[Surgical repair of postinfarction ventricular septal rupture].

OBJECTIVE: To study the characteristics of occurrence of postinfarction ventricular septal rupture (VSR) and preliminarily evaluate the proper timing of operation and its effect. METHODS: Twenty patients with postinfarction VSR were divided into two groups: surgical repair (14 patients), and conventional treatment (6 patients). Clinical data were analyzed and the 14 patients were followed up for 4 months-14 years. RESULTS: The 6 patients died within 6 hours-7 days. Of the 14 patients treated surgically, however, 4 had their ruptures closed by the technique of plication and the rest by prosthetic patches. Concomitant procedures included resection of postinfarction ventricular aneurysm in all 14 patients and coronary artery grafting bypass in 3. Five patients (35.7%) died during peri-operation, and 14.3% had residual ventricular septal defects. 2 patients died of heart failure 7 and 9 years after the operation respectively, 1 was lost to follow-up, and the rest were in good condition. CONCLUSIONS: The proper timing of operation determined by the cardiac function of patients. It is necessary to improve condition of circulation before operation. The mortality rate of emergency operation is high within 3 days after VSR, and residual VSR tends to develop in a week after the repair. Operation should be performed a week after VSR. The early operative mortality is closely related to preoperative cardiogenic shock and postoperative complications.

[A report on investigation and clinical application of corneal storage media].

OBJECTIVE: To develop intermediate term storage media suitable for Chinese eye banks. METHODS: Corneal buttons of rabbits were stored in DX solution freshly prepared. After storage for several days, the survival rate of the corneal endothelial cells was examined, enzymohistochemical staining and ultrastructural examinations were carried out for the buttons. Thirty-seven human corneas stored in DX solution for 3 - 11 days were used for corneal transplantation, clinically. RESULTS: There was no significant difference between rabbit corneal buttons stored in DX sol. and in Optisol in any indices studied. 37 grafts were all transparent in 1 week after transplantation, and the mean endothelial cell density was (2,204.56 +/- 689.56) cells/mm(2). CONCLUSION: As a safe and efficient intermediate term (about 1 week) storage medium, DX sol. is appropriate for Chinese eye banks.

[Reducing incidence and mortality from rectal cancer by polypectomy: a prospective cohort study].

OBJECTIVE: To address the hypothesis that colorectal cancers often arise from precursor lesion(s), either adenomas or non-adenomatous polyps. METHODS: A population-based mass screening for rectal cancer using 15cm rigid endoscopy was conducted in Haining County, Zhejiang province, from 1977 to 1980. Of over 230,000 participants, 4,076 of them were diagnosed with precursor lesions, either adenomas or non-adenomatous polyps, which were then removed. All individuals with precursor lesions were followed and re-examined by endoscopy every two to five years through 1998. RESULTS: After the initial screening, 952 metachronous adenomas and 417 non-adenomatous polyps were detected and removed from the cohort. Furthermore, 27 cases of colorectal cancer were detected and treated. Log-rank tests showed that the years of survival significantly increased among those cancer patients who had undergone mass screening compared to other rectal cancer patients (P < 0.001). According to the population-based cancer registry in Haining County, both age-adjusted incidence and mortality rates from rectal cancer decreased from 1977 to 1996. Observed accumulative incidence and mortality rates decreased to 68.6% and 82.4% comparing with expected numbers during the last 20 years. CONCLUSION: Mass rectal cancer screening in the general population followed by routine endoscopy for high-risk patients could decrease both rectal cancer incidence and mortality rates.

[The prevention of primary liver cancer by selenium in high risk populations].

OBJECTIVE: To study the preventive effects of selenium on primary liver cancer. METHODS: After screening of blood samples in 18,000 males from 20 to 65 years-old in Qidong, Jiangsu province (a high risk area for liver cancer), 2,065 cases of HBsAg positive, AFP negative and normal liver function (normal ALT values) were found. The subjects were randomly divided into two groups, based on their residence areas; 1,112 subjects (experimental group) received one tablet of sodium selenite (0.5 mg Se) every day and 953 subjects (control group) received one placebo tablet every day. RESULTS: During three years of intervention and follow up, the blood selenium concentration and glutathione peroxidase activity of the subjects in the experimental group were increased and had significant difference as compared with those of the control group (P < 0.01). At the same time, the prevalence rate of micronucleus cells in peripheral blood lymphocytes in the experimental group was significantly lower than that of the control group (P < 0.01), and the incidence of new liver cancer in the experimental group (3 057.55/10(6), 34 cases out of 1,112 subjects) was significantly lower than the control group (5 981.11/10(6); 57 cases out of 953 subjects) (P < 0.01). CONCLUSION: The results confirms that selenium supplementation in general populations lived in high risk is effective in the prevention of liver cancer and the using of selenium tablets is simple and feasible.

