Growth hormone secretory dynamics over the menstrual cycle.

Whether GH secretion in women varies over the menstrual cycle is uncertain. Previous investigations have led to conflicting conclusions; some studies suggested that there is an estrogen mediated rise in GH during the periovulatory (PO) and luteal (L) phases whereas others indicated no change in GH axis over the cycle. Differences in conclusions could relate to heterogeneity of the study populations, GH sampling paradigms or sensitivity of the GH assays used. In order to investigate whether GH secretion varied over the cycle, 24-h GH profiles using every 10-min sampling were obtained in 6 ovulatory women during the early follicular (EF), PO and L phases of the cycle. The TSH response to TRH, GH response to GRH and fasting plasma IGF-I were measured on each occasion. There was a trend toward higher integrated GH concentration (IGHC) during the PO phase, although this difference was not statistically significant (3284+/-721 vs 4542+/-872 vs 4071+/-699 microg/min/L; EF vs PO vs L; p=0.09). Similarly, deconvolution estimated GH secretion did not vary over the cycle (p=0.56). There were no differences in GH pulse amplitude or frequency. There were no correlations between IGHC and sex steroids. Serum IGF-I was constant over the cycle (272+/-38 vs 277+/-31 vs 265+/-38 microg/L; p=0.89). The TSH response to TRH and GH response to GRH did not vary over the cycle. We concluded that the effect of changes in the ovarian steroid milieu on the GH axis during spontaneous menstrual cycles is minimal.

[Low expression of nm23-H1 mRNA and its protein in breast cancer].

OBJECTIVE: To explore the expression of nm23-H1 mRNA and its protein in breast cancer and its correlation to the biologic behavior. METHODS: RT-PCR and immunohistochemical method were employed to detect the expression of nm23-H1 mRNA and its protein in 58 breast cancer tissues. RESULTS: Low level expression of nm23-H1 mRNA and protein was shown in 44.8% and 48.2% of examined cases respectively. The low expression of nm23-H1 mRNA was correlated, but not always parallel to the protein expression. The low level expression of nm23-H1 mRNA or the protein was significantly associated with lymphatic metastasis of the cancer. CONCLUSION: Examination of nm23-H1 expression may be of significance in evaluating metastatic potentiality and prognosis of breast cancer.

[Study on the p53 gene mutation and microsatellite instability of gastric carcinoma].

OBJECTIVE: To observe the relationship between p53 gene mutation and microsatellite instability (MSI) of the preoperative staging of gastric carcinoma by endoscopic ultrasonography(EUS). METHODS: A total of 73 cases of gastric carcinoma were taken for the preoperative staging with EUS. Silver staining PCR-SSCP method was used to detect mutations in exons 5, 6, 7, 8 of p53 gene and MSI at 4 loci on chromosomes 2, 5, 17 in the 73 paraffin-embedded biopsy specimens, and the relationship between them was studied further. RESULTS: The overall mutated rate of p53 gene was 54.8%, with 6.8%, 15.1%, 19.2% and 13.7% in exons 5 to 8, respectively. Analysis of the relation of the mutation with the preoperative staging by EUS showed that it was significantly higher in T3(64.3%) and T4(67.9%) than in T1(0) or T2(25.0%)(P<0.05), and it was 67.6% in cases with lymph node metastasis, which was significantly higher than that without metastasis (41.7%)(P<0.05). The overall detective rate of MSI was 37.0%. The detective rate of MSI had no relation with the depth of invasion, which was 40.0%, 33.3%, 39.3% and 34.5% at T1 to T4 stages (P<0.05), respectively, but it was significantly higher in cases with lymph node metastasis(51.4%) than those without metastasis(22.2%)(P<0.05). Although the detective rate of MSI showed an increasing trend in the mutated cases of p53 gene as compared with the no mutation cases, there was no relationship between MSI and p53 gene mutation (P<0.05). CONCLUSION: These results suggest that the gene mutation of p53 and MSI may represent the different mechanisms of carcinogenesis. They can reflect the cytobiologic malignant behavior of gastric carcinoma in varying degrees, which may be of reference significance in analysing the prognosis of the patients in clinics.

