The HealthCare Chaplaincy, Inc., New York, New York.

The HealthCare Chaplaincy is a not-for-profit center for health-related pastoral care, clinical education and research. With a professional staff of 24 chaplains and 11 CPE Supervisors, "The Chaplaincy," as it is called, has developed a multifaith, multi-institutional "Partnership Model" providing pastoral care leadership for a growing number of medical and health-related facilities (see the appendix for a current list). At the same time, these institutions constitute a campus for educating annually over 250 students representing 30 or more different denominations and faith groups. In connection with its new, seven-floor center in the heart of Manhattan, it has recently begun specialized community outreach programs, including sponsorship of a Jewish Institute for Pastoral Care, seminars for local clergy, and bereavement as well as other support groups. In a 13-year period of growth, the budget has risen from less than $300,000 per year to over $4 million in 1998.

Traumatic hemipelvectomy: a survivable injury.

Traumatic injury remains an unfortunate and all-too-common event in the United States military population. Traumatic hemipelvectomy is a rare but devastating injury with few survivors recorded in the medical literature. We report a surviving case of traumatic hemipelvectomy and review an approach to management.

Angiosarcoma at the site of a Dacron vascular prosthesis: a case report and literature review.

Four of 32 reported sarcomas related to the aorta have arisen around previously placed aortic vascular prostheses suggesting that the graft may have been an etiologic factor. Our recent experience with such an angiosarcoma arising around a Dacron aortic graft prompted a review of the lesion to identify risk factors, diagnostic approaches, and treatment options. The diagnosis of these sarcomas is seldom made before operation. Animal studies have implicated plastic polymers including Dacron as carcinogenic materials capable of inducing sarcoma in 7% to 50% of exposures. Because of the rarity of these tumors and the thousands of vascular implants used over the past 30 years, it is unlikely that this degree of risk can be extrapolated to humans. However, a tumor should be included in the differential diagnosis of any mass or thromboembolic event associated with a vascular prosthesis.

Communicating across cultures: improving translation to improve complex emergency program effectiveness.

Translation is a vital activity in Complex Emergencies (CEs) in which the responders and the affected populations do not share the same language or culture. This particularly applies to CEs in developing countries in which a lack of local resources usually results in the importation of foreign aid workers. This paper describes many of the common issues surrounding translation that can affect CE response effectiveness, issues that frequently are not appreciated by aid workers, including clinicians. The authors describe how these issues can arise, their effects, and outline approaches to addressing them.
