[Pruritometer 1: Portable measuring system for quantifying scratching as an objective measure of cholestatic pruritus].

Objective assessment of subjective symptoms such as pruritus always presents problems, which can often be resolved only indirectly. The objectification of pruritus was necessitated by a study on the efficacy of a serotonin antagonist used as treatment of cholestatic pruritus. In the present paper, a portable measuring system for the indirect objective assessment of pruritus via the quantification of scratching is described. A piezoelectric scratch-vibration sensor for attachment to the middle finger of the patient's dominant hand was developed. A sensor interface detects the scratching signals and generates pulses that are then summed in an adapted sports watch. The entire system-Pruritometer 1- is worn by the patient like a wrist watch, and is characterized by ease of handling. Acceptance by the patient is reported to be good. A statistically good correlation between measured (Pruritometer) and visually counted scratches was demonstrated. Pruritometer 2, which will enable scratch frequency and intensity distribution over time to be determined, is presently being developed.

Assessment of structurally related chemicals: toxicity and ecotoxicity of acrylic acid and acrylic acid alkyl esters (acrylates), methacrylic acid and methacrylic acid alkyl esters (methacrylates).

BUA compiled the available data on toxicity and ecotoxicity for several acrylic and methacrylic acid esters and their corresponding acids. A comparison of these data revealed a qualitative similarity in the toxicological and ecotoxicological properties of the compounds considered. The data indicate that methacrylates are less reactive than the corresponding acrylates.

Monitoring of FES-induced muscle activity by continuous EMG-recording.

Functional Electrical Stimulation (FES) requires information on the stimulated muscle for adjustment of the stimulation current, avoidance of muscle fatigue during the conditioning period and long term follow-up. Several applications of chronical FES are in clinical practice, but a system for direct registration of muscle activity under FES still does not exist. In six sheep the right Latissimus Dorsi Muscle (LDM) and Thoracodorsal Nerve were exposed. Stimulation electrodes were applied to each nerve and 3 EMG-applied sensing electrodes were placed into each LDM. The LDM tendon was connected to a force transducer. Burst stimulation was applied and the amplitude was increased from 0 to 4 mA in steps from burst to burst. EMG (M-wave) was amplified and recorded continuously via modified instrumentation amplifier, oscilloscope and tape recorder. Isometric muscle tension was recorded using force transducer, A/D interface and PC. Continuous EMG-recording was performed in all cases. Simultaneous recording of muscle tension and EMG revealed a close correlation (IrI=0.95, p < 0.0001) between the muscle strength and amplitude of the M-wave. Continuous recording of the EMG seems to be a reliable method for direct monitoring of the stimulated muscle. Three intramuscular electrodes can provide enough information to monitor FES induced muscle activity.

Characterization and toxicological behavior of synthetic amorphous hydrophobic silica.

During almost three decades of experience with hydrophobic silicas, no adverse health effects have been observed in manufacturing and applications with appropriate handling of the materials. The oral LD50 for rodents is > 7.9 g/kg body wt. Fumed or precipitated hydrophobic silicas do not produce inflammation of the skin or mucous membranes. Likewise, acute and chronic oral tests yielded no adverse systemic effects. A limited carcinogenesis study in rats did not induce tumors and the Ames test of a toluene extract was negative. Reproductive or developmental toxicity was not observed. In general, hydrophobic silicas provide a toxicological profile essentially the same as common silicas.

Long-term results of nervous tissue alterations caused by epineurial electrode application: an experimental study in rat sciatic nerve.

In order to evaluate the long-term effects of epineurial electrode application for functional electrical stimulation (FES) the left sciatic nerve of seven rats was exposed. Four ring-shaped stainless steel wire electrodes were sutured to the epineurium of each nerve in the same manner as performed clinically for carrousel stimulation in man. The nerves were reexposed 1 year after implantation and the stimulation threshold to obtain a tetanic contraction in the lower limb was determined for each electrode. Afterwards the animals were sacrificed. The electrodes were excised and cross sections of the sciatic nerve directly at site of the electrodes, 2-mm proximal and 2-mm distal to them were harvested for histologic and planimetric assessment of nerve lesions. The area of damaged neural tissue was expressed as a percentage of the total cross-sectional area within the perineural sheath. The sciatic nerves of the right side served as controls. The values for the stimulation thresholds ranged between 0.1 and 1.0 mA (mean 0.43 mA). By morphometric examination five of seven nerves were seen altered, the altered areas captured between 1% and 4.8% of the total cross-sectional area of the nerves within the perineural sheath. Besides two specimens, all altered nerve segments exhibited distinct signs of nerve fiber regeneration. The clinical implications of the results for long-term electrical stimulation, such as phrenic pacing, are discussed.

Risk factors for bilateral testicular germ cell tumors. Does heredity play a role?

