[Evaluation of diagnosis of endometrial hyperplasia by curettage].

OBJECTIVE: To evaluate the accuracy of diagnosing endometrial hyperplasia (EH) by curettage. METHODS: 150 cases who had been diagnosed as EH by curettage and received hysterectomy shortly after were studied retrospectively. All of the specimens obtained from curettage and operation underwent pathological examination. The results of pathological diagnosis of these two kinds of specimens were compared. Proliferating cell nuclear antigen (PCNA) expression was examined by immunohistochemistry in 38 cases diagnosed as complex atypical hyperplasia (CAH). RESULTS: Fifty-three cases were diagnosed as simple hyperplasia (SH), eleven cases as complex hyperplasia (CH), twenty-six cases as simple atypical hyperplasia (SAH), and sixty cases as CAH by curettage. Rediagnosis was made for all patients after hysterectomy. Pathological examination of the specimens from operation diagnosed 65 cases as SH, 7 cases as CH, 15 cases as SAH, 29 cases as CAH, and 34 cases as EC. The general accuracy of histological diagnosis by curettage was 76.7% approximately 92.6%. Coexistence with EC was more common in cases with SAH and CAH than in cases with SH and CH (chi(2) = 26.3, P < 0.01). Coexistence with EC was more common in cases with CAH than in cases with SAH (chi(2) = 9.78, P < 0.005). Among the CAH cases, coexistence with EC was more common in postmenopausal patients than in premenopausal patients (chi(2) = 3.93, P < 0.05). Among the CAH cases, the positive rate of PCNA expression was higher and strength of positivity greater in those cases with EC (Uc = 3.66, P < 0.05). CONCLUSION: The accuracy of curettage is rather high in diagnosing SH and relatively low in diagnosing CAH. CAH often coexists with EC. Examination of PCNA expression may help differentiate between CAH and EC.

Establishment of an animal model of chronic atrophic gastritis and a study on the factors inducing atrophy.

OBJECTIVE: To establish a rat model of chronic atrophic gastritis and explore the factors inducing atrophy. METHODS: In accordance with repeated orthogonal design of L8(2(7)), 60% alcohol and 20 mmol/L sodium deoxycholate (served as factor A), 0.05%-0.1% ammonia water (factor B), 0.05% indomethacin (factor C) were given, alone or in combination, to rats in three experiments for 3 months, 6 months or 9 months respectively. Then the rats were dissected, and their pathologic changes of the gastric mucosa were assessed. RESULTS: Typical signs of chronic atrophic gastritis (CAG) were found in all rats which were treated with factor A, B, C alone or in combination for 6 or 9 months. No significant difference of pathologic changes of gastric mucosa was found between the rats treated for 6 months and those for 9 months. No obvious CAG signs were found in the rats treated with factor A, B, C for 3 months. CONCLUSION: Sixty percent of alcohol, 20 mmol/L sodium deoxycholate, 0.05%-0.1% ammonia water and 0.05% indomethacin given to Sprague-Dawley rats for 6 months can successfully establish the animal model of CAG. Prolongation of the model-establishment time is not able to further facilitate the atrophy of gastric mucosa.

[The effect of epidermal growth factor on the pathologic changes of gastric mucosa in SD rats with chronic atrophic gastritis].

OBJECTIVE: To study the effect of epidermal growth factor (EGF) on the pathologic changes of gastric mucosa in rats with chronic atrophic gastritis(CAG). METHODS: The established rat models of CAG were divided into therapy group and control group. The rats in the therapy group received EGF 10 microg/kg subcutaneously (SC), whereas these in the control group the same volume of normal saline SC. 12 weeks later, all rats were killed by cervical dislocation and their gastric mucosa were examined with microscope. RESULTS: The grade of inflammatory cell infiltration in the therapy group was lower than that in the control group (P < 0.01). The thickness of gastric mucosal gland layer was (215.0 +/- 20.7) microm in the therapy group and (139.2 +/- 13.8) microm in the control group (P < 0.01). The ratio of the thickness of gastric mucosal glands and muscularis mucosa(L(1)/L(2)) was 2.70 +/- 0.34 in the therapy group and 1.27 +/- 0.27 in the control group (P < 0.01). The number of gastric glands in 1 mm length of mucosal layer was 26.20 +/- 1.27 in the therapy group and 19.90 +/- 1.78 in the control group (P < 0.01). In the therapy group, the gastric glands were rearranged in order, without signs of malignant proliferation. The width of the expression of proliferating cell nuclear antigen (PCNA ) of gastric mucosa was higher in the therapy group than in the control group [(77.70 +/- 4.16) microm vs (54.40 +/- 4.54) microm, P < 0.01]. CONCLUSIONS: EGF played a therapeutic role in reversing the gastric mucosal atrophy of the rats with CAG. It promoted the expression of PCNA, which induced a protective proliferation of the gastric mucosal lesions in the rats with CAG.

