[The influence of oxygen and bicarbonate administration to the parturient on maternal and fetal blood gases and acid base balance (author's transl)].

The effects of oxygen and bicarbonate administration to parturients were studied by analysis of maternal and fetal blood gas parameters. Oxygen inhalation resulted in a significant increase of the fetal pO2 and appears to be a practicable mean of intrauterine reanimation of hypoxic fetuses. Intravenous bicarbonate was unable to raise the fetal pH. Simultaneous bicarbonate and oxygen administration had no desirable effects on the fetal acid-base-balance.

[Rheobase, pelvic score and oxytocin sensitivity test before spontaneous onset of labour (author's transl)].

Electrodiagnostical investigations measuring the rheobase and chronaxy as well as Oxytocin Sensitivity Tests (OST) and estimations of the pelvic score have been carried out on 19 healthy pregnant women from the 34th week on weekly and in daily intervals 1 week before the calculated date of delivery. Generally there was no trend to a decrease of rheobase until 2 days before delivery, whereas a significant drop was found between day -2 and the day of the onset of labour. At the same time the pelvic score ascended highest, but in individual cases there was no direct correlation between pelvic score and rheobase value. The OST became positive in more than 50% before the rheobase decline. Practical aspects of the combined estimation of pelvic score, OST and rheobase are discussed especially in view of the elective induction.

[Urinary catecholamine excretion in women with a normal menstrual cycle (author's transl)].

The daily excretion of epinephrine and norepinephrine was analysed throughout one menstrual cycle in 8 normal women under every day circumstances. Blood levels of LH, FSH, prolactin, oestradiol, progesterone, testosterone and cortisol were determined by RIA every other day, or daily around the time of ovulation. No characteristic cyclic variations were found in epinephrine except for a peak in the premenstrual period, whilst norepinephrine was increased during menstruation and at about the time of ovulation in all women. The cortisol patterns showed no characteristic cyclic changes. Possible correlations are discussed between cyclic catecholamine changes and the well known psychological and emotional changes corresponding with the phases of the menstrual cycle.

[Clinical and pathological aspects of primary carcinoma of the fallopian tube (author's transl)].

A report is presented of 24 cases of primary carcinoma of the Fallopian tube, histologically confirmed between 1956 and 1971 at the laboratory of the 2nd Gynaecological Department, University of Vienna, Austria. All patients underwent surgery. Total hysterectomy and bilateral salpingo-ophorectomy was performed in 16 cases. All patients received radiotherapy after surgery. All patients in whom the carcinoma was not confined to the Fallopian tubes (stages II to IV) died within 5 years (average 2.6 years). Patients with stage I lesions survived longer than 5 years only in those cases with more highly differentiated carcinomas (grades I and II). Histological grading, therefore, seems to enable a more reliable prognosis of stage I carcinoma of the Fallopian tube.

[Basic statements to standardize electrodiagnostical investigations in obstetrics (author's transl)].

The use of electrodiagnostical investigations became more and more accepted in obstetrics within the last decade; namely rheobase measurements as a means of recognising a threatening premature labour and of checking the effectiveness of tocolytic treatment, and further as an indication of a change in the neurovegetative excitability immediately before delivery. Not only the excitability of the uterine musculature increases before onset of labour but also at the same time, the excitability of the striated skeletal musculature is raised. In obstetrics the electrical excitability usually is tested on the left musculus tibialis anterior but there is a difference in applying the electrodes (unipolar and bipolar technique). Both techniques and the basis of Electrodiagnosis are reviewed in this paper. The advantages and standardisation of the bipolar technique shall be shown.

[Neuromuscular excitability and duration of labor. Rheobase before and after amniotomy for induced labor].

In 12 primiparae who--on term--did not have spontaneous contractions but a Bishop score greater than 7, Rheobase was measured and there after labour induced by amniotomy. A second evaluation of Rheobase followed one hour after amniotomy. Results show that one hour after rupturing the membranes the Rheobase values decreased significantly (p less than 0,01). The correlation between initial Rheobase values and duration of delivery was still more significant (p approximately 0,001). The possible importance of Rheobase evaluation in the selection of pregnant women on term for elective induction (baby by appointment) is discussed.

[Electrodiagnostical determination of strength-duration curves during pregnancy, labour, puerperium, and their evaluation (author's transl)].

