[Parturient studies of the cardiotocogram score and transcutaneous oxygen pressure in hypotrophic and eutrophic fetuses].

Semiquantitative and quantitative principles of cardiotocogram evaluation are necessary for the evaluation of subacute and chronic oxygen deficiency in fetuses. By means of tcPO2 measurement it is possible to check these evaluation principles for their relation with fetal oxygen pressure. From 59 tcPO2 cardiotocograms, a total of 99 30-minute-intervals were analysed. 27% of the fetuses showed growth retardation (below the tenth weight percentile according to Kyank). Fetal oxygen pressure was statistically significantly (alpha less than 0.05) correlated with changes in the CTG score according to Hammacher. In the group with hypoxemic oxygen pressure (below 11 mmHg/1.47 kPa) the correlation was particularly high, and the CTG score of 5 points was significantly poorer than the 3 points in the nonhypoxemic control group. Our investigations essentially confirm the grading of the CTG score by Hammacher. Hypotrophic fetuses showed a significantly poorer CTG score, and their oxygen pressure of 12.9 mmHg (1.7 kPa) was significantly below that of the control group (15.4 mmHg/2.1 kPa).

[Comparison of transcutaneous oxygen pressure measurement with computer automatic CTG analysis sub partu].

514 labors from 20 tcpO2-cardiotocograms with about 10 000 events were computerized. By means of tcpO2-measurement we found a higher correlation between the fetal condition basing on a discriminant function (DF) than to the quantitative solitary parameters (residual-bradycardia, fetal heart frequency, dip area, lag time and bradycardia-index). The boundary ranges from the quantitative CTG-interpretation: "normal", "praepathological" (warning signs) and "pathological" (hypoxie signs) were confirmed for practice. The tcpO2-curve basing on quantitative analysis of their parameters (base line, slope of pO2 referring to labors and the delay time) is good adapted for examination of solitary CTG-parameters in relation to clinical practice.

[Significance of tcPO2 limit values in classification of cardiotocograms].

The continuous transcutaneous measurement of oxygen pressure has got an important method for the improvement of CTG-changes. A quantitative evaluation of the tcPO2-curve allows a better prediction of hypoxemia and acidosis with help of the CTG. Precondition is the examination of bound-values from the tcPO2-parameters: Baseline of oxygen pressure (PB), decrease of oxygen pressure (PD) and its delay time (DL). For this purpose we used 1 027 contraction patterns from 48 tcPO2-CTGs with 12 000 single parameter-values. It seems, that 11-12 mmHg (1,47-1,6 kPa) is a significant bound. CTG-pattern with PB greater than or equal to 15 mmHg (2 kPa) are not connected with hypoxemia and PB-values less than or equal to 10 mmHg (1,33 kPa) show the hypoxemic image of the CTG. The contraction-related decrease of oxygen pressure (PD) and its delay time (DL) are useful for estimation of the importance of variable decelerations. PD less than 2 or 3 mmHg (0,27-0,4 kPa) and DL higher than 46 seconds allow a classification of variable decelerations to the not hypoxemic type. PD equal to or greater than 5 mmHg (0,67 kPa) and DL between 40 and 20 seconds are suspicious for the hypoxemic type. These investigations are the supposition for a computer-aided classification of CTG-pattern with bound-values of the tcPO2-curve.

Pharmacokinetics of mepindolol sulfate in the rat, dog, and rhesus monkey. Studies on species differences.

14C-labeled mepindolol was rapidly and almost completely absorbed in the rat and dog after oral administration of 0.4 mg/kg, achieving maximum concentrations in the plasma after 1-2 hr. In the rhesus monkey, absorption was distinctly slower. Higher doses (25 mg/kg) given to rats and dogs exhibited maximum levels at 6 hr. The bioavailability of low doses was 1-2% in the rat and 40% in the dog. Plasma levels of unchanged drug declined biphasically in the dog with half-lives of 8 min and 3 hr. Excretion of radioactivity was mainly biliary in the rat. In the dog, elimination was equal in feces and urine, and in the rhesus monkey it was mainly renal. Although in the rat and rhesus monkey 14C-active compounds were completely eliminated, substantial amounts of metabolites were retained in the liver of the dog even 10 days after administration. After repeated administration, labeled substances accumulated in the dog, reaching maximum concentrations in the liver.