[The association between polymorphism of endothelial nitric oxide synthase gene and diabetic nephropathy].

OBJECTIVE: To investigate the association between a 894 G-->T mutation at exon 7 and a 27 base pair(bp) repeat polymorphism in intron 4 of the eNOS gene and diabetic nephropathy in Chinese. METHODS: A case control study of 228 Chinese subjects (including 143 type 2 diabetes mellitus with or without nephropathy and 85 normal controls) was performed. The number of 894 G-->T mutation allele and 27 bp repeat polymorhism alleles were determined by polymerasechain reaction restriction fragment lenth polymorphism(PCR-RFLP) method and PCR combined with 4% agarose electrophoresis. RESULTS: The frequency of the T allele and TG genotype at exon 7 and the a allele and ab genotype in intron 4 were significantly higher in diabetic nephropathy positive (DN+) group than in diabetic nephropathy negative (DN-) group and control subjects(P < 0.05). Diabetic patients with coexistence of the T and a alleles had a higher incidence of diabetic nephropathy(P < 0.05) than those with only one of the two alleles or without any of the two alleles. Multivariate logistic regression analysis showed that blood glucose, GHbA1c, SBP, TC, 894 G-->T mutation at exon 7 and a 27 base pair(bp) repeat polymorphism in intron 4 of the eNOS gene are independent risk factors for diabetic nephropathy. CONCLUSIONS: The T allele at exon 7 and a allele in intron 4 are related to diabetic nephropathy in Chinese patients with type with 2 diabetes mellitus. The incidence of diabetic nephropathy is higher in patients who have both T and a alleles than patients who have either T or a allele alone.

[Clinical characteristics of primary biliary cirrhosis with Sjögren's syndrome of 20 cases].

OBJECTIVE: Primary biliary cirrhosis (PBC) is a chronic non-suppurative destructive cholangitis of unknown etiology. It has been reported that patients with PBC suffer from xerostomia and keratoconjunctivitis at high prevalence and PBC has been shown to be associated with Sjögren's syndrome. This study investigated the relationship between PBC and Sjögren's syndrome. METHOD: 20 cases diagnosed as PBC consented to the evaluation for Sjögren's syndrome which included clinical and serological features, Schirmer's test, measurement of parotid flow rate and labial minor salivary gland biopsy. RESULTS: The main manifestations of PBC are jaundice, pruritus, hepatosplenomegaly. M2 subtype of antimitochondrial antibody is of great specificity for PBC. Many PBC patients are suffering from xerostomia and keratoconjunctivitis sicca but do not have typical serological and pathological presentations of primary Sjögren's syndrome. Corticosteroids and immunosuppressive agents have disappointing effects in the treatment. Ursodeoxycholic acid is effective to certain extent. CONCLUSION: It was indicated that Sjögren's syndrome associated with PBC appears to be a secondary form differing from primary Sjögren's syndrome.

[Study of pharyngo-cutaneous fistula after total laryngectomy].

OBJECTIVE: To explore the factors which lead to pharyngo-cutaneous fistula after total laryngectomy. METHOD: A retrospective study of 125 patients who underwent total laryngectomy during a 16-year period was carried out. RESULT: The rate of post-laryngectomy pharyngo-cutaneous fistula is 16.0% (20/125), it appears reduced tendency. With single-factor analysis, the factors effect on the rate of post-laryngectomy pharyngo-cutaneous fistula include clinical tumor stage, tumor types, pre-operative radiotherapy dose, intraoperative blood transfusion, operative duration. With all variable logistic model analysis, the factors effect on the rate of pharyngo-cutaneous fistula are clinical tumor stage and pre-operative radiotherapy dose, but pre-operative radiotherapy is very possible to be a risk factor (P = 0.0566). With logistic model stepwise regression analysis, the factors effect on the rate of pharyngo-cutaneous fistula are clinical tumor stage and operative duration. CONCLUSION: Pharyngo-cutaneous fistula appears easily in patients with T3, T4 tumor, and have pre-operative radiotherapy, especially have large dose preradiotherapy. For these high risk patients, the duration of operation should be as short as possible to reduce the rate of post-laryngectomy pharyngo-cutaneous fistula.

[Effects of the expression of mouse metallothionein-I gene in human HeLa cell line on drug resistance].

