Posterior compartment syndrome of the calf resulting from misdiagnosis of a rupture of the medial head of the gastrocnemius.

Rupture of the medial head of the gastrocnemius muscle has frequently been confused with thrombophlebitis. The distinction between these two entities is important and may be difficult to ascertain. Misdiagnosis may result in unnecessary complications. An illustrative case, in which venography, MRI, and compartment pressure measurements were diagnostic, is presented.

Management of severe hand trauma with a mini external fixateur.

Maintenance of length, alignment, and mobility while affording ready access for wound care has traditionally been a problem in the management of severe hand trauma. However, miniaturization of existing components of external fixation has provided a significant step in the solution of this problem. Twenty-eight fractures in severely traumatized hands in 24 patients have been managed using a Hoffman mini external fixateur. The patients' mean age was 26.5 years. The dominant hand was involved 60% of the time, and 80% of the population was male. Most fractures (80%) were open injuries with marked comminution, often intra-articular, frequently the result of gunshot wounds (60%). Mini external fixation has afforded proper wound care and maintenance of desired alignment. Complications have been few, while improved results (alignment, range of motion, strength, fracture union), have been demonstrated. Our surgical technique, results, and some illustrative cases are presented.

Post-traumatic arthrosis of the index carpometacarpal joint. A rationale for treatment and report of two cases.

Although a relatively rare entity, post-traumatic arthrosis of the carpometacarpal joint of the index finger can be a disabling problem. Its diagnosis must be differentiated from other sources of local pathology. A rationale to the diagnosis and treatment of this entity is presented, demonstrating the usefulness of test injections of xylocaine, an intra-articular injection of a steroid preparation when a relatively congruous joint exists, and arthrodesis of the joint when an incongruous joint is encountered.

Bilateral intrinsic compartment syndrome of the hands in an 18-month-old child.

Intrinsic compartment syndrome of the hands is a relatively rare phenomenon. When it does occur, prompt recognition and early treatment are essential for a favorable outcome. The bilateral occurrence of this phenomenon is extremely rare; to the best of our knowledge it has not previously been described in a young child. We present a case of bilateral occurrence involving all intrinsic compartments of both hands in an 18-month-old child. Early diagnosis, prompt surgical decompression, and early postoperative rehabilitation provided a favorable, functional outcome.

Triplane fractures of the adolescent ankle--a report of three cases.

Three cases of triplane fractures of the distal end of the tibia are presented. There are usually two or three fragments and it occurs in adolescents in whom there is little growth potential left; hence there is little growth disturbance despite premature closure of the physis. The key to successful treatment is to assess the degree of articular deformity and to restore articular congruity.

The Jones' fracture in the nonathlete.

A review of the literature concerning Jones' fractures yields a relatively recent description of the entity as a chronic condition frequently arising as a stress fracture in young athletes and not infrequently requiring surgical correction. Over a 5-year period, we have treated 53 patients with Jones' fracture. All patients were nonathletes. Each described, as Jones did originally, an acute injury. Thirty-six patients were followed to union. There were two delayed unions and no nonunions.

Perforation of the rectum by a Smith-Petersen nail.

The use of internal fixation devices in the treatment of hip fractures has proved to be a major advance in modern surgical therapy. However, as with all surgical procedures, many complications of internal fixation have been described (3, 5, 11). Perforation of the rectum by an internal fixation device has only once to our knowledge been previously reported in the literature. This paper is a report of such a case.

Acute calcium deposit in the hand of an 11-year-old girl.

A case of acute calcific deposit in the hand of an 11-year-old girl is presented. This is the first report of this entity in a child. The orthopedic surgeon and pediatrician are alerted to its existence and occurrence in the pediatric population.

Medial triplane fracture delineated by computerized axial tomography.

Triplane fractures of the adolescent ankle are relatively rare but should be suspected when a forceful rotatory injury is sustained. A case of the extremely rare medial triplane variant is presented. The importance of a careful history and physical examination and precise radiographic demonstration with computerized axial tomography is demonstrated.
