Beta7 integrin-deficient mice: delayed leukocyte recruitment and attenuated protective immunity in the small intestine during enteric helminth infection.

The ontogeny and function of gut-associated-lymphoid tissue is known to be critically dependent on the beta7 integrin subfamily. We have investigated the development of intestinal inflammation and pathogen-specific protective immunity to enteric helminth infection in beta7 integrin knockout (KO) mice. During Trichinella spiralis infection of the small intestine there was a significant delay and reduction in the magnitude of intestinal eosinophilia and mastocytosis in the absence of P7 integrin, resulting in impaired host protection. Aberrant distribution of mast cells was also observed in the small intestine of infected KO mice. Adoptive transfer of primed wild-type mesenteric lymph node cells into T. spiralis-infected beta7 KO mice did not restore the intestinal mast cell response, suggesting that the defect in intestinal mastocytosis is due to the absence of beta7 expression on this population rather than an indirect consequence of reduced T cell numbers. In contrast, no impairment in leukocyte recruitment or protection against Trichuris muris infection of the large intestine was observed in KO mice. Taken together the data provide the first description of reduced leukocyte homing and attenuated protective immunity against helminth infection in beta7 KO mice. Furthermore, these results suggest that beta7 integrin-independent adhesion molecule interactions are deployed in the large but not small intestine during intestinal inflammation.

Force probe measurements of antibody-antigen interactions.

The surface force apparatus has been used to quantify directly the forces that govern the interactions between proteins and ligands. In this work, we describe the measured interactions between the antigen fluorescein and the Fab' fragment of the monoclonal 4-4-20 anti-fluorescyl IgG antibody. Here we first describe the use of the surface force apparatus to demonstrate directly the impact of the charge composition in the region of the antibody binding site on the antibody interactions. Several approaches are described for immobilizing antigens, antibodies, and proteins in general for direct force measurements. The measured force profiles presented are accompanied by an extensive discussion of protocols used to analyze the force-distance curves and to interpret them in terms of the antibody structure. In addition to long-range electrostatic forces, we also consider short-range forces that can affect the strength of adhesion between the Fab' and immobilized fluorescein. The latter investigations demonstrate the influence of interfacial properties on the recognition of surface-bound antigens.

Risk factors and determinants of neurodevelopmental outcome in cystic periventricular leucomalacia.

UNLABELLED: The aim of the study was to determine risk factors for the development of cystic periventricular leucomalacia (PVL) and to correlate ultrasound findings with neurodevelopmental outcome. By means of a retrospective case-control study (matched for gestational age, birth weight, sex, and year of birth) and a cohort analysis of all preterm infants with cystic PVL documented by ultrasound scans hospitalised at a local tertiary care centre between 1988 and 1998, 98 preterm infants with a gestational age ranging from 26 to 35 weeks were diagnosed as having cystic PVL. The mean day of diagnosis of periventricular echodensities was 3 +/- 2 days (range 1-11 days), and of cystic PVL 21 +/- 8 days (range 2-47 days). Of 79 infants (1988-1997) eligible for neurodevelopmental follow-up (91%), hemi-, di-, or tetraplegia was diagnosed in 61 (77%), normal mental outcome in 22 (28%), associated visual disorders in 41 (52%) and seizure disorders in 12 (15%) infants. Significant risk factors associated with the development of cystic PVL were premature rupture of membranes, chorioamnionitis, and hyperbilirubinaemia (odds ratios 4.665, 6.026, and 2.460 respectively). Subgroup analysis according to gestational age (26-28, 29-32, 33-35 weeks) revealed similar results despite spontaneous labour (26-28 weeks; odds ratio 4.808) and pre-eclampsia (33-35 weeks; odds ratio 3.517). Multiple pregnancy was associated with a twofold increased risk (odds ratio 2.075). The white matter damage probably accounted for the significantly higher prevalence of apnoeas (P < 0.001) and neonatal seizures (P < 0.001). Cysts located bilateral or parieto-occipital were associated with a higher risk of cerebral palsy (odds ratios 6.933 and 4.327 respectively). Solely anterior located cysts were associated with normal neurological outcome. Increasing size of the cysts was associated with increasing risk of cerebral palsy with a cut-off value of 10 mm (odds ratio 3.300 and above) and all infants with cysts of more than 20 mm diameter had cerebral palsy. CONCLUSION: The high prevalence of premature rupture of the membranes and chorioamnionitis further supports the role of intra-uterine infection in the pathogenesis of periventricular leucomalacia. The overall prognosis of cystic periventricular leucomalacia is poor.

