[Postmortem changes after 50 years immersion (1939 reconstructed suicide)].

In September 1989 an old motor car was accidentally discovered in a lake, 50 meters below the water surface. The automobile contained the torso of an adult woman with extensive adipocere formation and the skeletonized body of a man. Forensic and criminalistic investigations lead to the identification of the deceased persons. In the year 1939 the persons together had committed suicide. The corpses were immersed for fifty years.

[Sudden death of a young asthmatic patient--allergic granulomatous angiitis (Churg-Strauss syndrome)].

Churg-Strauss syndrome has three main criterions: 1) severe asthma with fever attacks, 2) extra- and intravascular granulomatous proliferations of epitheloid and giant cells, 3) inflammatory infiltrates rich in eosinophilic leukocytes. All these features were given in the case of a 19-year-old woman, who died suddenly. This paper deals with the morphological findings of the forensic autopsy.

[Premature craniosynostosis--cause of sudden death in children and young adults].

Primary premature craniosynostosis ordinarily leads to characteristic skull abnormalities, if the ossification of the sutures happens in early childhood and involves all sutures, the elevation of intracranial pressure may not cause severe clinical features and the head morphology may be normal. A disproportion between skull capacity and brain volume will be the result. Three cases are reported, where these preconditions combined with exterior effects like minimal trauma, physical activity or consumption of alcohol or medicaments caused sudden death. Important autoptic findings are ossified cranial sutures with the absence of suture lines and increased scalloping of the internal table of the skull (impressiones digitatae).

[Post-traumatic hydrocephalus--result of repeated abuse of a young child].

Serious physical and psychic abuse of children is a much discussed theme in forensic, juridical and sociological literature. This paper deals with the pitiable case of a meanwhile four years old infant having been battered by his stepmother. At the age of two years the boy was unconscious admitted to the hospital. Computertomography of the brain did not show any pathological findings. Because of multiple effusions of blood of variable age located at the child's body the stepmother was noticed and accused of assault and battery, but the trial ended in acquittal. Two years later the boy suffered from serious neurological symptoms and computertomography showed high-pathological alterations such as hydrocephalus, parietooccipital hygroma and a damage of the brain.

[Attempted infanticide with a happy ending. Report of infanticide in Tirol and Vorarlberg].

The paper presents an analysis of infanticides which happened during the last 20 years, investigated by the Institute of Forensic Medicine at the University of Innsbruck. On the basis of some casuistries forensic and criminalistic aspects of infanticides will be discussed. Special attention is payed to two cases of "survived infanticides" with re-establishing of the relationship between mother and child.

[Skin damage caused by fatal benzene fume inhalation].

Two young men tried to stop a leaking patrol pipe of a private car in a workshop pit. One of them died from petrol-vapour-poisoning. Toxicological analyses by means of headspace gas chromatography permitted the identification of gasoline vapour. The other victim lost consciousness for a few hours and survived. By investigation of both victims alterations of skin were observed and interpreted as combustions with burns and blisters. Therefore the law-enforcement agency supposed an explosion or deflagration of gasoline vapour. In reality the experts on burnings could not gain any indication of an explosion and we proved the skin alterations to be a dermatotoxic effect of petrol.

[Fatalities in Alpine skiing--autopsy results and forensic medicine conclusions].

135 corpses of persons died during alpine skiing were dissected between 1978 to 1987 on the Institute for Forensic Medicine, University of Innsbruck. 83% are masculine sex. 74 percent died as a consequence of an accident. 26 percent died a natural death. The causes of death and of the accidents which could partly be reconstructed are discussed. It is pointed out, that the forensic autopsy not least in demanded in connection with assurance questions.

[Fatal accidents of motorcycle riders. Comparison of the craniocervical injury picture before and following introduction of the legally sanctioned protective helmet rule].

From 1982 to 1987 79 bodies of motorcycle riders have been dissected. Accident investigations by police proved, that 33 of them used protective helmets in the moment of accident, 40 of the victims were unhelmeted. In case of 6 accidents police investigations could not assign victims to one of those groups definitely. Our study demonstrates the change in injury patterns of the craniocervical region before and after introduction of liability for using protective helmets. We could determine, that the establishment of this law resulted in a significant recession of serious skull and brain injuries. On the other side we found a coincidental increase regarding injuries of craniocervical location, but mostly without fatal consequence.

