Evidence of duplication of the human salivary amylase gene.

Isoelectric focusing of human parotid saliva reveals different alpha-amylase patterns reflecting qualitative and quantitative variations. A puzzling pattern, which shows three different amylase gene products, was found in four individuals. Based on this observation a model is presented in which the salivary amylase gene is duplicated. Family studies show that the AMY1*A2 gene forms a haplotype with the normal gene, AMY1*A1, whereas the AMY1*A3 gene still exists in a single form. The absence of homozygote 2-2 in offspring of 1-2 X 1-2 marriages and in population material, and the fact that the variant protein makes up about only 20-30% of the total amylase protein in heterozygotes can be considered as additional evidence supporting the hypothesis. The possibility that cis-acting regulatory variants are involved in the patterns with quantitative variation is discussed.

[Clinical double blind comparison of diclofensine and placebo in geriatric patients with depressive syndromes].

The objective of this double-blind trial was to assess the effect of diclofensine on geriatric patients with depressive syndromes in comparison to placebo. 40 patients (age 60-75 years) were treated for 3 weeks with a mean daily dose of 57.5 mg diclofensine or placebo. Diclofensine was found effective in 18 (= 90%) cases (placebo 60%) after 2.9 days on an average (placebo, 4.2 days), mostly having a mood elevating effect and in only 2 cases (placebo 1) having a stimulatory effect. The results of the global assessments, the Hamilton depression scale, the d2-test, the block design test and the mood scale (Befindlichkeitsskala) demonstrated a statistically significant superiority of diclofensine over placebo. The critical flicker frequency, on the other hand, was not able to differentiate significantly between the two drugs. The tolerance was considered as very good in all cases. Unwanted effects were observed in 13 patients (= 65%) of the diclofensine group and in 9 patients (= 45%) of the placebo group. They disappeared in most cases within one week after their appearance. According to this trial diclofensine can be described as a well tolerated drug without pronounced anticholinergic side effects, capable of improving depressive states faster and more effectively than placebo.

[Valve replacement for mitral in incompetence in a patient with osteogenesis imperfecta (author's transl)].

Reports on aortic and mitral valve alterations in patients with osteogenesis imperfecta, a congenital connective tissue disorder, are sparse in the literature. Aortic valve incompetence is more common than mitral valve dysfunction. Though the clinico-pathological and histological features of valve alterations are very similar to those seen in Marfan's Syndrome, valve dysfunctions in osteogenesis imperfecta are rarer than in Marfan's Syndrome. In a 23-year-old patient with typical clinical feature of osteogenesis imperfecta, mitral incompetence was detected by echo- and angiography. Patient was provided with a St.-Jude-Medica prosthesis and postoperative course was without any complications.

[Antirheumatic treatment in old age and its effect on kidney function].

25 geriatric patients with normal renal function and ten patients with impaired renal function were treated with piroxicam (Felden) for osteoarthritis of the major joints. The objective was to determine the efficacy and toleration of the drug and, in particular, renal parameters and--in the case of the patients with renal insufficiency--the plasma levels of the drug. Piroxicam improved both pain and restricted movement in both groups and it caused only few side effects. The specific laboratory values did not change in the patients with normal renal function, while renal parameters sometimes even improved in the patients with renal insufficiency during the treatment with piroxicam. Piroxicam did not accumulate in these patients despite the impaired renal function; the plasma levels corresponded to those found in patients with normal renal function.

Radiosensitivity of microscopic tumours of a transplantable mammary adenocarcinoma in mice.

Evidence is presented that microscopic tumours (of a transplantable murine mammary carcinoma, M8013X) grow faster than larger, palpable, tumours. Microscopic tumours are also more radiosensitive than larger tumours. The decrease in radiosensitivity in larger tumours is prevented to a large extent by misonidazole, which has no significant effect on the radiosensitivity of microscopic tumours. The retardation in growth rate which occurs after the fast microscopic growth is probably related to the appearance of hypoxic cells. Both the decrease in growth rate and the progressive development of hypoxia may be caused by the relatively poorer blood flow in larger tumours. Part of the radioresistance in "large" tumours ( approximately 250 mm3) seems to be due to factors other than hypoxia; maybe cell-kinetic factors also play a role. The intrinsic radiosensitivity of tumour cells in microscopic tumours was assessed by means of a modified latency test: the Dq and Do were 2.2 and 2.5 Gy respectively. A number of factors which may influence the reliability of these estimates are discussed.

[Dihydroergot plus in the long-term therapy of orthostatic hypotonic dysregulation in older patients].

42 patients aged 75 /+- 12,5 years with orthostatic dysregulation and partly with hypotension were treated with the new combination of 20 mg etilefrine and 2 mg dihydroergotamine (Dihydergot plus) t.i.d. for 2 months in a single blind controlled study. The efficacy of Dihydergot plus was proved by a standardized orthostatic test and evaluation of subjective orthostatic symptoms. The resting blood pressure of the patients with hypotension increased from 106/63 mmHg to 114/67 mmHg. After changing from supine to standing position all patients showed a stabilized haemodynamic reaction. The fall of systolic and increase of diastolic blood pressure as well as the increase of heart rate were reduced and the subjective symptoms improved. There were also an improvement of the patients mood and activities of daily living. Dihydergot plus was well accepted by all patients during the treatment period.

[Treatment of severe respiratory and urinary tract infections in older patients].

In a clinical study 20 elderly patients with severe respiratory tract infections and cystopyelonephritis resp. were treated with the penicillin combination product Optocillin. In each case the infectious organism was isolated. The infections were exclusively mixed infections involving a high proportion of staphylococci and gramnegative problem organism. Most of the isolated organisms were sensitive to the antibiotic component of Optocillin. The preparation was well tolerated. Considering the severity and chronic nature of the cases under investigation the clinical effect is regarded as very good.

[Therapy of cerebrovascular insufficiency. A psychometric double-blind trial with a slow-release form of xantinol nicotinate (author's transl)].

Of 80 geriatric patients with cerebral insufficiency, 40 received 1000 mg of a slow-release form of xantinol nicotinate (XN) and 40 a placebo twice daily for 12 weeks. According to the co-variant analytical evaluation of the placebo/verum differences with the initial values as covariable after 12 weeks' application, XN influenced most of the investigated geriatric complaints favorably and improved the general intellectual capacity. Central activation and concentration are already improved after 6 weeks' treatment with XN.

[Double-blind long-term study on a combination of tetracycline, theophylline, doxylamine succinate, etafedrine, phenylephedrine and guaifenesine in chronic bronchitis (author's transl)].

The investigations presented were undertaken to find out whether a long-term treatment of chronic bronchitis with a combination of tetracycline, theophylline, doxylamine succinate, etafedrine, phenylephrine and guaifenesine could reduce the frequency of recurrent infections of the respiratory system. Out of 57 patients who all had suffered from at least one exacerbation in the year preceding the trial only 12 showed clinical signs of an ixacerbation during the study. 12 exacerbations occurred during the placebo phase, 6 during the treatment with Ditenate Tetra. It is specified why such a long-term treatment with an antibiotic should be reserved only for patients who are especially endangered by the exacerbation. The results of numerous microbiological investigations of sputa, nose and throat swabs before and during the long-term study are interpreted under certain aspects and questioning.
