Does cyclosporin A influence sex hormone level?

Cyclosporin A is an effective immunosuppressive substance which is extensively applied in various conditions. A well known side effect of high dose Cyclosporin A treatment is the occurrence of hypertrichosis, often referred to as hirsutism. The purpose of the study was therefore to investigate ovarian, adrenal and pituitary hormones as possible mediators of increased hair growth. In 5 female and 11 male patients who had developed hair overgrowth during Cyclosporin A treatment serum androgens (testosterone, dehydroepiandrosterone-sulfate, androstendione), sex hormone binding globuline (SHBG), prolactin (HPRL), 17-hydroxyprogesterone (17-OHP), 17 beta-estradiol (17 beta-E) and cortisol (F) were determined by standard radioimmunoassay methods. The same number of age matched patients with Azathioprine treatment served as control. Both patient groups received additive cortisone treatment of the same dosage. No significant differences of serum androgens were noted between Cyclosporin A treated patients and controls. In addition, normal prolactin levels were defected. 17-OHP was in the low normal range and cortisol below normal due to the additive cortisone treatment. SHBG was within normal range. The findings thus indicate that increased hair growth caused by Cyclosporin A seems not to be caused by an action of the substance on the hormonal level.

Response of multiple syringomas to isotretinoin.

Two female patients with multiple syringomas were treated for 5/6 months with cumulative doses of 9 and 11 g isotretinoin respectively. Clinically, the appearance of the lesions in both patients improved from the third month on. After therapy, the lesions were flattened, softened, and skin coloured. Histologically, a marked reduction of size and numbers of syringomas was observed, and the contents of cystic lesions seemed reduced 6 months after therapy. Ultrastructurally, changes in the cystic and ductal epithelium correspond with the keratin modulating efficacy of the drug, but also point at a drug-induced change in intracellular lipid composition.

Culture versus direct specimen test: comparative study of infections with Chlamydia trachomatis in Viennese prostitutes.

To evaluate the sensitivity and specificity of the direct specimen test with monoclonal antibody conjugated with fluorescein isothiocyanate (FITC), cervical smears from 700 Viennese prostitutes were investigated by immunofluorescence microscopy and compared with the results of cultures for Chlamydia trachomatis in McCoy cells. Inclusion bodies in cell culture and elementary bodies on the smear slide were counted and evaluated in four categories. Cultures were obtained from 12.6% (88) of all women investigated, whereas the direct specimen test gave positive results in 10.4% (73). The results indicate that in asymptomatic people with only few inclusion bodies and latent infections, culture on McCoy cells is the more adequate method of identifying Chlamydia trachomatis.

Basement membrane components outline the tumour islands in cylindroma.

The main histological feature of cylindroma is the deposition of sheaths of a 'hyalinized' material contiguous to the tumour cell clusters. Although ultrastructural studies of this material have revealed a basement membrane-like structure, its exact nature has remained unclear. Using immuno-staining with affinity-purified antibodies directed against distinct basement membrane components, we have shown that type IV collagen and laminin are major constituents of this zone. In addition, cell culture studies indicated that both proteins are synthesized by the tumour cells. The immunohistological data make it clear that the tumour matrix between the tumour cell islands is composed not only of basement membrane components, but also is composed of other connective tissue constituents, i.e. type I and III collagen and fibronectin.

13-cis-Retinoic acid in rosacea. Clinical and laboratory findings.

The paper presents clinical and laboratory data on 13-CIS-retinoic acid treatment in rosacea. Good-to-excellent results are reported for severe forms of rosacea. Side effects were tolerable and could easily be controlled. The role of sebaceous gland function was documented by both histological and skin sebum parameters. Diminution of sebaceous gland size and the decrease of skin sebum with alteration of single sebum fractions correspond with findings in acne. According to the findings the alteration of sebaceous gland function is one model of drug action in rosacea. Thus sebaceous gland dysfunction may play an important role in the pathogenesis of rosacea.

Computer-assisted volumetric analysis of cutaneous malignant melanomas.

The total number of neoplastic cells seems to be an important factor in interactions between host and neoplasm. Studies of patients with leukemia indicate that cure of the disease occurs after reduction of leukemic cells below 10(6). With this concept in mind, a calculation of the number of neoplastic cells in cutaneous malignant melanoma was performed by computerized volumetric analysis. Serial sections of three entire lesions were assessed by a system of Videoplan Morphometry. The results were compared with conventional prognostic variables (level, thickness, mitotic index). This method of assessing volume did not improve the prognostic accuracy afforded by simpler methods.

[NK cells and the nervous system].

