[Lipid and apoprotein profile without and with oil load].

In 20 normolipaemic patients and 1 patient with hyperlipoproteinaemia (HLP type IV) the lipid profiles without and with oil load are taken (triglycerides, HDL-cholesterol, apolipoprotein A and B). For the oil load test at 7.00 a.m. 100 g sun-flower oil are given and then till 15.00 every two hours 2 ml venous blood are taken. The lipid profiles without oil load (in normal nutrition) take place in the same temporary course. Of the investigated parameters only the triglycerides show large and characteristic deviations in the profiles with oil load as well as in normal nutrition. For males (n = 10) in contrast to women (n = 10) larger variations in the TGL-profiles are found. According to the size of their TGL deviation the test persons can be divided into 4 groups (A to D), which at the same time possess clearly gradated HDLC levels. In group A (high HDL cholesterol) scarcely variations in the TGL profile (without and with oil load) are recognizable. Group D (relatively low HDL cholesterol) shows high peaks in the TGL curves (under oil load of more than 4 mmol/l). After a dietary and regadrin treatment of 6 months the HLP patient shows a remarkable improvement in the TGL and HDLC profile. The results are discussed from the point of view of the pathogenesis of arteriosclerosis and in this case particularly the role of the HDL cholesterol is concerned.

[Determination of HDL-cholesterol].

For the clinical practice methods of the determination of HDL-cholesterol made their way which are based on the precipitation of apolipoprotein-B-containing lipoproteins and a determination of cholesterol following. The expensive methods of the ultracentrifugation serve as reference methods. The most-spread precipitation techniques (heparin/MCl2, dextran sulphate/CaCl2 or MgCl2 photungstic acid/MgCl2) are comparatively observed with regard to their effectiveness, practicability and methodical and technical conditions (influence of the concentration of the precipitation reagents, pH-value, temperature, incubation and centrifugation conditions). Results of own investigations as well as data from literature are presented to the problem of the harmonization of the cholesterol determination with the precipitation technique. According to the opinion of the authors for the enzymatic determination of cholesterol by means of the CHOD-PAP-method the phosphotungstic acid precipitation well stood the test, whereas for the chemical determination of cholesterol after Liebermann-Burchard in manual or automatized works the precipitation by means of dextran sulphate/CaCl2 (40 g/l, 2.0 mol/l) is to be recommended. The superabundant precipitations with phosphotungstic acid and dextran sulphate/MgCl2 (20 g/l, 2.0 mol/l) achieve higher results in Liebermann-Burchard's reaction likely on account of interferences.

Ruthenium hexammine trichloride (RHT)-mediated interaction between plasmalemmal components and pericellular matrix proteoglycans is responsible for the preservation of chondrocytic plasma membranes in situ during cartilage fixation.

Treatment of cartilage tissue with the cationic dye ruthenium hexammine trichloride (RHT) prior to fixation has been shown to prevent the detachment of chondrocytic plasmalemmata from the pericellular matrix and the aqueous extraction of proteoglycans during the subsequent fixation procedures. However, plasmalemmal rupture is prevented only by the simultaneous addition of RHT and the dialdehydic fixative glutaraldehyde. It is proposed that RHT forms an electrostatic cross-linkage between anionic components within the chondrocytic plasmalemma and proteoglycans of the pericellular matrix; experimental support for this hypothesis is presented. The precise nature of the plasmalemmal components with which RHT interacts is unknown. However, since their anionic properties are apparently lost following treatment with chondroitinase ABC, it seems likely that they represent chondroitin sulfate groups of membrane intercalated proteoglycans.

[Effects of cimetidine and ranitidine on blood coagulation].

The influence of cimetidine and ranitidine on blood coagulation and plasmatic fibrinolysis were assessed in vitro and in vivo. After an intravenous bolus of 200 mg cimetidine a marked prolongation of plasmatic thrombin time was observed, which however did not occur after 50 mg of ranitidine. In vitro addition of cimetidine (0.12 mg/ml) and ranitidine (0.06 mg/ml) again led to prolongation of plasmatic thrombin time only after cimetidine, reaching double the initial value. This requires application of cimetidine in acute intestinal haemorrhage not as a bolus, but only by drip infusion. Continuous application of 600 mg over 12 hours did not result in changes of coagulation values. In contrast, ranitidine may be administered as a bolus. Bolus injections of 50 to 100 mg did not result in any prolongation of plasmatic thrombin time. The antifibrinolytic effect of both H2-receptor antagonists was investigated in the streptokinase-induced fibrinolysis spindle of the thrombelastogram. Clear-cut antifibrinolytic properties were not demonstrable for either preparation with this method.

[Trace elements and lipid parameters in arteriosclerotic circulatory disorders by polyene fatty acids].

In 106 patients with peripheral arteriosclerotic obturating disease low values of serum magnesium as well as an increase of the relation of vasoaggressive to vasoprotective lipoproteins could be established. Under a polyene fatty acid diet (40 ml sunflower oil or linseed oil a day) the serum copper values significantly decreased, whereas of the indices of arteriosclerosis only in the linseed oil group of apolipoprotein-B/A-quotient, in the sunflower oil group the triglyceride level decreased statistically significantly. Therefore the favourable influences of physical conditioning as well as of pharmacotherapy on the disturbed lipoprotein metabolism of patients with vascular diseases should additionally be used.

