[Hypernatremia as a rare complication in change from lithium to valproate].

This paper is about a female patient who suffered from bipolar disorders for several decades. Due to a change in medication from lithium to valproic acid by infusion, the patient fell into a coma. Hypernatremia, which had developed unnoticed, was later discovered as the reason. This hypernatremia was due to renal diabetes insipidus stemming from the decades of lithium therapy, with accompanying polydipsia and polyuria, and also to the failure to give enough fluid to the somnolent patient.

Implementing the North Karelia Project in Scotland.

A comprehensive, determined and theory-based community programme can have a meaningful and positive effect on risk factors and life styles. In much the same way as the North Karelia Project was rolled out to cover the whole of Finland, there is no reason why our intervention schemes could not be rolled out to cover the whole of Scotland. The North Karelia Project proved that a major national demonstration programme can be a strong tool for favourable national development in chronic disease prevention and health promotion.

Stimulation of the thrombin receptor of human glomerular mesangial cells by Ser-Phe-Leu-Leu-Arg-Asn-Pro-Asn-Asp-Lys-Tyr-Glu-Pro-Phe peptide.

We have studied the effects of thrombin (alpha-thrombin) and Ser-Phe-Leu-Leu-Arg-Asn-Pro-Asn-Asp-Lys-Tyr-Glu-Pro-Phe (SFLL), a peptide agonist of the platelet thrombin receptor in cultured human mesangial cells, and find that SFLL can reproduce the biochemical and morphological effects of thrombin. Treatment of mesangial cells with cAMP-elevating agents causes fragmentation of stress fibers, loss of the vitronectin receptor from sites of focal adhesion, and produces a change in shape from a flat to a more arborized configuration. These effects are prevented by both thrombin and SFLL. Thrombin and SFLL also initiate biochemical signaling events in mesangial cells by stimulating the metabolism of phospholipids. Both thrombin and SFLL stimulate release of inositol phosphates from [3H]inositol-labeled cells, elevation of cytosolic calcium, the formation of [3H]myristic acid-labeled diacylglycerol, an increase in the mass of diacylglycerol, 32P incorporation into phospholipids, and release of unesterified [3H]arachidonic acid from cells prelabeled with [3H]arachidonic acid. When present together, the effects of SFLL and thrombin on diacylglycerol formation, arachidonic acid production, and inositol phosphate production were not additive. This suggested that SFLL and thrombin were acting on the same receptor. This was further supported by our observations that cells pretreated with SFLL and subsequently exposed to thrombin (or vice versa) did not show elevated cytosolic calcium. We also show that phospholipase D is activated by demonstrating production of radiolabeled phosphatidylethanol when cells are treated with SFLL in the presence of ethanol. These findings indicate that SFLL can be used to study the receptor-mediated effects of thrombin in mesangial cells, thereby avoiding thrombin's proteolytic actions.

[Modulation of granulocyte phagocytosis].

Non-specific complications are encountered relatively often in patients with below normal granulocytic phagocytosis rates. We tested vitamin C, transfer factor, gamma-globulin, BCG, PPD, UV irradiation of the subject's blood (UVB) and hyaluronidase with the aim of stimulating granulocytic phagocytosis with rice starch in autologous serum. In our experiments, only the addition of hyaluronidase caused any increase in phagocytotic activity. The in vitro experiments with hyaluronidase yielded promising results and will be followed up since they may be relevant to practical questions.

[The status of coagulum choledocholithotomy in choledochus revision].

By means of the coagulum choledocholithotomy the surgeon has an additional possibility to reduce the number of residual concrements. The indication must be exact. Multiple small not incrusted stones in the biliary duct system are most suitable for the application of the coagulum lithotomy.

[Clinical and prognostic aspects of multiple cancers].

The anamnestic data, course and prognosis for 76 patients with multiple carcinomas were statistically analyzed in a retrospective study undertaken jointly with the oncological working group. The importance of multidisciplinary care for this group of patients over a period exceeding five years must be emphasized. If the general status of a patient deteriorates, the doctor should always consider the possibility of a second or third carcinoma.

[Oncologic after-care of patients with prostate cancer].

The aim of the examinations is the proof of the clinical value of the granulocyte phagocytosis test in the after-care of the palliatively treated patients with a carcinoma of the prostate. On the basis of the analysis of the own patient and the observation of the course the routine control of the granulocyte phagocytosis is recommended, since unspecific complication are recognized earlier and thus may be treated better.

[Significance of preoperative prognosis].