Light- and electron-microscopic studies of human, mouse, rat and guinea pig tissue subjected to PAP and ABC immune reactions revealed, that a monoclonal antibody against human natural killer cells (LEU 7) reacted also specifically with neural elements. In man, not only NK cells, but also myelin sheaths, oligodendrocytes, neurones, astroglial and ependymal cells as well as some enterochromaffin cells were labelled. Similar results, with the exception of negative ependymal cells, were obtained in the laboratory animals investigated. Controls and experiments using another monoclonal antibody against human natural killer cells (VEP 13) were negative. The presence of an antigen shared by human natural killer cells and neural elements could be of importance for the pathogenesis of demyelinating disorders.

[The leopard syndrome, a cardio-cutaneous syndrome].

A patient with the "leopard" syndrome presented with cardiomyopathy and a large arteriovenous shunt of the left renal vessels. The skin manifestations included multiple lentigines, junctional naevi, blue naevi, and one malignant lentigo. This syndrome is a rare entity, but in all patients with multiple lentigines the possibility of associated cardiovascular changes should be considered. In addition, regular monitoring of the pigmented lesions should be performed because of the increased risk of development of malignant melanoma.

[ Fournier gangrene: a rare complication of multiple myeloma].

A 69-year-old male patient with multiple myeloma developed a dolent swelling of the right side of his scrotum at the time of maximal leukocyte depression after cytostatic drug therapy. Within a few hours the scrotal skin became gangrenous. After surgical debridement of the necrotic tissues and simultaneous antibiotic therapy the necrosis showed a favorable course.

[Genital Chlamydia infections in female patients at risk of having sexually transmitted diseases].

Investigations for Chlamydia infections of the genital tract were performed in 270 Viennese registered, as well as in 51 non-registered prostitutes. The results were compared with findings obtained in 56 female patients attending an out-patient STD clinic and 48 pregnant women seeking antenatal care in a department of obstetrics. Diagnosis of Chlamydia infection was the organisms on cultured McCoy cells after treatment with cycloheximide. The highest infection rate (31%) was found in non-licensed prostitutes. In registered prostitutes, 20.4% of the cervical smears proved to be positive for Chlamydia. The control groups showed lower infection rates of 9% and 4%, respectively. These results indicate that women who frequently change their sexual partners are considerably more at risk of contracting Chlamydia infections. Hence, this group of persons should be more regularly investigated and--if necessary--given appropriate treatment in order to prevent further spread of the disease.

Gastric blood flow, mast cell degranulation and micromorphology of gastric mucosa following experimental haemorrhagic shock in dogs.

Ischaemia of the gastric mucosa in haemorrhagic shock appears to be one of the principal factors underlying acute bleeding from the upper gastrointestinal tract. In the present experimental study on dogs the changes of blood flow in the upper gastrointestinal tract were recorded by direct flow measurement in the pertinent vessels. Fourteen mongrel dogs were subjected to haemorrhagic shock lasting for 3 and 4 h. A decline of 46 per cent cardiac output was observed while coeliac artery blood flow decreased by 40 per cent and gastric artery blood flow by 60 per cent. All stages of stress ulcers were documented by light and electron microscopy. In addition, pronounced degranulation of mast cells preceding major tissue damage was observed. In the light of these findings a cascade of events is thought to be present resulting in the development of stress ulcer.

Nasal glioma.

Light and electron microscopic studies in a case of nasal glioma revealed a tumor composed nearly entirely of astrocytes. No ganglion cells or neurons were present and the mass was not surrounded by a capsule reminiscent for meningeal tissue. The vasculature resembled dermal patterns rather than typical central nervous system. No bone defect or connection to endocerebral tissue could be detected in the present case. Nasal glioma represents an ectopic focus of astrocytes, comparable to a hamartoma, rather than a true tumor or a herniation of brain tissue.

[Polyamine levels in blood and plasma of patients with malignant melanoma].

The polyamines putrescine, cadaverine, spermidine, spermine, and the amine histamine in whole blood and plasma of 42 patients with malignant melanoma were analysed after cold acid extraction and ion-exchange chromatography by fluorescence detection. Depending on the degree of tumor spreading the patients were grouped into 5 subpopulations; results were statistically compared with the values obtained from a normal population. The mean values of polyamine concentrations in the patient groups were generally not distinguishable from the mean values of the controls. Solely the mean of the spermidine values of postoperative patients with or without medical therapy and the spermine mean value for postoperative patients in tumor stage I and III were elevated. Cadaverine was never detected. The individual frequency of elevated polyamine levels in the plasma of single patients and variable, depending on patient subpopulation and polyamine under consideration. Spermidine was most frequently elevated (44%) in patients of tumor stage I under immunetherapy. We conclude that due to the low frequency of polyamine elevations to be found in plasma of melanoma patients by our method no diagnostic or prognostic significance and only limited importance for monitoring patients under therapy can be claimed for.
