[Epimetaphyseal pseudarthroses].

Epi-metaphyseal pseudarthroses are rare. They occur in about 1% of cases after osteosynthesis and osteotomies and may cause deviation as well as painful limitation of motion and instability. Several different treatment methods can be advocated, including decortication and bridging of bone and articular defects, stable osteosynthesis and arthroplasty to regain motion. In selected cases endoprosthetic replacement can be a reliable procedure instead of an arthrodesis.

[Shock room management of polytrauma].

The emergency room functions as a junction between preclinical and early clinical treatment of patients with multiple trauma and should have defined technical and room possibilities. The personal staff should continue resuscitation measures and perform clinical and technical diagnostic procedures according to trained algorithms. The recognition of life-threatening injuries, the set up of correct priorities and application of respective surgical procedures characterize a good emergency room management.

[Strategy of surgical management of polytrauma].

Operative procedures in multiple injured patients consist in the first stage in life-saving operations such as control of bleeding and cerebral decompression. Operative measures during the urgent second operative phase have to be undertaken under consideration of the development of a multiple organ failure syndrome. Early stabilization of relevant extremity fractures and thorough care of soft-tissue are of particular importance (day-1-surgery). Only second look procedures to optimize soft-tissue injuries and prevent infections may be allowed during this early intensive care period. Delayed operative procedures should only be performed after stabilization of the overall patient situation to prevent enhancement of the systemic inflammatory response. The required operative procedures of the multiple injuries have to be attributed to the respective operative phases.

[Perspectives of polytrauma management].

Despite a high standard of preclinical, clinical and rehabilitative trauma care, the number and degree of injuries will remain a social and medical challenge. Improvements of trauma care in the future may be possible in respect to social, economic, organisational, clinical and research issues. Prevention of trauma will be of major impact and improvements may be possible by preventive legislation and technical developments. Organisation of trauma care should integrate high level trauma centres early-on including a national, regional and local quality control system. The future preclinical and clinical individual care of multiple trauma patients should consider the complex pathophysiological mechanisms at all levels, while the understanding of these mechanisms is increasing continuously.

[Primary malignant bone and soft tissue tumors of the pelvis--oncologic and functional outcome].

Surgical treatment of primary malignant bone and soft tissue tumors involving the innominate bone has to be considered an extensive procedure with high incidence of complications. It requires the knowledge of different techniques of resection and reconstruction of bone, joints, soft tissue and intrapelvic organs, as well as a multidisciplinary management. Therefore, it should be limited to specialized centers. The best oncological and functional results can be achieved by continuity resection and pelvic reconstruction in low grade malignant bone tumors [1-3].

[Spiral CT of the thorax in primary diagnosis and intensive treatment of polytrauma patients].

The performance of a rapid spiral chest CT scan during primary diagnosis and intensive care leads, in a high percentage of cases, to clinically relevant diagnoses. In particular, occult ventral (tension-) pneumothoraces and lung contusions have been found in one third of polytraumatized patients both initially and during the sequential course. Additional insertion of chest tubes has been observed to be the major consequence.

Differential expression of heat shock protein 70 in well healing and chronic human wound tissue.

Heat shock protein 70 (hsp 70) is an important member of the heat shock protein family, which is induced by different forms of stress. We attempted to find out if hsp 70 is also involved in wound healing, which likewise resembles a stress situation for cells too. Therefore we collected tissue samples from well healing and chronic human wound tissue. We used Northern- and Western-blot analysis to study the expression of hsp 70. At the protein level we found a strong correlation between well healing wounds and high expression of hsp 70, whereas chronic wounds showed no or weak expression. Interestingly hsp 70 mRNA did not show this significant correlation, displaying a variant expression pattern in the same kind of wound tissue, possibly due to unknown posttranscriptional regulating step, which has to be investigated in further studies. To localize hsp 70 mRNA and protein was used insitu hybridization and immunohistochemistry. Both displayed an overexpression in endothelial cells of capillary vessels.

[Diagnosis and spontaneous course of non-traumatic localized myositis ossificans].

Non-traumatic localized myositis ossificans is a rare variant of pseudotumoral muscular ossification. In the early stage it may be misdiagnosed as soft tissue sarcoma or some kind of osteosarcoma. Myositis ossificans itself does not always require therapy; when diagnosed on the basis of the typical features revealed by meticulous non-invasive investigation, biopsy is not mandatory for confirmation of the diagnosis. The diagnostic procedures needed - radiography, ultrasound, MRI, and isotope bone scan - are discussed with reference to three clinical case reports.

