Presurgical angiographic localization of small bowel bleeding site with methylene blue injection.

The source of gastrointestinal hemorrhage is found in 94-95% of patients who undergo radiologic and/or endoscopic examinations of the upper or lower bowel. However, the exact presurgical localization of small bowel bleeding sites, which account for the other 5-6%, remains diagnostically difficult. We describe a previously reported and uncommon but useful angiographic technique used in conjunction with surgery which assists in this localization.

Radioactivity in environmental samples taken in the Sellafield and Ravenglass areas of West Cumbria, 1977-1982.

Terrestrial foodstuffs and other materials of agricultural importance have been analyzed for fission products and actinides discharged in waste from the Sellafield processing works in west Cumbria . Results obtained between 1977 and 1982 are summarized and it is concluded that the internal radiation exposure of the general public due to consumption of locally produced food in no instance amounts to more than a small fraction of recommended limits. Deposition of seaborne sediment is shown to be the main route by which radionuclides of Sellafield origin reach grazing land bordering a nearby tidal estuary.

The effect of decreased renal function with and without reduction in renal mass on 1,25-dihydroxycholecalciferol production in rats.

Decreased production of 1,25-dihydroxycholecalciferol (1,25-(OH)2D3) by the kidney may be responsible for vitamin D resistance in renal failure and contribute to renal osteodystrophy. The present studies investigate the effect of reduced renal mass and reduced renal function on conversion of 3H-25-hydroxycholecalciferol to 3H-1,25-(OH)2D3 in vitamin D-deficient rats. Renal mass was reduced by partial or total nephrectomy. Renal function was reduced by ureteral ligation. At comparable degrees of renal function, reducing renal mass decreased 1,25-(OH)2D3 formation approximately proportional to the amount of renal tissue removed. Ureteral ligation without reduction of renal mass also reduced production within the first 24 hours. This may have been caused by the marked hyperphosphatemia which occurred with ureteral ligation. When partial nephrectomy and ureteral ligation were done together the results were additive. The decreased 1,25-(OH)2D3 formation produced by unilateral nephrectomy was not improved 4 weeks after surgery, although renal function had returned to normal.

Adoption of Thayer-Martin culture medium by physicians in office-based practice.

In the summer of 1972, 26.4% of 900 physicians surveyed in the United States reported that they had performed one or more endocervical cultures for gonorrhea during a week of study. Obstetrician-gynecologists (62.8%) were considerably more likely than others to employ this diagnostic technique. The use of Thayer-Martin cultures was associated with the number of women examined and type of laboratory used (public, private or hospital), but not region of practice (East, Midwest, South, of West), area of practice (urban, suburban, or rural), or month of interview (July, August, or September). Results of this survey are consistent with the diffusion of innovation model proposed by Rogers.