[Postnatally acquired hypothyroidism in an infant a variant of congenital hypothyroidism (author's transl)].

We are reporting a case of hypothyroidism in an infant which developed at the end of the second quarter of the first year of life on the basis of a dysgenetic thyroid gland. By the time of its diagnosis in the ninth month of life it had led to bone age retardation and dysgenesis of the second lumbar vertebra. Clinical picture, longitudinal growth and psychomotor development were minimally or not affected. Pathogenetically this case of relatively mild postnatally acquired hypothyroidism has to be classified under the spectrum of congenital hypothyroidism. Clinically it is important that it would not have been picked up by a neonatal screening program and still might have led to impairment of the cerebral development.

[Congenital hypothyroidism].

The clinical picture of congenital hypothyroidism in the newborn period as well as in infancy is presented. Those signs and symptoms are particularly emphasized which in spite of being clearly present in the newborn are frequently not registered and mostly not correctly interpreted. This is meant to aid in earlier clinical diagnosis of congenital hypothyroidism. Furthermore, guidelines for diagnostic work-up, therapy and follow-up are presented. For an optimal mental and neurological prognosis early treatment is of paramount importance.

3H-Cymarol in man. Excretion pathways and serum protein binding.

In 10 patients, 5 having received 3H-cymarol i.v., 5 orally, the radioactivity in plasma, urine and in the feces of some patients also was determined. After oral administration the plasma levels rose rapidly reaching maximum levels 1--2 h after administration. After i.v. injection about 30% of the given radioactivity were excreted in the urine. The remaining radioactivity was found in the feces suggesting a high biliary excretion. Only 10% of the radioactivity excreted in the first 24 h were chloroform-extractable. The radioactivity found in the urine after oral administration of the drug amounted to 17.6%. Between 51.1 and 58.5% of the drug were bound to serum proteins.

[Gunshot injury caused by a training bullet].

In the course of a scuffle a 50-year old man has been shot by a revolver. The culprit and witnesses state, there has been only one shot, whereas the victim declared having been shot twice. The medico-legal investigation revealed a gunshot wound that passed through the left thigh, a grazing shot of the right hand and a superficial lesion of the skin at the forehead with sprinkled gunshot residue in the epidermis. The investigation confirmed the statement of the victim: the first shot to the forehead was caused by a plastic training cartridge, the second to the right hand and thigh by a cartridge loaded with an ordinary bullet.

[Fatal exsanguination from a small venous injury].

Report on a 34 year old man who bled to death sitting in an empty bath-tub by suicidal cutting in his groin. The source of the bleeding was a small lesion of the femoral vein with a diameter of 2 mm. No endogenous or exogenous coagulopathies could be established.

["Self-asphyxia"--a forensic medicine-criminal challenge].

In judicial and police circles - sometimes even in circles of medical professionals - it is still often thought that suicidal strangulation by ligature is impossible. Reviewing the forensic literature we could find more than 200 cases that have been published earlier. We report another 4 cases of suicidal strangulation. The main problems of criminalists and medical examiners in such cases are discussed. Primary investigation of the corpse in original position by a forensic scientist is of high importance.

[The "inner gunshot residue"--a gunshot entry marker in gunshot of ridged skin].

Gunshot residue below the surface of the skin is typical of a contact gunshot wound. However, in a case of our own, when a taxi-robber was shot by the cab driver through his right hand at a distance of several yards, this finding provoked quite a confusion. Only when experimental shots at thick glabrous skin were performed, it could be established that long range shots led to intraepidermal gunshot deposit too. The explanation of this phenomenon lies in the tenacity of thick skin. Unlike the thin hairy skin, which is punched out by the penetrating projectile, the thicker and more tenacious plantar/palmar epidermis gets displaced radially, the stratum germinativum tears and the projectile casts its gunshot residue in the pouch formed in such a manner. Therefore, intraepidermal gunshot residue is proof of an entrance gunshot wound but not always a sign of contact shot if the wound is located in the palmar or plantar region. It is typically produced by shots at thick skin.
