Biological meaning of the prospective epidemiological study on chronic obstructive lung disease and aging.

An epidemiological follow-up study on the occurrence of the chronic obstructive lung disease (COLD) in the elderly was carried out in a probability sample of Cracow inhabitants. The research dealt with defining the high risk groups. The health status of the population studied was appraised by standardized interviews and spirometric measurements. Prevalence of COLD was rather strongly related to age both in men and in women but the increase with age was not uniform, being linear below 50 yr and steep in older age groups. The logistic curve appeared to fit quite well to the empirical data regarding the frequency of COLD in relation to age. The overall relative risk of developing COLD estimated by odds ratio was also affected greatly by smoking and by respiratory symptoms. Against expectations, the study showed that the elderly run a substantial risk of developing COLD even without any respiratory symptoms and being a non-smoker. The data obtained indicate that age groups above 50 yr are more vulnerable as the pace of lung function deterioration is much higher than in younger persons. The changes produced by relatively mild pulmonary symptoms like productive cough adding to pre-existing changes caused by aging may precipitate definite chronic obstructive lung disease.

Interaction of smoking and urban air pollution in the etiology of lung cancer.

Surveillance of lung cancer incidence based on mortality was carried out over 6 years in Cracow. It appeared that lung cancer death rates among Cracow inhabitants were higher than average rate in the population of Poland but this difference in the large extent could be explained by the greater prevalence of smoking habit in Cracow than in whole Poland. Very intriguing was a substantial excess of lung cancer deaths only in male residents of the city center having the highest level of the air pollution. Since this excess in the lung cancer deaths could not be exclusively explained by smoking or occupational hazards the air pollution should be assumed as a responsible factor. Lack of the similar phenomenon in females living in the city center can be explained by the fact that the air pollution alone is not sufficient cause in the etiology of lung cancer but that in combination with other adverse factors like smoking or occupational hazards it develops its carcinogenic effect.

A follow-up study on chemoimmunotherapy (5-fluorouracil and BCG) in advanced gastric cancer.

Two hundred and seventeen patients with advanced gastric cancer were classified according to the resectability of the tumour into two groups: I, resectable (non-radical surgery), 99 patients, and II, non-resectable carcinoma, 118 patients. Within each group patients were randomly assigned to receive 5-fluorouracil (5-FU) + BCG, 5-FU, or no further treatment (surgery only). BCG was given by scarification. A 2-year follow-up is reported. The group of patients with resectable tumours and receiving chemoimmunotherapy had a statistically significant prolongation of survival compared with the 5-FU and surgery only groups. No differences in survival were observed between these treatment modalities in patients with non-resectable tumour. These observations indicate that chemoimmunotherapy may be of benefit for a selected group of patients with gastric cancer.

Styrene and methyl methacrylate in the industrial environment as a risk factor of chronic obstructive lung disease.

The main objective was to assess the biological effect of styrene and methyl methacrylate on the respiratory system in the industrial population. A total of 454 males from the exposed group and 683 workers from the control group were examined. The health status of the workers was evaluated by standardized interviews on chest symptoms and lung function testing. The environmental measurements were performed in all work places where the persons under study were employed. There was practically no differences in the prevalence of chronic chest symptoms in both groups of workers but the frequency of lung obstruction appeared to be more than twice as high among the exposed persons in comparison with the controls. A large proportion of cases with lung obstruction did not show any chronic chest symptoms. It appeared from the study that the environmental factor developed a much more harmful effect on lung function than smoking, and one effect was independent of the other.

[Results of subjective and endoscopic studies on the incidence of upper digestive tract pathology among industrial workers].

This paper has been aimed at an evaluation of the prevalence of upper alimentary duct pathologies in an industrial population, as well as at an attempt to evaluate the value of subjective symptoms in screening tests. Altogether 975 subjects have been examined, but only 235 of them underwent endoscopic studies. The data on subjective symptoms have been collected from all the subjects, by way of a special questionnaire. In the group of those subjected to endoscopic studies, 38,7% have been found to have developed various diseases of the upper alimentary duct. Those predominating have been ulcers and deformations of pylorus and hiatal herniae. Ulcers and deformations of duodenum in smokers have been nearly twice those in nonsmokers. The results demonstrated that the prevalence of upper alimentary duct diseases in the industrial population concerned, has been higher than expected. To account for this, further epidemiological analytical studies must be undertaken. It has been found that within the screening studies of populations, aimed at the detection of upper alimentary duct diseases, questionnaire on abdominal pain is more reliable than the questions on dyspeptic symptoms.
