Comparative study of various intracervically administered PG gel preparations for termination of first trimester pregnancies.

In a randomized, double-blind study, 30 healthy, nulliparous women of similar gestational age were given intracervical applications of 0.5mg PGE2, 0.05mg Sulprostone or 0.1mg Sulprostone gel in order to soften the cervix prior to curettage for first trimester termination of pregnancy. The preparations were administered 8 hours before curettage. The number of complete and incomplete abortions, ease of passage through the cervical canal, as measured by a tonometer before and 8 hours after the administration of prostaglandin, the degree of pain experienced and the quantity of analgesics required, plus the frequency of systemic side effects were all always assessed by one trialist. With regard to the rate of abortion and cervical softening, the administration of 0.1mg Sulprostone gel proved the most effective method. However, in comparison with the others, it also caused the greatest degree of pain and necessitated the greatest use of analgesics. The softening effect of the prostaglandin E2 gel was significantly less and in this group there were two cases of cervical lesion due to tenaculum laceration. The intracervical application of 0.05mg Sulprostone gel is to be recommended for pre-operative ripening of the cervix before termination of pregnancy in the first trimester, as it effectively dilates the cervix and does not cause systemic side effects or pain in the lower abdomen, enough to make treatment necessary.

The use of innervated flaps for the closure of ischial pressure sores.

Since 1971 we have been covering pressure sores and unstable scars with transposition-rotation--muscle and musculocutaneous flaps, thereby padding the cavity resulting from excision of bone and scar tissue. The long term results compare favourably with classical random pattern skin flaps. From the beginning of 1980 we have been using the sensory innervated tensor fascia lata myodermal flap for the ischial region. This provides sensation for a formerly insensitive area in patients suffering from a spinal cord lesion below L3/4.

Comparison of spinal cord injuries in females and in males, 1979-1981 Basle.

The statistics regarding age, marital status, nationality, paraplegic: tetraplegic ratio and causes of accident of the 92 female and 238 male patients in our Centre in the years 1979-1981 show, in comparison to our own previous studies and those of other countries, the worldwide highest female-male rate of 1:2.59. This is usually due to the higher occurrence of traffic accidents for women. The fact that in the age group below 30 years more women than men suffer from spinal cord injuries implies adapting rehabilitation and management to the specific needs of women.

[The effect of metoprolol on fenoterol-induced kidney function disorders].

The effect on renal function of a combination of a beta 2 receptors stimulating tokolytic agent (Fenoterol) with a so-called cardioselective beta 1 blocker (Metoprolol) which is able to antagonise the cardiac side effects of beta 2 stimulation, was examined in femaLe rabbits in a 24-hour test. Infusing 31.5 ml/h isotonic solution, 20 micrograms/kg/min Fenoterol and 3.125 micrograms/kg/min Metoprolol, excretion, creatinine clearance, electrolyte elimination, the serum electrolyte levels, Hb, HK and the weight were determined. The combination of Fenoterol and Metoprolol antagonised almost completely the massive water retention occurring under Fenoterol alone. At the latest after 12 hours, normal values were obtained in respect of the creatinine clearance reduced under Fenoterol, the urine excretion rate and, consequently, the Hb concentration and haematocrit reading. The reduction of sodium elimination caused by Fenoterol was completely reversed under the combination, whereas the reduced excretion of potassium was not influenced. Water retention in tocolysis with beta 2 stimulants is one of the most important causes for the information of a pulmonary oedema. The danger of the occurrence of such pulmonary oedemas, especially in risk pregnancies (gestosis), is drastically reduced by antagonising water retention, and hence the combination of Fenoterol and Metoprolol seems to be very favourable from both the cardiac and renal viewpoints. In a group of 17 pregnant women, the drastic improvement of urine excretion under the combination could be confirmed.

Mechanism studies of enzymatically formed tolidine blue and determination of peroxidatic activities.

