortality was 2.7%. In 1.4% intrauterine foetal death was observed, all associated with a birthweight below the 3th percentile. The average duration of pregnancy was 34 + 4 weeks. A high perinatal mortality of 13.1% and an overall mortality of 14.7% was observed. 11.4% of the premature babies with IUGR showed deformities and about 2% presented chromosome aberrations. 46.4% of children have been transferred to the Neonatal Intensive Care Unit. 57.1% of the postnatal complications have been related to the lungs, 26.5% to the cardiovascular system and 14.3% to the cerebrum. Maternal complications were referred to: SIH/EPH-gestosis (65.4%), HELLP-syndrome (5.8%), nicotine abuse (31.4%), pregnancy anaemia (17.3%) and gestational diabetes with insuline therapy (7.7%). On the placental site the most noticeable conditions have been placental insufficiency (40.6%) and placental infarction (28.7%). CONCLUSION: Children with IUGR are exposed to high perinatal mortality and postnatal morbidity. Premature babies in association with an IUGR are at high risk. The surveillance of the pregnant women and the new-born children should be performed in a perinatal centre.

[Placental morphometry in pregnancies with reversed enddiastolic flow in the umbilical artery or fetal aorta].

BACKGROUND: Fetuses with a reversed enddiastolic flow in the umbilical artery and/or aorta are at risk pregnancies and show a higher incidence of perinatal morbidity and mortality. The aim of this study was to evaluate the pathological changes in the terminal villous tree in fetuses with a reverse flow in the fetal aorta or umbilical artery. MATERIALS AND METHODS: In this retrospective study, 16 cases with a reverse flow in the fetal aorta or umbilical artery (Gr. 1) were compared with gestational age matched 16 healthy pregnant women (Gr. 2). The following morphometric parameters were evaluated in 50 sections in each placenta (1600 measurements): mean vessel diameter, volume density of the villous tissue, stem villi and terminal villi. Measurements were performed using a computerized Video Image Analysis system. RESULTS: The mean gestational age at birth was similar in both groups (30 + 4 weeks gestation vs. 30 + 6 weeks gestation, respectively, for Gr. 1 and Gr. 2). The birth weights (985 g vs. 1780 g) and the placental weights (216 g vs. 385 g) were significantly lower in the reverse flow group. There was a significant reduction in the proportion of total villous tissue (19 % versus 45 %) and in the diameter of tissue at the terminal villi (2.1 % versus 7.6 %) in the reverse flow group. Although the mean number of vessels at the terminal villi was lower than the control group (4.1 versus 5.6), the mean and total areas of the vessels were not different. CONCLUSION: Our observations showed a significant association between the placental morphometric parameters and reversed enddiastolic flow in the umbilical artery or fetal aorta. The reduced number of functional placental units is responsible for the diminished exchange function of the placental vessels in "reverse flow" cases. This could partially explain the adverse outcome in this clinical situation.

[Fetal and neonatal ascites: a report of 15 cases and a review of the literature].

Fifteen patients with non-immune fetal or neonatal ascites and absence of generalized edema are described. Nine patients were diagnosed prenatally. Nine patients were preterm. Artificial ventilation was necessary in twelve infants immediately after birth. In five cases, including two cases of transitory ascites, the etiology could not be established. In accordance with previous reports, we found the following underlying disorders in our patients: urinary tract obstruction, polycystic disease of the kidneys, chylous ascites, sepsis, intrahepatic atresia of the biliary ducts, hepatic and renal failure following severe asphyxia. 8 out of 15 patients died. A survey of the literature shows a variety of underlying disorders. Therefore, several diagnostic tests are necessary, in order to identify the etiology. Causative treatment is possible in some cases of urogenital , gastrointestinal, infectious and chylous ascites. Sophisticated perinatal management is essential for the outcome of patients with fetal or neonatal ascites; nevertheless the mortality rate is still about 50%.

[Non-immunologic hydrops fetalis--report of 14 cases and literature review].

With the advent of prenatal diagnosis and the feasibility of intrauterine treatment, non-immune hydrops fetalis (NHIF) is gaining increasing importance. 14 patients are reported 10 of whom having been diagnosed prenatally. 13 were born prematurely severe asphyxia being a frequent occurrence. 7 out of 14 infants survived. In 5 cases no definite etiology could be established. In 7 patients NHIF was attributable to cardiovascular diseases, predominantly associated with brady- or tachycardia. A survey of the literature reveals that etiology remains unidentified in about 40%. For the rest fetal, maternal or placental diseases are the primary causes. The abundance of underlying diseases are operative via three pathophysiological pathways: hemodynamic disturbances, decreased oncotic pressure and increased capillary permeability. By ultrasound examination, NHIF can be detected prenatally permitting intrauterine treatment in selected cases. In combination with improved postnatal management, this might contribute to a better outcome of NIHF which is still associated with a high mortality rate. Meticulous diagnostic work-up facilitates genetic counseling and the pursuit of prenatal diagnosis in subsequent pregnancies.

