Although it is generally accepted that schistosomal granuloma formation results from a T-cell dependent host response towards the parasite egg, attention has recently been focused on the involvement of B-cells in the induction of schistosome-induced pathology. In this study we investigated the involvement of two functionally different B-cell populations in the formation of the Schistosoma mansoni granuloma: naive and antigen-stimulated B-cells. In liver granulomas two distinct B-cell populations were found, namely unstimulated B-cells at the periphery of the granuloma and antigen-stimulated, syndecan-1 positive B-cells in the inner part near the deposited egg. Intestinal granulomas differed by their relative lack of unstimulated B-cells at the granuloma periphery, but like hepatic granulomas intestinal granulomas also have syndecan-1 positive B-cells. From our results, we conclude that B-cells are important constituents of the S. mansoni granuloma.
During the period 1980-1989 35 patients with successfully replanted digits could be selected. Only 17 patients with 32 long fingers and 2 thumbs replanted were prepared to cooperate for this retrospective study, looking for the late functional results. Mobility of the replanted digits was measured by the TAM-value. Power was indicated by grip strength and tip pinch. Weber two-point discrimination test and Von Frey test were used to investigate sensibility. Most patients show cold-intolerance. Mobility was worse for the more proximal replantations. Replantations distal to PIP do functionally very well. Therefore single digit amputations should be considered as an indication for replantation, especially amputations distal to the pip. Ring and little finger replantations have a better mobility than index and middle fingers. Mobility in younger patients is also better. Grip strength and tip pinch are respectively 66% and 73% of the normal values. All patients (except 2) could return to their work in a reasonably short time (average 5 months).

Infection of a susceptible host with the blood fluke Schistosoma mansoni results in the formation of periovular granulomas and subsequent fibrosis in the target organs. Granulomogenesis and fibrogenesis are mediated by immunological events which require cell-cell and cell-matrix interactions. In this review, the role of adhesion and co-stimulatory molecules in the genesis of the schistosomal pathology (granulomogenesis and fibrogenesis) is outlined. These molecules provide essential immunological interactions not only for the initiation of granuloma formation but also for the maintenance and modulation of the schistosomal granuloma during chronic infection. Furthermore, the role of secreted soluble adhesion molecules in the different clinical forms and in the modulation of the schistosomal granuloma is discussed. Recent new insights into the role of adhesion molecules for the induction of pathology by other developmental stages of the parasite (other than eggs) will be presented.