Biotransformation of the stable prostacyclin analogue, iloprost, in the rat.

The metabolic pathway of the stable prostacyclin analogue, iloprost (ZK 36 374), was studied in the rat, both in vivo and in vitro by a rat liver perfusion model. Metabolites were isolated from both experiments by preparative high performance liquid chromatography and identified by GC/MS and NMR analysis. In vitro, iloprost was metabolized by consecutive beta-oxidation of the upper side chain. Both dinor- and tetranoriloprost could be isolated from the perfusion medium. The metabolic pattern in bile was similar to that in the perfusion medium. In vivo, iloprost was totally metabolized by beta-oxidation of the upper side chain and by subsequent hydroxylation and conjugation. The compounds identified in rat urine were tetranoriloprost which represented about 3/4 of all metabolites, hydroxylated tetranoriloprost with the additional hydroxyl group presumably at position 17 and a conjugate of tetranoriloprost. Dinoriloprost and unchanged drug were not observed. beta-Oxidation of the upper side chain was stereoselective to give a 6 alpha-H/6 beta-H ratio of 86:14.

[Androgen substitution in the andrological disease picture].

Androgen deficiency is due to an insufficient synthesis of testosterone by the Leydig cells. This is either caused by a primary defect (primary hypoandrogenism) or by an inadequate hypophyseal stimulation (secondary hypoandrogenism). The clinical symptoms may include a late puberty, diminished terminal hairs, small testes, sexual impotence, and abnormal seminal fluid, but none of these conditions as an unique sign allows the diagnosis. This is achieved by low testosterone plasma levels (or low urinary excretion). The primary hypoandrogenism is additionally characterized by high gonadotropin levels, while the secondary form shows low FSH and LH. Hormone substitution is done with hCG and hMG in secondary hypoandrogenism only when fertility is aspired. In all other cases and in primary hypoandrogenism testosterone is given, either as oral form (testosterone undecaonate) or as depot injection (testosterone enanthate). Synthetic derivates like methyltestosterone are hazardous (hepatomas) or, like mesterolone, are too weak in their androgenic effect.

[Early and late morbidity of underweight (LBW) newborn infants following breech presentation in relation to the mode of delivery].

The influence of type of delivery on neonatal mortality, frequency of disturbances in adaptation to extrauterine life, and incidence of deviations in psychomotoric development was examined retrospectively in 126 low birthweight infants born in breech presentation. 65 infants were born by cesarean section, 61 by vaginal delivery. The control group involves 270 low birthweight infants born spontaneously in vertex presentation. In the weight categories of 1500 to 2500 g neonatal mortality, frequency of pathologic Apgar-scores, acidosis morbidity, incidence of severe respiratory distress, and percentage of psychomotoric handicaps were found to be lower in infants born by cesarean section compared to vaginally born newborns. However, only some of the differences between both groups were statistically significant. In newborn infants weighing less than 1500 g at birth an improved outcome after abdominal delivery could not be demonstrated.

[Morbidity of term and low-birth weight infants born in breech presentation by vaginal or abdominal delivery].

182 children born in breech presentation were investigated at the age of 5 to 6 years with regard to late morbidity after vaginal delivery and caesarean section. From case historical data, development in early childhood was assessed comparing the criteria of walking, speaking and "dryness". An attempt was made to find a relation between neonatal data and late morbidity. 156 children (85,7%) had a birthweight of not less than 2 500 g. 26 children (14,3%) were preterm and/or small for date. 80 term babies and 8 preterm and/or small for date babies were delivered by caesarean section. The birth weight of the preterm and/or small for date babies was 2 212,5 +/- 286,5 g for those delivered by caesarean section and 2 070,5 +/- 312,0 g for those delivered vaginally. The investigation program consisted of the following items: A. Assessment of intellectual development by means of the Raven-test, B. Examination of total and minute motor activity by means of the Rostock- Oseretzky -scale, C. Assessment of concentration abilities according to Koch- Pleissner . Statistically significant advantages in all items investigated were found in the group of term as well as preterm and/or small for date babies delivered by caesarean section. Except for intellectual development, analogous results were found for term delivered babies. Concerning development in early childhood, as reflected in walking, speaking and "dryness", there were no statistically significant differences between children delivered vaginally and those delivered by caesarean section. No relation could be found between neonatal data (Apgar-score, pH-value in the umbilical artery) and late morbidity. Problems of birth management are discussed.