The granulocyte phagocytosis test, Karnofsky index, and ASA standard were used in the context of 287 patients for general surgery, for the purpose of retrospective elucidation of the relationship between presurgical prognostication and postsurgical complications. Sensitivity, specificity, and "predictive values" were calculated. The highest information potential in terms of prognostication was recorded from objectively measurable granulocyte phagocytosis. Presurgical prognostication was found to be of clinical relevance only to surgical problem patients.

[Simultaneous investigations on granulocyte phagocytosis using rice starch and on granulocyte migration--a study in the preoperative definition of the defense situation of the organism].

Information which is more extensive than the unspecific value of the granulocytophagocytosis could not be found out by the simultaneous investigations of the granulocytophagocytosis and the granulocytomigration. The application of AHLG and tumor-associated antigens in the migration test system did not show specific results for the oncology. The granulocytomigration in a home-made modified Boyden chamber did not show any sexual distinctions. No statistical reliable relations could be found out between the rate of migration and ingestion of the granulocytes. The granulocytophagocytosis does not obtain a higher value for practice by the simultaneous definition of the migration.

[Modification of granulocyte function by lymphocytes].

Results of investigations are presented which are to show the impact of lymphocytes on granulocytophagocytosis . Lymphocytes in leukocyte suspensions are damaged by irradiation and by anti-human lymphocyte globulin. A slightly negative impairment of the phagocytic granulocyte function seems to be detectable. These findings are not unequivocal and not significant.

[Simultaneous studies of granulocyte phagocytosis, serum immunoglobulins, acute phase proteins and the 3d complement component in patients with malignant tumors].

In a clinical study granulocyte phagocytosis, serum levels of IgG and IgA and of the acute phase proteins ceruloplasmin, alpha-1-antitrypsin, transferrin, and of the 3rd component of complement (C3) are simultaneously determined in tumor patients and compared with healthy probands of the same age and with patients being under general surgical treatment. There was found a negative correlation between granulocyte phagocytosis and the IgG level and a positive correlation between phagocytosis and the C3 level in each group. Despite of some significant differences between all groups of probands with respect to the measured parameters no new essential information for clinical use could be received, thus the determination of granulocyte phagocytosis remains the only recommended method.

[Modification of granulocyte phagocytosis by extracts from tumor antigens].

The results with the granulocyte phagocytosis test adding various tumor antigen preparations are represented. In comparison with clinically healthy probands and patients having benign diseases there could not be proved any malignancy and organ specificity of granulocyte phagocytosis in patients with malignant tumors of different stages and various localization of organs.

Flow cytometric measurements of phagocytosis. I. A methodical and comparative study.

A flow cytometric technique is described for measurement of phagocytic activity in human leucocytes using FITC-labelled latex particles (0.77 micron). Cell suspensions separated from peripheral blood by dextran sedimentation were hemolyzed to avoid particle uptake by erythrocytes. The leucocytes were incubated for various time intervals with a standardized solution of 2 x 10(7) FITC-labelled latex particles per 2 x 10(5) leucocytes in 0.6 ml medium. Relative fluorescence per cell was measured on the basis of 10(5) cells, and a simple technique has been described to evaluate data characteristic for different steps of phagocytosis and to discriminate between phagocytizing and non-phagocytizing cells. In comparison, rice starch phagocytosis has been measured microscopically. A positive correlation between both methods has been found in 11 patients with different disease-related phagocytic capacity.

[In-vitro studies of granulocyte phagocytosis in patients with malignant tumors].

Investigations are presented about granulocyte phagocytosis with rice starch in patients with clinically curable malignant tumours and with those treated palliatively. A significantly reduced granulocyte efficiency of phagocytosis indicating a bad prognosis of the disease can be found in those patients being in the advanced tumour stage.

[The value of the granulocyte phagocytic test in oncology follow-up care].

This work represents the results of the granulocyte phagocytic test in tumor effected individuals by observing the course over a period up to three years. In tumor patients, which could be operated radically, a normal phagocytosis activity of granulocytes indicates the undisturbed course. In patients with progressive carcinoma growth the average of phagocytosis is lowered obviously. These results revealed information for the prognosis of malignant diseases and the nonspecific accompanying diseases.

[The normal state of granulocyte phagocytosis].

There are demonstrated results of the granulocyte phagocytosis with rice starch with healthy patients. The early phase of the phagocytosis seems to be more sensitive on changes of the unspecific defence. There were not found any significant differences between men and women. Patients who are older than 75 years have not a significant lower phagocytosis than younger ones.