Twenty-three bilateral testicular germ cell tumors (four synchronous and 19 sequential tumors) were investigated for potential risk factors. The incidence of maldescensus testis was not found to be higher than in patients with unilateral disease. The histologic findings of the first tumor did not have any effect on the incidence of the second tumor. In 21 patients (four synchronous and 17 sequential tumors), histocompatibility antigens (HLA) were determined; HLA-B14 was increased significantly in the sequential tumor group. Tendencies toward an increase of HLA-DR5 and HLA-DR7 also were found. The HLA-DR1, HLA-DR3, and HLA-DR4 showed a tendency toward a decreased frequency. Therefore genetic factors might be important in the development of sequential bilateral testicular cancers.

A laryngeal pacemaker for inspiration-controlled, direct electrical stimulation of the denervated posterior cricoarytenoid muscle in sheep.

Bilateral recurrent nerve palsies were induced in four sheep and unilateral nerve palsy in one sheep. Vocal cord abductions were achieved by direct electrical stimulation of the posterior cricoarytenoid muscles (PCMs). By means of the diaphragmatic myogram, respiratory synchronous activation of the glottis opening musculature was shown possible. In all cases an adequate glottis opening for respiratory function was achieved during a maximum period of stimulation of 5-8 h in each animal. Abductor movements of the vocal cords were documented with video laryngoscopy. Using digital image processing, the areas of the open glottis were measured at rest and during bilateral direct electrical stimulation of the PCMs.

First experimental application of multichannel stimulation devices for cardiomyoplasty.

The effect of long-term application of epineural electrodes to nerves was investigated in rat experiments. Neural damage reached a maximum of 6% shortly after implantation and decreased to 3% after 1 year. Impedance and threshold of epineural electrode were investigated in sheep experiments for up to 12 months. The mean impedance was in the range of 1 kohm, while the threshold less than 1 mA. The reduction in fatigue produced by multichannel stimulation was demonstrated by sequential isometric contractions of rectus muscles in sheep. The decrease in force was only 10% after 60 minutes of multichannel stimulation as compared to a reduction of 50% for single channel stimulation. Studies of cardiomyoplasty with single channel stimulation confirmed results reported by other investigators. In acute experiments with sheep, we demonstrated fiber-selective stimulation which led to isolated contraction of the left or right distal part or the right proximal part of the latissimus dorsi muscle. Potential advantages in the application of implantable multichannel stimulation devices as compared to single channel stimulation for cardiomyoplasty include: (1) fatigue-free stimulation at submaximal force level; (2) selection of hemodynamically effective electrode combinations; (3) potential for consecutive activation of muscle fiber groups, thereby allowing better simulation of the physiological contraction of the heart muscle; (4) redundancy of electrodes in case of technical failure or dislocation; and (5) stimulation of more than one muscle, if necessary.

Histological assessment of nerve lesions caused by epineurial electrode application in rat sciatic nerve.

The left sciatic nerve of 36 rats was exposed and four ring-shaped stainless steel wire electrodes were sutured to the epineurium of each nerve in the same manner as performed clinically for "carousel stimulation" in man. The rats were sacrificed 10 days (Group 1), 3 weeks (Group 2), or 3 months (Group 3) after implantation. The electrodes were excised, the nerves were embedded in Epon, and semithin sections were obtained for histological and planimetric assessment of lesions caused by the epineurially sutured electrodes. The right sciatic nerves served as controls. The total area of neural tissue within the perineurium was determined at three levels: at the site of the electrodes, 8 mm proximal, and 8 mm distal. The area of neural tissue damaged by the surgical procedure was expressed as a percentage of the total area. In Group 1, nine of 12 nerves showed lesions ranging from 0.39% to 25.39% of the total area of neural tissue, in Group 2 eight of 11 sciatic nerves showed lesions ranging from 0.24% to 13.03% of the total area, and in Group 3 five of 12 nerves showed lesions ranging from 0.21% to 4.96% of the total area. The pathologically altered areas in Groups 2 and 3 exhibited distinct signs of nerve fiber regeneration. The reasons for the decrease in damage from Group 1 to Group 3 and the clinical implications of the results for long-term electrical stimulation are discussed.

[Respiratory aid using a diaphragmatic pacemaker in high paraplegia].

Spastic tetraplegia due to cervical medullary injury above the origin of the phrenic nerve neurons means complete respiratory paralysis. In this case a phrenic pacemaker can be considered as an alternative to long term mechanical ventilation. An implantable 8-channel stimulation device has been developed for functional electrical stimulation of the phrenic nerves. The "Karussellstimulation" provides fatigue free stimulation for 24 hours a day. We report the case of a 12 year old girl with complete ventilatory insufficiency who underwent implantation of a phrenic pacemaker. 18 month after implantation she is independent of a conventional respirator and limits rehabilitation could be achieved. Electrophrenic respiration is of value in the rehabilitation management of tetraplegic patients with ventilatory insufficiency.

[The clinical value of NMR tomography for retroperitoneal diagnosis].

NMR-tomography is a noninvasive diagnostic alternative to conventional imaging modalities. For clinical purpose there is no significant improvement in staging of kidney tumors and lymphnode metastases. The diagnostic value for complicated cysts and adrenal tumors is superior to all alternative imaging modalities as CT and sonography. Tumors of vena cava could be staged with an accuracy rate of 100%.