Electrodiagnostical determination of Rheobase and Chronaxy has been carried out on 10 women during pregnancy, beginning labour and puerperium. Rheobase and Chronaxy were marked in a Strength-Duration Curves scheme as well as some other points within that scheme. As a base of rise of the gained curves the actual rise at the point of the chronaxy was calculated. Our results show that with all 10 women the t-Difference-Test showed a significantly much lower curve during labour than that obtained during pregnancy. The same applied to the rise of the curve obtained during labour, which was significantly much lower than that during puerperium. There was no significant difference between the curves obtained during pregnancy and puerperium. The significance of the decline of the actual rise at the point of the Chronaxy of the Strength-Duration Curve before starting of labour as a physiological method of functional diagnostic will be discussed there under.

[Electrodiagnostical investigations during pregnancy, labour, puerperium, and in impending early delivery (author's transl)].

Electrodiagnostical investigations measuring the Rheobase and the Chronaxy have been carried out on 100 women, 36 during pregnancy, 21 during labour, 29 in puerperium and 14 with impending early delivery. As the results show, the rheobase values, which were determined during pregnancy, were significantly much higher, than those during labour or impending early delivery. The rheobase values were significantly lower during 19th--30th week of pregnancy than during 30--40th week, and the latter were significantly much higher, than those during labour or impending early delivery. In puerperium there was no significant difference of the rheobase values to those during labour, but a significant difference to the values obtained during pregnancy. The chronaxy values show no significant differences during pregnancy and labour and during pregnancy and impending early delivery. Only the chronaxy values in puerperium were significantly lower than in pregnancy as well as in labour.

[Holt-Oram syndrome in combination with reciprocal translocation, lung hypoplasia and cardiomyopathy].

A now two years old girl had developed respiratory insufficiency shortly after birth requiring prolonged artificial ventilation. A hypo- and dysplasia of the right lung was identified as the underlying cause. Further diagnostic evaluation revealed a malformation of bones, i.e. shortness of the humerus, aplasia of the radius, shortness of the ulna and a congenital anlage of four metacarpal bones in fixed malposition. In addition, an atrial septal defect of the sinus venous type with left-to-right shunting was present. This, in combination with the sceletal abnormalities pointed to the diagnosis of Holt-Oram syndrome. In her first year of life the patient also developed a hypertrophic non-obstructive cardiomyopathy. Cytogenetic analysis of her family revealed a reciporke translocation between chromosome 1 and 11 (t -1, -11 (1p13, 11q13)) in the patient herself, her father and the father and a sister of her father. However, this finding as well as the hypertrophic cardiomyopathy has to be regarded as being independent from Holt-Oram syndrome.

An extraordinarily low diploid chromosome number in the reptile Gonatodes taniae (Squamata, Gekkonidae).

Mitotic and meiotic chromosomes of the gekkonid lizard Gonatodes tanieae from Venezuela were analyzed with conventional staining, differential banding techniques, and in situ hybridization using a synthetic telomeric DNA probe. The karyotype of this species is distinguished by the extraordinarily reduced diploid chromosome number of 2n = 16, which is the lowest value known for reptiles. In contrast to most other reptilian species, G. taniae has exclusively bi-armed (meta- and submetacentric) macrochromosomes and no microchromosomes. Examination of the mitotic chromosomes with AT and GC base pair-specific fluorochromes shows the absence of multiple banding patterns in euchromatic regions. The n = 8 diakinetic bivalents of male meiosis present terminal, subterminal, interstitial, and pericentromeric chiasmata. Comparison of the karyotypes of G. taniae and G. vittatus (also collected in Venezuela), and with those of three other Gonatodes species, indicates that the exceptional diploid number of 2n = 16 is the result of repeated centric fusions. In addition to the telomeres, all pericentromeric regions of the G. taniae chromosomes contain considerable amounts of the (TTAGGG)n telomeric sequence. These are probably not only relics of the centric fusions, but also a major component of the highly repetitive DNA in the C band-positive heterochromatin.

A fixed stimulation protocol for in vitro fertilization (IVF) without determinations of hormones in blood.

