The pharmacokinetics of milrinone in pediatric patients after cardiac surgery.

BACKGROUND: Milrinone has been shown to increase cardiac output in children after cardiac surgery, but pharmacokinetic analysis has not been used to identify effective dose regimens. The purpose of this study was to characterize the pharmacokinetics of milrinone in infants and children and to apply the results to the issue of dosing. METHODS: Twenty children were studied after they underwent repair of congenital cardiac defects. Control hemodynamic measurement was made after the children were separated from cardiopulmonary bypass, and each patient was given a loading dose of 50 microg/kg progressively in 5 min. Hemodynamic measurements were recorded again at the end of the loading dose and when a blood sample was taken to determine milrinone plasma concentrations. Further blood samples were taken during the next 16 h for milrinone plasma concentration analysis. The pharmacokinetics of milrinone were analyzed using the population pharmacokinetic program NONMEM. RESULTS: The loading dose of milrinone resulted in a mean decrease in mean blood pressure of 12% and a mean increase in cardiac index of 18% at a mean peak plasma concentration of 235 ng/ml. The pharmacokinetics of milrinone were best described by a three-compartment model. In the optimal model, all volumes and distribution clearances were proportional to weight, and weight-normalized elimination clearance was proportional to age; ie., Cl1 = 2.5 x weight x (1 + 0.058 x age) where Cl1 is expressed as ml/min, and the units of weight and age are kg and months, respectively. CONCLUSIONS: A loading dose of 50 microg/kg effectively increases cardiac index in children after cardiac surgery. Simulations indicate that the peak plasma concentration can be maintained by following the loading dose of 50 microg/kg with an infusion of approximately 3 microg x kg(-1) x min(-1) for 30 min and then a maintenance infusion, which may require adjustment for age.

Pharmacodynamic comparisons of levofloxacin, ciprofloxacin, and ampicillin against Streptococcus pneumoniae in an in vitro model of infection.

The increasing frequency of penicillin-resistant pneumococcus continues to be of concern throughout the world. Newer fluoroquinolone antibiotics, such as levofloxacin, have shown enhanced in vitro activity against Streptococcus pneumoniae. In this study, the bactericidal characteristics and pharmacodynamic profiles of levofloxacin, ciprofloxacin, and ampicillin against four isolates of S. pneumoniae were compared by using an in vitro model of infection. Standard antibiotic dosing regimens which simulated the pharmacokinetic profile observed in humans were used. Control and treatment models were sampled for bacterial CFU per milliliter over the duration of each 24- or 48-h experiment. In addition, treatment models were sampled for MIC determinations and drug concentration. Regrowth of all isolates as well as an increase in MICs throughout the study period was observed in the ciprofloxacin experiments. A limited amount of regrowth was noted during levofloxacin therapy for one isolate; however, no change in MIC was detected for any isolate. Ampicillin showed rapid and sustained bactericidal activity against all isolates. In this study, ratios of effective fluoroquinolone area under the concentration-time curve (AUC):MIC values ranged from 30 to 55. Levofloxacin, owing to its larger AUC0-24 values, has excellent and sustained activity against different pneumococcal strains superior to that of ciprofloxacin.

Aerosolized GM-CSF ameliorates pulmonary alveolar proteinosis in GM-CSF-deficient mice.

Surfactant proteins and phospholipids accumulate in the alveolar spaces and lung tissues of mice deficient in granulocyte-macrophage colony-stimulating factor (GM-CSF), with pathological findings resembling the histology seen in the human disease pulmonary alveolar proteinosis (PAP). Previous metabolic studies in GM-CSF-deficient [GM(-/-)] mice indicated that defects in surfactant clearance cause the surfactant accumulation in PAP. In the present study, GM(-/-) mice were treated daily or weekly with recombinant mouse GM-CSF by aerosol inhalation or intraperitoneal injection for 4-5 wk. Lung histology, alveolar macrophage differentiation, and surfactant protein B immunostaining returned toward normal levels in the GM-CSF aerosol-treated mice. Alveolar and lung tissue saturated phosphatidylcholine and surfactant protein B concentrations were significantly decreased after treatment with aerosolized GM-CSF. Cessation of aerosolized GM-CSF for 5 wk resulted in increased saturated phosphatidylcholine pool sizes that returned to pretreatment levels. In contrast, PAP did not improve in GM(-/-) mice treated daily for 5 wk with larger doses of systemic GM-CSF. Aerosolized GM-CSF improved PAP in the GM(-/-) mice, demonstrating that surfactant homeostasis can be influenced by local administration of GM-CSF to the respiratory tract.