[Long-term analysis of cement-free implanted PCA knee endoprostheses with 5-8 year follow-up].

A total of 54 PCA knee-joint endoprostheses were followed up over a period of 5-8 years. All the uncemented implants (52 femoral components, 52 patellar components, 54 tibial components) were subjected to radiological analysis of the anchorage zone between prosthesis and bone. The results were evaluated on the basis of the postoperative radiographs and the follow-up radiographs, which were divided into four groups. It was necessary to create a 5th group for radiographs not suitable for evaluation. The use of fluoroscopy is therefore essential in order to ensure that all radiographs can be evaluated. The best results were obtained with the uncemented femoral prosthesis. Complete "incorporation" with close contact between prosthesis and bone was observed in all cases, even if implantation was not optimal. Radiological loosening of the uncemented patella occurred in 3 patients, and it was necessary to perform one patellectomy 6 years postoperatively. In the case of the tibia implant, the follow-up showed radiological "incorporation" of the prosthesis in all cases with technically adequate implantation: the tibia implant was inadequately implanted in 8 cases. Radiological loosening of the tibia implant was observed in 4 of these 8 patients. There was a higher rate of radiological failure in the group of patients over 70 years of age, and among patients whose primary disease was primary chronic polyarthritis. However, given technically exact implantation, satisfactory results could be obtained even in these two groups. Clinical pains occurred only several years after evidence of radiological loosening. Radiological follow-up analysis therefore offers a means of obtaining early information about the prosthesis-bone interface of uncemented total knee-joint endoprosthesis.

Sperm morphology and chromatin condensation before and after semen processing.

Semen analysis constitutes the most important investigation of male infertility. However, the true anomalies present in defective sperm cells have been only partially characterized. The integrity of the sperm chromatin may play the most important role, particularly in ICSI, where most of the natural selection mechanisms are bypassed. This study was carried out to characterize sperm morphology (strict criteria), to evaluate chromatin condensation and sperm count in native semen as well as after semen preparation by the swim-up technique, and to eventually evaluate any correlation between these parameters. Semen from 90 men was analyzed for the above parameters in both the fresh and processed semen. Whereas the sperm count decreased after sperm preparation by the swim-up technique in comparison to the value in the fresh semen (p < .001), there was an increase in the percentage of morphologically normal (p < .001) and chromatin-condensed sperm (p = .99). However, there was no correlation between sperm morphology, chromatin condensation, and sperm count either in the fresh or in the processed semen samples. These results suggest that sperm morphology, sperm count, and chromatin condensation are independent parameters that should be evaluated separately in the assessment of male fertility in an assisted reproduction program.

Chromatin decondensation of human sperm in vitro and its relation to fertilization rate after ICSI.

The inability of sperm chromatin to decondense has been implicated in the failure of fertilization, This study was undertaken to identify the relationship between sperm chromatin decondensation in vitro after incubation with follicular fluid at various points in time and fertilization or pregnancy rates after intracytoplasmic sperm injection. Moreover, an attempt was made to determine whether this test could be used as a predictive test for the outcome of ICSI. Thirty-two infertile couples undergoing ICSI therapy were included in this prospective study. One milliter of semen from each sample was mixed with 1 mL of follicular fluid obtained from ICSI patients at the time of oocyte retrieval and incubated for 24 h. Many smears were made directly after semen liquefaction at the following time intervals: 30, 60, and 120 min and 24 h. Chromatin decondensation was evaluated with acridine orange staining. The mean percentage of uncondensed chromatin of spermatozoa in the native semen samples was 25 +/- 18.3%, which increased within 24 h to 91 +/- 9.5%. On the other hand, the fertilization and ongoing pregnancy rates were 64 +/- 21.7% and 20%, respectively. However, no correlations were found between chromatin decondensation at various point of time (30, 60, and 120 min and 24 h) and fertilization rate. No correlation was shown between the chromatin decondensation and sperm counts in the ejaculate. morphology, or the percentage of condensed chromatin. In light of this study, chromatin decondensation in vitro cannot be recommended for predicting the fertilization potential of spermatozoa and pregnancy rates in the ICSI program. Further research is necessary, especially in cases where ICSI is being considered as a therapeutic option.

Detection, quantitation and contrast enhancement of myocardial infarction utilizing computerized axial tomography: comparison with histochemical staining and 99mTc-pyrophosphate imaging.

The potential role of the CAT scan in detecting and quantitating acute myocardial infarctions was assessed in the excised hearts of 25 dogs with 48-hour-old myocardial infarctions. The CAT scan detected the infarct in every instance and distinguished subendocardial from transmural infarctions. While the CAT scan slightly underestimated the volume of infarcted tissue, this volume had a close linear relationship to the true volume determined by histochemical staining. On the other hand, the 99mTc-pyrophosphate scintiscans grossly overestimated the volume of infarcted tissue. Intravenous administration of contrast material prior to sacrifice produced contrast enhancement either of the rim or diffusely throughout the infarct. Thus CAT should have an important role not only in detecting but also in estimating the volume of acute myocardial infarctions.

