Based on new scientific knowledge, new aspects of emergency medicine have arisen. They are built on practical organizational structures, are practised both inside and outside medical facilities and have proved their usefulness. Nevertheless, limits to current emergency medical therapeutic measures exist and are to be taken into account. Doctors working in research and daily practice are urged to advance the development of emergency medicine for the benefit of the patient.
Potential organ donors can be expected in every hospital. In the GDR, the annual number of donors nearly corresponds to the number of hospitals, e.g. 1985:500. Even if they work as donor hospitals only once every 1 to 2 years, medical and organizational problems are unavoidable. Taking this aspect into consideration, a regional coordinating centre for the District of Magdeburg with 1.3 million inhabitants and 24 hospitals was established in 1981 at the Clinic for Anaesthesiology and Intensive Therapy of the Magdeburg Medical Academy. This centre advises the "donor-hospitals" and admits potential donors if they are transportable. In 8 years, the centre was consulted in 148 cases. Fifty-six of the resulting organ explantations were performed at the centre. The main advantages of such a centralized service are: permanent presence of specialists who are experienced in organ donor procedures, close local and organizational links to special groups for diagnosing brain death, performing organ explantation, artificial organ perfusion and organ transportation, the possibility to admit patients for "donor conditioning".

Choice and application of anaesthesia in patients undergoing endoscopies of the airways have to take into consideration several specific aspects, starting with preanaesthetic investigation and premedication. On principle local anaesthesia may be administered as well as general anaesthesia or a combination of both. Using modern forms of general anaesthesia, optimal oxygen supply, sufficient carbon dioxide elimination, complete muscle relaxation and good sleep induction conditions can be provided. On the other hand reduction of reflex activity and analgesia are to be improved. The importance of a well trained and experienced anaesthetist and careful intra- and postanaesthetic supervision of the patient is emphasized.
Cardiovascular disorders during endoscopy of the upper respiratory tract have since long induced anaesthetists and laryngologists to find a suitable anaesthetic procedure. It was the purpose of the present study to determine the influence exerted by different kinds of premedication and different modes of anaesthesia on the cardiovascular system, pO2, and the acid-base status. The results obtained show that a hypertensive-tachycardiac response to direct laryngo-tracheoscopy could not be prevented in any of the groups under investigation. Lowest blood pressure and heart rate increases were seen in those groups of patients which received an analgetic drug (Fentanyl or nitrous oxide) in addition to hexobarbital. During endoscopy, respiratory acidosis occurred in five groups of patients. All patients were sufficiently oxygenated.
Pre-operative fear is not only a psychologically stressing phenomenon for the patient, but also a considerable risk factor due to its somatical correlative. In a study, 150 patients who were to undergo planned surgery under general anaesthesia were questioned about their fears and worries concerning the anaesthetic. Self-rating scales and rating scales by doctors and a complaints analysis curve were used in addition to an interview system on illnesses and specific previous experiences. 54% of the patients were, according to specific criteria, assessed as being anxious or very anxious (for example diffused anxiety, premature weakening of the anaesthetic effect, worry about not waking up out of the anaesthetic). Several factors which could have an anxiety-lessening or increasing effect and which are of importance for the psychological preparation for an operation were deduced from the investigation.

The experience gained in the ascertainment of the brain death in 45 cases permits to draw the following conclusions: 1. The irreversible functional loss of the brain is in the first place diagnosed on the basis of the clinical-neurological findings. 2. The clinical ascertainment of the brain death requires a considerable amount of experience and should be carried out jointly by neurologists and anaesthesiologists. 3. By using cerebral angiography for proving the arrest of the cerebral circulation it is possible to ascertain the brain death earlier, to cut the pending time and to carry out a possibly envisaged removal of an organ in the period before the dropping of temperature and blood pressure.
General anaesthesia is also justifiable in dental and maxillofacial interventions on an outpatient basis. Having dealt with the indications, the authors concern themselves with preliminary examinations and premedications. Finally, they comment on, and evaluate from the viewpoint of practice-relevance, anaesthetizing methods, such as intravenous short anaesthesia, ketamine anaesthesia and mixed anaesthesia, which have proved a success for a total of 20 years of interdisciplinary co-operation of dentists and maxillofacial surgeons and anaesthetists.
