[Clinical criteria and evaluation of the pathology and progress of lympho-nodular toxoplasmosis].

We aimed at analysis of the clinical pattern and the course of lympho-nodular toxoplasmosis taking into account the intensity and duration of the illness. Ninety seven patients (69 adults and 28 children) with lympho-nodular toxoplasmosis were examined. Toxoplasmosis was diagnosed by indirect immunofluorescence; the titres of antibodies of IgG and IgM classes were also estimated. Depending on the duration of the illness three periods: early, regression of symptoms and protraction of symptoms were defined; depending on the intensity of signs and symptoms of toxoplasmosis three courses of the disease: acute, subacute and chronic were defined. The analysis showed that clinical picture of lympho-nodular toxoplasmosis was characterized by swelling of various peripheral lymph nodes as well as by general clinical symptoms (88.7% of cases) the intensity and duration of which, to a large extent, determined course of the illness. No correlation between clinical course and duration (early and regression period) of lympho-nodular toxoplasmosis and titres IFA-IgG (before therapy) was found. Pathomorphology of lymph nodes in patients with toxoplasmic lymphadenopathy is a valuable complementary examination which renders it possible to determine the period of the disease.

Pathophysiological aspects of Trichinella infection in man.

The author presents significant elements of the pathomechanism of general disorders resulting from trichinellosis. Moreover, the pathophysiology of the gastrointestinal phase is also outlined while special emphasis is put on disorders in the early stage of invasion, in the acute stage of the disease associated with other clinical symptoms, and disturbances after other clinical symptoms have subsided. The pathophysiology of muscle invasion deals also with the essential morphological changes within muscle fibers and with biochemical and bioelectric disturbances in muscle tissue. An outline of cardiopathophysiology in the course of trichinellosis is also presented.

The sequential appearance of IgG subclasses and IgE during the course of Trichinella spiralis infection.

Earlier studies have shown that IgG1 and IgG4 are the dominant IgG subclasses in the specific response during a chronic helminthic infection. It has also been suggested that IgG4 production results from chronic or repetitive antigenic stimulation and a correlation between IgG4 and IgE levels exists. An outbreak of Trichinella spiralis infection in Poland provided the opportunity to follow the sequential appearance of the IgG subclass and IgE responses in 15 patients during the early stage of Trichinella infection and to compare these observations in sera obtained one year later from the same patients. The results show that the sequential appearance of the IgG subclasses were IgG1 before IgG3 and IgG3 before IgG4. IgG1 antibodies dominated the immune response in all patients. A statistically significant increase in the number of IgG4 positive sera was observed in patients during the chronic stage compared to the findings during the early stage of infection (13% vs 73%; p less than 0.001), supporting the view that IgG4 results from a chronic antigenic stimulation. A correlation between the appearance of IgG4 and IgE was not found. The highest levels of IgE were seen in the first serum samples obtained, with a decrease during the course of infection.