[Establishment of a rabbit model of non-thermal high voltage electrical injury].

OBJECTIVE: To establish a rabbit model of non-thermal high voltage electrical injury, which was accompanied by progressive tissue necrosis for the further study of electrical injury. METHODS: Seventy-five New Zealand rabbits were employed, in which 45 were used for the selection of the size of electrode plate, the inflicting time, the intervals and the injury degree. Five groups of rabbits were used for the study of the model. The study was carried out by means of clinical anatomical exploration, categorization by the Index of Deep Burn Injury (IDBI) and (99)Tc(m)-MDP isotope scanning and gamma photography. RESULTS: The optimal injury indices selected were as follows: the electric field strength was 17 000 volts/m, the mean current intensity was 554 mA, and the average current density was 137 mA/cm(2) for small electrode and 21 mA/cm(2) for big one, and average increase of tissue temperature was 1.73 degrees C during injury process. This excluded the possibility of thermal injury. Five models were created, i.e. mild, moderate, severe, extra severe and destructive ones. There was no obvious cutaneous necrosis. Nevertheless, there was loss of injured extremities on 5th, 7th and 12th post-injury days in the severe, extra severe and destructive groups. CONCLUSION: Non-thermal factor was the major cause of electric injury in the model with typical clinical features.

[Clinical observation on treatment of pediatric intractable nephropathy with modified taohong siwu decoction].

OBJECTIVE: To observe the clinical effect of Modified Taohong Siwu Decoction (MTSD) in treating pediatric intractable nephropathy (PIN). METHODS: Ninety-five cases of PIN were divided into 2 groups at random. The 60 cases in the treated group were administered with MTSD and the 35 patients in the control group were treated with heparin. The clinical therapeutic effect and levels of thromboxane B2(TXB2) and 6-keto-prostaglandin F1 alpha before and after treatment were observed. RESULTS: The total effective rate of the treated group was 81.7%, which was similar to that in the control group (80.0%, P > 0.05). The levels of TXB2 and TXB2/6-ketoprostaglandin F1 alpha ratio were higher in both groups of patients as compared with those of the healthy control (P < 0.01), After treatment, the two criteria were significantly improved in the two groups, as compared with those before treatment, the difference was significant (P < 0.01), while in comparson between the treated and the control group, no significant difference was found (P > 0.05). CONCLUSION: MTSD has good effect in treating PIN, it could improve the metabolic unbalance of thromboxane and prostacyclin.

[Effects of yanggan xifeng recipe on changes of neurological behavior of rat of Parkinson's model].

OBJECTIVE: To investigate the effect of Yanggan Xifeng Recipe (YGXFR) for nourishing Liver to calm down endogenous Wind on neurological behavior of Parkinson's disease (PD) rat model. METHODS: PD rat model was established by destroying the substantia nigra condense area (SNc) and ventral tegmental area (VTA) unilaterally with 6-hydroxy dopamine (6-OHDA). The rats for control were injected in the same site with the same dose of normal saline. The animals were randomly divided into 3 groups (7 in each group), the control group, the model group and the TCM treated group. Staircase test and rotation test induced by apomorphine were performed in all groups and compared. RESULTS: The staircase test demonstrated that, after being treated with YGXFR, the food pellets intake were increased compared to model group (P < 0.05), and their rotation induced by apomorphine was lessened (P < 0.01). CONCLUSION: YGXFR could recover the neurological behavior deficits of PD rat models.

[A pharmacodynamic research on chromone glucosides of fangfeng].

OBJECTIVE: To investigate the pharmacological activity of prim-O-glucosylcimifugin and 4'-O-beta-D-glucosyl-5-O-methylvisamminol con tained in Fangfeng. METHODS: Analgesic, antipyretic and antiinflammatory effects were assessed using the models of yeast-induced rat fever, acetic acidinduced peritoneal pain thermalpain, and dimethylbenzol-induced mouse ear edema. RESULTS: Prim-O-glucosylcimifugin and 4'-O-beta-D-glucosyl-5-O-methylvisamminol could reduce the body temperature of rats in a model of yeast-induced fever. Both components could decrease the spasm action induced by peritoneal injection of acetic acid, increase the pain-threshold in a thermal-pain model, relieve the mouse ear edema caused by dimethybenzol and significantly inhibit ADP-induced platelet aggregation. CONCLUSION: Prim-O-glucosylcimifugin and 4'-O-beta-D-glucosyl-5-O-methylvisamminol have significant analgesic, antipyretic, anti-inflammatory and anti-platelet aggregation actions.

[The expression of trkC in spared dorsal root ganglion following unilateral dorsal root rhizotomy].

