Prep-A-Gene: a superior matrix for the purification of DNA and DNA fragments.

A new chromatographic matrix, Prep-A-Gene, is described for the isolation and purification of high quality DNA suitable for restriction analysis, ligation, transformation and sequencing protocols. This matrix selectively binds DNA greater than approximately 200 base pairs in length, while RNA, proteins, cellular components, agarose and other contaminants are washed free in minutes. This eliminates the need for time-consuming and laborious RNase treatments, gel extractions and phenol extractions. The DNA that is desorbed from the matrix is available immediately as a substrate for subsequent protocols. DNA purified in this manner exhibits no detectable shearing, even with more fragile chromosomal DNA.

Identification of T-cell receptor V beta deletion mutant mouse strain AU/ssJ (H-2q) which is resistant to collagen-induced arthritis.

Our laboratory is involved in investigating the role of T-cell receptor (Tcr) in collagen-induced arthritis (CIA). During these studies we found AU/ssJ (H-2q) mice to be resistant to CIA like SWR (H-2q), as compared with other H-2q strains with wild-type Tcr like DBA/1 and B10.Q. Upon screening with monoclonal antibodies F23.1 and KJ23a, AU/ssJ was found to be F23.1 negative (V beta 8 Tcr negative) and KJ23a positive (V beta 17a Tcr positive). Southern blot analysis on liver DNA using specific Tcr-V beta probes confirmed the deletion of V beta 8 gene family and also showed that AU/ssJ mice have deletions of V beta 9, V beta 13, V beta 12, and V beta 11 genes of Tcr. Further, these mice show a restriction fragment length polymorphism pattern with V beta 10, V beta 6, and V beta 17 probes similar to SWR mice as compared with B10 mice. Since SWR and AU/ssJ are from different backgrounds, these studies indicate that specific variable region beta chain genes of Tcr are crucial for susceptibility to CIA in mice. Furthermore, these studies identify an additional inbred strain which has also deleted 50% of its Tcr-V beta genes.

Central retinal artery occlusion with an irido-embolus following carotid endarterectomy.

Endarterectomy is often the treatment of choice in carotid artery disease displaying significant stenosis of the internal carotid artery. This procedure is not without its complications, the most visually devastating being central retinal artery occlusion and secondary neovascularization. Such a case is presented here along with the interesting finding of a cholesterol irido-embolus, apparently the first citation in the ophthalmic literature to date.

Posttraumatic glaucoma.

Following ocular trauma, it is not uncommon for many patients to develop some form of secondary glaucoma. Studies have indicated a 2-10% incidence of late onset glaucoma following blunt trauma. The trauma can be either penetrating or nonpenetrating in nature. Penetrating injuries can lead to severe intraocular damage and inflammation which may cause anterior synechiae, a pupillary block or a trabeculitis. These processes can ultimately produce a glaucomatous condition. Severe blunt ocular trauma (an ocular contusion) can also lead to a glaucomatous state by producing an angle recession, hyphema, inflammation, and lens dislocation and ruptures. All of these different types of injuries and subsequent glaucomas will be discussed along with their treatment plans.

Long-standing venous stasis retinopathy with resultant cystoid macular edema.

A 62-year-old Hispanic male presented with decreased left eye vision secondary to long-standing, chronic cystoid macular edema as a result of venous stasis retinopathy. Treatment of venous stasis retinopathy is generally directed at reducing or eliminating the macular edema to avoid permanent visual acuity loss. There is still debate as to which forms of treatment are effective as well as which type of treatment is best. Argon laser photocoagulation, and to a certain extent steroid therapy, however, appear to give the best results. In the presented case, treatment was not initiated because of a poor prognosis for improvement due to a poor level of visual acuity and the chronicity of the cystoid macular edema.

Proliferative vitreoretinopathy.

Proliferative vitreoretinopathy is the recently adopted term for a complicating condition which occasionally follows retinal detachment. The major processes involved are cellular proliferation and membrane formation. These developments greatly reduce the surgical success rate. A case of proliferative vitreoretinopathy is presented here; we briefly review the mechanism of development of this condition, as well as current treatment concepts.

Cancer patients' knowledge, beliefs, and behavior regarding pain control regimens: implications for education programs.

