[Open diaphyseal tibial fractures in children].

In a collective study of 22 surgical clinics 138 open diaphyseal tibial fractures in children from 0 to 14 years were examined. The incidence of open fractures, in relation to all tibial shaft fractures comes up to 6.9%, one third (2.3%) are second degree and third degree open fractures. 80% of these fractures are caused by traffic accidents. In 33% there exist multiple fragments. In first degree open fractures conservative treatment generally is practiced, while in second and third degree open fractures osteosynthesis often is performed.

[Tibial fractures in children (author's transl)].

In a collective study of 22 surgical clinics 1235 closed diaphyseal tibial fractures in children from 0 to 14 years were analysed. The incidence of this type of fracture in relation to all fractures in childhood, was 6.2%. In pre-school children isolated tibial fractures and incomplete fractures predominated, while in children from 6 to 14 years tibial plus fibular fractures as well as displaced fractures occurred more frequently. Closed reduction and plaster cast were used as standard treatment. Traction was used only exceptionally. In only 3.0% osteosynthesis was performed under the following indications: irreducible fracture, multiple fractures, concomitant chest or abdominal injury, polytrauma.

Antibacterial activity of N-(beta-styryl) formamides related to tuberin.

A series of para-substituted N-(beta-styryl)formamides, analogues of tuberin (4a), has been prepared and assayed for antibacterial activity. The methylthio, ethoxy, and methyl analogues 4e, 4j, and 4t were about twice as active as tuberin against Mycobacterium phlei. Although tuberin lacks activity against Staphylococcus aureus, several of the analogues described were found to inhibit this organism. The phenyl group of tuberin is not a prerequisite for activity since analogues based on naphthyl or ferrocenyl groups were also active. A quantitative structure-activity relationship further implied that an aromatic group need not be present, suggesting the synthesis of the cyclohexyl and n-amyl analogues which were found to possess high activity.

Effect of manual lymph drainage massage on urinary excretion of neurohormones and minerals in chronic lymphedema.

Treatment of 29 cases of chronic lymphedema of various origins, mostly of the lower limbs, by manual lymph drainage massage resulted in significant changes of neurohormone excretion in the urine, whereas the secretion of 17-KS, thyroxine, minerals, and creatinine was not significantly changed. Comparison of the values of urinalysis before and after manual lymph drainage of the patients showed the following changes: 17-KS; -3.5% (non significant); 17-OH: -31% (significant); adrenaline: +50% (significant); noradrenaline: +19% (significant); serotonin: -22% (significant); 5-HIAA: +21% (significant); histamine: +129% (highly significant); thyroxine: -17% (nonsignificant); creatinine: -17% (nonsignificant); sodium: -1% (nonsignificant); potassium: -14% (nonsignificant). The corresponding values for ten controls were all non significant. These findings underline the importance of adrenaline and noradrenaline release by manual lymph drainage, which improves circulation. On the other hand, our results indicate the involvement of histamine and perhaps of serotonin in lymphedema formation, and suggest a combination of manual lymph drainage massage with antihistamine and antiserotonin treatment.
