An 8-year-old boy presented with a white pupillary reflex. This clinical picture suggested a diagnosis of retinoblastoma or Coats' disease. Ultrasonography did not reveal the characteristic signs often seen in retinoblastoma. However, diagnostic vitrectomy and subsequent enucleation confirmed the diagnosis 'retinoblastoma'.
Cardiac manifestations are reported in 0.3%-4.0% of European patients with Borrelia burgdorferi (B.b.) infection. Usually symptoms disappear within 6 weeks. We report a case with persistent impairment of atrioventricular (AV) conduction. Diagnosis was confirmed by demonstration of IgM antibodies and increase of IgG antibody titers against B.b. in serum, by isolation of the spirochete from skin biopsy material and by the typical clinical combination of erythema migrans, Bannwarth syndrome (meningoradiculitis), and complete heart block. Despite immediate antibiotic therapy with ceftriaxone, first degree AV block and second degree block Wenckebach with atrial pacing at 100 beats/minute persisted for 2 years. We conclude, that Lyme carditis can cause long-standing or irreversible AV conduction defects despite adequate and early antimicrobial therapy.
Direct intravitreal injection of antibiotics plays an important role in the management of bacterial endophthalmitis. In the present study we investigated the toxicity and clearance of intravitreally injected cefotetan in a rabbit model. No toxic ocular side effects could be detected by electroretinography (ERG) or light and electron microscopy up to and including a single intravitreal dose of 1000 micrograms. Intravitreal injection of 2000 micrograms cefotetan resulted in mild degeneration of photoreceptor outer segments and, sporadically, in cataract formation. After intravitreal injection of 4000 micrograms, moderate toxic degeneration of photoreceptors occurred, with displacement and mitochondrial swelling of inner segments. In addition, lysosomal lamellar inclusion bodies could be detected in pigment epithelial cells. After a single intravitreal injection of 1000 micrograms cefotetan, concentrations greater than the minimum necessary for the inhibition of most commonly occurring intraocular pathogens (except Pseudomonas aeruginosa and Staphylococcus epidermidis) were maintained in the vitreous humor for greater than 48 h. Cefotetan may be a potentially important drug for intravitreal injection, especially in cases of gram-negative and suspected anaerobic endophthalmitis.
To fix parameters for the pharmaceutical characterization of AWD 26-06 test methods converting identification, purity, content and stability as well as their results are submitted. Spectroscopic investigations are intended to give information on the constitution of the molecule.
Using the Octopus G1 program, we investigated the visual fields of amblyopic and healthy contralateral eyes in 21 patients with strabismic amblyopia and 14 patients with anisometropic amblyopia. All subjects had a visual acuity of 0.7-0.06. A central scotoma was detected in 85.7% of the patients with strabismic amblyopia and in 79% of those with anisometropic amblyopia. The mean maximal depth of central scotomas in subjects with strabismic amblyopia was 6.81 +/- 4.74 dB (mean +/- SD); in those with anisometropic amblyopia, it was 6.64 +/- 4.34 dB. This difference was statistically not significant. However, in patients with anisometropic amblyopia, the visual field indices MD (mean defect) and CLV (corrected loss variation) were significantly higher than in those with strabismic amblyopia (P less than 0.05 vs P less than 0.05); this was predominantly due to additional flat defects that occurred paracentrally and peripherally in subjects with anisometropic amblyopia. In both groups of patients, we found a significant negative correlation between visual acuity, on the one hand, and the maximal depth of the scotomas and the visual field indices (MD and CLV), on the other.
Preexisting supplies of a local public health office and other institutions are taken into account in developing main activities of an AIDS-counsellor within the AIDS-programme at public health offices in Western Germany. School education and individual AIDS counselling are predominant. Sexology at schools in Bavaria is restricted by the education privilege of parents and ethical principles supported by the catholic church. Further activities for domestic nursing personnel and medical doctors were introduced successfully. The controversy of epidemic hygiene predominating over psychological counselling is discussed. Efficacy of working at schools and future developing topics are discussed.
4,382 blood samples were examined for HIV antibodies between August 1985 and the end of December 1988 at the Public Health office of the city of Augsburg. The prevalence of positive tests was 0.57% (n = 25). The rate at which various consultation offers were made use of was calculated for 1988. From 1985 to 1988 approximately 200 registered female prostitutes were regularly tested for HIV. It was only in 1985 that the HIV antibody test was positive in two women. Compared with 1984-1986 as basis there was a marked drop in the incidence of gonorrhoea to 71.7% and in 1988 to 6.5%.

The concentration of guanidine was determined in blood serum and urine of healthy humans following oral administration of guanidine hydrochloride. The biological half life is 12 h, the renal clearance is 40--50 ml. An improved method for the determination of guanidine in body fluids is described.

A computer analysis of 87 penetrating kertoplasties done in a uniform manner by a single surgeon revealed the following: 1. On a statistical basis small grafts (7.0 mm) provided better visual acuity than larger grafts (8.0 mm). 2. On a statistical basis small grafts will be clearer postoperatively than larger grafts. 3. On a statistical basis the age of the donor is not related to either graft clarity or visual acuity.

Using a random selection procedure, 100 patients were evaluated in order to determine the value of prophylactic antibiotics in cesarean section. All patients were evaluated for febrile morbidity, and specific infection sites were recorded. Significant differences were found between the treated and control groups and between cesarean sections performed prior to the onset of labor and after the onset of labor.
Discontinuous acidic anodic polyacrylamide gel electrophoresis enables the separation of bovine beta-caseins from those of ovine and caprine. Interfering protein bands as a consequence of ripening or processing have not been detected. After evaluation of the stained gel by laser densitometry, quantification was performed with calibration standards on the same gel by the ratio of the peak areas from bovine to ovine and bovine to caprine, respectively. Thus, independence from the extractability of proteins affected by denaturation and ripening (which might in some cases raise the limit of detection) is achieved. The range of quantification extends from 5 to approximately 70% bovine casein in relation to total casein.