Direct and reflex myocardial effects of intracoronary administered contrast materials in the anesthetized and conscious dog: comparison of standard and newer contrast materials.

The direct and indirect actions on left ventricular dynamics of contrast material (sodium meglumine diatrizoate) currently used for coronary arteriography, modified ionic material (sodium meglumine calcium metrizoate) and non-ionic material (metrizamide) were assessed in conscious and anesthetized dogs. In both anesthetized and conscious animals, the diatrizoate compound caused an early (3--10 sec after injection) decrease in peak dp/dt and dp/dt/LVP40, followed by late (10--20 sec after injection) increases in these variables. The predominant early and later effects of the calcium metrizoate compound were increases in parameters of LV contractile state. Metrizamide produced no significant early alterations, but later induced a small increase in these variables. The positive inotropic actions of each of the contrast materials were attenuated by beta adrenergic blockade. The early effects of the contrast materials were similar in the presence of segmental ischemia. The late positive inotropic effects in response to the diatrizoate compound and metrizamide were not observed in the ischemic state, while the positive inotropic response induced by the calcium metrizoate compound was significantly reduced. Thus intracoronary administration of sodium meglumine diatrizoate produced direct myocardial depression, followed by adrenergically mediated myocardial stimulation. Calcium metrizoate caused prominent direct and adrenergically mediated augmentation in contractile state. Metrizamide induced the least alteration in LV contractile state.

Uptake of contrast materials by experimental acute myocardial infarctions: a preliminary report.

The concentration of iodine within infarcted and normal myocardium after intravenous administration of contrast material was determined by fluorescence excitation analysis in seven dogs at 48 hours after coronary arterial ligation. The iodine concentration of infarcted myocardial tissue was several times greater than normal myocardium after administration of meglumine/sodium diatrizoate, iodipamide, and an experimental polymer of iothalamic acid.

Role of iodinated contrast material in the evaluation of myocardial infarction by computerized transmission tomography.

In vivo and in vitro studies have shown that areas of myocardial infarctions can be delineated on CT scans after intravenous administration of iodinated contrast material to dogs with experimental myocardial infarctions. Within the first minute after intravenous administration of contrast material (initial myocardial perfusion phase), the infarct appears an as area of decreased x-ray attenuation (cold spot image). Studies using radiolabeled microspheres indicate that the relative attenuation numbers throughout the myocardium in this phase reflect myocardial perfusion (Hessel et al, 1978). Delayed scans obtained at 5 minutes and later showed a reversal in the attenuation values of normal and infarcted myocardium with the infarct appearing as an area of homogeneous or mottled increase in x-ray density compared to normal myocardium. (Siemers et al, 1978; Carlsson et al, 1977; Higgins et al, 1979). This phenomenon of delayed contrast enhancement of myocardial infarctions ensues as early as 8 hours after coronary occlusion (Higgins et al, 1979) and is present in evolved infarcts as old as 51 days (Newell et al, 1979). Direct measurements of iodine concentration in infarcts, normal myocardium, and organs surrounding the heart were performed in dogs with 48-hour-old myocardial infarctions using fluorescent excitation analysis (Higgins et al, 1978, 1979). These measurements of iodine concentration in tissue samples obtained at 10-180 minutes after intravenous administration of contrast material indicated that the highest iodine concentration in the infarct was at 10 minutes, but the highest ratio of iodine concentration of the infarct to normal myocardium was at 180 minutes. At all time intervals, the concentration of iodine within the infarct was fivefold greater than in the normal myocardium.

Association between sperm cell chromatin condensation, morphology based on strict criteria, and fertilization, cleavage and pregnancy rates in an IVF program.

In this study, a total of 95 ejaculates from infertile patients were investigated morphologically according to Kruger's strict criteria and 78 of the 95 ejaculates were stained for chromatin condensation with acidic aniline blue. Patients were divided into two groups based on the percentage of morphologically normal spermatozoa as follows: Men with normal sperm morphology < 14% (Group 1), and men with normal morphology > 14% (Group 2). The relationship between percentage of normal sperm morphology and fertilization, cleavage and pregnancy rate was analysed. The rates were 33.7%, 57.1% and 0.0% respectively, in the first group. The corresponding values for the second group were 76.1%, 68.2% and 32.1%. The fertilization and pregnancy rates correlate significantly with morphologically normal spermatozoa. In regard to the percentage of morphologically normal spermatozoa stained with aniline blue, patients were divided into two groups: patients with 0-20% stained spermatozoa (Group I) and those with > 20% (Group 2). Fertilization and pregnancy rates were higher in the first group than in the second group (79.9%, 52.8% vs. 58.8%, 29.5%). The results demonstrate that chromatin condensation visualized by aniline blue staining is a good predictor for IVF outcome and should be considered besides morphology by sperm assessment for patients undergoing IVF treatment.