Effect of bromocryptine in oligozoospermic men with hyperprolactinaemia.

13 patients with oligozoospermia, showing high prolactin serum levels but no signs of a prolactinoma, were treated with the specific prolactin inhibitor bromocryptine. After a 50 day treatment period an increase of sperm count in 6 cases, a decrease in 2 cases and no alteration in 5 cases was observed. Other semen parameters were not altered. Serum prolactin was significantly lowered. No explanation for the influence of inhibition of prolactin release on spermatogenesis can be given at present. The value of this therapy therefore is doubtful.

[Computerized analysis of intrapartum cardiotocograms].

307 monitored labors were investigated by means of a discrimination function (DF) in relation to qualitative CTG-parameters. In this connection we could confirm on the one hand well defined qualitative CTG-criteria in their clinical importance by computation corresponding to "normal", "prepathological" (warning-signs) and "pathological" (signs of hypoxia). On the other hand we could demonstrate the exactly recognition of qualitative CTG-parameters in attendance of computation. The boundary ranges of discrimination function resulting from the automatic CTG-analysis respecting fetal condition turned out to be reproduced and clinically reliable. The prediction with regard to neonatal parameters (pHNA, APGAR-score 5 minutes after delivery) is justifying the employment of present soft ware in clinical practice.

[Evidence of bacteria in ejaculate].

Infections caused by bacteria strike primarily the prostate and vesical glands, and secondarily the epididymis. Essential first is to give evidence of bacteria in germ numbers exceeding 10(5) pro ml ejaculate. In addition, the diagnosis of prostatitis requires the demonstration of other signs of infection, for example increased numbers of leukocytes in the ejaculate, in the prostatic secretion and in the urine; the clinical symptoms in relation to these indicators are not necessarily characteristic. It is essential for the microbiological diagnostic, that evidence of all the germs be found in native material. It is therefore necessary to first disinfect the external urethral orifice and the glans penis, in order to avoid super-imposition of germs from the surroundings. Every demonstration of bacteria must be connected with a determination of resistance. The relationships between the ejaculate parameters which deviate from the norm, and evidence of bacteria in ejaculate are unclear. Reports about disturbances in spermatozoa motility or morphology during infections of the adnexa of man are to be evaluated with care.

Biotransformation of proterguride in the perfused rat liver.

The metabolic pathway of the dopaminagonistic ergoline derivative, proterguride, was studied in vitro in a rat liver perfusion experiment. Metabolites were isolated by preparative HPLC and identified by MS and NMR analyses. In total, seven compounds could be identified. The metabolic steps involved in the biodegradation of proterguride are N-deethylation, N-oxidation, hydroxylation, and/or dehydrogenation and oxidative cleavage of the indole ring.

Biotransformation of the antidepressant DL-rolipram. I. Isolation and identification of metabolites from rat, monkey, and human urine.

The metabolic pathway of DL-rolipram was studied in two animal species and in man. Metabolites were isolated from rat, rhesus monkey, and from human urine by preparative HPLC and identified by MS and NMR analysis. In total, the structures of 7 degradation products could be elucidated. Rolipram was metabolized by ether cleavage at the methoxy and cyclopentyloxy groups and by hydroxylation in positions 2 or 3 of the cyclopentyloxy ring followed by sulphation. Additionally, but exclusively in man, the 5-position of the pyrrolidone ring was hydroxylated.

Vulvar amyloidosis mimicking giant condylomata acuminata in a patient with multiple myeloma.

We report a case of unusual cutaneous amyloidosis involving the vulva in a patient with multiple myeloma. Genital examination revealed a dense agglomeration of verrucous papules and pedunculated condyloma-like tumors. The correct diagnosis was established by immunohistochemical examinations that visualized large amounts of lambda light chains, whereas no reaction was detected for kappa light chains or human papilloma virus. In this way, the differential diagnosis of condylomata acuminata could be ruled out. Condyloma-like lesions have been described in patients suffering from multiple myeloma, but the present case is unusual because of the extensive involvement. Vulvar amyloidosis should be added to the list of possible presentations of myeloma-associated systemic amyloidoses.