Androgen-stimulated human prostate epithelial growth mediated by stromal-derived fibroblast growth factor-10.

It has been suggested that prostate homeostasis is regulated indirectly by androgens through stromal-epithelial interactions in part by factors from the stromal cells acting on receptors in epithelial cells. In this report, the role of fibroblast growth factor (FGF)-10 in prostatic epithelial proliferation was investigated. The expression of FGF-10 mRNA was apparent in primary-cultured stromal cells, but not in epithelial cells derived from human tissue from patients with benign prostatic hyperplasia (BPH). The mitogenic activity of human recombinant FGF-10 assessed by 5-bromo-2'-deoxyuridine (BrdU) incorporation was demonstrated in isolated epithelial cells, but not in cultured stromal cells. No mitogenic activity of dihydrotestosterone (DHT) for either epithelial or stromal cells could be demonstrated, but quantitative PCR (real-time PCR) with a double-labeled fluorogenic probe demonstrated that expression of FGF-10 in stromal cells was enhanced 5.3-fold at a DHT concentration of 100 pM. Androgen receptor mRNA levels showed no significant change with DHT at concentrations less than 100 pM, but were reduced to 50% of control levels at a DHT concentration of 10 nM. These results suggest that stromal-derived FGF-10 stimulates human prostatic epithelial growth and its mRNA expression is induced by androgens, without an increase in the androgen receptor mRNA. Moreover, FGF-10 may be involved in the development or support of human BPH.

Ubiquitous induction of p53 in tumor cells by antisense inhibition of MDM2 expression.

BACKGROUND: The MDM2 oncogene functions as a negative feedback regulator of the p53 tumor suppressor. Abnormal expression of MDM2 in tumors may attenuate the p53-mediated growth arrest and apoptosis response, resulting in increased cell proliferation and resistance to chemotherapy. MATERIALS AND METHODS: We have developed phosphorothioate antisense oligodeoxynucleotides optimized for inhibition of MDM2 expression and investigated the role of MDM2 in a large panel of tumor cell lines. RESULTS: Inhibition of MDM2 expression in 15 tumor types containing wild-type p53 results in a significant induction of nuclear p53 accumulation. The increase in p53 level is due to prolonged half-life and is associated with an increase in p53 transcriptional activity, growth inhibition, or apoptosis. Inhibition of MDM2 expression is also sufficient to induce nuclear p53 accumulation in several cell lines with cytoplasmic p53. CONCLUSIONS: The MDM2 negative feedback loop is important for maintenance of p53 at a low level by promoting p53 degradation. Nuclear export and degradation by MDM2 may contribute to the p53 nuclear exclusion phenotype. Inhibition of MDM2 expression can effectively activate p53 in most tumor types, including those without MDM2 overexpression, and may have broad anti-tumor potential.

Suitable location and optimum bladder width for the occluding cuff used for indirect arterial blood pressure measurements at the wrist.

Using a finite element method (FEM) and a volume oscillometric method with a photo-electric plethysmograph, we tried to determine which location(s) and which bladder width(s) of the occluding cuff would allow the most accurate indirect blood pressure measurements at the human wrist. Nuclear magnetic resonance imaging of the wrist allowed us to determine the geometry of two-dimensional (2D) finite element models. FEM analysis showed that the pressure transmission ratio (calculated tissue pressure: externally applied pressure) from the cuff to the radial artery (RA) was 100% when the cuff was placed over the site (L1) at which the RA crosses the most protuberant spot on the volar aspect of the distal end of the radius. In fact, the mean blood pressure (BPM) measured at this site in human subjects coincided with that measured at the upper arm (BPMA). In addition, a three-dimensional (3D) FEM was constructed by extending the 2D FEM at site L1 in the longitudinal direction to investigate the relation between pressure transmission to the RA and cuff bladder width. FEM analysis using the 3D finite element model showed that for all cuff widths greater than 0.25 times the diameter of the wrist, the external pressure was transmitted almost completely to the RA beneath the cuff center. Moreover, when the cuff width was as large as 0.45 times the diameter of the wrist, the BPM measured at site L1 in human subjects was similar to the BPMA. The theoretical and experimental results in the present study both suggest that for accurate indirect blood pressure measurements at the wrist, the L1 site is a suitable location for the occluding cuff and a cuff whose bladder width is more than one-third but less than one-half the wrist diameter can be used.

