Vaginosonographic detection of endometrial cancer in postmenopausal women.

In a prospective study 155 postmenopausal women not on hormone treatment underwent a vaginal ultrasound investigation. All women were routine patients of the regular ambulance. In addition to uterus biometry, the endometrial thickness was also measured. An endometrium of greater than or equal to 4 mm thickness was clarified histologically by means of a curettage. In total we found 7 endometrial cancers and 1 cervical cancer. Vaginal sonography proved to be a very sensitive (81%) and specific (89%) method for the detection of endometrial neoplasia.

Evaluation of atherosclerotic lesions using NMR microimaging.

Magnetic resonance imaging (MRI) has been proposed as a potential tool in the evaluation of atherosclerotic lesions. However, two basic difficulties have to date prevented the full exploitation of the potentials of this technique: the poor spatial resolution of the conventional tomographs and the wide variety of the lesions as well as their intrinsic dishomogeneity. In this study the in vitro morphology of normal and atherosclerotic vascular tissue specimens has been evaluated using a high resolution spectometer equipped with a microimaging device. Morphological features of the vessel walls as small as 10(-1) mm have been detected and the distribution of lipids and of calcified or necrotic regions has been evidenced in atherosclerotic plaques. Different techniques, such as local spectroscopy performed on volumes of 1 mm3 and localized magnetization recovery measurements, have been employed to characterize specific regions of the vessel walls from the chemical and the physical point of view. The good agreement of NMR findings with histological data allows us to conclude that NMR microimaging represents a suitable technique for the in vitro detection and characterization of atheromatous lesions.

[Clinical management of cystic ovarian tumors].

In a prospective study at the University of Göttingen 206 cystic ovarian tumours were examined vaginosonographically. Basing on a graduated schedule, the individual therapy of each ovarian tumour was determined. By use of this clinical management, all blastomas could be detected correctly and in 6.8% of all cases a follicle or corpus luteum cyst was operated per laparotomiam. Basing on the created data, the authors suggest a simple and effective clinical management of cystic ovarian tumours.

[Laparoscopic treatment of tubal pregnancy with prostaglandins].

Fifteen patients with laparoscopically diagnosed tubal pregnancy and constant or rising plasma beta-hCG levels were treated with prostaglandin F2 alpha and prostaglandin E2. Prostaglandin F2 alpha (5 mgms diluted in 10 cc of isotonic sodium solution) was injected transabdominally with a 22 gauge spinal needle during laparoscopy into the Fallopian tube. Prostaglandin E2 (500 micrograms ms) was given intramuscularly during three consecutive postoperative days. The treatment was defined as successful if plasma beta-hCG levels declined below the lower limit of detection and no further intervention other than prostaglandin application was required. The treatment was successful in eight patients. Six patients underwent laparotomy and salpingotomy because of rising beta-hCG levels. None of the treated patients displayed any adverse reactions following prostaglandin F2 alpha application. One patient underwent explorative laparotomy during the second postoperative day because of lower abdominal pain. During operation, no pathological change could be found. This patient was excluded from the study. In the group treated successfully (n = 8) seven out of eight patients had beta-hCG levels below 2500 mlU/ml preoperatively. In the unsuccessfully treated group (n = 6), four out of six patients had beta-hCG levels above 2500 mlU/ml preoperatively. Mean duration of beta-hCG decline to 10 percent of the maximum preoperative value was 15.8 +/- 8.64 days (mean +/- S.D.). Postoperatively, hysterosalpingography was performed in six out of eight successfully treated patients after three menstrual cycles (one patient had an intrauterine pregnancy, one patient refused written consent). The Fallopian tubes were patent bilaterally in all six patients.

[CA-125 serum level in early pregnancy follow hMG/hCG stimulated and unstimulated cycles].