Metallothionein-I (MT-I) gene was inserted into EcoRI site by using pSV2-neo plasmid vector. Recombiant plasmid was transfected into HeLa cells by DNA-calcium phosphate precipitation technique. MT-I expression colones were growing in medium including G418. The amount of MT-I expression in transfected cells was found 2.6 times higher than that of non-transfected ones. In order to observe the relationship between the expression of MT-I gene in cells and drug resistance, cells were treated with different concentrations of cisplatin and adriamycin respectively. The results indicated that cisplatin (0.1 mumol/ml) inhibited the growth of both transfected and non-transfected cells. The inhibitory rates were 34% and 82% respectively(P < 0.05). IC50(50% inhibitory concentration for cell growing) was 0.144 mumol/ml and 0.061 mumol/ml and the ratio of them was 2.36: 1 after the treatment of cisplatin 72 h later. The cells were treated with adriamycin 72 h later, the inhibitory rates of transfected and non-transfected cells were 18% and 25% separately. The rates showed no significant difference (P > 0.05). The results indicated that MT was related to drug resistance of tumor cells.

[Histopathological study of local invasiveness of transglottic cancer].

OBJECTIVE: To explore the characteristics and relevant factors of transglottic cancer invading paraglottic space (PGS) and pre-epiglottic space (PES). METHODS: Whole-organ serial sections of 50 transglottic cancer specimens were studied histopathologically. RESULTS: Transglottic cancer invading PGS was observed in 39 of the 50 (78.0%) specimens. In 32 of the 39 specimens, the invasion outstripped PGS, involving elastic cone in 26(81.3%), thyroid cartilage in 24(75.0%) and cricothyroid membrane in 20(62.5%). Transglottic cancer invading PES was seen in 23 of the 50 specimens (46.0%). With the increase in tumor staging and tumor size, PGS and PES became increasingly invaded. There were two possible routes of invasion to PES: a) extending anteriorly and upward via PGS; b) extending upward via anterior commissure to destroy the stem of epiglottic cartilage. CONCLUSION: PGS is easily invaded by transglottic cancer. Partial laryngectomy is recommended in patients with T3, T4 transglottic cancer larger than 20 mm in diameter.

[Determination of ferulic acid and peoniflorin in siwu decoction prepared by different methods of yellow rice wine].

OBJECTIVE: To compare the influence of different methods with YRW (yellow rice wine) on the composition of Siwu decoction. METHOD: TCL-SM was used to determine the ferulic acid, paeoniflorin and matters dissolved in ether in Siwu decoction. RESULT: The Siwu decoction with YRW preparation has the highest contents, while the Siwu decoction containing Chinese Angelica root and Rehmammia root prepared with YRW has the lowest contents. CONCLUSION: The Siwu decoction prepared with YRW or added with YRW is more helpful for ferulic acid and paeoniflorin to be dissolved out than the Siwu decoction containing Chinese Angelica root and Rehmannia root prepared with YRW.

Clinical investigation of traditional Chinese medicine and pharmacy in treating respiratory tract infections in AIDS.

Based on the difference in severity of the disease and complications, 80 cases of acquired immune-deficiency syndrome (AIDS) accompanied with cough were divided into 2 groups: One treated by traditional Chinese medicine (TCM group), including 58 cases and the other treated by Integrated Chinese and Western medicine (integrated group), 22 cases. The therapeutic results show that in the TCM group there were 18 cases (31.03%) with clinical cure of the respiratory tract infection, 7 cases with therapeutic effect (12.07%) and 33 cases without any effect (56.90%), for a total rate of 43.10% therapeutic effectiveness; while in the integrated group there were 6 cases with clinical cure (27.27%), 2 cases with therapeutic effect (9.09%) and 14 cases without any effect (63.64%), the total rate of therapeutic effectiveness being 36.36%, slightly lower than in the TCM group and possibly related to greater severity of the disease or complications.

Prospect for study on treatment of AIDS with traditional Chinese medicine.

In this paper, studies on single Chinese medicinal herbs, composite prescriptions, and their active principles, which have been used against HIV both in China and abroad, are highlighted; and the results from clinical treatment of 158 HIV infected patients performed by a China-Tanzania Coordinating Group are also reported. Six of the patients displayed seronegative conversion and remained so in the follow-up for 10 to 15 months. The author assumes that the prescriptions and drugs strengthening the patients' resistance could enhance the efficacy of AIDS treatment.

Neurohypophysial AVP concentration in stroke patients.

Neurohypophysial arginine vasopressin (AVP) concentrations were determined by RIA in 9 patients with cerebral infarction, 11 patients with intracerebral hemorrhage, 5 patients with subarachnoid hemorrhage and 5 control subjects. The results showed that neurohypophysial AVP concentration in patients with cerebral infarction increased by 225.8% as compared with the control subjects (P < 0.05), and the AVP concentrations in patients with intracerebral and subarachnoid hemorrhage did not change significantly. Brain herniation as a result of intracranial hypertension, gastrointestinal bleeding and hyperglycemia (two clinical manifestations of stress) was seen more frequently in hemorrhagic stroke patients than in ischemic stroke patients. These findings suggest that the ischemic brain damage may contribute to the elevation of neurohypophysial AVP concentration in patients with cerebral infarction.