Intense noise-induced apoptosis in hair cells of guinea pig cochleae.

Cells can die by two distinct pathways: apoptosis and necrosis. To explore whether intense noise can induce hair cell (HC) death via the apoptotic pathway, we systematically examined morphological changes in guinea pig cochlear HC nuclei stained with Hoechst 33342, a fluorescent dye specifically labelling the nuclear DNA. A narrow band noise centred at 4 kHz with levels at 110 dB, 115 dB or 120 dB (SPL) was applied for 4 h and the exposed cochleae were collected at various intervals (3 h, 3 or 14 days) after the noise exposure. Auditory function was monitored by measuring thresholds of auditory brain stem responses. In the noise-damaged cochleae, there were two major types of nuclear changes, nuclear condensation appeared as karyorrhexis or karyopyknosis and nuclear swelling. Karyorrhexis and karyopyknosis predominately appeared in the severely damaged cochlear region in the animals exposed to 120 dB noise and examined 3 h after the noise exposure. In contrast, swelling of nuclei occurred in all of the noise-exposed cochleae, and was the feature change in the animals exposed to 110 and 115 dB noise. This pathological change persisted at least for 14 days after the noise exposure. The typical changes of karyorrhexis and karyopyknosis noted in the animals exposed to 120 dB noise were morphologically similar to those nuclear changes described in previous studies for apoptosis, suggesting that the apoptotic process may be involved in intense noise-induced HC death.

Retinal fascin: functional nature, subcellular distribution, and chromosomal localization.

PURPOSE: To investigate the functional properties, subcellular localization, and chromosomal location of retinal fascin. METHODS: Recombinant retinal fascin protein was prepared by using a baculovirus-insect expression system. Actin-binding and -bundling assays were performed with chick actin purified from skeletal muscle. Western blot analysis and immunohistochemistry were performed with a polyclonal antibody raised against bovine retinal fascin. A human retinal cDNA library was screened with an expressed sequence tag cDNA fragment. Chromosomal location was determined with fluorescent in situ hybridization. RESULTS: The actin-binding and actin-bundling activities of retinal fascin were demonstrated by high- and low-speed centrifugation assays. Formation of filamentous (F)-actin bundles by retinal fascin in vitro was also morphologically confirmed by fluorescence microscopy and electron microscopy. Immunohistochemical analysis revealed that retinal fascin protein was localized specifically in the outer and inner segments of the photoreceptor cells in the retina. Two splicing variants of human retinal fascin cDNA were also located. One clone encoded 492 amino acids, and the other encoded 516 amino acids. The gene encoding retinal fascin was localized to human chromosome 17, region q24 -25. CONCLUSIONS: These results suggest that retinal fascin may play a role in formation of unique morphologic structures of the photoreceptor cells and is a candidate gene for retinal degenerative disorders.

Hydroquinone, a benzene metabolite, increases the level of aneusomy of chromosomes 7 and 8 in human CD34-positive blood progenitor cells.

Benzene is an established human carcinogen, producing leukemia, hematotoxicity and perhaps lymphoma. Its carcinogenicity is most likely dependent upon its conversion to phenol and hydroquinone, the latter being oxidized to the highly toxic 1,4-benzoquinone in the bone marrow. Exposure of human lymphocytes and cell lines to hydroquinone has previously been shown to cause various forms of genetic damage, including aneusomy and the loss and gain of chromosomes. However, the target cells for leukemogenesis are the pluripotent stem cells or early progenitor cells which carry the CD34 antigen (CD34(+) cells). In this study, human cord blood, which is particularly rich in CD34(+) cells, was exposed to hydroquinone for 72 h in a medium that favored CD34(+) cell survival and growth. CD34(+) and CD34(-) cells were then isolated. Fluorescence in situ hybridization was employed to determine the level of aneusomy of chromosomes 7 and 8 in both cell types. CD34(+) cells were generally more susceptible to aneusomy induction by hydroquinone than CD34(-) cells. Increased trisomy and monosomy of chromosomes 7 and 8 were observed in CD34(+) cells (P(trend) < 0.001), whereas in CD34(-) cells only an increased level of monosomy 7 was detected (P(trend) = 0.002). Particularly striking effects of hydroquinone were observed in CD34(+) cells on monosomy 7 and trisomy 8, two common clonal aberrations found in myeloid leukemias, suggesting that these aneusomies produced by hydroquinone in CD34(+) cells play a role in benzene-induced leukemogenesis.