The CA-125-antigen could not only be identified in the epithelium of serous ovarian carcinoma, but also in the regular surface epithelium of the endometrium, the decidua and the peritoneum. The aim of this study was, to examine the pattern of CA-125 in the serum of women at early stages of pregnancy, after hMG/hCG-stimulation and subsequent insemination, IVF or GIFT. In particular, it was differentiated between simple or twin pregnancies, ovarian hyperstimulation syndrome and mole pregnancies. 16 patients, who spontaneously became pregnant after cyclus-monitoring, served as a control group. Among the total of 77 patients, 16 women with single pregnancies without stimulation showed an increase of CA-125 values to maximum amounts of 42.9 +/- 18.8 U/ml in the 8th week. 21 patients with single pregnancies after hMG/hCG-treatment and IVF, exhibited maximums of 97.5 +/- 66.8 U/ml in the 6th week, in cases of twins (n = 10) of 216.8 +/- 85.1 U/ml in the 7th week. Considering the group with the hMG/hCG-stimulation and following insemination (n = 20), maximum values of 91.4 +/- 57.3 U/ml were found in the 6th week, just as in the IVF-group. In cases of ovarian hyperstimulation syndrome (n = 5) as well as in such of a mole pregnancy (n = 5), drastically elevated values of 9519 and 1496 U/ml respectively, have been demonstrated. The decidua is regarded as a presumable source of high antigen levels in normal pregnancy, while the peritoneum is assumed to be such as for the ovarian hyperstimulation syndrome. Concerning mole pregnancy, the antigen levels could possibly emerge from the amniotic fluid.(ABSTRACT TRUNCATED AT 250 WORDS)

[Vaginal ultrasound as a screening method for detection of adnexa tumors in postmenopause?].

424 ovaries of 212 postmenopausal women not on hormone treatment were examined by vaginal ultrasound. 155 patients had no clinical complaints, 57 patients showed atypical uterine bleeding. 48 pathological findings could be detected sonographically (11.3%). Among the histologically clarified cases were 2 asymptomatic ovarian cancers. Altogether, the vaginosonography proved to be a very simple and effective method to detect ovarian tumors in postmenopausal women.

[Local treatment of malignant pleural effusion in gynecologic tumors].

Pleural effusions are a frequent complication of malignant gynecologic diseases, especially in breast cancer with an incidence of about 50%. Pleural effusions usually occur at a stage of the disease where no further curative therapeutic options exist. Therefore, the treatment should be effective and have a low side effect profile. The intrapleural administration of mitoxantrone in comparison with other pleurodesis techniques has been demonstrated to meet these requirements.

A randomized double blind study of a new ready-for use prostaglandin gel (Org 2436) for cervical ripening prior to first trimester pregnancy termination.

In a placebo-controlled double-blind randomized study involving 50 primigravidae scheduled for termination of first trimester pregnancy 500 micrograms PGE2 gel or placebo gel were applied intracervically 6 hours before curettage. The priming effect was verified in all patients by means of comparative examinations with a special tonometer before application of the gel and immediately before the surgical procedure. No abortion or vaginal bleeding occurred in any of the patients. After 500 micrograms PGE2 gel the mean free passability (i.e. force less than or equal to 1 Newton) was 7.8 (+/- 2.0) mm, the corresponding value for the placebo group 5.2 (+/- 1.2) mm (p less than 0.0001). The prostaglandin pretreatment led to a significant increase in dilation of 5.9 +/- 2.3 mm versus 2.0 +/- 1.2 mm in the placebo group (p less than 0.0001). 17 of the 25 PGE2-treated patients complained of lower abdominal pain, gastrointestinal side effects were observed in one of these patients. In the placebo group there were no undesired concomitant symptoms. No intra- or postoperative complications occurred in the PGE2 gel-treated group. After application of the placebo gel cervical lesions and incomplete curettages were demonstrated in 4 cases. According to this preliminary experience, the intracervical application of the new ready-for-use-PGE2 gel represents a practicable and efficient method for preoperative cervical priming. In view of the intraoperative complications in the placebo group the frequency of side effects after PG application are within tolerable limits.

Urinary protein electrophoresis profile in normal and hypertensive pregnancies.

