OBJECTIVE: To report the results of microsurgical treatment of tumors medially located in the middle skull base. METHODS: The preoperative MRI features, operative approaches, results of operation and postoperative complications in 26 cases were reviewed. RESULTS: Total resection of the tumors was achieved in 12 cases, subtotal resection in 9 cases, partial resection in 6 cases. Two patients died of operation. Cranial nerve palsy remained unchanged in 5 cases and became exacerbated in 5 cases. Clinical improvement was observed in 15 cases. CONCLUSION: The combined frontotemporal-zygomatic approach to remove medially located tumors of the middle skull base is adequate.
OBJECTIVE: To improve the successful rate of acupuncture anesthesia for supratentorial craniocerebral operations. METHODS: On the basis of previous study of acupuncture anesthesia for craniocerebral operations, 72 cases of front fossa and temporo-parieto-occipital region lesions were operated on by acupuncture anesthesia with new modified acupoints and adjuvants from 1993 to 1998. RESULTS: One hundred percent of successful rate was achieved in all 72 cases by the new methods. CONCLUSION: Acupuncture anesthesia has distinct advantage and should be widely used as one of the effective anesthesia for supratentorial craniocerebral operations.
OBJECTIVE: To evaluate the survival rates, functional outcome, and complications between the combined and non-combined treatment of primary osteosarcoma of the extremities. METHOD: From 1977 through 1992, 170 patients with high-graded, nonmetastatic osteosarcoma were treated. Their average age at diagnosis was 21 years old (ranging from 6 to 52). Tumors were observed at distal femurs in 80 patients (47%), proximal tibia in 51 (30%), proximal humors in 10 and other locations in 22. Combined therapy (en bloc resection of tumor with preoperative and postoperative neoadjuvant chemotherapy) was given 104 patients and non-combined therapy (tumor resection only) to 66. Ninety-four patients underwent limb salvage surgery. In 76 patients who received amputations or disarticulations 33 received chemotherapy. RESULTS: The 5-and 10-year-survival rates were 61% and 53% in the combined therapy group and 28% and 26% in the non-combined therapy group. Local recurrence rate was 23% in limb salvage surgery group and 5.2% in amputation group. Functional evaluation of 80 patients showed that the patients who underwent salvage surgery had higher functional scores than those who had an amputation according to MSTS scoring system (1993). The average scores reached 71% in the salvage surgery group and 53% in the amputation group. CONCLUSIONS: In this study, long-term that combined therapy raised the survival rate of the patients with osteosarcoma. The ase of en bloc resection of bone tumor and reconstructive technique, has enable limb salvage procedures possible to produce excellent functional outcomes. The patients who well have responses to preoperative chemotherapy may have a higher survival rate.
OBJECTIVE: To study alterations of molecular cytogenetics of bone tumor serially. METHODS: Chromosomal analysis, immunohistochemistry, molecular hybridization, nested-PCR/SSCP, DNA sequence analysis, and gene transfection techniques were used to investigate the common bone tumors. RESULTS: Chromosomal aberrations were found in 50 cases of aggressive bone tumors, with a chromosome number aberration rate of 86.96% and a structure aberration rate of 93.94%. However, there were differences among different kinds of bone tumors. The incidence of chromosomal instability in 53 patients with bone tumors was higher than that in normal controls. Meanwhile, the fragile sites of chromosomes of the patients were closely correlated with the high nonrandom chromosome breakpoints of tumor cells and related oncogene loci. The data on p53 and Rb from 305 cases and 89 cases, respectively, indicated that abnormalities of p53 and Rb were common events in bone tumors. The accumulation of p53 protein and loss of Rb protein were more common in aggressive bone tumors. Structures and their products of related proto-oncogenes (ras, myc, fos, bcl-2, met) were studied in 124 cases. Bone tumors presented multi-oncogenes abnormalities with differences in different pathological kinds. Exogenous wild-type p53 gene transfected into osteosarcoma cell line induced growth inhibition and apoptosis of tumor cells. CONCLUSIONS: Abnormalities of chromosomes, proto-oncogenes, and tumor suppressor genes frequently exist in bone tumor cells and are correlated with oncogenesis.