A clinical investigation on zhi ling tang for treatment of senile dementia.

Zhi Ling Tang (ZLT [symbol: see text], a TCM prescription designed for replenishing essence, supplementing marrow, invigorating qi, warming yang, removing blood stasis and phlegm, tonifying the brain, and invigorating mental activity) was used in 32 cases of senile dementia (SD). After treatment, the levels of serum cholesterol (Tch), triglyceride (TG), and plasma lipid peroxides (LPO) were lowered; the content of high density lipoprotein (HDL) and the activity of superoxide dismutase (SOD) in RBC significantly elevated, the cerebral blood flow was increased; latent period of P300 and P3 waves was shortened while the amplitude of P3 elevated; and topographic electroencephalogram, revised Hasegawa dementia scales (HDS) scores (P < 0.05-0.01) and clinical symptoms were improved. The total effective rate was 81.3%, indicating that ZLT exhibits relatively good therapeutic effects for treating senile dementia.

[Investigation of immunization status in children of floating population in Henan Province].

OBJECTIVE: To understand the immunization status in children of floating population in Henan Province. METHODS: Floating people was divided into four types based on their geographical distribution and characteristics of settlement and randomly sampled from each type in 17 cities and prefectures of Henan Province. Immunization status was investigated in 2,141 children from floating population by household visits. RESULTS: Immunization coverage in children of floating population was less than 60% and coverage of complete immunization with four kinds of vaccine was only 32%, far lower than that in local children. Coverage for children in large-size bazaars and construction sites and from peddlers' families was the lowest, 20.8% and 25.1%, respectively, which indicated that immunization for children in floating population had restricted the in-depth development of Expanded Program on Immunization (EPI) in Henan Province. The main reasons for that were floating population had no stable residence and job. The key problems in immunization for floating population at present were their unawareness of EPI, lack of initiative and certain defects in EPI service existed in some areas. CONCLUSION: It is necessary to strengthen publicity for immunization, to improve the mode of EPI service and to enhance the management in immunization for floating population.

Hemostatic abnormalities associated with acute promyelocytic leukemia and corrective effects of all-trans-retinoic acid or arsenic trioxide treatment.

OBJECTIVE: To study in vivo effect of all-trans-retinoic acid (ATRA) or arsenic trioxide (As2O3) on the expression of tissue factor (TF) and the hemostatic disorders, a series of parameters were measured in bone marrow blasts and plasma from acute promyelocytic leukemia (APL) patients. METHODS: The plasma variables were measured by ELISA or chromogenic study. The TF transcription was assessed using reverse transcription-polymerase chain reaction technique (RT-PCR). RESULTS: The blast cell procoagulant activity (PCA), TF antigen of APL cell lysates, as well as the transcription of APL TF mRNA elevated at diagnosis, were reduced after ATRA or As2O3 therapy. The plasma level of platelet alpha-granular membrane protein-140, soluble fibrinomonomer complex, thrombomodulin, tissue plasminogen activator and D-dimer significantly increased, fibrinogen, antigen level of protein C, plasminogen, alpha 2-plasminogen inhibitor and plasminogen activator inhibitor decreased at diagnosis, were restored to normal after complete remission but protein C activity and protein S remained elevated in ATRA group. CONCLUSIONS: There existed activation of platelets and consumption of anticoagulants as well as activation of coagulation and fibrinolytic system before treatment. Both ATRA and As2O3 therapy down-regulated the expression of TF mRNA, decreased the PCA and TF level in APL cells, inhibited coagulation activation, secondary hyperfibrinolysis and recorrected other hemostatic abnormalities, thus greatly improved the bleeding symptom in early stage of the treatment.

[Cloning and antitumor activity of angiogenesis inhibitor HIAF-1].