By using of modified urine preparation and a highly sensitive SDS-polyacrylamide gel electrophoresis (SDS-PAGE) we determined the urinary protein profile in 21 healthy males, 25 healthy females, 64 patients with uncomplicated pregnancy and 110 hypertensive pregnant women. The urinary protein patterns were similar in controls and in women with a normal pregnancy. There were no increase in the number of protein bands from the 1st trimester to term, and the electrophoresis pattern did not change in the postpartum period. In both groups, an intensively stained protein band with an apparent molecular weight of 105 kD was detected. The 105 kD band was significantly reduced or completely absent in 91 (83%) out of 110 hypertensive pregnant women. The urinary protein electrophoresis profile correlated significantly with the severity of the disease. The 105 kD band disappeared just before or simultaneously with the onset of clinical symptoms in 18 out of 32 hypertensive pregnant women followed throughout pregnancy. Postpartum the 105 kD in urine reappeared at 2 to 14 days after delivery in 49 of the 53 patients. Using a silver staining and Western blot, the 105 kD band was identified as Tamm-Horsfall protein, which is identical to the immunosuppressive glycoprotein uromodulin. The findings in the SDS-PAGE may reflect a transitory tubular dysfunction in cases of pre-eclampsia, which is usually reversible after delivery. The results of our study support the hypothesis of an immunological basis for this disorder.

[Immunotherapy of couples with habitual abortion].

After habitual abortions and the exclusion of nonimmunological causes of miscarriage, 26 patients were treated with lymphocytes of their partners of prophylaxis of abortion. The first treatment was done during early pregnancy in eight patients. Of the remaining 18, 12 have become pregnant up to now. Five pregnancies have gone to term, one is in the 13th week at the moment, and five are in the 20th week. Two patients lost their baby after the 20th week due to nonimmunological causes. Again seven patients had an early spontaneous abortion. Thus, the success rate of the immunological therapy is 63.2% at the moment.

[Vaginal sonography: a screening method for early detection of ovarian tumors and endometrial cancers?].

In total we performed a vaginosonographic measurement of 212 patients with or without a postmenopausal bleeding. Altogether 424 ovaries were sonographically examined. An endometrial thickness greater than 4 mm was histologically clarified by means of a curettage and all detectable ovarian tumors by means of a laparotomia. In total we found seven asymptomatic endometrial carcinomas, one cervical carcinoma and two ovarian cancers. The vaginosonography showed to be a very sensitive and acceptable method for the early detection of postmenopausal ovarian and endometrial tumors.

[Septic ovarian vein thrombosis--diagnosis and therapy].

Septic ovarian vein thrombosis is commonly the sequela of puerperal purulent endomyometritis. The incidence has been reported to be 1 in 600-6000 deliveries. The predominant location is the right ovarian vein according to the puerperal uterine drainage. The diagnostic and therapeutic experience in 9 cases is described. The leading symptoms were abdominal pain localized at the psoatic muscle and a tender, barrel-shaped tumor. Ultrasound and computed tomography (CT) added in the correct preoperative diagnosis. Antibiotic therapy and anticoagulation are recommended in an uneventful course; in complicated cases surgical intervention becomes mandatory.

[Documentation of surgical data with dBASE-III].

By the development of microelectronic it is nowadays possible to manage even large amounts of data by personal computers (PC). Quality control and a prescribed operationstatistics compels to use a computer. In most cases, however, the employment of a computer fails because of the lack of adequate soft-ware. As a basis of such a use the relational data bank system dBASE-III offers it's services. The introduced system OPSTAT serves to fill in and to control the operation manual and makes it possible to register and to analyse all operations, diagnoses, complications and special risk factors.

Vaginal sonographic visualisation of a cervical carcinoma.

In the present case, the vaginal sonographic picture of a cervical (collum) carcinoma, which was clinically non-apparent, but was histologically established through a fractionated curettage, led to a change in the surgical procedure in the sense that a primary operation according to Wertheim--Meigs instead of a knife conisation was performed. The postoperative histology showed a good correlation between sonographic tumor boundaries and the actual expansion of the carcinoma.