OBJECTIVE: To explore the clinical characteristics and surgical results of retinal detachment in eyes with anterior proliferative vitreoretinopathy (PVR). METHODS: 83 patients (83 eyes) of retinal detachment with grade C-D PVR were studied. Different surgical methods were used in treating anterior PVR and posterior PVR, respectively. RESULTS: Among the 83 patients, anterior PVR was in 34 eyes (41.0%) and posterior PVR 49 eyes (59.0%). The anterior PVR occurred in younger patients, mean age 28.5 years, more in male patients (85.3%) and more in grade D of PVR (55.9%); in the posterior PVR, the mean age was 38.9 years, 63.3% of patients were male and 24.5% of patients were in grade D of PVR. Eighty-three patients had a mean postoperative follow-up of 12.9 months. Complete retinal re-attachment occurred in 24 eyes (70.6%) with anterior PVR and in 38 eyes with posterior PVR (77.6%; chi(2) = 0.51, P > 0.1). The final corrected visual acuity >or= 0.05 was in 11 eyes with anterior PVR (32.4%) and in 22 eyes with posterior PVR (44.9%; chi(2) = 1.32, P > 0.1). CONCLUSIONS: Eyes with anterior PVR have different clinical characteristics and worse surgical conditions when compared with eyes of posterior PVR. However, by the use of modern vitreoretinal surgery, the outcome in eyes with anterior PVR may become similar to that of posterior PVR.
OBJECTIVE: To understand the distribution and related risk factors of HIV infection among drug users in Guangxi. METHOD: Three hundred and twenty five drug users were tested for serum HIV antibody, with enzyme linked immunosorbent assay. RESULTS: One hundred and sixteen drug users (35.7%) were found to be HIV positive. The main risk factors associated with HIV infection were intravenous drug users and needle sharing among drug users. The HIV prevalence was 40.9% (114/279) among IDUs, and only 4.3% (2/46) among oral drug users. The phenomenon of sharing injecting equipment (76.7%) among intravenous drug users in Guangxi represents a very serious problem. CONCLUSION: There is a high risk of HIV infection among drug users in Guangxi. Prevention programs urgently need to be implemented.
OBJECTIVE: To study the risk factors of epidemic hemorrhagic fever (EHF). METHODS: A 1:1 matched case-control study was conducted to analyse EHF risk factors. RESULTS: One hundred and eighty-five matched pairs were investigated. Five factors were associated with risks of the disease, including rodent activity in house, farm working, travel, annual average income of an individual and ground material of the house, when chi(2) test and logistic regression model were processed respectively. The coefficients of rodent activity in house, farm working, travel, annual average income of an individual and ground material of the house in multivariable Logistic regression model were 1.377, 1.687, 1.630, -1.216 and -0.888, respectively, when P value < 0.05 was observed. CONCLUSIONS: Factors as high average income of individuals, brick or cement used as material of the ground in the house, seemed to be protective factors of EHF, which were important to the control and prevention of EHF.
OBJECTIVE: To observe the safety and the preventive effects of three kinds of vaccines as Mongolian gerbils kidney vaccine, mouse brains vaccine and hamster kidney vaccine inoculated 5 years ago. METHODS: Field study and laboratory tests were carried out by random grouping and IFAT, MCPENT, ELISA, HI tests. RESULTS: The antibody-dependent enhancement did not appear in all individuals who received vaccines after four or five years. The seroconversion rates of MCPENT were 8.97%, 11.76% and 18.75% while the seroconversion rates of IFAT were 20.73%, 30.22% and 23.40% respectively for Mongolian gerbils kidney vaccine, mouse brains vaccine and hamster kidney vaccine. The protection rates were 100% for all three kinds of inactive vaccines which showed good epidemiological efficacy. CONCLUSION: The vaccines can protect clinical infection of EHF effectively after four or five years of the initial vaccination.
