Calcium channel blockers in cirrhotic patients with portal hypertension.

Four calcium channel blockers, i.e. nifedipine, verapamil, cinnarizine and tetrandrine are currently available and used widely in treating cardiovascular diseases. To confirm the effects, if any, of calcium channel blockers on cirrhotic patients with portal hypertension, a study was performed on esophageal variceal pressure and rebleeding rate of esophageal varices after 2 years by using calcium channel blocker in 321 patients from some 23 hospitals. The results demonstrated that the calcium channel blockers could significantly reduce the esophageal variceal pressure and the portal blood flow in cirrhotic patients with portal hypertension. The proportion of patients with no recurrent gastrointestinal bleeding after 2 years medication of tetrandrine was 87.9% in tetrandrine group, significantly higher than those in the other 4 groups (P < 0.05). It is suggested that tetrandrine should be more effective for cirrhotic patients with portal hypertension in preventing recurrent variceal bleeding.

[Investigation and experimental study on the standards and quality of leech pieces].

It has been confirmed by investigation that Whitmania pigra is the main medicinal breed of leech, and filiform pieces are the best for extraction. The quality of leech processed by 3 different methods has been studied qualitatively and quantitatively in terms of amounts of extraction, TLC, electrophoresis of protein and anti-thrombin action.

[The injury of liver and kidney of rats sustaining delayed fluid resuscitation of burn shock].

Delayed fluid resuscitation of burn shock may lead to many harmful effects. We investigated the injury of liver and kidney of rats sustaining non-fluid perfusion, immediate perfusion, and delayed perfusion of burn shock. The electron spin reonance (ESR) was used to determine the existence of oxygen free radicals (OFR) successfully. We tested the activity of ATP enzyme in kidney, lactate dehydrogenase isoenzyme 5 (LDH5), GPT and GOT. We also tested the contents of malonaldehyde (MDA) and ATP in liver and kidney, urea nitrogen (BUN) and creatinine (Cr) in blood. We found that OFR plays an important role in the injury of liver and kidney sustaining delayed fluid resuscitation of burn shock. Immediate fluid perfusion can not protect the liver and kidney perfectely. And some OFR scavengers should be added to the fluid resuscitation of burn shock.

Multi-target PCR analysis by capillary electrophoresis and laser-induced fluorescence.

Quantitative analysis of polymerase chain reaction (PCR) amplified HIV-1 DNA or cDNA fragments is attained using an automated system that combines capillary-gel electrophoresis (CGE) for high-efficiency separation and laser-induced fluorescence (LIF) for high-sensitivity detection. This system enables the detection of PCR-amplified multiple target DNA or cDNA in the same tube by a single injection with high precision.

In vivo and in vitro 31P magnetic resonance spectroscopic studies of the hepatic response of healthy rats and rats with acute hepatic damage to fructose loading.

The hepatic response to a fructose challenge for control rats, and rats subjected to an acute sublethal dose of carbon tetrachloride (CCl4) or bromobenzene (BB), was compared using dynamic in vivo 31P MRS. Fructose loading conditions were used in which control rats showed only a modest increase in hepatic phosphomonoester (PME), and a small decrease in ATP, Pi, and intracellular pH after fructose administration. Both CCl4 and BB-treated rats showed a much greater fructose-induced accumulation of PME than did controls. Trolox C, a free radical scavenger, prevented most of this PME increase. BB-treated rats, given sufficient time to recover from the hepatotoxic insult, responded to the fructose load similarly to controls. Liver aldolase activities of control, toxicant-treated rats, and toxicant plus Trolox C-treated rats correlated inversely with PME accumulation after fructose loading (correlation coefficient: -0.834, P < 0.05). Perchloric acid extracts of rat livers studied by in vitro 31P MRS confirmed that the PME accumulation after fructose loading is mainly due to an increase in fructose 1-phosphate. These studies are consistent with the aldolase-catalyzed cleavage of fructose 1-phosphate being rate-limiting in hepatic fructose metabolism, and that the CCl4 and BB treatment modify and inactivate the aldolase enzyme.

Serological and genotypic characterization of group A rotavirus reassortants from diarrheic calves born to dams vaccinated against rotavirus.

