The investigation was carried out in a representative sample of subjects of the Federal Republic of Germany, including West Berlin (n = 1952). The subjects were 20--64 years old. The following were investigated: 1. Social and abusive drinking patterns 2. Relations between drinking behavior and sociodemographic data 3. Relations between drinking behavior and subjective feeling. Of the subjects, 47% consumed alcohol daily or several times a week, 6% called themselves teetotallers. Beer was the preferred beverage. The drinking behavior varied among income classes and vocational groups. Of the whole sample, 8% drank regularly whilst working; 25% of the skilled workers did so. Comparing alcohol consumption and subjective feeling, there was a slight association between elevated mood and frequent alcohol consumption. Of the subjects, 4% were identified as alcohol misusers (7% of the males, 1% of the females). Persons at high risk were men living alone (especially widowed and divorced subjects) and persons of medium income. Regular consumers of hard liquor were at higher risk than consumers of beer or wine. The highest percentage of misuse was found among unskilled and skilled workers and among inhabitants of large cities. There was a slight association between high alcohol misuse index and depressive states, subjective complaints, and paranoid attitudes.
From the extensive literature on alcoholism 250 relevant diagnostic items were selected and tested for their selectivity in 3 independent inquiries among 1335 patients (661 of them alcoholic), both outpatients and inpatients. The outcome is a two-part test: a section of 7 questions answered by the doctor and a self-assessment section comprising 24 questions answered by the patient. Its validity (validity of the whole test r=0.85) and reliability (reliability of the self-assessment section r=0.94) are high. A diagnosis of alcoholism made according to the usual clinical aspects was confirmed in 90% of 675 unselected medical, surgical and psychiatric patients and alcoholism was suspected in a further 8%.
The following neuropsychiatric disorders have been briefly described: alcohol withdrawal syndrome, delirium tremens, alcohol hallucinosis, Wernicke-Korsakow syndrome, seizures, tremor, Marchiafava-Bignami disease, central pontine myelinolysis, alcoholic amblyopia, alcoholic cerebellar degeneration cerebral atrophy, alterations of personality in chronic alcoholics, alcoholic polyneuropathy. The pathogenetical aspects as well as the pathological findings have been reviewed with special emphasis on nutritional factors.

The conditioning problems of suicidal acts of aged people were described by demonstrating the results of investigations in 125 patients (over 65 years). The control group consisted of 3004 suicidal patients below 65 years. Depression has been the prevailing psychiatric diagnosis (50% in contrast to 33% in the control group). 24% of the patients alleged conflicts with partners or other relatives as a motivation for the suicidal act. Fear of somatic illness or social isolation follows next. It is remarkable that vocational or economical conflicts were not mentioned. At last prevention problems were discussed. The need for modifying the attitude toward the aged was stressed.
This short questionnaire for alcoholics (KFA) has so far been used as an aid for the clinical diagnosis of alcoholism. In the presented empirical study the KFA was tested on its efficiency as an independent diagnostic instrument for the identification of alcoholics. The questionnaire was given to 120 male alcoholics and 80 male controls (nonalcoholic inpatients). Its validity (correlation of test scores by independent criterion) of r = 0.81 with a reliability of r = 0.94 is fairly high, considering the nature of our control group being inpatients in contrast to the normally used healthy controls. A further improvement of discrimination could be achieved in weighting certain items. On this basis given a cutting score of 6 points the misclassifications in both alcoholics and controls could be reduced to 4%. Allowing this error rate the KFA can be considered a suitable test for the identification of alcoholics.
UNLABELLED: An attempt was made to establish the prevalence of alcoholism in the State of Bremen in 1973. A sample of the adult population was sent a questionnaire. The selection of questions was based on the principles of the WHO definition of 1952 and on dependence on alcohol. Amount of drinks, behavior, psychosocial consequences and dependence on alcohol were gone into. The assessment of answers was weighted. RESULTS: When very strict criteria are applied, alcoholism existed among 2-2.5% of the population samples. With slightly less strict criteria which probably correspond more closely with reality, it was 3-4%. This, calculated for the whole population of Bremen, shows a prevalence of at least 1.6-2%, probably 2-3%. Alcoholism is not evenly distributed between sexes, age groups, social status and income-groups. The relation between men and women is 2 to 1. Most alcoholics are found among those aged 16-29 years and among unskilled workers, in the income group between 800 and 1399 DM p/month and among those not stating their income. The smallest number of alcoholics was in the group between 40 and 49 years old, among people not in top positions of the civil service or in business and in the lowest income group (less than DM 800 p/month). It is remarkable that no difference was found to be related to educational standards.

After a survey of the particularly great risk of suicide among alcoholics and drug addicts, the psychopathologically common base of suicidal tendencies and addiction which arise from the advances of the theory of narcism are presented. Comparative clinical investigations in two groups of patients who had attempted suicide (with and without alcoholism) show that every 4th suicidal patient has alcohol problems. Alcoholic patients present a more pathological self-rating, they relatively frequently live alone in poor accommodation and even considerably worse social and economic conditions.

In a controlled study during two years we have collected psychological and sociological data on partners of chronic female alcoholics by making use of a semistandardized interview (80 items were evaluated by program Dial and the input scales of Spitzer and his decision logic), of the Freiburger-Persönlichkeits-Inventar (FPI) and of the Hawie-Dahl-test, a German abbreviated version of the Wechsler-Bellevue-Intelligence-test. Furthermore we investigated the interpersonal perception in the marriage of the female alcoholic by a semantic differential. The relatively small number of subjects and controls emphasizes the considerable problems as far to the methodial implications of such a study. Therefore we have to interpretate cautiously our results. We may plead for the hypothese, that the husband of the chronic female alcoholic already from the beginning of the marriage had fifficulties in the realisation of his male role and beyond this that the partnership of the chronic female alchoholic is evidently disturbed by distortion of the interpersonal perception. Finally the interpretation of our results is disucssed with regard to the special approach of examination.