Benzene increases aneuploidy in the lymphocytes of exposed workers: a comparison of data obtained by fluorescence in situ hybridization in interphase and metaphase cells.

Benzene is an established human leukemogen that increases the level of chromosome aberrations in lymphocytes of exposed workers. Numerical aberrations (aneusomy) can be observed by fluorescence in situ hybridization (FISH) in both interphase and metaphase cells. Whereas interphase FISH allows nondividing cells to be analyzed, one advantage of metaphase FISH is that it can also detect structural changes. The present study compares the abilities of metaphase and interphase FISH to detect aneusomy of chromosomes 7 and 8 in healthy benzene-exposed human subjects. Metaphase and interphase cells from the peripheral blood of 43 workers exposed to benzene (median = 31 ppm, 8-hr TWA) and 44 frequency-matched controls were analyzed by FISH. Normal diploid cells contained two hybridization signals, whereas those that were potentially monosomic contained one, trisomic 3 and tetrasomic 4. The frequency of cells with one hybridization signal for chromosome 7 in metaphase spreads rose from 2.72 +/- 0.19 (%, mean +/- SE) in controls to 3.79 +/- 0.63 in workers exposed to 31 or fewer ppm benzene and 5.9 +/- 0.85 in those exposed to more than 31 ppm (P(trend) < 0.0001). No similar dose-dependent increase in the frequency of cells with one hybridization signal was observed by interphase FISH, probably because of probe overlap artifact. Although significant dose-dependent increases in the frequency of cells with three hybridization signals for chromosome 7 were detected by both methods in the higher-exposed group, a larger, more significant difference was detected by metaphase FISH between controls and workers exposed to 31 or fewer ppm. Similar data were obtained for chromosome 8. Interphase and metaphase FISH were moderately correlated for three hybridization signals but not for one hybridization signal in chromosome 7 or 8. In general, metaphase FISH was more sensitive in detecting both monosomy and trisomy in the lymphocytes of exposed workers.

[Effects of A, D-genome chromosomes on photosynthetic carbonassimilation in common wheat] [In Process Citation]

Photosynthetic traits of ditelosomic lines with A, D-genome in CS (Chinese Spring) were investigated in this paper. The both arms of 4A-chromosome have the positive effects on photosynthetic rate, active photosynthetic duration(APD), chlorophyll content and mesophyll conductance. The short arms of 1A and both arms of 6D also have positive effects on photosynthetic rate and APD. But the long arms of 4D have negative effects on photosynthetic rate, APD and RuBPCase activity. The short arms of 2A, long arms of 5A, 6D and 7D negatively affect on RuBPCase content. So, 4A, 4D and some other chromosomes have shown important effects on photosynthesis in common wheat.

Corneal topographic analysis after excimer photorefractive keratectomy.

OBJECTIVE: To evaluate the corneal surface changes and visual quality after excimer photorefractive keratectomy (PRK) for myopia. METHODS: Corneal topographic analysis was performed on 23 patients (38 eyes) with myopia after PRK at the 1-, 3-, 6- month postoperative follow-up visits. RESULTS: The corneal sphericity was changed after excimer PRK. As time went on, the mean surface regularity index (SRI) and the surface asymmetry index (SAI) decreased gradually, and the corneal surface became more smooth; mean simulated keratoscope readings (SimK) showed a gradual restoration. At the 6-month postoperative examination, corneal topography showed four main patterns of ablation: round or oblong (50%), collar-button (23.68%), semicircular (18.42%) and central island (7.9%). Patterns of ablation were correlated with visual acuity. CONCLUSION: The quantitative analysis of corneal topography is essential for evaluating corneal surface changes after PRK and helpful in the surgical design of PRK and in predicting the refractive outcome with greater precision.

