A trial of isolating a tumor-like form of pulmonary cavities infected with aspergilli. II. Intra- and postoperative, morphologic and serologic data.

In ten specimens of resected pulmonary tissue, a little known and underscribed form of aspergillosis called tumor-like blocked pulmonary cavity with liquid content infected with aspergilli was observed. This designation was based on the finding in the specimens of tumor-like cavitary structures with blocked bronchus and contents from which abundant growth of aspergillus fumigatus was invariably obtained in cultures. Nine of the ten specimens were bronchiectasis, and one was a bronchogenic cyst. The scant literature was review, emphasizing features characteristic of this form of mycetoma and differences from the typical and frequently encountered form of ordinary pulmonary aspergilloma. The serologic response and its long duration were attributed to biological properties of aspergilli in this form caused by inaccessibility of oxygen and by occlusion of the bronchial lumen.

The test of non-specific cell-mediated response in animals with transplantable tumours stimulated with calf thymus extract.

Investigations were carried out on the non-specific cell-mediated response in mice with transplantable tumours: LLC in mice C57Bl and F1(C57Bl X C3H), and SAL-l tumour in Balb/c mice. The malignant cells were implanted subcutaneously for observation of changes in the non-specific cell-mediated response at the time of the local growth of the tumour, and injections into the vein or into the foot-pad were made for studying the non-specific cell-mediated response at the time of tumour dissemination. In each series of the experiments the animals received an extract of calf thymus--the preparation TFX-Polfa. The control groups had the tumour cells implanted or injected but were not given TFX. For determining the non-specific cell-mediated response the animals were immunized with picryl chloride. In both experimental models, that is with subcutaneous implantation and with injections intravenously or into the foot-pad a decrease of the non-specific cell-mediated response was observed progressing gradually with extension of the tumours. Long-term administration of TFX in doses of 0.1 mg per mouse prevented the decrease of the non-specific cell-mediated response in the first place in mice with subcutaneous implantations. In mice with malignant cells injected intravenously or into the foot-pad TFX administration had no evident effect on the value of the test. This observation suggests that TFX prevents the development of a secondary immune deficit in mice with subcutaneous transplanted tumours--that is mainly at the time of local growth of the tumour, but is of little use in the phase of tumour dissemination.

Certain parameters of oxygen pulmonary toxicity during hyperoxia.

The results of the investigations on the effect of highly concentrated normobaric oxygen on rabbit lung are reported. Detailed pathomorphological analysis demonstrated that rapidly developing extensive changes of the structural elements of the capillaries and other vessels were the prevalent lesions. These changes are connected, in the light of earlier experiments carried out under identical conditions, with the observed disturbances in the metabolism of pulmonary lipids and suggest pathogenetic correlations between prostaglandins and pulmonary surfactant.

A trial of isolating a tumor-like form of pulmonary cavities infected with aspergilli. 1. Preoperative data, diagnostic difficulties.

A retrospective analysis was made of the case histories, clinical course and results of auxiliary examinations in 10 patients with tumor-like lung shadows, which preoperatively were suspected of aspergilloma, tuberculoma, malignant tumor or "tumor". The postoperative diagnosis being known, the preoperative diagnosis difficulties were analyzed. Aside from only one patient in whom a correct diagnosis was reached, and despite numerous examinations indicated by suspicious, diagnoses were imprecise, with two or three alternatives. Although not performed systematically, the preoperative mycologic sputum studies and serologic tests were negative. Nevertheless, in four cases, fungal etiology of the tumor was suspected, three of them with single bronchiectasis and blocked bronchus. Examination of operative specimens led to the isolation of a little known form of pulmonary aspergillosis, which in the second part of this study (6) was named tumor-like blocked cystic space with liquid contents infected with aspergilli (TBLA).

[Our modification of ergograph for studying muscle fatigability in myasthenia].

The authors designed and made an ergograph of own modification and elaborated normal values of fatiguability half-time on the basis of 51 healthy subjects comparing the results with those in a group of 41 myasthenics subjected to thymectomy. The fatiguability half-time was determined in groups of muscles in the hand, axilla and popliteal area and masseters before and 14 days after thymectomy. The ergographic record was obtained without application of drugs as well as at the peak of activity of drugs and at the peak of activity of those after 5 mg Tensilon dose. The investigation made it possible to determine the muscle power before and after tymectomy. The Tensioln test at the peak activity of drugs permitted to establish the necessary dose of anticholinesterase drugs, especially after the operation.