OBJECTIVE: Safety and immunogenicity of inactivated bivalent EHF vaccine in humans were evaluated in the epidemic area of Zhejiang province, China. METHODS: Susceptible persons with negative anti-EHF were selected in Jiande county, Zhejiang province to receive 3 doses of inactivated bivalent EHF vaccine at 0, 7, 28 days. A booster injection was given one year after the primary immunization. Antibody responses were measured in human volunteers by IFA and MCPENT. Local and general reactions were recorded within 72 hours after each vaccination by physicians. RESULTS: Two weeks after the primary vaccination, 99.04% of the subjects developed significant hantavirus antibody titre measured by IFA which had a 37.34% drop one year after the primary vaccination. Seroconversion rate increased to 100% two weeks after the booster dose. Neutralising antibody titres paralleled this trend with 100% of vaccine recipients producing neutralising antibody two weeks after the primary doses. However, it dropped to 80% one year after the primary vaccination. One hundred percent of the vaccine recipients started to respond two weeks after boosting. The geometric mean titre (GMT) of neutralising antibody against 76 - 118 and UR were 18.27 +/- 2.21 and 12.47 +/- 2.16 respectively after the primary injections, but it increased to 37.09 +/- 2.24 and 32.61 +/- 2.05 respectively after the secondary immunization. General and local reaction rates were 0.46% and 1.98%, with no severe side effects observed in the vaccinees. CONCLUSION: The vaccine was well tolerated and could induce good humoral immune response.
OBJECTIVE: To detect the rearrangement of T cell receptor gamma chain gene in single H/R-S cell as an indication of early T lymphocyte differentiation. METHODS: CD30 positive H/R-S cells were isolated with micromanipulator from the frozen sections and analyzed by single-cell PCR. TCR-gamma chain gene rearrangement was detected through semi-nested PCR. In the first round of PCR, a pair of TCR-gamma primers which amplify the segments between Vgamma and Jgamma region (including the N region) of TCR-gamma gene mixing with a pair of beta-globin primers were used. In the second round, only the products positive beta-globin were amplified with the primers for TCR-gamma gene. RESULTS: A 268 bp beta-globin gene specific product was obtained in 24 out of 56 H/R-S cells from 6 cases of Hodgkin's disease. After the first round of PCR, no rearrangement of the TCR-gamma gene was noticed at a single cell level. CONCLUSION: H/R-S cells from the selected cases were not derived from the T-cell lineage but possibly from the B-cell lineage.
OBJECTIVE: To investigate the relationship of expression of NPM-ALK cohesive gene protein with the subtypes and prognosis of anaplastic large cell lymphoma (ALCL). METHODS: Nineteen cases of ALCL with CD30 positivity were studied. Immunohistochemical staining was performed in paraffin sections by using polyclonal antibody anti-p80 and other monoclonal antibodies. The clinical data of these patients were analyzed and all patients were followed-up. RESULTS: The neoplastic cells in 9 out of 19 ALCL expressed p80 protein. Histopathologically, all 9 cases with p80 expression were common type and small cell type. None of these 9 cases were Hodgkin like type (P < 0.05). Immunologically, all the 9 positive cases were T and null cell phenotype and none of them was B cell or other phenotype (P < 0.05). Clinically, the patients with p80 expression were younger than those without p80 expression (P < 0.05). The prognosis of the former was worse than in the later (P < 0.05). CONCLUSIONS: p80 positive ALCL could be a new clinicopathological entity. The polyclonal antibody anti-p80 might be an useful marker for the differential diagnosis and prognosis estimation of ALCL.
