[Fatal pulmonary embolism after lysis therapy in post-traumatic renal vein thrombosis].

A 30-year-old man was hospitalized because of increasing dyspnoea for 4 weeks. Chest X-ray demonstrated an infiltrate in the right upper lobe and enlargement of the central pulmonary arteries. Lung perfusion scintigraphy revealed, typical of embolism, absent perfusion of the entire right upper lobe, as well as segmental embolism in the left upper and basal lobes. Phlebography of the legs and pelvis was unremarkable. Intravenous heparin treatment was begun (initially 1,250 IU/h, then dosage adjusted according to the partial thromboplastin time). Nonetheless the patient's condition deteriorated the next day and the respiratory failure increased (pO2 61 mm Hg despite oxygen supply). Streptokinase was then infused in ultra-high dosage, 9 million units over 6 hours. But the patient died of cardiocirculatory failure 4 hours after the streptokinase infusion had been finished. Autopsy revealed fulminant recurrent pulmonary embolism with occlusion of the right main pulmonary artery. The emboli had their origin in renal vein thrombosis extending into the inferior vena cava, which had probably been caused by slight trauma to the flank during a game of squash 6 weeks previously.

[Familial hypertrophic obstructive cardiomyopathy].

The diagnosis of hypertrophic obstructive cardiomyopathy in a 50-year-old man was confirmed by left-heart catheterization and echocardiography, the diagnosis having first been made some years earlier. Both his brother and his children have the same disease. The patient had several cardioversions for recurrent atrial fibrillation with a rapid ventricular rate. Because conservative treatment failed to give a satisfactory result, transaortic myotomy and myectomy were performed. Clinical examination eight months postoperatively showed that the patient's general condition had markedly improved, there was sinus rhythm, and the signs of left ventricular hypertrophy had regressed.

Long term treatment of Huntington disease with L-glutamate and pyridoxine.

Decreased levels of gamma aminobutyric acid (GABA) and its synthetic enzyme, glutamic acid decarboxylase, have been found in the brains of patients with Huntington disease. In an attempt to augment GABA-mediated neurotransmission, daily doses of 25 gm of L-glutamate (the substrate for glutamic acid decarboxylase) and 500 mg of pyridoxine, its cofactor, were given to five patients with Huntington disease. This regimen was continued for 2 years. Assessment of motor and behavioral function indicated no improvement on this regimen.

Quantitative extraction of morphologic cell parameters from the diffraction pattern.

A new technique for the fast and quantitative extraction of cell parameters from diffraction patterns was applied to cervical gynecologic material. Based on radial scans through the diffraction pattern, this technique permits the simultaneous determination of the nuclear and cytoplasmic diameter by Fourier analysis of the radial scanning signals after compensation for the intensity falloff by an amplification which is proportional to the third power of the radial position in the diffraction plane. In addition, a correlation between the angle of the highest amplitude of the scanning signals and the coarseness of microstructures in the cell was observed. Illustrative examples of measurements on exfoliated cells of different type are presented.

[Comparative morphological studies on the vascular systems of testes in cattle, swine, horse and dog under functional conditions].

The metacrylate and latex corrosion techniques were used to establish that the vascular system of testes is based on one coherent principle in common domestic mammals. The cone-shaped Plexus pampiniformis consists of numerous venous rami, between 0.25 mm and 1.0 mm in thickness and forming a dense vascular network, which practically encase the spiral-shaped A. spermatica interna (cooling coil principle). The testicular veins and arteries in the Tunica albuginea constitute a somewhat voluminous layer of vessels for dissipation of heat, with rami branching off radially into the testicular parenchyma. Most of the artieral rami with radial penetration of the testicular parenchyma turn towards the surface in the mediastinum testis for three-shape ramification. The vascular rami are characterised by countless meanders, primarily for temperature control, pulse flattening for more regular and even blood flow, and blood reflex pumping.
