[Delayed results after implantation of bifurcated prosthesis in aorto-iliac incompetence].

Early results and those seen after a 5-year follow up are discussed. Three hundred fifty one patients with aortoiliac incompetence were treated with the implantation of bifurcated aortoiliac prosthesis. Very favourable early result was achieved in 284 (81%) patients. Very favourable delayed effect was noted in 133 patients, i.e. in 75% of patients who reported for the control examination. In the follow up period, amputation of the limb was performed in 30 (8.5%) patients because of progressing ischemia. Hundred thirty seven (39%) followed up patients died during 5 years. Causes of so high mortality rate are discussed in view of risk factors and postoperative complications.

[Indications for and results of treatment of carotid artery stenosis].

The authors discuss own experience with the treatment of carotid artery stenosis. Diagnostic principles and indications to surgical treatment, technique of block anesthesia, an operation and obtained results are also discussed. Internal carotid artery stenosis was treated surgically in 90 patients within 1985-1988. Block anesthesia enabling intraoperational control of patients' consciousness was used in 72 cases. Satisfactory early results were noted in 92.9% of patients. Two patients died after surgery whereas 5 patients suffered from neurological complications. Favourable late results (follow-up period ranged from 6 to 48 months) were noted in 64 out of 70 patients which reported to clinical examination (91.4%).

Polish vascular prostheses.

Vascular prostheses have been manufactured in Poland since 1960. Double velour crimped tube and bifurcation prostheses are currently available under the Trade Mark Dallon. The Authors present and discuss 812 grafting procedures for the aorto-iliac occlusive disease with the use of 457 Polish and 355 foreign-made vascular prostheses. Wound infection occurred in 7% of the patients with implanted Polish prostheses and in 6.8% of the patients with implanted foreign-made vascular grafts. The acute thrombosis of the operated artery was similarly frequent in case of Polish and foreign-made prostheses implantation, 7% and 6.8% respectively. Haemorrhagic complications were more frequent in patients with implanted Polish prostheses (1.7%) than those with foreign-made vascular grafts (0.8%). Early results of the grafting procedures (6-month follow-up) were nearly the same in both groups and the results reported in the available literature. The Authors conclude that the Polish knitted vascular prostheses have properties similar to the foreign-made ones.

Application of transcutaneous oxymetry in vascular surgery.

Our study is aimed at the assessment of transcutaneous oxygen pressure tension measurements (tcpo2) application in relation to evaluation of results of vascular operations upon chronic ischaemic limbs and choice of amputation level if necessary. 269 patients were examined. All measurements were performed with the help of Hellige Oxymonitor SM 361 and a Polish one. A Clark-type electrode was attached to the surface of the skin at the dorsum of the foot and 1/3 upper tibia level (10 cm below lower margin of the patella). In several cases 1/3 lower thigh level was examined too. The clinical status of patients appeared to reflect tcpo2 values. Correct process of healing was observed to start from tcpo2 critical value of 40 mmHg. Tcpo2 method seems very useful for assessment of results of blood-flow reconstruction operations and choice of most advisable level for chronic ischaemic limb amputation.