This study sought to explore the expression of trkC in spared dorsal root ganglion(DRG) following unilateral dorsal root rhizotomy. 15 cats were divided into three groups: the intact group, the 3-day and 10-day groups after operation (unilaterral L1-L5 DRG and L7-S2 DRG were sectioned, but L6 DRG was spared). The animals were sacrificed on the 3rd day and 10th day after operation, respectively. The L6 DRG from the three groups were taken and cut into frozen sections (20 microns). The sections were stained using specific antiserum for trkC by the immunohistochemical ABC method. The numbers of large sized neurons (> 57 microns) and small sized neurons (< 42 microns) for trkC-IR were counted respectively. The results showed that the number of positive large sized neurons for trkC of the 10-day group after operation decreased significantly than that of the intact group and the 3-day group after operation, while the number of positive small sized neurons for trkC of the 10-day group increased apparently than that of the intact group and the 3-day group after operation (P < 0.05). This experiment demonstrated that unilateral partial dorsal root rhizotomy could change the expression of trkC might in large neurons and small sized neurons, and the results suggested that trkC might involve in the spinal cord plasticity following partial dorsal root rhizotomy.

[The correlation of serum levels of insulin-like growth factors and intrauterine growth: report of a study in rats].

This animal experiment was designed to investigate the relationship of the serum insulin-like growth factors (IGFs) with fetal intrauterine growth. The serum concentrations of IGF-I, -II in 21 fetal rats were measuerd and their relationships with fetal birth weight, length and tissue weights of brain, lung, liver were analysed respecively. The results showed that both of the serum concentrations of IGF-I, -II were positively correlated (P < 0.01) with fetal birth weight, length and tissue weights of brain, lung, liver respectively. These data suggest that both IGF-I and IGF-II may play an important role in fetal intrauterin growth and their decreased concentrations in serum may be factors contributing to fetal intrauterine growth retardation.

[Purification and characterization of an endo-beta-1, 4-mannanase from Bacillus subtilis BM9602].

An extracellular neutral endo-beta-mannanase(endo-beta-1, 4-D-mannan mannanohydrolase, EC 3.2.1.78)of Bacillus subtilis BM9602 was purified to electrophoretic homogeneity by ammonium sulfate precipitation and DEAE-cellulose DE22 chromatography with 45.5 fold and 5.9% yield. It's molecular weight and pl value were 35 kD by SDS-PAGE and 4.5 by isoelectric focusing, respectively. The enzyme was the most active at pH 5.8. The optimum temperature of the enzyme activity was 50 degrees C. The enzyme was stable at pH 6.0-8.0 and below 50 degrees C. The activity of the enzyme was inhibited by Hg2+, Ag+ strongly. For various substrates, such as locust bean gum, guar gum, sesbania gum and konjac gum, Km and Vmax value of the enzyme were 3.8, 14.9, 11.3, 2.4 mg/mL, and 24.5, 86.5, 38.4, 19.8 mumol.min-1.mg-1, respectively. The enzyme hydrolize various plant beta-mannans, with valuable oligosaccharides and without monosaccharide.

[The study on Rhizoma Alpiniae officinarum and other herbs as penetration enhancer for the permeation of 5-fluorouacil].

OBJECTIVE: To study the effects of volatile oils from Rhizoma Alpiniae Officinarum, Pericarpium Zanthoxyli, Herba Asari, cineaol and ethanol extracts on the percutaneous pentration of 5-fluorouracil. METHODS: By Valia-Chienhorizontal diffusion cell and HPLC, the effects of volatile oils from Rhizoma Alpiniae Officinarum, Pericarpium Zanthoxyli, Herba Asari, cineol and ethanol extracts on the percutaneous penetration of 5-fluorouracil were observed and compared with azone. RESULT: 1%, 3% volatile oils from Rhizoma Alpiniae Officinarum, Pericarpium Zanthoxyli and cineol possessed strong promoting action on percutaneous absorption of 5-fluorouracil. The effects of volatile oils from Rhizoma Alpiniae Officinarum was stronger than that of azone; the effects of volatile oils from Pericarpium Zanthoxyli was the same as that of azone. CONDUSION: The volatile oils from Rhizoma Alpiniae Officinarum, Pericarpium Zanthoxyli, Herba Asari can enhance effectively the skin permeation of 5-fluorouracil. They deserved to be researched and developed further.

[Studies on c-myc gene expression and p16 gene inactivation in nasopharyngeal carcinoma].

OBJECTIVE: The alterations of c-myc and p16 genes and their relationship with the development and progression of nasopharyngeal carcinoma were studied. METHODS: Sixty-nine biopsies of nasopharyngeal carcinoma were examined for the homozygous deletion methylation and reduced expression of p16, and the overexpression of myc family oncogenes using multiple PCR, restriction endonuclease coupled PCR, reverse transcriptase PCR and immunohistochemistry. RESULTS: The homozygous deletion and methylation of p16 gene were found in 7 and 11 cases respectively. The total inactivation rate was 26.1% (18/69). The negative expression of p16, as shown by immunohistochemical examination was found in 42 out of 69 cases (60.9%). The overexpression of myc family oncogenes was found in 51 cases (73.9%). CONCLUSION: It suggests that the inactivation of anti-oncogene p16 and the activation of myc family oncogene may play an important role in the development of nasopharyngeal carcinoma.