[Split skin and full-thickness skin grafts].

Skin grafting is one of the earliest described surgical procedures. In the age of microsurgery and free-tissue transfer, it remains a utilitarian means of achieving soft tissue reconstruction. Free skin grafts can be classified into three types: full-thickness skin grafts, split-thickness skin grafts, and composite grafts. The indications, techniques, donor site considerations, postoperative complications, and results of each type are reviewed. The full-thickness skin graft for facial and cervical reconstruction is usually harvested from the upper arm or postauricular region and is applied under a tie-over dressing. When comparing the functional and aesthetic results of full-thickness skin grafts and split-thickness skin grafts in terms of morbidity, skin elasticity, skin sensitivity, matching, and scar recurrence, full-thickness skin grafting seems to be the most adequate technique.

[Laparoscopic latero-lateral pancreaticojejunostomy].

Side-to-side pancreatojejunostomy for patients with chronic pancreatitis and dilated pancreatic duct is an established procedure, but so far there has been no report of this operation being performed as minimally invasive surgery. A 23-year-old woman with a history of seven years of recurrent pancreatitis with pancreas divisum underwent a latero lateral pancreatojejunostomy with Braun anastomosis. For access, three ports were used; the anastomoses were produced with Endo-GIA staplers. The operation time was 115 min and the postoperative course was uneventful. During 8 months of follow-up no dilatation of the pancreatic duct or signs of recurrent pancreatitis were seen. The patient feels fine. Using improved and newly developed surgical devices (i.e., ultrasonic shears, endo-staplers) more difficult procedures can be done in a reasonable operation time. Under ideal conditions, patients will have full benefits with minimally invasive surgery.

[Unstable carotid stenosis--an inflammatory disease?].

Arterioarterial thromboembolism from extracranial internal carotid artery (ICA) stenosis is an important pathogenic mechanism of ischemic stroke. However, even a high-grade ICA stenosis carries a greatly variable annual risk of stroke, as high as 13% following a recent occurrence of transient or minor cerebral ischemia or as low as 1-2% in clinically asymptomatic patients. There is increasing evidence that inflammatory processes play a central role in atherosclerosis and particularly in plaque destabilization converting chronic atherosclerosis into an acute neurological disorder. In thromboendarterectomy specimens from patients with high-grade ICA stenoses, the extent of inflammatory infiltration and the expression of matrixmetalloproteinase-9 correlated to clinical and ultrasonic features of plaque destabilization such as cerebral microembolism. Inflammation might become a new therapeutic target in symptomatic carotid artery disease.

[VRATS--Virtual Reality Arthroscopy Training Simulator].

The subject of this paper is a highly interactive medical training system for arthroscopic surgery; this is based on computer graphics and virtual reality (VR) techniques and offers an alternative to conventional training methods. To provide the virtual environment, a realistic 3D representation of the knee joint is derived from 2D medical image data. The use of tracking techniques guarantees an intuitive handling of the surgical instruments. The system allows navigation via a virtual camera and interaction with the virtual anatomical structures. First approaches for the simulation of tissue deformation caused by collisions with the instruments are implemented. One important advantage over conventional training systems is the possibility of verifying the training progress. Work is in progress on the realization of tactile feedback with the aim of providing a higher degree of interactive realism.

