[Magnetic resonance: imaging of the heart and mediastinum].

The anatomic structures of the heart and mediastinum can be demonstrated with MRI in three dimensions in their topographic order without contrast medium or heart catheterization. Furthermore, differentiation of the tracheobronchial system and the mediastinal lymph nodes can be discerned with MRI. MRI offers the advantages of reducing invasive diagnostic procedures, eliminating radiation hazards and avoiding the risks of contrast media. Only in the area of the structures adjacent to the diaphragm an improvement of the images by means of additional respiratory-gating is desirable. Concerning tumors in the mediastinum, MRI supplies essential information for the planning of therapy.

[Medical history impressions of Karl Marx 1983].

Excerpts of his London era first published on the occasion of the Karl Marx testimonials of 1983 gave rise to extend the memory of the fundamental achievements of Karl Marx to medico-historical aspects. In this case Karl Marx paid special attention to the working and living conditions of the working class and an analysis of his adequate statements and records shows multifarious details which give a research basis also for the history of medicine. Marx and Engels had friendly contacts with several physicians who shared the opinions of the two classics: their way of life is shown in the most important points.

[Eisleben and the Mansfeld area in the medical history aspect. A contribution to Luther year 1983].

The homeland of the reformer Martin Luther which since the middle ages has been acting as the centre of a mining industry is at the same time also one of the starting points for the organisation of a mining medicine with its medical measures directed to the treatment of the miners. On the occasion of the Luther memorial year 1983 is reported on several physicians and pharmaceutists of the Eisleben-Mansfeld district who distinguished themselves by particular activities and thus for ever stay in the history of science.

[Branches of natural science in the Halle Medical College program in the 18th and 19th centuries].

The subjects of natural sciences belonging in the 18th century to the complex of medicine in the early 19th century change over to the complex of reliability of the faculty of philosophy. The future physician passes a tentamen philosophicum. In the sixties of the 19th century the increase of knowledge in the field of natural sciences causes an increased consideration on these subjects for the basic medical training. On these conditions the tentamen philosophicum becomes the tentamen physicum, i.e. examination in natural sciences, the precursor of the preliminary medical examination of our time.

[History of continuing medical education within the scope of regional associations and societies].

In Halle the associations and societies with the aim of postgraduate medical education can look back on a tradition lasting for centuries. The beginnings which are to be proved for the 18th century led to the formation of associations and societies which besides the mediation of knowledge also dealt with problems of the medical profession. In Halle on the basis of regional societies as well as in form of clinico-scientific soirées is made allowance for the demands of modern medical postgraduate education, the latter institution existing since 1959 can at present refer to 200 arrangements performed and forms a not unimported partial complex for the regional postgraduate educational system.

Neuronal correlates of sleep, wakefulness and arousal in a diurnal insect.

The discovery that various states of sleep, rest, wakefulness and arousal in man can be correlated with specific forms of the electroencephalogram1 has led to intensive studies of these states, mostly in mammals2-5. Today it is generally accepted that circadian sleep-wakefulness cycles occur in mammals and birds2,3,6. Behavioural observations on sleep in moths have also been published7; many other invertebrates demonstrate rest/activity cycles8. Circadian sensitivity fluctuations in both central9 and peripheral10-15 components of the visual system of various nocturnal arthropod species have been demonstrated. We now report that long-term, extracellular, single-unit recordings from optomotor interneurones in the optic lobes of forager honey bees reveal an oscillation in their sensitivity to moving visual stimuli16, 17. The oscillation displays properties typical of a circadian rhythm6, 18. The sensitivity of the neurons is higher during the subjective day than during the subjective night. The locomotor activity of individual, fixed walking forager bees shows a similar circadian oscillation and is also higher during the subjective day. Visual and mechanical stimuli can act directly on the interneurones and restore their sensitivity during times of reduced neuronal responsiveness. A comparison with results available for mammals makes it likely that the neuronal phenomena presented here are correlates of the bee's circadian sleep-wakefulness rhythm.