OBJECTIVE: A new angiogenesis inhibitor HIAF-1 (human inhibitor angiogenesis factor-1) was cloned, expressed in E. coli and its antitumor activity was studied. METHODS: HIAF-1 was amplified by RT-PCR from human fetal liver tissue, then cloned into pET30a(+) vector and expressed in E. coli BL21:DE3 after sequencing. In vitro endothelial proliferation inhibiting activity of HIAF-1 was examined by MTT method. In vivo antitumor activity was studied in a murine model of IVTA2MA-891. RESULTS: HIAF-1 was first cloned from human fetal liver tissue. Sequence analysis of the inhibitor revealed identity to a c-terminal fragment of human collagen XVIII. HIAF-1 was effectively expressed in E. coli with a yield of 88 mg/L. Recombinant HIAF-1 protein could inhibit endothelial cell proliferation in vitro with an IC50 value of 7.5 micrograms/ml. In vivo studies showed that HIAF-1 inhibited growth rate of the primary tumor by 46.6% and metastasis by 68.9%. CONCLUSION: The cloned and the bio-engineering product of HIAF-1 is an angiogenesis inhibitor, capable of inhibiting tumor growth and metastasis.

[Clinical and experimental studies on clearing away stomach-heat purging fire and nourishing yin principle in treating patients of recurrent aphthae].

OBJECTIVE: To observe the therapeutic effect of Kouchuangping granule (KCP) on recurrent aphthae and explore its mechanism. METHODS: One hundred and forty-eight patients treated with KCP were observed and compared with 49 patients treated with tripterygii multiglycoside tablet as control in controlling relapse, prolonging intermittent period, improving local symptom and the influence on humoral and cellular immunity. In animal experiment, the effects of KCP on immunologic function, anti-exudation, anti-inflammation and analgesia were studied. RESULTS: The clinical markedly effective and cure rate of the KCP group was 62.8% and the total effective rate was 89.9%, which were significantly higher than those of the control group (40.8% and 75.5%, P < 0.01, P < 0.05). In the KCP group, the intermittent period of aphthous ulcer recurrence was prolonged to 6 months and the local symptoms were markedly improved, the differences were significant as compared with those before treatment and those in the control group (P < 0.01 or 0.05). The KCP showed effects in suppressing humoral immunity and enhancing cellular immunity. Results of experimental study showed that KCP could inhibit the supernormal reaction of cellular immunity to prevent and treat the hypersensitivity caused erosion and ulceration of mucosae, inhibit capillary permeability and WBC migration and abate the 5-hydroxytryptamine (5-HT) and histamine induced local swelling and pain. CONCLUSION: KCP has a significant effect in treating recurrent aphthae, the mechanism might be related with its effect on immunoregulation, anti-inflammation and anti-exudation.

[Effects of all-trans retinoic acid and arsenic trioxide on tissue factor expression of acute promyelocytic leukemia cells].

OBJECTIVE: To investigate the effects of all-trans retinoic acid (ATRA) and arsenic trioxide (As(2)O(3)) on both tissue factor (TF) expression and procoagulant activity (PCA) of acute promyelocytic leukemia (APL) cells in vivo and in vitro. METHODS: The PCA of APL blasts freshly isolated from the APL patients treated with ATRA or As(2)O(3) was detected using a one-stage clotting assay; the TF antigen was detected by ELISA and TF mRNA by RT-PCR. The maturation sensitivity (NB4) or resistant subclones (NB4-R1) of the promyelocytic NB4 cell line as well as U937 cells transfected with pMSCV-PML-RARa treated with or without ATRA or As(2)O(3) were also examined. RESULTS: Both ATRA and As(2)O(3) time-dependently down-regulated the TF antigen, its mRNA transcription and membrane PCA of APL cells in vivo and in vitro. The TF antigen level in PML-RARa(+) U937 cells was significantly higher than that in U937 cells transfected with retrovirus vector (890 pg/8 x 10(5) +/- 80 pg/8 x 10(5) cell and 728 pg/8 x 10(5) +/- 86 pg/8 x 10(5) cell, respectively). Both ATRA and As(2)O(3) down-regulated the TF antigen of the U937 cells transfected with or without PML-RARa. CONCLUSION: Tissue factor expression and PCA of APL cells can be down-regulated by ATRA and As(2)O(3). By down-regulating the TF expression, As(2)O(3) might also be used to improve the DIC-related hemorrhage of APL. It is also suggested that elevated TF antigen in the PML-RARa(+) U937 cells may be related with the fusion protein PML-RARa, while the down-regulation effect of ATRA and As(2)O(3) on the TF expression of U937 cells might not be involved in the fusion protein.