Improperly placed anterior cruciate ligament grafts: correlation between radiological parameters and clinical results.

Despite increasing knowledge on knee biomechanics and refined operative techniques, an increasing number of patients are being seen with failed anterior cruciate ligament (ACL) reconstruction. Failure of the reconstruction and further damage to the knee are correlated with improper placement of the graft, which interferes with graft biology and biomechanical demands. Between 1994 and 1995, 63 patients with improperly placed ACL grafts were referred to our institution because of persistent knee instability and pain. A method for analysis of the femoral drill hole on radiography was developed. Before reoperation the radiograph was evaluated by our method, noting the clinical aspects according to the recommendations of the International Knee Documentation Committee (IKDC). The femoral placement of the ACL graft could easily be defined on the lateral and anteroposterior tunnel radiography. The most common error was a femoral placement anterior to the anatomical insertion of the ACL. A significant correlation (P < 0.05) was found between femoral placement of the graft in the sagittal plane and clinical results: the IKDC score declined with increasing distance of the graft from the most isometric bundle of the ACL in the anteroposterior direction.

Free nerve endings in the medial and posteromedial capsuloligamentous complexes: occurrence and distribution.

Free nerve endings (FNEs) of type IVa play a distinctive role in the articular nociceptive and sensorimotor system of the knee. This study qualitatively and quantitatively analyzed FNEs in the medial and posteromedial capsuloligamentous complexes. Biopsy specimens from ten precisely defined anatomical locations were taken from seven fresh cadaver knee joints. The specimens were fixed with 4% formaldehyde solution and stained with hematoxylin-eosin. The results were examined using immunohistochemistry. The occurrence of FNEs is described in combination with their specific pattern of distribution. A high number of FNEs were found in all investigated elements with a maximum relative density in the insertion of the semimembranosus muscle in the direct attachment on the tibial margin. The number was lowest in the superficial medial collateral ligament. The results were correlated with anatomical and biomechanical functions of the stabilizing effect of the medial capsuloligamentous complex. Our findings indicate that lesions and surgical procedures can alter normal sensory feedback and coordination by modifying the use of muscle fiber during specific movements.

Magnetic detection of injury-induced ionic currents in bean plants.

A superconducting quantum interference device (SQUID) multichannel magnetometer was used to measure the temporal and spatial evolution of the magnetic field accompanying stimulation by burning and/or cutting of Vicia faba plants. These magnetic fields are caused by ionic currents that appear after injury in different parts of the plant. All measured V. faba plants responded to the burning stimulation with detectable quasi-d.c. magnetic signals. In order to measure these signals, a suitable modulation had to be used. The covariance method was applied to analyse the measured data. The results demonstrate a dipolar-like magnetic signal, exponentially decreasing in time, above the cutting type of injury. After the burning stimulation, the magnetically detected activity was concentrated predominantly above the leaves/petioles and less above the stem. Possible mechanisms for this behaviour are suggested. A comparison with previously known electrical measurements of plant injury is given.

The gap effect in pro-saccades and anti-saccades in psychometric schizotypes.

Schizophrenic patients and their first-degree relatives exhibit deficits in the anti-saccade task. In the present study, anti-saccade task performance was examined in subjects with 'high' and 'low' expressions of the schizotypal personality trait. For that purpose, the SPQ-G, the German adaptation of the Schizotypal Personality Questionnaire (SPQ; Raine, 1991), was filled in by 489 university students. Twenty and 21 participants with 'high' and 'low' SPQ-G scores, respectively, were compared with respect to saccadic eye movements elicited under the overlap and 200 ms gap conditions of the pro- and anti-saccade tasks. Each task block comprised 150 trials, 75 to either side in random order. The order of presentation of the task blocks was counterbalanced across the participants of each group. Saccadic reaction times were slower during the anti- as compared to the pro-saccade task and under the overlap as compared to the gap condition. Direction errors occurred almost exclusively during the anti-saccade task, express saccades mainly under the pro-saccadic gap condition. High-schizotypal participants did not differ significantly from low-schizotypal participants in any of these measures. While these results might suggest normal anti-saccade task performance in schizotypal personality as defined by the SPQ-G, the sampling strategy adopted in the present study is the more plausible explanation for the lack of group differences.