Two strains of bovine rotavirus (BRV), designated strain Nebraska Scottsbluff-1 (NS-1) and NS-2, were isolated from 2 neighboring cow-calf beef cattle ranches where dams had been vaccinated with a commercial vaccine containing group A BRV strain Neonatal Calf Diarrhea Virus (NCDV)-Lincoln (P1:G6). Northern blot hybridizations using whole genomic RNA probes indicated that strains NS-1 and NS-2 had identical group A RNA electrophoretic patterns and were homologous at all gene segments. Strain NS-1 was compared with reference group A BRV strains using serological and genotypic methods. In vitro virus neutralization assays indicated that strain NS-1 was neutralized by a G6-specific neutralizing monoclonal antibody (mAb) and guinea pig hyperimmune serum (GPHS) raised against BRV strain B641 (P5:G6), but not by G10-specific neutralizing mAb or GPHS raised against BRV strain B223 (P11:G10). Nucleic acid hybridization experiments using whole-genomic RNA probes revealed that gene segment 4 of strain NS-1 differed from BRV strains NCDV-Lincoln and B223, but hybridized with strain B641. Conversely, gene segment 5 of strain NS-1 hybridized with BRV strain B223, but not with BRV strains NCDV-Lincoln and B641. A G-specific cDNA probe produced by reverse transcription polymerase chain reaction (RT-PCR) amplification of strain NS-1 hybridized specifically only with G6 strains NCDV-Lincoln and B641, but not with G10 strain B223. Co-electrophoresis experiments using strains NS-1, B641, and B223 further confirmed these results, suggesting that strain NS-1 was a naturally-occurring reassortant BRV between strains B641 and B223. Taken together these results indicated that a naturally-occurring group A BRV reassortant with a P gene different from the vaccine virus was responsible for the diarrheal syndrome observed on both ranches. Results from this study also indicate the existence of at least 2 different gene segments 5 among group A BRV infecting cattle.

Prompt pressure applied to peculiar points in the treatment of spasmodic infantile cerebral palsy--a report of 318 cases.

Infantile cerebral palsy is a nonprogressive central motor disturbance of varied etiology. The spasmodic type is its main form, accounting for 50-60% of the infants afflicted with the disease. The causal factors include multifarious conditions arising during parturition and in the pre- and postnatal periods. As yet there is no specific treatment for the disease. We have used a method of applying prompt pressure to peculiar points in treating it and received satisfactory results. The present group comprised 318 patients treated for the disease by the method of prompt pressure applied to peculiar points combined with appropriate orthopedic manipulations, resulting in a 73.27% rate of good to excellent therapeutic effect. Follow-up exam was done in 52 cases, among which the efficacy was assessed as stable in 21 cases, and the remaining 31 cases as being in an improved condition. Infantile peculiar points are points peculiar to infants, mostly located in the head and extremities, and constitute an important component part of the treasure-house of traditional Chinese medicine. The method recommended here is simple and convenient, causing little pain in the invalid yet showing a marked therapeutic effect, and therefore can be regarded as a new way for treating infantile cerebral palsy.

[Vascularized outer-table calvarial bone flaps to repair depressed disfigurement of face].

Utilizing the superficial temporal artery as the pedical, seven vascularized outer-table calvarial bone flaps have been raised for the reconstruction of a variety of craniofacial deformities. Satisfactory results were obtained. The advantages of the flap include: 1. proximity to the operative field; 2. it is a membranous bone similar to that of the facial bone; 3. it is more or less malleable, so that it can be formed into different shapes, in different sizes and thickness; 4. donor-site scar is inconspicuous; 5. no postoperative pain; 6. no resorption.

[Studies on magnetic responsiveness and renal embolization with magnetic microspheres in dogs].

The responsiveness and transcatheter embolization with magnetic gelatin microspheres (MG-ms) in dog kidney were reported. In experiments with magnet using a constant flow apparatus such as roller pump, the percent retention was determined by counting the carrier retained by the magnetic field and divided by the total starting counts. Factors influencing the percent retention of MG-ms include: velocity of medium flow, magnetic field intensity, alpha-Fe2O3 content in the MG-ms, viscosity of the medium and so on. Transcatheter embolization with MG-ms (10-30 microns) was performed under external magnet control in dog kidney. The result of angiogram and histological section showed that the MG-ms were in the arteries, arterioles, and glomerular capillaries with no adverse reactions. The embolized effect with magnet field was more prominent than the group without magnet field. These results show that the MG-ms is a promising embolic agent for treatment of renal cancer under external magnet control.