In aqueous solution peroxidase catalyzes the conversion of o-tolidine to tolidine blue by hydrogen peroxide. This reaction of practical significance for analytical tests was studied by optical and ESR spectroscopy. For the blue dye formed in aqueous solution a meriquinoidic structure was proposed which is in equilibrium with an instable radical compound. This equilibrium is shifted to higher radical concentrations by ethylene glycol. Naphthene derivatives stabilize the meriquinoidic structure by means of non-covalent interactions resulting in a decrease of the radical concentration. In crosslinked gelatin the dye formation runs analogously, so that this system is suitable for providing evidence for H2O2-forming reactions. Naphthene derivatives substituted by sufficiently long aliphatic groups are diffusion stable in crosslinked gelatin. By interacting with the dye they prevent its rapid chemical decomposition and diffusion into the sample solution. It was shown, furthermore, that by means of such systems with a suitable structure of the layers both substrates and enzymes participating in the reaction may be determined analytically. This does provide a basis for developing new analytical test variants.

[Plasma hormone concentrations in induced abortion with local prostaglandin administration in the 1st trimester].

Abortion was performed by curettage on 71 women with pregnancies between the 7th and the 13th week of gestation seven to eight hours after intracervical application of a tylose gel containing 3mg prostaglandin F2 alpha. Prior to the application of the prostaglandin and immediately before the surgical intervention a sonographic examination for determining the vitality of the pregnancy was carried out.--Plasma progesteron, estradiol and HPL levels were determined radioimmunologically prior to the application of prostaglandin, at four-hour intervals on the day of intervention, and 24, 48 and 72 hours after the intervention. In 22 women a complete or an incomplete abortion occurred; in two cases a blighted ovum was observed; 47 pregnancies, according to sonographic examination, remained intact until curettage. After seven to eight hours duration of the effect of the prostaglandin gel, progesterone levels were found to be reduced to 60.5 per cent and 17-beta-estradiol to 31.4 per cent of the initial values, whereas the HPL values fell below the specificity of the testing procedure (12.5 ng/ml). Comparative investigations of the pregnancies which, according to sonographic findings, remained intact until curettage and those which were aborted after the application of prostaglandin did not, in spite of low plasma progesterone and estradiol levels in the abortive group, reveal any statistically significant differences. The abortive effect--even with local application--of the prostaglandins was confirmed. Conclusions regarding the effective mechanism of the prostaglandins upon the fetoplacental unit and the function of the corpus luteum remain subject to speculation.

Effects of the tocolytic agent fenoterol on body weight, urine excretion, blood hematocrit, hemoglobin, serum protein, and electrolyte levels in nonpregnant rabbits.

Infusion of an isotonic solution and increasing doses of the beta 2-sympathomimetic agonist fenoterol (0.6, 20, or 80 micrograms/kg/min) into female rabbits led to a reduction in urine excretion and a concomitant dose-dependent increase in body weight. Although an infusion at 2.5 ml/hr of an isotonic solution that contained 0.6 or 20 micrograms/kg/min of fenoterol did not significantly change the hematocrit, hemoglobin, or serum protein levels, they were significantly decreased by a similar infusion that contained 80 micrograms/kg/min of fenoterol and by giving 20 ml/hr of an isotonic solution that contained 0.6, 20 or 80 micrograms/kg/min of fenoterol. The central venous pressure remained unchanged in animals that received 2.5 ml/hr of an isotonic solution alone or in combination with 0.6 to 80 micrograms/kg/min of fenoterol. It was significantly elevated in animals treated with 30 ml/hr at similar doses. These data support our hypothesis that the development of pulmonary edema which has been reported with the use of tocolytic therapy is mainly due to hypervolemia related to the antidiuretic effect of the beta 2-sympathomimetics. The intravascular hypervolemia leads to an increased fluid content of the lung and may explain the development of pulmonary edema.

Induced hydrolytic activity of yeast phenylalanyl-tRNA synthetase by tRNAPhe-CC.

"Induced hydrolysis" a new hydrolytic activity, was found by measuring AMP-production during aminoacylation of tRNAPhe-CCA by yeast phenylalanyl-tRNA synthetase in the presence of tRNAPhe-CC under conditions of low ionic strength at pH 8.5. Experiments using the elongation factor Tu . GTP provide evidence that transfer of phenylalanine to the tRNAPhe-CCA is followed by rapid hydrolysis in the presence of tRNAPhe-CC. A simple mechanism shows good agreement with the experimental data.