[Endoluminal stent-graft repair of aortic aneurysms].

OBJECTIVE: To study the clinical value of the treatment of aortic aneurysms with endovascular stent-graft prosthesis. METHODS: After a unilateral surgical arteriotomy, Talent, Vanguard and Chinese stent-graft were advanced through the femoral arteries and placed in the proper position of the aneurysm sac under X-ray fluoroscopic guidance. RESULTS: Four patients with descending aneurysm, 1 patient with abdominal aortic aneurysm (AAA) involving the renal artery, superior mesenteric artery and celiac artery, and 1 patient with right common iliac artery aneurysm received straight stent-grafts. Five patients with infrarenal AAA received bifurcated stent-grafts. CT and MRA were performed during a follow-up of 3 - 19 months. Aortic aneurysms were completely excluded from the circulation. Five patients prolonged fever was noted and 1 patient was found to have a leakage 3 months later, and hemiplegia in 1 after procedure. CONCLUSIONS: Endovascular treatment of aortic aneurysm is technically feasible and can effectively exclude aortic aneurysms from the circulation. Endoluminal repair may serve as an interventional strategy to treat aneurysm, especially in patients at high surgical risk, but long term effect needs further study.

[Abdominal aortic aneurysm treated by endovascular stent-graft and conventional surgical repair: a comparison].

OBJECTIVE: To evaluate the feasibility of abdominal aortic aneurysm (AAA) treated by endovascular stent-graft. METHODS: The clinical data of 52 patients with AAA treated by endovascular stent-graft (n = 20) and conventional surgical repair (n = 32) were analysed retrospectively. Patients conditions, operative hours, blood loss, function recovery and complications were compared. RESULTS: No significant difference was observed in sex, age, other disease, anesthesia risk category, aneurysm type, aneurysm diameter, technical success rate, and mortality rate between the groups (P > 0.05). The patients who underwent intraluminal treatment had significant reductions in operative time, blood loss, intensive care unit and hospitalization. But there was a high complication rate in the endovascular stent-graft group. All complications were connected with the interventional technique; endo-leak was the chief complication after operation. CONCLUSIONS: Endovascular treatment has marked merits such as reduced trauma, short hospitalization stays, and early functional recovery. It is suitable for the patients who can't undergo open surgical repair. But the complications caused by this technique needs further study.

Relations of tumor suppressor gene p16 and RB to early diagnosis of lung cancer.

OBJECTIVES: To study the incidence and mechanism of the tumor suppressor gene p16 and RB inactivation in lung cancer, and to investigate their relations to biological characteristics, clinical pathological diagnosis and gene classification. METHODS: Immunohistochemistry, double in situ hybridization, PCR, PCR-SSCP, and sequencing were used to study the changes in the p16 and RB genes in 106 patients with lung cancer. The specimen of normal lung tissue from the patients with lung cancer and specimens of the 23 patients with benign lung diseases were studied in contrast to those of the same patients. RESULTS: The total expression rate of the p16 and RB genes in the lung cancer tissues of the 106 patients was lower than that in normal lung tissues and tissues of benign lung diseases and was specially related to histological type, metastasis, and clinical stage of lung cancer. In stage I and II lung cancer, the obvious inactivation rate of the tumor suppressor gene p16 or RB was 32.6% or 28.3%. The p16 inactivation appeared largely in non-small cell lung cancer (50.1%), and the RB inactivation largely in small cell lung cancer (88.2%). Homozygous deletion, methylation, and mutation were three mechanisms for the p16 gene inactivation. The deletion rate of the p16 exon 1 and/or exon 2 was 25.8% in the patients with non-small cell lung cancer. Fifteen patients (16.9%) showed methylation at the SmaI sites of CpG island of the p16 gene. PCR-SSCP and sequencing revealed p16 gene mutation in 9 of the 106 patients with lung cancer. CONCLUSIONS: The p16 and RB genes may play an important role in genesis and progression of lung cancer. The inactivation of the p16 or RB gene may be an early sign of lung canceration, that is important for the early diagnosis. A new gene classification model for lung cancer diagnosis would be set up based on the research of the p16 and RB genes.

