Geriatric respite care--present practice and the potential for improvement.

A prospective Audit of patients admitted for geriatric respite care was undertaken. Information was obtained from patients, staff and carers for 125 consecutive admissions over a 20 week period. The 87 clients admitted had a mean age of 79.6 years and 83% had a high physical/mental dependency. In contrast to some previous studies mortality was very low (less than 2%). One hundred and nine (87%) admissions were discharged home. Respite admission bed use was classified as shared care (971 days, n = 19), planned respite (937 days, n = 50) and crisis respite (809 days, n = 18). The shared care and planned group did not show significant differences in patient dependency or carer profile. On a population basis, geriatric respite care was required on a bed per 1,000 population aged 75 and over. Patients viewed as inappropriate for geriatric care were most frequent in the crisis respite group, with the Geriatric Unit at times meeting shortfalls in other services. The sharing of key information between families/carers and community/hospital was, at times, deficient. The need for increased publicity about the service and for a carer support group was identified. Most families were satisfied with the care received but specific ways of improving the service were suggested.

Pyogenic liver abscess in the elderly.

Seventeen patients over the age of 60 years, admitted to the Aberdeen Teaching Hospitals between 1980 and mid-1988, were diagnosed as having pyogenic liver abscess. Eleven were women and the mean age was 73.5 years. The diagnosis of liver abscess was made by needle aspiration in seven patients and was an unexpected finding at laparotomy in three. Six patients were thought to be suffering from advanced malignant disease until autopsy. The overall fatality was 53%. Misleading results from special investigations reduced the chance of a correct diagnosis in several patients. Even in frail elderly patients, a filling defect in the liver should not be attributed to metastatic malignancy when there is no known primary site, until liver abscess has been excluded by aspiration biopsy under ultrasound or computerized tomographic scan guidance. The high fatality of pyogenic liver abscess seems to be at least in part due to a lack of clinical suspicion.

"Liver cancer ... or is it?".

There has been a gradual change in the pattern of presentation of pyogenic liver abscess with an increasing incidence in the elderly. At the same time an improvement in mortality with early diagnosis and treatment has been recognised. We describe two patients in whom the diagnosis of liver abscess was not suspected until autopsy in one and aspiration of pus during biopsy of a liver "tumour" in the other.

Prescribing patterns observed in registered nursing homes and long-stay geriatric wards.

Details of medication prescribed to 400 elderly patients in 18 registered nursing homes and 233 patients in 11 long-stay geriatric wards were obtained. Excluding laxatives, there was a mean of 2.15 medications prescribed for the long-stay patients, compared to 2.62 items for the nursing-home population. There was apparent over-prescribing of certain drugs within the nursing-home population.

Long term amantadine treatment. The danger of withdrawal.

Three elderly Parkinsonian patients on long term amantadine therapy (mean nine years) are presented. In each case abrupt withdrawal of amantadine resulted in rapid deterioration in the patient's condition which was corrected by reintroduction of amantadine.

Nursing dependency in registered nursing homes and long term care geriatric wards in Edinburgh.

There has been growing interest and public investment in registered nursing homes, apparently based on the assumption that these homes are the private equivalent of hospital long term care. We have tested this hypothesis in a survey comparing 400 patients in 18 registered nursing homes with 217 patients in 11 geriatric long term care wards in Edinburgh. The nursing home patients formed a distinct and separate group: 362 (92%) were women, 392 (98%) were single or widowed, and 358 (90%) were self financing, whereas in the geriatric long term care group 148 (68%) were women and 35 (16%) were still married. Patients in nursing homes were also far less dependent than those in geriatric long term care wards (p less than 0.005). This study suggests that there may be large differences between the patients in these two types of institution, particularly with regard to nursing dependency. This finding has important implications in the future planning of long term places for the dependent elderly.

A survey of registered nursing homes in Edinburgh.

Between February and November 1984, 18 registered nursing homes in Edinburgh were visited and information was obtained about their 400 patients. Details of the facilities provided by these homes are presented here. Admission and biographical details were obtained for each patient and the dependency of the nursing home population was determined; 28% of the total population appeared to be independent in self-care, though there were large variations between homes. Forty-two patients (10.5%) were found to be receiving Department of Health and Social Security supplementary benefit. The characteristics of this group are compared with those of the rest of the nursing home population. The implications of these observations and recent legislative developments are discussed.

Spontaneous pneumothorax: a retrospective review of aetiology, pathogenesis and management.

A retrospective analysis was made of all spontaneous pneumothoraces admitted to a Respiratory Unit between 1976 and 1981. There were 117 individuals responsible for 148 admissions, with a male to female ratio of 2: 1, giving a higher incidence amongst females than has been previously reported. Significantly fewer admissions were noted during May, June and July. Of the total survey population 89 per cent had smoked and in 40 per cent there was pre-existing lung disease. There was a history of two or more pneumothoraces in 36 per cent. Tetracycline pleurodesis, while attempted in 19 patients, was successful in only ten. Twelve patients underwent thoracotomy with a preponderance following right sided lesions. The duration of admission was more than 20 days for 17 per cent of the population. The aetiology, pathogenesis and management of pneumothorax is discussed.

Alteration of pharmacokinetics after halving a slow-release theophylline tablet.

A study was carried out in 10 healthy volunteers to determine the effects on the bioavailability of theophylline from a biconvex-shaped, slow-release 400 mg theophylline tablet formulation when the tablet was taken whole or as two separate halves in a single dose. The subjects took one or other dosage regimen on different occasions separated by a 7-day washout period. The results showed that absorption was faster and plasma levels were higher when the 400 mg tablet was taken as 2 halves.