[Orbicularis oculi muscle suspension and orbital septum fixation in blepharoplasty].

OBJECTIVE: The study is to correct all the deformities of the lower eyelid. There is a regressive keletonization of the orbit as the soft-tissue covering of the lower eyelid skin and orbicularis oculi descend downward due to loss of tone, and the orbital fat herniates through a weakened septum orbital, resulting in an infraorbital crease. METHODS: We suspend the orbicularis oculi muscle and fix the orbital septum in blepharoplasty. RESULTS: This method has been used in 32 patients with satisfactory results in recent years. CONCLUSION: The technique can improve all the deformities of the baggy eyelids and achieve long-lasting youthful countenance.

[Study of 6 cases of AIDS with pulmonary complication].

OBJECTIVE: To improve and to increase the awareness of pulmonary complications of AIDS. METHODS: Six cases of confirmed AIDS from may 1992 to August 1997 were analyzed. RESULTS: Of the six cases, there were five complicated with pneumocysitis carinii pneumonia (PCP) (in which one had PCP as the first occurred manifestation). The clinical presentations of PCP were fever(5/5), dyspnea on exertion or at rest(4/5), and hypoxemi with mean PaO2 of 58.1 mm Hg. Chest X-ray showed bilateral diffuse interstitial or alveolar infiltrates. Pulmonary tuberculosis, tuberculous lymphadenities (fast-acid staining positive but PPD negative) and bronchial fungal infection were found in three cases. CONCLUSIONS: AIDS patients are at high risk of suffering from pulmonary complications, of which PCP is more common. If young pateints who were healthy in the past suddenly suffered from pneumonia and respiratory failure, PCP should be considered. When opportunistic pulmonary infections are diagnosed under special circumstances, one should be alert to the possibility of AIDS. In these cases serum HIV antibodies should be checked immediately.

[Helicobacter pylori infection and expression of PCNA, p53, c-erbB-2 in carcinoma and precancerours lesions of the stomach].

OBJECTIVE: To study the role of Helicobacter pylori (Hp) in gastric carcinoma. METHODS: Hp was examined in 70 patients with gastric carcinoma, 62 patients with pre-malignant lesions and 16 controls by improved Warthin-Starry method and PCNA, p53, cerbB-2 protein examined immunohistochemically. RESULTS: The frequency of Hp infection in gastric carcinoma and its precusor lesions was significantly higher than that in the controls. There was no significant difference in Hp infection rate between gastric carcinoma and its precursor lesions, nor between the intestinal type and diffuse type of gastric carcinoma. Hp infection correlated well with expressions of PCNA, c-erbB-2 and with co-expression of PCNA, p53 and c-erbB-2 in gastric carcinoma and its precusors. p53 protein was expressed only in gastric carcinoma, and its expression level was higher in the presence than in the absence of Hp infection (P < 0.01). CONCLUSION: Hp might act as a tumor promoter in the genesis of gastric cancer.

[A study on the etiology of Budd-Chiari syndrome].

OBJECTIVE: To investigate the etiology of Budd-Chiari syndrome (BCS) with finding in surgical operation. METHODS: Thirteen patients with BCS undertook direct vision membranectomy and reconstruction of inferior vena cava. The specimens were studied with light microscopy, electron microscope and immunohistochemistry. RESULTS: Membranous occluding changes, which thickness varying from 2 to 20mm, were found in the posthepatic segment of inferior vena cava during the operation. Thrombi in inferior vena cava were found in 7 cases, 3 of them with thrombi evenly extending to the root of hepatic vein. The compromised venous wall showed proliferation of fibrous connective tissue, hyalinization or mucoid degeneration under light microscopy, 9 of the 13 cases showed chronic infiltration of inflammatory cells. Copious collagenous fibers, few endothelial cells and mitochondria swelling were found with electron microscope. No immune complex (IgA, IgG, IgM) was observed by immunohistochemistry. CONCLUSION: It is concluded that in these cases of BCS, occluding and stenosing lesions in the inferior vena cava were the outcome of thrombosis and its sequelae and local inflammation might be one of the reasons of thrombosis. There was no evidence of congenital malformation.

