Ranitidine-associated hallucinations.

Adverse reactions involving the central nervous system have been rarely reported with ranitidine, a H2-receptor antagonist. A 72 year old female is described that developed visual hallucinations shortly after initiation of ranitidine therapy. Rechallenge with the drug resulted in both visual and auditory hallucinations following a single dose.

Clinical study of mastitis-metritis-agalactia in sows in Illinois.

Mastitis-metritis-agalactia (MMA) occurred in 6.9% of 16,405 farrowings recorded over 1 year (1981) in 31 Illinois swine herds. Prevalence of MMA among herds ranged from 1.1% to 37.2% and was not correlated to herd size. There was a significant positive correlation between the prevalence of MMA and the housing system wherein sows were moved from pasture gestation to crates immediately before farrowing. A significant seasonal peak of MMA was found for the third quarter of the year (10.3%). Mean prevalence of MMA was 13% in pluriparous sows and 4.2% in primiparous sows. The difference was not significant. Sows with MMA gave birth to more live pigs per litter compared with healthy sows (+0.7 pigs per litter). The number of stillbirths and pigs found dead after birth also was higher in sows with MMA (+4.6%). Total pig mortality until 1 week of age was 56% in litters of pluriparous sows with MMA and 17% in litters of healthy pluriparous sows and, respectively, 41% and 15% in litters of primiparous sows. The mean number of pigs per litter surviving at 1 week of age was 5.6 in pluriparous sows with MMA and 9.3 in healthy sows and, respectively, 6.2 and 8.3 in primiparous sows. For all herds investigated, mean litter size at birth (including stillbirths) was 11.0. Total pigs mortality until 1 week of age was 31%. Pig losses attributable to MMA, at 6.9% prevalence of the disease, were 4% of all pigs born and 12.9% of the pig mortality until 1 week of age.

Pharmacological differentiation of postsynaptic alpha adrenoceptors in the dog saphenous vein.

The dog saphenous vein has postsynaptic subpopulations of both alpha-1 and alpha-2 adrenoceptors which are easily demonstrable using selective agonists and antagonists. Specific alpha-1 (methoxamine and SK&F 89748) or mixed alpha-1, alpha-2 (l-norepinephrine and M7) agonists as well as the specific alpha-2 agonist, BHT 920, cause concentration-related contraction of this tissue. However, maximum contractions produced by alpha-2 activation are significantly less than maximum contractions produced by alpha-1 or combined alpha-1, alpha-2 adrenoceptor activation. SK&F 89748-induced contractions are competitively inhibited by prazosin (pA2 = 7.74) and rauwolscine (pA2 = 6.63); BHT 920-induced contractions are unaffected by prazosin but inhibited by rauwolscine (pA2 = 8.93). Contractile responses to l-norepinephrine are inhibited by prazosin, rauwolscine or phenoxybenzamine in a manner that suggests that norepinephrine interacts with two subtypes of alpha adrenoceptors in this tissue. These data indicate that the dog saphenous vein strip is a suitable in vitro preparation for study of drug action at both postsynaptic adrenoceptors inasmuch as either subpopulation of alpha adrenoceptor can be studied independently using specific agonists or antagonists.

Reorganizations of health agencies by local government in American urban centers: what do they portend for "public health"?

The major health agencies of local urban governments--public health department and public hospital--are often dismissed as "irrelevant" or attacked as "unresponsive". The rush to reform through reorganization hasn't often addressed the underlying problems of an eroded tax base, an indifferent federal policy, and intra-professional disputes. Newer corrective steps are called for if complex demands for public and personal service are to be met.

Are rectogenital chlamydial infections a marker of sexual abuse in children?

In order to examine the occurrence of chlamydial infection in sexually abused children, we cultured 51 abused children and 43 controls, 2 to 14.5 years of age. Only seven of the children were male. Chlamydia trachomatis was isolated from the vagina or rectum of two abused girls, 10 and 12 years of age, at their follow-up examination several weeks after the suspected abuse. C. trachomatis was also isolated from the vaginal culture of two controls and from the throat and rectum of another girl enrolled as a control. The two control children with vaginal infection were sisters, 7 and 10 years of age, who had been abused by their step-father 3 years previously. An antichlamydial immunoglobulin greater than or equal to 1:32 or a fourfold rise in the titer of immunoglobulin G was found in 13 of 49 (27%) abused children and 2 of 40 (5%) controls. This difference was statistically significant. The isolation of C. trachomatis from a rectogenital site was not limited to children with recent sexual abuse in this population. Serology may also be of limited use since serologic evidence of recent chlamydial infection was present in 5% of controls.
