[Erythema elevatum diutinum. II. Immunoelectronmicroscopical study of leukocytoclastic vasculitis within the intracutaneous test reaction induced by streptococcal antigen (author's transl)].

A leukocytoclastic vasculitis was induced by intracutaneous injection of streptococcal antigen in a patient with erythema elevatum diutinum (E.e.d.). The immunoelectronmicroscopical demonstration of C3 was performed by use of the peroxidase-antiperoxidase multistep technique 24 h after the injection of the antigen. Deposits of C3 were found between endothelial cells, on the outer surface of endothelial cells, pericytes, and smooth muscle cells, as well as within the multilayered basal lamina of small vessels. Intact and disintegrating neutrophils accumulate within the vessel walls and in their surroundings. Necrosis and fibrin deposition are present in advanced stages. The findings demonstrate the sequence of events in leukocytoclastic vasculitis at the ultrastructural level. They also support the hypothesis that in E.e.d. an Arthus type reaction induced by bacterial antigens may be of pathogenetic significance.

Immunoelectronmicroscopic examination of early lesions in histamine induced immune complex vasculitis in man.

In 4 cases of allergic vasculitis circulating immune complexes (IC) were demonstrated. Spontaneous and histamine induced vascular changes were studied by immunofluorescence microscopy. The early events in IC vasculitis were investigated at the ultrastructural level by immunoelectronmicroscopy using the peroxidase-antiperoxidase multistep technique. Our findings support the concept that human IC vasculitis is triggered by the deposition of circulating IC in the walls of postcapillary venules between endothelial cells, pericytes and the layers of the basal lamina. Tissue destruction is only secondary due to local complement activation and the release of lysosomal enzymes from chemotactically attracted leukocytes.

[Immunoelectron microscopy in dermatology].

The principle of immunoelectronmicroscopic studies using horseradish perpoxidase is described. This method, especially the peroxidase-antiperoxidase multistep technique, reveals more details about the exact localization of immunophenomena in different dermatological diseases. The results of immunological investigations performed on the ultra-structural level in bullous diseases, lupus erythermatosus, vasculitis, and psoriasis are summarized and compared with the immunofluorescent and classical electromicroscopic findings.

[Generalized acute pustulosis. An unusual presentation of leukocytoclastic vasculitis].

After an acute course of pharyngitis and bronchitis treated with Bactrim (trimethoprim and sulfamethoxazol), a 52 year old woman suddenly developed a widespread, sterile pustular eruption with predilection of the distal parts of the extremities. The histological and electron microscopical examinations showed intraepidermal pustules with underlying distinct leukocytoclastic vasculitis. Direct immunofluorescence revealed the prescence of intra- and perivascular deposits of C3 in the upper part of the dermis. Drug- and/or infection allergy is suggested to be responsible for the etiopathogenetic mechanism of leukocytoclastic vasculitis.

Electron microscopical study of psoriasis pustulosa.

Five cases of psoriasis pustulosa were examined by electron microscopy. The main features within the dermis were dilated capillaries filled with polymorphonuclear leukocytes and red cells. Neutrophils passed out of the vessels through the gaps and fenestrations and migrated towards the epidermis throughout the distinctly edematous corium. Lymphatic vessels could be found. Histiocytes containing Langerhans granules were observed in the dermis and epidermis. The morphological changes of the epidermis depented on the place examined and were most evident near fully developed pustules in the upper layers. The presence of previously described morphological findings could be confirmed. Perinuclear cytolysis and other changes in the keratinocytes, however, seem to be secondary, resulting from edema and accumulation of neutrophils within the epidermis. Immunological phenomenona may be responsible for the latter event.

Immunoelectronmicroscopical demonstration of in vivo bound complement C3 in psoriatic lesions.

The presence of in vivo bound complement (C3) within the stratum corneum of 4 psoriatic cases is demonstrated ultrastructurally by the use of the peroxidase-antiperoxidase multistep technique. The positive reaction product was located in the intercellular space and on the surface of the parakeratotic cells within the horny layer. The membranes of the horny cells were not altered and peroxidase granules could also not be detected within their cytoplasm. The explanation of this finding as well as its pathogenetic significance is discussed.

Electron microscopic studies in dermatitis herpetiformis in relation to the pattern of immune deposits in the skin.

Electron microscopic studies were made in 12 cases of dermatitis herpetiformis: 6 of them with a continuous immunofluorescence line of IgA deposits at the dermo-epidermal junction, and the other 6 with granular IgA deposits in the dermal papillae. Six cases of bullous pemphigoid with a continuous immunofluorescence line of IgG deposits at the dermo-epidermal junction were examined similarly for comparison. In dermatitis herpetiformis with the continuous IgA line the ultrastructural characteristics both of dermatitis herpetiformis and bullous pemphigoid were present, even when the histological and clinical features as well as response to sulphapyridine and sulphones were typical of dermatitis herpetiformis. The ultrastructural pattern was essentially the same as in the cases with clinical and histological characteristics of the mixed dermatitis herpetiformis-bullous pemphigoid form, although in the latter there was some predominance of the characteristics of bullous pemphigoid.

The ultrastructure of granuloma annulare.

Nine cases of granuloma annulare at different stages of development were investigated electron microscopically. Special attention was given to collagen changes, to the cellular infiltrate, and to vascular changes. The necrobiotic changes of the collagen were: loosening of the fibers at the borders of the bundles; swelling and loss of cross-striation of the fibrils; and complete degradation of the collagen, with precipitation of mucinous and fibrin material and of glycogen. In 2 cases "cross-banded filamentous aggregations" were present. In later stages new collagen synthesis occurred. In the infiltrate, histiocytes/macrophages were seen developing into epithelioid and giant cells. In addition, numerous active lymphocytes and, at later stages, some neutrophils were found. Vascular changes consisted of endothelial swelling and duplications of the basal lamina. A perivascular infiltrate was present, but no distinct vasculitis was seen. Our results indicate that the necrobiosis of the collagen without primary vasculitis may be an initial event in granuloma annulare. A delayed hypersensitivity mechanism is discussed with regard to pathogenesis.

Pathogenesis of pemphigus erythematosus.

Immunofluorescence studies were made by the indirect method in 54 cases of pemphigus erythematosus, in 50 of which skin specimens from light-exposed and unexposed regions were investigated also by the direct IF method. IF Band was shown to be demonstrable in skin specimens from exposed regions in 81% of cases and from unexposed regions in 23%. ANA were found in some 31% of patients, though usually in titers below those of IC antibodies. There were 2 cases each of coexistence with myastenia gravis and thymoma and with SLE. Virus-like particles, however, were found by electron microscopy only in 1 case with coexisting SLE. Detection of IF Band in skin specimens from a significant majority of patients with pemphigus erythematosus, presence of ANA in some, and occasional coexistence of SLE suggest some relation of the disease with lupus erythematosus.

Studies on the role of C-type viruses in the development of epithelial tumors induced with DMBA.

Epithelial tumors were induced using 0.5 per cent solution of DMBA in two strains of mice--one infected with leukoviruses (Swiss mice), and the other is free of these viruses (hairless mice). Tumors from 15 mice of each strain were examined light- and electron-microscopically. Depending on the period of administration of the carcinogen, benign growths of the type of papilloma or keratoacanthoma were obtained, or malignant tumors. In the tumors in Swiss mice electron microscopy revealed a distinct increase in the numbers of viruses in comparison with surrounding skin and intact skin of healthy mice of this strain. In spite of certain histologic differences between tumors produced in Swiss and hairless mice, the results argue against a role of leukoviruses in the pathogenesis of experimentally induced epithelial tumors in mice.