A new fixed schedule of ovarian stimulation for IVF was developed, which is not only simpler and easier to handle for the patients but also yields better fertilization and pregnancy rates. The period and beginning of stimulation are shifted by means of a contraceptive pill, so that stimulation can generally be started on a Sunday. The patient takes 100 mg clomiphene for 5 days and 7.5 mg prednisolone for 30 days to suppress possible exaggerated adrenal androgens; she also receives 150 IU of human menopausal gonadotropin (HMG) i.m. every other day from her family physician. From the 8th day of stimulation onwards, follicular growth is registered by daily ultrasound at the IVF center. When the dominant follicle exceeds 18 mm in diameter, 5000 IU human chorionic gonadotrophin (hCG) is given. Blood sampling for hormone estimations is not necessary, and only one sample of urine is needed for the LH estimation before hCG. In comparison to clomiphene citrate (Clomid) alone, Clomid plus HMG/FSH and HMG/FSH alone, this protocol resulted in significantly higher fertilization and pregnancy rates per follicular puncture. The rate of abortions and extrauterine pregnancies, on the other hand decreased. When comparing repetitive IVF cycles with the first IVF cycle, a significant reduction of oocytes in repetitions was found, but no difference was found in the number of fertilized eggs. The pregnancy rate, on the other hand, increased in the repeat cycles.(ABSTRACT TRUNCATED AT 250 WORDS)

Monitoring of Clomid-hMG-hCG controlled ovulation by ultrasound and a rapid (15 minutes) total urinary estrogen assay.

A simple and rapid assay for estimation of total urinary estrogens is presented. The test is based on a commercial latex agglutination inhibition assay designed for urinary estrogens in pregnancy. To obtain the appropriate sensitivity range for the application of the test it was necessary to concentrate the urine using self contained C 18 mini cartridges. A fairly good correlation was found between the obtained values from the urine and estradiol-17 beta estimated by radio-immunoassay in serum (correlation coefficient r = 0,77). Comparing those results and regarding the number of follicles present at ultrasonic examination, and/or laparoscopy a value of 23,79-31,29 mg/l TE in urine/follicle was calculated according to 300-400 pg/ml serum/follicle, at which ovulation should be induced. Even withstanding the inherent limitations of this semiquantitative test and some loss of precision in terms of absolute values, when combined with ultrasound, excellent guidelines were established by this rapid and simple test to determine ovulation induction by hCG.

The importance of the FSH/LH ratio in ovarian stimulation therapy regime for in vitro fertilization.

In a retrospective study batches of hMG with different FSH/LH ratios, as well as pure FSH were used in two different stimulation protocols (Clomiphene/hMG and hMG only). The aim of this paper was to study the effect and importance of the different FSH/LH ratios if any, in ovarian stimulation during the follicular phase on the success of IVF. The results of our preliminary study indicate that as far as ovarian stimulation with Clomiphene/hMG is concerned, there seems to be no significant beneficial effect on IVF success of using pure FSH or gonadotropin preparations with higher FSH/LH ratios. With gonadotropin only for ovarian stimulation however, the use of high FSH/LH ratio seems to significantly increase the pregnancy rate.

A simplified technique for fertilization and culture of human preimplantation embryos in vitro.

The paper describes simplification of existing methods of culture in human IVF. Focusing attention on the need to streamline procedures, the mode of housing the egg/embryo in a constant accessible environment is examined. We now culture with B2/B3-Menezo medium in a multidish under 5% CO2 in air at 37 degrees C in an open system within a CO2 gassing incubator. The advent of the multidish as a specific housing system is highlighted. The 80% fertilization rate combined with a stable osmolarity demonstrates that previous success is maintained with changes introduced to improve convenience and simplicity of procedures.

[Clinical course of pregnancy and labor following in-vitro fertilization].

Data of 11 pregnancies (1 twin-pregnancy - 12 deliveries) achieved by in-vitro-fertilization and embryotransfer on infertile patients are reported. For most patients pregnancy was almost normal; 2 patients had threatened premature labour. In 5 cases there was evidence of moderate EPH-gestosis. In many of the women excessive weight gain was observed although estriol and human chorionic somatotrophin values were within the normal range. Intrauterine growth was normal according to echographic cephalometry. In 4 pregnancies delivery was by cesarean section after spontaneous onset of labour. All newborn babies were healthy and developed normally. The sex ratio was 7: 5 girls to boys (including one set of female twins). From fertilization the duration of the pregnancies was 265 +/- 5 days which corresponds with previous calculations based on the monitoring of ovulation. Calculated from implantation, presumably 5 days after fertilization, duration of pregnancy was 260 +/- 5 days.
