[The acute hemorheologic effect of calcium dobesilate].

In a series 40 subjects without manifest vascular disorders received 1000 mg Calcium dobesilate (Doxium). Before and 3 hours after application the viscoelasticity of whole blood as well as plasma viscosity and the hematocrit were determined. The statistical analysis was performed by Wilcoxon-test. The viscosity of whole blood and the hematocrit decreased with a significance of p less than or equal to 0.001, whereas the elasticity decreased in a significance manner of p less than or equal to 0.01. Plasma viscosity remained uncharged. The results show an effect of hemodilution, which could be very important for treatment of hyperviscosity.

Calcium dobesilate and its effects on hemorheology and microcirculation.

In series, 50 patients suffering from cerebral (n = 33) and peripheral (n = 17) diseases were treated with 3 x 500 mg calcium dobesilate (Doxium) per day. Before onset of therapy and after 14 days the viscoelasticity of whole blood and plasma as well as hematocrit and microcirculation at the finger pad were measured. Viscoelasticity of whole blood, plasma and hematocrit decreased in a significant manner (p less than or equal to 0.001), while microcirculation increased significantly (p less than or equal to 0.001). The results justify the application of calciumdobesilate for ischemic diseases with elevated viscosity.

[Effect of celiprolol compared to propranolol on thyroid metabolism].

In series of 20 patients suffering from atrial arrhythmias, hypertension or hyperkinetic heart syndrome in euthyroid subjects the effect of the new cardioselective beta-blocker celiprolol was examined when orally administered in doses of 300 mg per day against propranolol in a dosage of 160 mg per day. Celiprolol caused a significant reduction (p less than 0.01) of both systolic and diastolic blood pressure while propranolol showed simply a marked decrease in systolic blood pressure. A significant decrease in heart rate was observed with both drugs (p less than 0.01). TT3 and TT4 decreased only slightly when administering either celiprolol or propranolol. A further not significant drop in FT4 due to celiprolol is contrasted to a similarly not significant increase in FT4 when administering propranolol. A striking decrease in FT3 was induced by both drugs, although in the case of celiprolol this reduction became apparent only within a period of 3 weeks. Since only in cases of hyperthyroidism the number of beta-receptors has increased, the reduction of 5'deiodinase is possibly not so pronounced in euthyroidism. In this way an explanation may be found for a mild inhibition of conversion and a consequently missing increase in RT3. Neither celiprolol nor propranolol caused significant changes of laboratory findings, especially when considering triglycerides and creatinine values. Side effects could be lessened in nearly all cases and deterioration was not observed so that therapy never had to be discontinued. Celiprolol has to be considered as a further favourable cardioselective beta-blocker with intrinsic sympathicomimetic activity, causing a reasonable reduction in blood pressure and heart rate and moreover a propranolol-like effect in the peripheral metabolism of thyroid hormones in euthyroid patients.

[Myocardial infarction while windsurfing (author's transl)].

A 40-year-old joiner collapsed while windsurfing. After hospitalization the ECG showed a posterior wall infarction (stage 1) of which the patient shortly died. Autopsy revealed a fresh and a rather older posterior wall infarction, otherwise the coronary artery system was unremarkable. From indirect anamnesis a Wolff-Parkinson-White syndrome with paroxysmal tachycardias and collapses could be established. The non-traumatic causes of death in sport generally and the problems of WPW syndrome, paroxysmal tachycardia, collapse and sport in particular are dealt with in more detail. Possible causes for infarction with unremarkable coronary artery system are briefly presented. Finally a connection between reinfarction and windsurfing is discussed.

Ossifying fibromyxoid tumor misdiagnosed as follicular neoplasia. A case report.

BACKGROUND: Fine needle aspiration of an ossifying fibromyxoid tumor (OFMT) at a prethyroidal location is initially misinterpreted as follicular neoplasia. Though the histopathologic criteria have been analyzed in detail, no experience with the cytologic features of OFMT is reported in the literature. CASE: A 50-year-old male presented clinically with a recurrent goiter. Aspiration cytology (Giemsa stained) was characterized by a predominant fine fibrillary, pink matrix and moderate cellularity. The nuclei were eccentrically located, round to oval and slightly plemorphic, with fine chromatin. Focally, mild nuclear crowding and a few rosettelike structures were present. CONCLUSION: The cytologic features were consistent with a soft tissue tumor of probably neurogenic origin, like OFMT. Differential diagnosis in this particular location not only included follicular thyroid neoplasms but also neurofibroma, neuroma, chondroma and, less likely, neuroepithelial tumors. However, since some diagnostic criteria of OFMT, like the nodular growth pattern and mature bone, can be found only histologically, the diagnostic value of aspiration cytology seems to be limited.