OBJECTIVE: To study the clinicopathologic features, the expression of cytotoxic-granule-associated protein TIA-1 and Epstein-Barr virus (EBV) infection in subcutaneous panniculitis-like T-cell lymphoma (SPTCL). METHODS: Totally 17 cases of SPTCL were studied. Immunohistochemical staining was done for TIA-1, CD45RO, CD3, CD20 and CD68, and in situ hybridization for EBV encoded small nuclear RNA (EBER1/2). RESULTS: In the 17 cases of SPTCL, the ratio of male-to-female was 1:1.1 and the median age was 24 years old. The main clinical manifestations were asymptomatic plaques, nodules and masses. Histologically, the pattern of lymphoid cells infiltration was panniculitis. The infiltrated lymphoid cells were pleomorphic and their mitotic figures were few (0 - 2/hpt). In the tumor tissue, there might be epithilioid granulomas, multinuclear cells, beanbag cells, small pieces of necrosis and small vessels with lymphoid cells infiltration in the vascular wall. Immunohistochemically, all the cases showed a T-cell phenotype and the expression rate of TIA-1 was 100% (16/16). By in situ hybridization, the positive rate of EBV-EBER1/2 was 29.4% (5/17). CONCLUSIONS: SPTCL has a specific histopathologic picture. It may originate from cytotoxic T lymphocytes or NK cells. Some of them may be accompanying with EBV latent infection.
OBJECTIVE: To analyse the characteristics of apoptosis sensitivity in U937-ASPI-3K cells which is stably expressing anti-sense ATM/PI-3K. METHOD: Annexin V-PI apoptosis detection kid and flow cytometry were used to determine apoptosis rate, Western blot to the expression level of cytochrome C protein and Bcl-2 protein. RESULTS: U937-ASPI-3K cells had a lowered threshold for triggering apoptosis in response to low dose irradiation. Cell apoptosis rate was 28% and (53.0 +/- 5.4)%, respectively, at 8 and 48 hours after 1.5 Gy (137)Cs irradiation, while in U937-pZEOSV2 (+) control group it was 4.2% and (11.0 +/- 3.3)%, respectively. Cytosolic cytochrome C protein band was gradually intensified in U937-ASPI-3K in 4 - 8 h after irradiation, but was not detected in control group. Bcl-2 protein gradually decreased in U937-ASPI-3K in 4 - 8 h after irradiation, but didn't change obviously in control group. CONCLUSION: Cell line U937-ASPI-3K acquired a phenotype with a lowered threshold for triggering apoptosis. It demonstrated that selective inactivation of ATM gene might be of considerable value in tumor treatment.
OBJECTIVE: To eliminate the back wall defect of external canal and opened mastoid cavity which was remained from radical mastoidectomy. METHOD: We performed the operation on 17 cases after radical mastoidectomy by using reconstruction of the canal wall and fill-in opened mastoid cavity and ossicular replacement prostheses made of homograft costal cartilage. The tympanoplasty had been made operated for all patients, 8 ears were PORP, the others were TORP. RESULT: All cases were followed-up for 6 months-2 years. The external canal of all patients are normal in appearance. The total effective rate of gain (> 15 dB) was 14 ears (82.4%). Only one patient whose tympanic membrane was perforated postoperation 3 months. CONCLUSION: Reconstruction of acusticus back wall and hearing after radical mastoidectomy with homograft costal cartilage is effective to prevent mastoid cavity infection and improve hearing.
Hydroxyl polycalcium sodium phosphate(HP) is a new substitute material for human hard tissues. The isolated rat osteoblasts from the parietal and frontal bones of 1-3d SpragueDawley rats were used to estimate the osteocompatibility of HP in this study. The effects of the materials on cell morphology, growth and proliferation were determined. The result showed that rat osteoblasts could attach, spread and proliferate on HP as normal cells. The proliferation of cells on HP was more favour able to the adhesion and proliferation of rat osteoblasts and could form osteointegration early.
OBJECTIVE: To reconstruct middle ear structure for open mastoid antrum with external auditory canal after radical mastoidectomy in one-stage. METHOD: 71 ears of post-mastoidectomy (discharging 53 ears and dried up 18 ears) were undergone with reconstruction of middle ear. The posterior wall of external auditory canal, mastoid cavity and chain of ossicles were reconstructed with homologous costal cartilage. RESULT: 69 ears of 71 cases were near normal structure followed up 6 months to 5 years after operations. The result showed hearing improvement over 15 dB were 55 ears (77.5%) and under 15 dB were 11 cases (15.5%). Five cases (7.0%) were failed. CONCLUSION: Reconstruction of middle ear with homologous costal cartilage is a ideal surgery to reconstruct hearing structure and avoid infection of middle cavity after radical mastoidectomy.