NO chemiluminescence in exhaled air: interference of compounds from endogenous or exogenous sources.

Nitric oxide determination in exhaled air using chemiluminescence analysers is increasingly used, but may be affected by various other components of the air sample. The influence of several compounds originating from endogenous or exogenous sources on NO readings has been studied. Defined amounts of water vapour, carbon dioxide, acetone, heptane, acetonitrile, oxygen, nitrous oxide and enflurane were added to air samples with NO concentrations 0-250 parts per billion. Marked and significant decreases in NO readings, which strongly depend on the concentration of the respective interfering compound, were found for water vapour (0.155% per 1% relative humidity), carbon dioxide (1.97% per 1% CO2 volume/volume (v/v)) and nitrous oxide (0.608% per 1% v/v N2O). While acetone in concentrations up to 8.5% v/v had no measurable effect on NO readings, heptane and acetonitrile led to marked decreases. Oxygen in concentrations of up to 95% (v/v) had no effect on NO determination. NO readings were markedly decreased by >10% per 1% (v/v) of the anaesthetic enflurane. However, due to large variations in NO values, these decreases were not statistically significant. Furthermore, enflurane reacted with the molybdenum converter of the NO(x) analyser in use, resulting in major damage to the instrument. Eliminating, or at least considering, interferences by compounds present in exhaled air is an urgent prerequisite for reliable and precise nitric oxide determination.

Acceptance and digestibility of some selected browse feeds with varying tannin content as supplements in sheep nutrition in west Africa.

In 1994 and 1995 leaves from 10 selected West African trees and shrubs with varying tannin content were tested to determine their suitability as an alternative and supplementary browse feed for West African dwarf sheep to improve productivity in small-scale holdings in Benin. Dry matter intake per kg metabolic body weight (DM g/kg W0.75) varied between the different browse feeds and between the different trials and ranged from zero (Leucaena leucocephala) up to 26.7 DM g/kg W0.75 (Margaritaria discoidea). The digestibility of the organic matter varied between 58.9% (L. leucocephala) and 68.2% (Mallotus oppositifolius). Agelaea obliqua showed the highest levels of total phenols (10.2%), tannin phenols (8.8%) and extractable condensed tannins (8.0%). Leaves from various browse feeds are a good and protein-rich supplementary fodder in addition to the grass Panicum maximum. However, feeding of A. obliqua and Cnestis ferruginea should be avoided due to toxic components.

Airborne gram-negative bacterial flora in animal houses.

The concentration and the species composition of airborne gram-negative bacteria were studied in four cattle houses, one pig house and one poultry barn. On average only between 0.02 and 5.2% of the total number of culturable aerobic bacteria were identified as gram-negative bacteria. Obligate anaerobic gram-negative bacteria were not isolated at all. In the airborne gram-negative bacterial flora the following bacterial families dominated: the Enterobacteriaceae, the Pseudomonadaceae and the Neisseriaceae. Within the family of the Enterobacteriaceae the species Escherichia coli and Enterobacter agglomerans were predominant. In animal houses using straw as bedding material Ent. agglomerans was most frequent, whereas in animal houses without litter E. coli was mainly found. Airborne Neisseriaceae were isolated very frequently in cow barns with Acinetobacter lowffii as the primary species. Airborne Pseudomonadaceae were found in high concentrations during periods of high air humidity. The results presented may also give some indications on the origin and sources of airborne endotoxins in animal housing.

