[Ruptures of the tendons of the extensor system of the knee joint].

Ruptures of the quadriceps and patellar tendon are comparatively rare lesions. From the number of cases from 1973 till 1983 17 ruptures of the femoral extensor tendon and 8 ruptures of the patellar tendon were controlled diagnostically, therapeutically and in view of their long-term results. Causally in the group of quadriceps tendons 13 spontaneous ruptures were facing 4 traumatically caused ruptures. In case of traumatic ruptures 3 of them were adolescents after extreme traumatic violence by rapid traumata. Degeneratively conditioned subcutaneous quadriceps tendon ruptures we only found in case of men who were on an average older than 50 years. In spite of physiologically extreme resistance to ruptures 6 of 8 ruptures of the ligamentum patellae were ascribed to the fact that by the progressing degeneration the remaining strain capacity of the tendon was reduced below the measure of normal load. In each individual case the aspects of expert opinion were traced - above all in case of tendon ruptures with osseous patella lesions. For the first time a temporally staggered bilateral spontaneous rupture of the patella was described. The operative approach has been discussed just as much as the necessity to examine the internal knee-joint structures arthroscopically in case of the plurality of accompanying retinaculum lesions.

Elective modules in the behavioural sciences.

An elective modules programme in the behavioural sciences was designed for the undergraduate medical curriculum at The Ohio State University College of Medicine. The programme provides a mechanism by which a broad range of behavioural science content can be introduced into the curriculum without increased allocation of teaching staff or budget. Student preference data indicate some clear differences with the more popular modules being those which focus on skills or behaviours that students perceive to be useful in the immediate practice of medicine. The elective aspect of the programme was part of its initial appeal, both to students and teaching staff. Two years of course evaluation data from the students indicate that most individual modules and the programmes as a total have been successful in achieving their intentions. The programme has now been included as a permanent component of the curricula.

Embolization of pelvic bone metastases from renal cell carcinoma.

4 patients were treated by embolization for pelvic metastases from a renal cell carcinoma. Embolization was carried out either preoperatively or was the only therapy. The method, the materials used and the results of 3 of these patients during 7-16 months of observation are presented.

[Results of stabilisation in pathologic fractures in the femoral shaft region by nailing (author's transl)].

The article reports on 11 patients with metastases in the femoral shaft. In 5 patients, a pathological fracture had occurred, whereas in 6 patients the femur, which was liable to fracture, was prophylactically stabilised by means of the femoral shaft nail. Eight of these patients were able to walk after the operation, whereas 4 patients died after an average period of 19 weeks following surgery of their primary diseases. The method employed in these cases was of a palliative nature, resulting in considerable relief from pain, improved ability to walk, and hence in an improvement of the quality of life of these patients.

[The crescent sign in childhood urography].

The crescent sign is a rare roentgenological symptom in excretory urography of infantile hydronephrosis. The orign is a presence of contrast material in the collecting renal tubule of the atrophic renal parenchyma. That is displaced at the renal circumference. The disappearance of the crescent sign represents the prognostic for the reparative potence of the renal parenchyma after the therapy of the obstructive uropathy in two own case.

A survey study of the use of electropupillogram in predicting response to psychostimulants.

To confirm the conclusions from a previous study supporting the usefullness of electropupillogram (E.P.G.) in predicting clinical response, data from three separate studies with hyperkinetic and learning disabled (L.D.) children treated with stimulants were surveyed. Change in extent of pupillary contraction (E.C.) after a test dose of stimulant as measured by E.P.G. did not correlate significantly with actual clinical rating change (with one exception out of 14 correlations calculated). These negative results are reported with a reservation regarding their validity because of technical difficulties in data collection.

A study of endogenous dopamine metabolism in Gilles de la Tourette's disease.

This longitudinal, blind study of Gilles de la Tourette's disease in a 44 year old male patient who was nonresponsive to Haloperidol therapy indicates that dopamine excretion is related to clinical response. An increased urinary excretion of dopamine and some of its metabolites was associated with the failure of Haloperidol therapy. Imipramine, administered to treat the patients' depressive mood that emerged in the course of treatment, decreased the urinary excretion of dopamine and moderately alleviated the symptoms of Tourette's syndrome. The results suggest that monitering urinary dopamine and 3-methoxytyramine excretion in Tourette's disease may predict the clinical response to pharmacotherapy, and that a dopaminergic mechanism may be associated with this type of motor hyperkinesia.