Zinc ions potentiate adenosine diphosphate-induced platelet aggregation by activation of protein kinase C.

Zinc deficiency has been linked to a bleeding tendency and impaired wound healing in several disease states. A number of investigators have suggested that zinc ions play a role in platelet aggregation in vitro as well as in in vivo studies. The purpose of the present study was to explore the mechanism by which adenosine diphosphate (ADP) and Zn2+ may act cooperatively during activation of blood platelets. We demonstrate that Zn2+ alone does not affect either formation of thromboxane A2 or intracellular calcium mobilization in platelets. On the other hand, we show that ADP and Zn2+ exert a cooperative effect on the phosphorylation of P-47 protein (pleckstrin), a substrate of protein kinase C in platelets. The inhibitory effect of this reaction by the compound Ro31, a specific inhibitor of the regulatory domain of protein kinase C, was compatible with our contention that Zn2+ may act directly on protein kinase C. Our study provides evidence that zinc ions present in plasma or platelets may modulate ADP-induced platelet aggregation. N,N,N',N'-tetrakis(2-pyridylmethyl)ethylenediamine (TPEN), a zinc chelator, blocked ADP-induced platelet aggregation. This aggregation was restored by 10 mumol/L of Zn2+ but not by other ions. Also, a Zn2+ ionophore, pyrithione, potentiated the ADP-induced platelet aggregation and this potentiation was blocked by TPEN. Experiments with the zinc ionophore suggest that intracellular zinc ions play an important role in activation of platelets, and in the absence of other platelet agonists it appears that it may be a requirement for ADP-induced platelet aggregation to occur.

Characterization of the [3Fe-4S] and [4Fe-4S] clusters in unbound PsaC mutants C14D and C51D. Midpoint potentials of the single [4Fe-4S] clusters are identical to FA and FB in bound PsaC of photosystem I.

In a previous paper we showed that the C51D mutant of PsaC contains a [3Fe-4S] cluster in the FA site and a [4Fe-4S] cluster in the FB site and that the C14D mutant contains an uncharacterized cluster in the FB site and a [4Fe-4S] cluster in the FA site [Zhao, J. D., Li, N., Warren, P. V., Golbeck, J. H., & Bryant, D. A. (1992) Biochemistry 31, 5093-5099]. In this paper we describe the electrochemical and electron spin resonance properties of the recombinant C14D and C51D holoproteins after in vitro reinsertion of the iron-sulfur clusters. Unbound PsaC shows no significant resonances in the oxidized state, but the unbound C14D and C51D mutant proteins show an intense set of resonances at g approximately 2.02 and 1.99 characteristic of an oxidized [3Fe-4S]1+/0 cluster. The Em' values for the [3Fe-4S]1+/0 clusters in C14D (FB*) and C51D (FA*) are -98 mV, and both represent one-electron transfers. After reduction with dithionite at pH 10.0, wild-type PsaC shows a broad set of resonances resulting from the superposition of FA- and FB- characterized by a low-field peak at an apparent g value of 2.051 and a high-field trough at an apparent g value of 1.898. The FB resonances in C51D were slightly narrower, with a low-field peak at an apparent g value of 2.039 and high-field trough at an apparent g value of 1.908.(ABSTRACT TRUNCATED AT 250 WORDS)

Arg15-Lys17-Arg18 turkey ovomucoid third domain inhibits human furin.

Turkey ovomucoid third domain with Leu18 in its reactive site is a potent inhibitor of many serine proteinases: subtilisins, chymotrypsins, and elastases. Previous studies showed that an L18K mutation made it a moderately strong inhibitor of trypsin, while an L18E mutation made it a strong inhibitor of Glu-specific Streptomyces griseus proteinase (GluSGP). For human furin substrates the consensus optimal sequence is RXKR decreases. Therefore the A15R, T17K, and L18R mutations were made in turkey ovomucoid third domain. The mutant inhibits human furin with a Ka of 1.1 x 10(7) M-1. As human furin catalyzes an obligatory step in human immunodeficiency virus proliferation, this inhibitor, along with the others already available, deserves further study.