Objective demonstration of cervical softening with a prostaglandin F2 alpha gel during first trimester abortion.

In a prospective study the cervix softening and dilating effects of intracervically applied prostaglandin F2 alpha gel were investigated in 100 patients during induced abortion in the 7-12th week of pregnancy. A specially designed tonometer was used to measure the resistance of the cervical canal before as well as 6-8 h, 4 days and 5-6 weeks after gel application. The cervix of 99 patients proved to be freely passable without use of undue force for at least Hegar 8 after treatment. Measurements in 96 patients 5-6 weeks after the operation showed good general agreement with those values obtained before gel application. No spontaneous or operation-induced lesions of the cervix occurred. Intracervical prostaglandin gel application proved itself to be a safe, practicable and gentle method for avoiding dilation-induced complications during first trimester abortions.

Social and vocational reintegration of paraplegic and tetraplegic patients in Switzerland.

The purpose of this statistical review (based on 90 acute SCI admissions into the Swiss Paraplegic Centre, Basle in 1979), is to give, in the form of an evaluation, consideration to the social and vocational significance of the patients' disability. Seventy patients (77.8 per cent), are working or will work, (of 35 tetraplegics, 25, of 55 paraplegics, 45); and 67.8 per cent drive a car. So far as marital status is concerned there have been no significant changes as yet. The important question of resettlement was ideally solved in 80 per cent of patients. The improvement of education following injury was in general relatively limited. Twenty-five per cent reported that they have improved their education subsequent to their disability, these coming from the group having initially more than basic education. Socio-economic conditions of our patients are satisfactory. Cases of hardship have been substantially supported by the Swiss Paraplegic Foundation.

Fenoterol-induced changes of urine excretion, body weight, blood hematocrit, hemoglobin, total protein values and serum electrolyte levels in pregnant rabbits.

Infusion of the beta 2-sympathomimetic agonist fenoterol into pregnant rabbits at a dose of 20 micrograms/kg/min + 2.5 or 30 ml/h isotonic solution significantly reduced urine excretion. Body weight changes of animals infused with 20 micrograms/kg/min fenoterol + 2.5 or 30 ml/h isotonic solution were inversely correlated to the reduction of urine excretion. The urinary potassium excretion per 24 h was not significantly altered after the infusion of fenoterol + 2.5 or 30 ml/h isotonic solution. On the other hand, the urinary sodium excretion per 24 h was significantly decreased in animals infused with 20 micrograms/kg/min fenoterol + 30 ml/h isotonic solution. Infusion of 20 micrograms/kg/min fenoterol + 2.5 ml/h isotonic solution did not significantly change hematocrit, hemoglobin or total protein values. In contrast, increasing the volume of isotonic solution to 30 ml/h resulted in significantly decreased hematocrit, hemoglobin and total protein levels at 6, 12 and 24 h after starting infusion. Serum potassium concentrations were reduced at 2-24 h after infusion of 20 micrograms/kg/min fenoterol + 2.5 or 30 ml/h isotonic solution was started. The serum sodium concentrations did not change significantly during the infusion of 20 micrograms/kg/min fenoterol + 2.5 or 30 ml/h isotonic solution. While in animals infused with isotonic solution alone or with 20 micrograms/kg/min fenoterol + 2.5 ml/h isotonic solution the central venous pressure did not change significantly, it was strongly increased in animals infused with 20 micrograms/kg/min fenoterol + 30 ml/h isotonic solution at 12-24 h after start of infusion. The results of this study are discussed with respect to those obtained from pregnant women under tocolytic therapy and to the possible pathomechanism of the development of pulmonary edema.

[3H]Diazepam displacing activity in human cerebrospinal fluid.

Pooled human cerebrospinal fluid was separated by Sephadex G-50 chromatography. The presence of three peaks, A, B and C, was demonstrated by monitoring absorbance at 254 and 280 nm. All peaks showed [3H]diazepam displacing activity in the membrane receptor test. Peak B was further separated on Bio-Gel P-4. At least two major fractions free of salt and GABA in the molecular weight range of approximately 700--3600 were shown to displace [3H]diazepam in the receptor test. This activity was enhanced by a factor of 3 in the presence of 10 microM-GABA.