[Surgical treatment of malignant esophageal tumors in PUMC Hospital].

OBJECTIVE: To study how to prolong the postoperative survival time of the patients with malignant esophageal tumors. METHODS: The clinical data of 1,098 patients with malignant esophageal tumors from 1961 to 1992 were retrospectively analyzed. The deletion of fragile histamine triplet(FHIT) gene(a tumor suppressor gene) in 30 fresh esophageal samples obtained in 1996 was detected with PCR and RT-PCR. RESULTS: The resectability was raised gradually and the operative morbidity and mortality decreased year-by-year, but there was no significant improvement on the postoperative 5-year survival rate. Delayed diagnosis and ir-radical resection influenced the long term survival. The deletion of cDNA of FHIT gene was 64.2% in esophageal cancer and 20% in the resected margin of the cancer. CONCLUSIONS: High atypical hyperplasia in esophageal epithelium and deletion of FHIT gene in esophageal cancer and its resected margin are pathological and molecular markers for early diagnosis of esophageal cancer respectively, and the latter may be one of the molecular markers for the resection. Early diagnosis and treatment, radical resection, and postoperative nutritional support are essential for the improvement of the postoperative survival time of the patients.

Final report about Stage II Collaboration Project on Leprosy Rehabilitation.

OBJECTIVE: To undertake the pilot experiments of prevention of disability (POD) in 14 different geographical areas to serve as examples for future development of rehabilitation work in China and in Asia. METHODS: According to the principles and national criterion, 27 000 people affected by leprosy were selected and assessed using disability record forms at beginning and followed up regularly for observing changes of different indicators. RESULTS: A total of 197 neuritis cases were detected and treated with prednisolone out of 1 407 new or active cases. Self-care training of eyes, hands and feet were conducted for 10 500 disabled people affected by leprosy. Comprehensive therapy was given to 1 804 cases having complicated ulcers of which 1 055 cases have got their ulcers healed. Out of 706 prostheses, 613 were given to patients with satisfactory results. Surgical treatment was given to 269 cases and 251 have shown good progress. CONCLUSION: Most of patients have got benefit from the project in function or appearance which is very helpful for their going back to the society and agreed by foreign experts during the final evaluation. The experiences from the project can be implemented in the whole country.

[Flotation and extraction spectrophotometric determination of trace silicate in water].

In HCl solution, silicate reacted with molybdate ammonium to produce silicomolibdic, then a yellow compound which was produced from the oxidation of TMB was simultaneously isolated to benzene phase by flotation and then isolated to dimethylsulfoxideformic acid by extraction. The compound gives a high absorption at 458 nm. The apparent molar absorptivity is 1.26 x 10(5) L.mol-1.cm-1. In the range of 0.02-1 mg.L-1 Si obeys Beer's law. The proposed method which combines with enrichment and measurement is simple, rapid, selective and convenient to determine silicate in water with satisfied results.

[Conformity of cancerous diagnosis of serum-fluorimetry under the influence of carotenoid fluorescence].

We observed that carotenoid had sensitization for fluorescence of porphyrin as cancerous diagnostic technology was applied in clinic by serum-fluorimetry. The Forster theory was used in order to analyse sensitized fluorescence between carotenoid and porphyrin in this paper. Transfer characteristic between energy donor and energy acceptor was calculated and discussed its influence on cancerous diagnosis.

[The structure of DL-homocysteic acid in D2O solution].

DL-Homocysteic acid (DLH) is an important free amino acid in central nervous system of human body. In this paper the structure of DLH in D2O solution was discussed. The FTIR and FT-Raman spectra of DLH, which were obtained by Nicolet Magna-IR 750 II FTIR spectrometer and 950 FT-Raman spectrometer, were compared between its solid state and its D2O solution. The results indicated that the H-bonding structure of DLH in solid state was rearranged in D2O solution. The appearance of 1,620 and 1,408 cm-1 suggests the partly ionization of -COOH. From above, we can conclude that a new H-bonding structure of DLH was formed in D2O solution. The H-bond of C-O...H-N was replaced by C-O...D-O, and the H-bonds of -NH3+ and -SO3- were also varied by D2O molecular. Furthermore, the peak shift and the intensity changes of -CH2 in FTIR and FT-Raman spectra were also proved that the skeleton structure of DLH was changed in D2O solution. In addition, this new structure did not alter with the change of concentration of DLH.