[A retrospective study of 3 cases of nephrotic syndrome with pulmonary thromboembolism].

OBJECTIVE: To study the nephrotic syndrome (NS) with pulmonary thromboembolism (PTE). METHODS: 3 cases of NS with PTE were reviewed. RESULTS: The fibrinogen (mean 7.8 g/L), alpha-2-maccoglobulin (mean 21%) and cholesterol (mean 7.2 mmol/L) in the blood were higher in the NS patients with PTE. 1 of 2 patients was found to be membranous nephropathy. Ventilation/perfusion lung scan and a postmortem examination showed that bilateral, multiple pulmonary thromboembolism was found in 1 case. It was effective to use heparin and/or urokinase for NS with PTE. CONCLUSIONS: NS exists in the hypercoagulable state in blood. It is easy to concomit PTE. Pulmonary artery angiogram should be done when NS with PTE was suspected. Besides glycocorticoids, the combination of anticoagulants and thrombolysis gave most favorable results for NS patients with PTE.

[Long-term efficacy of transurethral electrovaporization of the prostate for symptomatic benign prostatic hyperplasia].

OBJECTIVE: To observe long-term efficacy of transurethral electrovaporization of the prostate for symptomatic benign prostatic hyperplasia. METHODS: Two types of electrode of transurethral electrovaporization of the prostate (TVP) were performed on 150 patients of symptomatic BPH (mean age 74.8 years). The patients were assessed at baseline for both efficacy and followup at 36 months. Efficacy parameters evaluated included operation time (minutes), I-PSS and quality of life score, digital rectal examination, transrectal ultrasound, urodynamics and postvoid residual urine. RESULTS: 36 months after operation, I-PSS was decreased from 24.8 to 5.4 and the quality of life score also was decreased from 4.9 to 2.1. The mean peak uroflow was increased from 9.8 to 18.5 ml/s and postvoid residual urine was decreased from 108 ml to 22 ml (P < 0.05). The mean operation time was 44.7 min. Urodynamic results of 139 cases (92.7%) showed no bladder outlet obstruction, the remaining 11 cases showed severe bladder outlet obstruction to mild obstruction after 36 months. Hematuria was the only complication of our procedure. CONCLUSIONS: Significant clinical improvement is maintained with minimal morbidity. The long-term efficacy and safety of TVP is satisfactory. TVP is a potential useful procedure for transurethral resection of the prostate.

[Genotoxicity and lipid peroxidation caused by organic extracts of chlorinated drinking water in rats and HepG2 cells].

The genotoxic effects of chlorinated drinking water(CDW) extracts were examined in human HepG2 hepatoma cells, liver cells and polychromatic erythrocytes (PCEs) of bone marrow of Wistar rats by using in vivo and in vitro micronucleus(MN) test and single cell gel electrophoresis (comet assay). The influence of CDW on the lipidperoxidation (LPO) in blood and tissue of rats was also studied. The results indicated a significant increase of the frequencies of MN in PCEs at the low concentration of CDW and in HepG2 at the high concentration of CDW. The increased DNA breakage caused by CDW was observed in rat liver cells and in HepG2 cells by using comet assay. The levels of malon dialdehyde(MDA) were significantly increased almost in all animals and in all organs tested. A dose-dependent increases of MDA were observed in rat liver. It was demonstrated that CDW extracts caused genotoxic and oxidative damage on animals.

[The properties of new affinity adsorbent for adsorbing the cycle immune complex in blood].

This study was designed to select an affinity adsorbent from 5 kinds of adsorbents available. The adsorbent AA3 was found being specific to the adsorption of pathologic cycle immune complex (CIC). The physical properties such as specific surface area, proe volume and average pore diameter of adsorbents were determined. A discussion on the factors that affect the adsorption of CIC is also presented in this paper.