Removal of immunoglobulins by a protein A versus an antihuman immunoglobulin G-based system: evaluation of 602 sessions of extracorporeal immunoadsorption.

Elimination of IgG can be achieved by extracorporeal immunoadsorption (IA) based on specific binding to either staphylococcal protein A (Excorim) or sheep polyclonal antibodies directed against human IgG (Therasorb). In 602 analyzed sessions of IA, elimination of IgG was 60% through 80% depending on the treated plasma volume, with no significant difference between the mentioned systems. However, the decrease of IgM and IgA was approximately 50% in the anti-IgG compared to 20-40% in the protein A system. Plasma albumin concentration decreased by 20% in the anti-IgG system compared to 15% in the protein A system, and hemoglobin values increased by 2% in the anti-IgG system and decreased by 6% in the protein A system. In conclusion, a clinical relevance for these findings cannot be ruled out, and the individual choice might depend on the clinical situation and laboratory findings.

Congenital sodium diarrhea is an autosomal recessive disorder of sodium/proton exchange but unrelated to known candidate genes.

BACKGROUND & AIMS: Congenital sodium diarrhea (CSD) is caused by defective sodium/proton exchange with only 6 sporadic cases reported. The genetics of the disease have not been established. We studied 5 infants with secretory diarrhea, identified in a circumscribed rural area in Austria, to define the mode of transmission and the involvement of candidate genes known to encode for sodium/proton exchangers (NHEs). METHODS: We collected clinical and laboratory data from 5 affected patients, analyzed the pedigrees of their families, and performed homozygosity mapping and multipoint linkage analysis studies in 4 candidate regions known to contain NHE genes. RESULTS: The diagnosis of CSD in 4 of 5 patients was based on daily fecal sodium excretion between 98 and 190 mmol/L, hyponatremia, metabolic acidosis, and low-to-normal urinary sodium concentrations. Pedigree analysis of the affected 2 CSD families revealed parental consanguinity and a common single ancestor 5 generations ago. Homozygosity mapping and/or multipoint linkage analysis excluded the NHE1 locus on chromosome 1, NHE2 locus on chromosome 2, NHE3 locus on chromosome 5, and NHE5 locus on chromosome 16 as potential candidate genes for CSD in this pedigree. Results on NHE4 were inconclusive because the precise chromosomal location of this NHE gene in humans is currently unknown. CONCLUSIONS: Our data indicate that CSD is an autosomal recessive disorder but is not related to mutations in the NHE1, NHE2, NHE3, and NHE5 genes encoding for currently known sodium/proton exchangers.

[Ultrasonically controlled percutaneous high frequency thermotherapy of liver tumors with perfused needle electrodes].

Percutaneous radiofrequency ablation (HFTT) is a new therapeutic technique for the treatment of inoperable primary and secondary liver tumors. We report our initial experience using a newly developed perfusion electrode. Twelve liver tumors (11 metastases of colorectal tumors, 1 hepatocellular carcinoma) were treated in 5 inoperable patients. The patients had 1 to 3 liver tumors. All lesions were cytologically confirmed and measured 12-47 mm. The technique was approved by the institutional review board and informed consent was obtained from all patients. A 12-mm-needle electrode with a 15-mm-active tip was introduced into the liver tumors under ultrasound guidance and tumors were coagulated with radiofrequency energy of 350 kHz. The needle electrodes were perfused with 0.9% saline during coagulation to increase the volume of coagulation necrosis without tissue vaporization. The serial changes in tumor size after therapy were evaluated with spiral CT imaging. Dynamic CT showed that unenhanced areas indicative of coagulation necrosis developed in all tumors. In 8 of 12 tumors no signs of recurrence appeared during the observation period of 7 (5-12) months. No major complications were observed. Our preliminary experience suggests that percutaneous radiofrequency coagulation can be a simple, safe and potentially effective treatment for selected patients with inoperable liver tumors. The results justify further studies to investigate the possible role in clinical practice.