Is pregnancy-associated plasma protein A a tumor marker?

Pregnancy-associated plasma protein A (PAPP-A) was measured in 159 patients with various nontrophoblastic tumors. In three patients with trophoblastic tumors, PAPP-A and human chorionic gonadotropin (hCG) were measured before operation and weekly thereafter. Circulating levels of PAPP-A in patients with nontrophoblastic tumors were not significantly different from the levels in the matched control groups. Circulating levels of PAPP-A and hCG were significantly elevated preoperatively in patients with trophoblastic tumors, whereas the postoperative concentrations of both proteins gradually decreased. The conclusion is that, probably, PAPP-A cannot be considered to be a marker for nontrophoblastic disease, and that measurements of PAPP-A in patients with trophoblastic cancers do not provide any advantage over measurements of hCG.

[The effects of an antiprogesterone steroid in women: interruption of the menstrual cycle and of early pregnancy].

RU-486 is a steroid which possesses a great affinity for the progesterone receptor, does not have a progesterone activity, but is indeed a strong antagonist of progesterone effects in animals. Oral administration induces the interruption of the luteal phase of the menstrual cycle and that of early pregnancy in women. Its mode of administration and its properties enable us to envisage a new methodology for menstrual cycle regulation and human birth control.

[Measurement of alanine aminotransferase and aspartate aminotransferase activity with a 2-channel automatic flow analyser].

A flow automaton for the estimation of the enzyme activities of the ALAT and ASAT is described: 1. In precision and exactness the automaton achieves results comparable to the manual UV-test. The VK are clearly below 5%. 2. Sera with enzyme activities greater than 200 U/l are diluted with human albumin solution (20 g/l) 1 : 5 or 1 : 10. 3. The speed of the analysis with simultaneous estimation of the activity of the ALAT and ASAT from one test is 40 tests/h. 4. The technological disadvantages of the colour test regarding the kinetic measurement are vastly evaded by the selected device parameters and conditions.

[Modification of special blood-level forming lipid metabolites by long-distance running in men 20-60 years of age].

The lipid metabolism and the lipoprotein metabolism was examined in 44 long-distance runners (people's athletics) and a group of 65 untrained persons as well as in 32 runners before and immediately after a 38-km and a 40-km-run, respectively. High density lipoprotein cholesterol (HDLC) was significantly increased in the runners, whereas very low density lipoprotein cholesterol (VLDLC) was found decreased. The quotient LDLC/HDLC (quotient of arteriosclerosis) is clearly decreased in runners. Total cholesterol and low density lipoprotein cholesterol (LDLC) of runners (people's athletics) and untrained persons were statistically not different. The post-heparin-lipase (pHL) and the rate of lipolysis pHL appeared clearly increased in persons participating in people's athletics. There were parallelities between the lipid values and the disc-electrophoretically established percental proportions of lipoproteins. The proportion of alpha-lipoprotein (Lp) of the runners was increased, the beta-Lp unchanged and the pre-beta-Lp decreased. The increase of the alpha-Lp was concomitant with a shift of the spectre of the alpha-Lp-subfractions. After the running (38 and 40 km) the triglycerides and VLDLC were increased, the pHL-activity and the rate of lipolysis pHL were decreased. In the other cholesterol parameters no significant shifts appeared after load. While Apo A does not change under the influence of the running, like HDLC, Apo B clearly increased. The results were discussed, taking into consideration the results of other authors.

Tissue and plasma concentrations of pregnancy-associated plasma protein-A (PAPP-A): comparison with other fetoplacental products.

Human placental lactogen (hPL), alpha fetoprotein (AFP), prolactin (PRL) and pregnancy-associated plasma protein-A (PAPP-A) were measured by radioimmunoassay in plasma, in homogenates of trophoblast, decidua, chorion, amnion and in amniotic fluid from 10 patients after non-complicated term delivery. Plasma samples and homogenates of trophoblast and decidua were also collected from 10 patients undergoing surgical termination of pregnancy between 7 and 12 weeks gestation. In addition, plasma and endometrial samples from 10 patients undergoing hysterectomy for other indications than malignancy were analysed for comparison. The highest tissue concentrations of hPL, AFP and PRL corresponded in each case to the known site of synthesis. For PAPP-A the highest concentration was found in maternal plasma at term [238.8 +/- 75.6 (SEM) micrograms/ml]. The highest tissue concentration was found at term in the decidua (57.0 +/- 2.0 micrograms/g), more than three times higher than that in the trophoblast (16.9 +/- 5.4 micrograms/g). The concentrations of PAPP-A in endometrial samples from non-pregnant women (1.9 +/- 0.6 micrograms/g) was 40 times higher than that in the corresponding plasma samples (0.05 +/- 0.02 microgram/ml). These observations point to the decidua as a possible source of PAPP-A.