[History of pre-academic medical education: the Gymnasium Illustre in Zerbst and the Philanthropinum in Dessau].

The memory of the foundation of the Gymnasium illustre in Zerbst in 1582 includes the memory of the medical propaedeutics which belonged to the school programme there. A series of graduates from this institution later had medical professionships at famous universities. Also the Dessau Philanthropinum attempted to carry out a kind of medical propaedeutics by a corresponding planning of the instruction. The two school system characterize a development the object of which was for different reasons to form a part of the grammar school education as preparatory medical instruction, in order to guarantee in this way an undisturbed transition to the university medicine.

Pharmacokinetics of metioprim in normal subjects and patients with impaired renal function.

The pharmacokinetics of metioprim, a new competitive inhibitor of bacterial dihydrofolate reductases, were studied after a single oral dose of 100 mg in two young healthy subjects, four elderly volunteers, and eight patients with impaired renal function. The pharmacokinetic parameters of unchanged metioprim were derived by analyzing plasma level curves using a two-compartment model. Plasma half-lives determined for the beta phase were 10.5 to 11.5 h in young volunteers, 12.0 to 12.9 h in elderly subjects, and 13.2 to 15.3 h in patients with renal impairment. A significant correlation between the overall elimination rate constant and the creatinine clearance was demonstrated.

[Sequelae of secondary jejunum interposition in gastrectomized patients].

Secondary interposition of the jejunum was performed in 45 patients who had 2/3 gastrectomy before. An additional a selective gastric vagotomy was done. All patients had a follow-up examination after 2 months to 6 years. Secondary disturbances like dumpingsyndrome were not observed. In 30 cases, which underwent sequential scintigraphic control examination no duodeno- gastral reflux could be observed. In 2 out of the 45 patients florid ulcers could be diagnosed by gastroscopy, in 3 of them silent ulcers.

[Unspecific tissue reactions caused by glove powder].

Macro- and histopathological examinations were performed on 200 Wistar rats of both sexes which had been intraperitoneally treated with either Brauns resorbable surgical glove powder or Bio-sorb surgical glove powder; control animals were injected with talcum powder. Talcum resulted in the well known foreign body granuloma along with adhesions. The amylaceous powders induced an initial inflammatory reaction with a subsequent non-reactive interval and thereafter solitary foreign body granulomas were formed. After terminating the experiment (12 weeks), no adhesions could be detected in animals treated with starch containing powder. A resorption of the powder was assumed in that the number of powder crystals was greatly reduced by the end of the experiment. The granulomas which developed after the 12th week were regarded as resorption granulomas. The development of adhesions was minimal with both types of amylaceous powder.

[100 years of the University Hospital in Halle. The development of the Halle University Hospitals in the first decade of their existence].

This year the clinics of the Martin Luther University of Halle-Wittenberg can look back on an existence of 100 years. Their establishment took place in a phase in which the natural-scientific principle and on this basis the hospital medicine began to make their way. Improved techniques and methods rendered a concentrated performance of medical care indispensary henceforth. In the phase of their foundation the new clinics which were established on the basis of French payments of reparations are reckoned the best institutions among the union of the Prussian universities. They get a high appreciation above all by the appointment of good scientists to professorships particularly of the theoretical fields. But already after few decades increasing insufficiencies of the clinical department show the beginning of a stagnation which should set in after the turn of the century and caused the loss of the first place the clinics held up to that time.

[100 years ago Robert Koch described the tuberculosis germ].

The beginning of the etiologic epoch in medicine is in general equated with the discoveries of Louis Pasteur and Robert Koch. The 24th march 1982 is the 100th anniversary of that memorable day on which Robert Koch announced his discovery of the tubercle bacillus. We remember this even in connection with the discussions on infectiosity and additional factors led before and after. The absence of specific possibilities of treatment and the failure of the tuberculin therapy induces regional and supraregional activities, which evoke the establishment of an organized care. At the instance of the situation of the changing of the century the correlation of endogenic and exogenic condition is explained, which was recognized also by Robert Koch.
