Septic atrial thrombosis. A potentially lethal complication of Broviac catheters in infants.
Even though the use of Silastic catheters for long-term parenteral nutrition has decreased the incidence of catheter-related thrombosis and infection, these serious complications still occur. We describe three infants with septic atrial thrombosis secondary to use of Broviac catheters. One of the infants had pulmonary embolism and Budd-Chiari syndrome. The use of echocardiography in the diagnosis of this potentially fatal complication and aggressive treatment--which included open heart removal of thrombi--are discussed. The three infants recovered fully from this complication.

Effect of cimetidine on healing and surgical treatment of gastric ulcers.
In this retrospective study, a cohort of gastric ulcer patients treated with antacids was compared with a well-matched group of patients treated with a combination of antacids and cimetidine. Cimetidine combined with antacids enhanced gastric ulcer healing, thereby significantly reducing the number of gastric ulcer operations. Cimetidine appears to act by providing a less acidic environment in which gastric ulcers can heal. The exact mechanism of cimetidine's action on gastric ulcers is not clear. Nonprophylactic administration of cimetidine apparently has no effect on the recurrence rate of gastric ulcers.

Effect of neostigmine and pyridostigmine on the plasma cholinesterase activity.
The effect of neostigmine 0.05 mg kg-1 or pyridostigmine 0.25 mg kg-1 on serum cholinesterase activity was investigated in 20 adult patients undergoing elective surgery. Both drugs produced marked depression of enzymatic activity. The maximal depression was observed in samples taken 5 min after injection. The maximal percentage depression of enzymatic activity was not significantly different in the two drug groups. However, at 30-60 min after injection, the degree of depression was less in the neostigmine group. This may be attributed to the different plasma clearances of neostigmine and pyridostigmine.