[A comparison between latanoprost and timolol in treatment of patients with primary open-angle glaucoma and ocular hypertension].

OBJECTIVE: To evaluate the therapeutic value of latanoprost on glaucoma. METHODS: In an open-label fashion, multicenter, randomized control clinical trial, the efficacy and adverse drug reactions of topical application of 0.005% latanoprost once daily were compared with that of 0.5% timolol twice daily for 12 weeks in patients with open-angle glaucoma or ocular hypertension. RESULTS: The study included 128 patients (63 patients in latanoprost group and 65 patients in timolol group) and 117 patients remained at the end of the study (60 cases in latanoprost group and 57 cases in timolol group). Comparing 12 weeks with baseline diurnal intraocular pressure (IOP), the IOP reduction (mean +/- standard deviation) in latanoprost group was (7.5 +/- 0.3) mm Hg (1 mm Hg = 0.133 kPa) (32%, t = 22.73, P < 0.0001) greater than the reduction in timolol group (6.1 +/- 0.3) mm Hg (26%, t = 17.94, P < 0.0001), the difference between the two groups being significant (F = 9.54, P = 0.0026). Two patients treated with timolol and none treated with latanoprost were withdrawn from the study because of inadequate IOP control; 3 patients with latanoprost had foreign body sensation. In latanoprost group, there was one patient whose eyelashes became darker and longer at the last visit (the 12th week). No ocular and systemic adverse events related to the two drugs were found. CONCLUSION: It is demonstrated that 0.005% latanoprost topically applied once daily is well tolerated and more effective in reducing IOP than 0.5% timolol topically applied twice daily. Thus, latanoprost has the potential to be a new first-line antiglaucoma drug.

[Investigation on E. coli O157 in Fujian, China].

OBJECTIVES: To understand the distribution of Escherichia coli O157 (E. coli O157) in human beings, livestock and fowls in Fujian Province, and biological features of the isolated strains. METHODS: Fecal specimens from diarrheal patients, livestock and fowls and meat specimens in Putian and Fuzhou were collected for detection of E. coli O157 during 1997 - 1998. RESULTS: A total of 2,725 specimens were detected, and 76 strains of E. coli O157 were isolated from pigs, pigeons, cattle, chickens, ducks, et al., as well as from the diarrheal patients. These strains could be classified into three types according to their serological tests, including 33 strains of O157: H7, 21 strains of O157: NM, and 22 strains of O157: H?. Detecting rate of E. coli O157 from pigs was the highest. The isolated strains could cause diarrhea in human and obvious pathological changes in the intestines of mice, but could not produce cytotoxin Vero, or heat-stable or heat-liable enterotoxin LT or ST. CONCLUSIONS: There were three types of E. coli O157 in Fujian. But, pathogenic factors of O157: H7 and O157: NM were different from those reported abroad. It suggested that there possibly existed some new pathogenic factors.

[Effects of holothuria glycosaminoglycan on the expression of tissue factor and thrombomodulin in stimulated endothelial cells].

OBJECTIVE: To study the mechanism of antithrombotic effects of holothuria glycosaminoglycan (GAG). METHODS: Endothelial cells from human umbilical vein which were pre-induced with 1 mg/L lipopolysaccharide, were treated by GAG (1 mg/L, 5 mg/L, and 10 mg/L, respectively) and 5 mg/L heparin as a control for 6 hours. Procoagulant activity (PCA), the expressions of tissue factor (TF) antigen and thrombomodulin (TM) antigen and their mRNA transcriptions were investigated. RESULTS: GAG could down-regulate the expression of TF antigen and mRNA, up-regulate the expression of TM antigen and mRNA, down-regulate the PCA of stimulated endothelial cells. CONCLUSION: Down-regulating TF expression and up-regulating TM expression of stimulated endothelial cells may be one of the mechanisms of GAG antithrombosis.