[The value of ultrasound for correction of clinical diagnostic errors in soft tissue tumors].

In doubtful clinical diagnosis, ultrasound investigation may represent a workable diagnostic means in revealing the nature of soft tissue alterations, as will be demonstrated by 6 clinical examples; in all cases a wrong clinical diagnosis delayed the necessary therapy and possibly deteriorated the prognosis of disease. In these patients correction of the clinical diagnosis was of decisive importance for consecutive diagnostics and therapy.

[The CT classification of intra-articular calcaneus fractures].

93 patients with 102 intraarticular calcaneus fractures (ICF) were examined by CT from 1986 to 1992. The images were evaluated with the use of a modified classification based on the number of fractured heel bone facets (2 facets in 4.8%, 3 facets in 53.9%, 4 facets in 32.3%, comminution in 8.8% of the fractures), the involvement of the calcaneus-cuboid joint (60.8%) and the fracture mechanism (tongue-type in 28.4%, joint depression in 62.7%) with the weight-bearing calcaneal compartments taken into special consideration. In that way, each intraarticular calcaneus fracture could be scored, enabling a fast diagnosis comprising factors relevant for the therapy and prognosis.

Analysis of morphology and gait function after intraarticular calcaneal fracture.

Open reduction and internal fixation is suggested by an increasing number of investigators as preferable treatment of displaced intraarticular calcaneal fractures. Assuming that quasianatomical reduction coincides with adequate function, many surgeons rely on morphological parameters (standard radiography, computed tomography) to demonstrate the effectiveness of surgery by achieving an optimum restoration of calcaneal geometry and joint surfaces. In order to correlate morphologic parameters and functional assessment, a prospective study was performed on 45 patients after surgical treatment of intraarticular calcaneal fractures using standard radiographic and computed tomographic scores, clinical evaluation, and gait analysis (dynamic pedography). Mean follow-up time after reconstruction was 23 months (range 18-50). Although clinical evaluation and assessment of gait function corresponded well with each other, radiographic scores showed a poor to moderate correlation with functional evaluation (r = 0.29-0.62); this was probably due to the missing analysis of soft tissue parameters. The comparison of clinical results and gait parameters with the individual radiographical parameters allowed us to identify those factors, with the greatest influence seen on the functional prognosis (i.e., calcaneal width, arthrosis in the neighboring joints). Morphologic analysis after calcaneal reconstruction based on radiographic techniques cannot predict subsequent function or substitute for functional assessment. However, it does allow for practical conclusions for surgical strategy in primary osseous reconstruction or secondary corrections.

[Impaired gait after base fractures of the 5th metatarsal bone].

Between 1982 and 1990, twenty fractures of the base of the fifth metatarsal were treated operatively and followed up for a mean of 4.5 years (range: 1-9 years). All patients were evaluated clinically, roentgenographically and with kinetic gait analysis. A scoring system was used to record and evaluate the clinical and radiological data. Nine patients had an excellent result and nine a good result; one patient had a fair and one a poor result. Examination by kinetic gait analysis showed a gait asymmetry in eleven of the twenty patients, with decreased load on the formerly injured areas of the involved foot and increased load on the corresponding areas of the contralateral foot. However, only three patients had a clinically visible gait disorder. We believe that the gait disorder described is an automatically adopted movement pattern. Its cause lies in the initial pain after the trauma and during the postoperative care and sometimes continues even after the cessation of pain. Kinetic gait analysis allows quantification of asymmetry of gait and clinically non-visible load disorder. Therefore, pain and established gait asymmetry are clinically relevant in such patients because they can be treated specifically.

[Tumor resection and reconstructive intervention in spinal tumors].

Modern techniques of resection and reconstructive surgery in patients with primary and metastatic spine tumors are presented. These techniques are delineated along an algorithm that allows us to develop an adequate oncological protocol, find a rational approach for preoperative embolization and for tumor resection and choose the proper materials and implants for bridging bone defects. The aim is to achieve short-term and long-term stability, respectively. These principles are illustrated by different examples and our own results in spinal metastasis surgery.

[Ventral stabilization of thoracic and lumbar spinal injuries].

An isolated ventral approach to the thoracal or lumbar spine seems sufficient in special burst fractures (type A2/3) or older compression fractures. The most effective ventral decompression of the spinal cord is followed by the fusion of the ventral body. Ventral plate instrumentation avoid dislocations of the interposed bone transplants. Our own clinical results are demonstrated.
