[Biliary peritonitis without perforation and idiopathic biliary peritonitis].

This is a report about an uncommon disease. Three forms of biliary peritonitis without demonstrable perforation are differentiated due to etiology as far as known. While the causes of biliary diffusion in the course of acute cholecystitis or pancreatogenic necrosis of the gall bladder are well known, there is still no information about the genesis of idiopathic biliary peritonitis, which never shows lesions or inflammation of the extrahepatic bile tract or gall bladder. Two cases are reported.

Accuracy of computed tomography in staging of kidney tumors.

Computed tomography was performed in 125 patients with kidney tumors for pretreatment staging. The accuracy of CT staging was then determined by correlating the CT findings with the pathologic findings (111 patients) or with the angiographic findings (14 patients). Perirenal extension was correctly predicted in 79 per cent of all the patients, lymph node involvement in 87, main renal vein involvement in 91, infiltration of the inferior vena cava in 97 and invasion of neighboring organs in 96 per cent. It is concluded that CT should be the baseline procedure for the assessment of the extent of spread of renal tumors.

[Treatment of pancreas lesions in childhood].

Although lesions of the pancreas are rare injuries in childhood, serious pancreatic lesions should be considered after blunt abdominal trauma initially. The typical feature of rupture of the pancreas is characterized by fall on a bicycle handlebar and a round circumscript sign in the upper abdomen. The management is determined by the situation: Nasogastric suction and i.v. alimentation in case of pancreatic contusion. Surgical intervention and local external drainage in case of parenchymal rupture and Roux'enteroanastomosis in the event of injury of the pancreatic duct are recommended. The implantation of a pancreatico-cutaneous fistula into the duodenum is reported.

[The "Perthes-Braun-syndrome". A sign of severe blunt thoracic trauma in childhood].

"Perthes-Braun-Syndrom" is told to be sign of severe blunt thoracic trauma. We report about 7 patients up to the age of 14 years, who showed symptoms of thoracic compression. The syndrome appears in different expression with main symptoms as subconjunctival bleedings and petechias of the upper part of the body. In 6 of the described cases the syndrome was associated with severe intrathoracic lesions. In is concluded that intensive supervision is indicated in patients with "Perthes-Braun-Syndrome" because of delayed appearance of cardial and pulmonary symptoms.

[Computer tomographic demonstration of so-called azygos continuation in the absence of the hepatic segment of the inferior vena cava. Three case reports (author's transl)].

Dilatation of the azygos vein is usually due to increased pressure or flow, rarely is it diopathic. Another cause is so-called continuation of the azygos in cases of absence of the hepatic segment of the inferior vena cava. The method of choice for the investigation of azygos dilatation nowadays is computer tomography. Anomalies of the inferior vena cava can be recognized in this way unequivocally by a non-invasive method. Computer tomography frequently shows important additional abnormalities (eg. situs inversus, polysplenia), since it demonstrates all organs situated in the plane of section.

[Adequate treatment of supracondylar fracture of the humerus with associated brachial artery and nerve injury in childhood (author's transl)].

Injury to the brachial artery and the nerve trunks, accompanying supra- and peri-condylar humerus fractures, occurs in children particularly in seriously dislocated fractures of Baumann Type II and III. Prompt treatment aims at avoiding late sequelae, especially ischaemic muscle contracture. Based on 6 of our own cases, the significance of brachial artery angiogram when the pulse does not become palpable, and of operative investigation, is expounded.

[The topical treatment of large second degree burns in childhood by tanning with silvernitrate and mebromide (author's transl)].

The tanning method using 7% silvernitrate and 2% mebromide solution for second degree burns in childhood is demonstrated. The advantages of this method are described as singular and painless application of the agent, little annoyance of the patient, bactericidal effect in second degree burns, low expenses and easy medical care. The procedure of tanning is preferred in treating burn injuries of the trunk and non mobile areas of the extremities. We report on indication, procedure of tanning and results in the treatment of 129 burned children.

[Sternum fracture in a 12-year-old boy].

A sternum fracture resulting from indirect insignificant trauma is described in a 12-year-old boy. Similarities between this case and aseptic osteochondronecrosis are revealed. Conservative treatment results in a quick recovery.

[Classification of stages of renal tumors by means of computed tomography scanning (author's transl)].

Fifty-eight patients with malignant renal tumors were submitted to computed tomography scanning. Fifty patients were operated. In eight patients, the disease was too advanced to allow a surgical treatment. The preoperative classification of stages was made according to the Robson scheme and compared with the results of the postoperative histopathological examination. There were identical results in 82% of all operated patients. An analysis of the individual results (fat tissue infiltration, lymph node metastases, infiltration of adjacent organs or veins) shows that the method is limited by the uncertain estimation of the renal vein as well as of the beginning fat tissue infiltration. The good results should give occasion to adopt the computed tomography scanning of the kidney into the staging catalogue of IUCC. Computed tomography scanning should be the basic examination for preoperative staging of renal tumors.

[Heart pacemaker stimulation within a coronary vein in persistent left superior vena cava].

Persistent left superior vena cava is a rare malformation that causes no functional disturbance. In one case of this anomaly associated with dextropositio ventriculi cardiac pacing was necessary using the approach from the left vena cephalica because of a variety of preceding problems and severe complications. The electrode could be stably positioned in the coronary vein, vena cordis media. The evidence of pacemaker malfunction conditioned by increase of threshold, of the occurrence of electrode perforation or electrode-induced coronary vein thrombosis has not been observed. The electrocardiogram during stimulation shows the feature of right bundle-branch block. The described method was used to avoid a repetitive intervention.