The inadequate control of cancer-related pain is a matter of great concern. Misconceptions about pain medications on the part of professionals and the public have been implicated as causes of this problem. Another cause may be noncompliance with prescribed regimens. A study of 82 cancer patients who were prescribed pain medications was conducted to further investigate the problem of pain control in this population. Patients were interviewed twice: on the day after they received pain prescriptions (T1) and again two weeks later (T2). There was a significant decrease in patients' reported pain between T1 and T2 (P = 0.0001). By their own report, the level of patients' compliance with prescribed pain control regimens was very high. At T2 77% of the patients recalled the correct names of their pain medications and 88% had been taking them. However, an important gap in patients' knowledge was revealed by their general inability to recall any of the common side effects of pain medications (constipation, nausea, and sedation). None of the measures of pain--pain level at T1 and T2 and change in pain between T1 and T2--was related to the patients' education, age, attending physician, prescribed medication schedule, or medication strength. More women than men reported increased pain during the study period (P = 0.04). In general, the patients' concern about possible addiction or tolerance to prescribed medications were low. However, there was a positive relationship between intensity of pain and concern about tolerance to drugs (P = 0.0003).(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of orally-administered ethanol on mammary gland morphology and functional efficiency in lactating rats.

The effect of ethanol on mammary gland morphology and functional efficiency in lactating rats was examined. When ethanol was orally administered from parturition throughout lactation, no effects were observed. When alcohol was administered from day 11 of gestation and throughout lactation, lower mammary gland weight and length, and pup weights were observed. Definite histological changes in the mammary gland were noted, especially at days 11 and 22 of lactation. It is speculated that ethanol disturbs the balance of the endocrine system relative to mammary gland development.

Ultrastructural observations on (pseudo-) exfoliation of the lens capsule: a re-examination of the involvement of the lens epithelium.

One lens each from 2 separate patients suffering from pseudoexfoliation and one lens from a normal eye were examined ultrastructurally. The normal lens capsule was internally lamellar in places and externally of a somewhat reduced electron density but otherwise of a homogeneous appearance and free of inclusions. Observations on the abnormal lenses revealed an electron dense and mainly granular but sometimes fibrillar material along the surface of the peripheral one-third of the capsule. Material of similar ultrastructural appearance was also found deeper in the capsule and in the immediately underlying epithelium. Since the capsule close to the pole of the lens was completely free of these abnormal inclusions and the underlying epithelium also lacked this material, it is concluded that the lens epithelium is a source of the surface debris in pseudoexfoliation. These findings therefore support previous authors who proposed a lenticular involvement in this disease. The present study does, however, not rule out the possibility of a uveal contribution of abnormal material but disputes its exclusive involvement in this disease, as has previously been postulated. Build-up of exfoliated material close to the capsular surface caused an apparent peeling of the lens capsule. It is therefore concluded that the anomalous lenses suffered from a true exfoliative process that may be more correctly termed 'exfoliation of the lens capsule.'

Vision symptoms with asteroid bodies in the vitreous.

Asteroid bodies in the vitreous present the doctor of optometry with a spectacular ophthalmoscopic view of numerous, minute yellow-white spheres in an otherwise normal vitreous. They have little or no association with systemic disease, and they are not often thought of as a cause for vision symptoms. We report four cases of vision symptoms that we believe were caused by asteroid bodies in the vitreous. The symptoms varied from minor impairments in visual acuity to floaters.

Arteritic and idiopathic ischemic optic neuropathy.

Ischemic optic neuropathy is a frequent cause of sudden visual loss in the older patient. There are two major causes of ischemic optic neuropathy; one, arteritic, due to an inflammatory process, and the other, idiopathic, possibly linked to hypertension. This paper presents four illustrative cases of ischemic optic neuropathy, one with an associated serous retinal detachment. The clinical appearance of the disorder, differential diagnosis of the two major types of ischemic optic neuropathy, and the pathogenesis and prognosis for vision are discussed. Currently accepted modes of treatment are also included.

Research in cancer patient education and compliance.

A study design has been presented which has the advantage of combining the research questions of education program evaluation and patient compliance. While it would be possible to evaluate education programs in other ways (e.g., using a sample of all program participants rather than a sample of all patients for whom a regimen was recommended), such an alternative design would not assess the potentially significant proportion of noncompliance which might occur when the regimen recommendation is made. It is the specification of inception cohorts as the study population that allows the accrual of more complete compliance data for the regimen as well as evaluation data for the education program. Given the recognized need for data on the compliance of cancer patients, the added cost of specifying inception cohorts for education program evaluations seems small. The proposed research design has the potential to contribute to cancer control programs: 1. a careful evaluation of new education programs for patients; 2. modifications of education programs based on feedback from professionals and patients during the initial implementation; 3. needed data on cancer patient compliance with extended detection and treatment regimens; and 4. increased understanding of the process of patient compliance, including identification of key variables in the health belief model which account for compliance among cancer patients.
