HCFA agenda. The new initiative under Medicare.

The Reagan administration's Medicare policy is based on maintaining a high quality of care, reducing government's direct involvement in medicine, increasing choices for both beneficiaries and health care providers, and promoting competition and programmatic efficiency. The private health plan option (PHPO), an approach based on marked economics, offers beneficiaries the choice of retaining traditional coverage or participating through private health plans. At the same time, physicians may join private plans of their own choosing, plans that can have a large degree of control over quality assessment, utilization, and payment. The PHPO contains guarantees of quality, notably government regulation of the plans that accept Medicare beneficiaries and outside quality review. The administration is seeking to expand the number and types of private plans and to give the PHPO a fair market test.

Norwalk virus outbreak at a college campus.

An epidemic of nonbacterial gastroenteritis affected nearly 100 students at a college campus in Jefferson County, Alabama. The outbreak closely resembled food poisoning, since there was a rapid occurrence of multiple cases within a short period. Vomiting occurred in 79% and diarrhea in 64%; fever was uncommon. We found a significant association between the illness and the eating of lettuce at a meal one day before the outbreak began. Paired serologic specimens showed evidence of Norwalk virus infection. Twenty acutely ill students had leukocytosis (mean WBC 12,780/cu mm) and lymphopenia--a pattern that may be characteristic of Norwalk virus gastroenteritis. Outbreaks of Norwalk virus infection as well as other nonbacterial gastroenteritis may closely mimic epidemics caused by more familiar foodborne pathogens such as staphylococci.

Legislative update ... quality of care under the Medicare program.

The following is the text of a statement made by William L. Roper, M.D., Administrator of the Health Care Financing Administration, before the Committee on Finance, U.S. Senate, on June 3, 1986. He discusses concerns about quality of care under the Medicare program and his commitment to ensuring that every Medicare patient receives high quality, medically necessary care. He delineates the mechanisms and activities that the Health Care Financing Administration uses to assure quality, new methods that are being explored, and the research program emphasis on the impact of the prospective payment system on quality as well as possible refinements to the system.

Emetic food poisoning caused by Bacillus cereus.

Symptoms of acute food poisoning developed in eight members of a group who ate lunch at a cafeteria. After brief incubation periods, all affected individuals complained of nausea and abdominal cramps. Four persons promptly experienced vomiting. None of those affected was found to have fever and all recovered with 48 hours. Epidemiologic investigation incriminated macaroni and cheese as a cause of the illness and samples of this food contained large numbers of Bacillus cereus. Previous outbreaks of B cereus emetic food poisoning have been associated with consumption of contaminated fried rice and may occur after ingestion of other foods.
