Fertilization and pregnancies following intracytoplasmic injection of testicular spermatozoa.

PURPOSE: Intracytoplasmic injection (ICSI) with testicular sperm was performed in 16 couples. All men had ductal obstruction and failed previous attempts of epididymal sperm microaspiration. METHODS: Testis tissue was obtained by excisional biopsies and incubated in HEPES buffered EBSS medium over 24 h at 37 degrees C. Motile sperm (Grade 1 to 2) were recovered in 13 patients and fertilized a total of 62 oozytes. Four pregnancies were achieved. RESULTS: One healthy boy and two girls (twin pregnancy) were born. CONCLUSIONS: The ongoing pregnancies revealed no fetal abnormalities on ultrasound scanning.

Air in the uterine cavity after embryo transfer.

OBJECTIVE: To examine the effect on the pregnancy rate (PR) caused by air bubbles loaded into the transfer catheter to bracket the embryo-containing medium. DESIGN: Prospective, randomized study. SETTING: One hundred ninety-six consecutive ETs in the Institute of Sterility Treatment. INTERVENTIONS: Air bubbles were loaded into the transfer catheter concomitantly with the medium in group I (n = 98), whereas in group II (n = 98) no air bubbles were used. RESULTS: The PR of group I equals that of group II. CONCLUSIONS: Bracketing the embryo-containing medium by air bubbles offers several advantages, especially the possibility of tracking the air on the ultrasound monitor to localize the embryos after the ET. For these reasons we recommend using the proven method.

Clinical application of volume estimation based on three-dimensional ultrasonography.

The development of three-dimensional sonography represents a milestone in the history of diagnostic ultrasound. It has become possible to evaluate a region of interest not only in the horizontal and vertical planes but also in a third, i.e. the frontal, plane. Among other possible applications, this technique greatly facilitates volume estimation. The objective of the present study was to evaluate the accuracy and precision of volumetry based on three-dimensional sonography. The results demonstrate a good correlation between estimated volumes and the volumes measured by transvaginal needle-guided follicle aspiration performed under sonographic visualization. Our findings confirm the conclusion of previous studies that three-dimensional sonography is making a valuable contribution to imaging techniques.

[Successful outcome of "intracytoplasmic sperm injection" (ICSI) within the scope of in vitro fertilization].

Poor function of spermatozoa accounts for half of all human infertility. "Intracytoplasmic sperm injection" (ICSI) is a new micromanipulatory method for the treatment of male infertility. "ICSI" was performed in cases of oligoasthenozoospermia in 12 IVF cycles, obtaining a fertilisation rate of 71.2% of all micromanipulated oocytes. So far four ongoing pregnancies occurred. Thus "Intracytoplasmic sperm Injection" represents a new encouraging technique to conceive, even in cases of severe male infertility.

Chromosome banding in Amphibia. XXI. Inversion polymorphism and multiple nucleolus organizer regions in Agalychnis callidryas (Anura, Hylidae).

Cytogenetic analyses were performed on several populations of the Central American tree frog Agalychnis callidryas, using conventional methods and banding techniques. The karyotype of this species is distinguished by an inversion polymorphism in chromosome 9, which is either submetacentric or telocentric. The populations examined are in Hardy-Weinberg equilibrium with respect to the two alternative morphs of chromosome 9. This is the first report of the occurrence of an intrapopulational chromosomal inversion polymorphism in the order Anura. In male meiosis, the two chromosomes 9 form a bivalent exhibiting a ring-like pairing configuration with terminal chiasmata in both arms, regardless of whether the paired homologs are heteromorphic or homomorphic. Furthermore, individual specimens of A. callidryas exhibit one or two unexpected 18S + 28S ribosomal RNA gene clusters, in addition to the standard nucleolus organizers. The chromosomal localization of these extra nucleolus organizers is identical in all metaphases from the same specimen and shows a specific intraindividual pattern. The karyotype evolution in the phyllomedusine hylids, the structure of the various classes of heterochromatin, and the occurrence and possible origin of the rare inversion polymorphisms and multiple nucleolus organizers in A. callidryas and a few other amphibian species are discussed.

[Incidence of ovarian hyperstimulation syndrome in in-vitro-fertilization treatment over a period of 11 years].

A total of 2915 cycles of controlled ovarian hyperstimulation (COH) were performed from 1981 to 1992 in an in-vitro fertilization--embryo transfer (IVFET) programme at our institute. In 325 (11.2%) cycles an ovarian hyperstimulation syndrome (OHSS) developed. It was defined if more than 10 follicles with a size of > 15 mm were found on the day of follicle puncture. OHSS was found to be more frequent in the 3rd quarter of the years. Soltriol or melatonin may play a role in this context. Additionally, we were able to confirm some risk factors that were previously reported to be associated with the OHSS: use of gonadotropin-releasing hormone agonist (GnRHa) protocols, and young age (< 35 years). The pregnancy rate was found to be higher among OHSS cases.

A comparison of the effects of two monophasic low dose oral contraceptives on the inhibition of ovulation.

Fifty-three women were randomly allocated to one of two combined low-dose monophasic oral contraceptives (20 micrograms ethinyl estradiol with 75 micrograms gestodene or 20 micrograms ethinyl estradiol with 150 micrograms desogestrel). The ability of these formulations to inhibit ovulation was compared using hormonal parameters and ovarian ultrasound. The effects on three treated cycles were compared with pre- and post-treatment cycles. No ovulations occurred in either group during therapy. Twenty-one percent of women were observed to show some follicle-like structures accompanied by raised serum estradiol in at least one treatment cycle. No significant differences between the two preparations were demonstrated on residual ovarian function. The secretion of estradiol and progesterone was significantly reduced throughout all three treatment cycles. Mean LH and FSH concentrations were comparable with both treatments. A secondary analysis of cycle control and tolerance was undertaken. Significantly less bleeding was seen in the gestodene group during cycle 2 (p = 0.02). There were no differences between the two treatments with respect to the other cycle control parameters. Approximately half the women recorded intracyclic bleeding during the first treatment cycle. This improved during cycles 2 and 3. Both formulations were tolerated well.

Relationship of the individual uterine size and the endometrial thickness in stimulated cycles.

At present there is no agreement regarding the usefulness of monitoring the endometrial thickness in stimulated cycles. We tested prospectively the hypothesis of a linear relationship of the given individual uterine size and the endometrial thickness in stimulated cycles using a 240 degrees transvaginal sector scanner. There was a strong correlation of the uterine dimension with the endometrial thickness in the midfollicular and the early luteal phase. Our findings suggest that the endometrial thickness is determined by the individual uterine architecture and therefore not predictive of the likelihood of implantation.

[In vitro fertilization in male subfertility--micromanipulation and special techniques for semen preparation].

Poor semen quality is increasingly the reason for human infertility. For couples who cannot be helped by conventional in vitro fertilization (IVF) treatment, micromanipulation methods and special sperm preparation techniques in in vitro fertilization hold considerable promise to enhance fertilization of the egg. "Partial zona dissection" (PZD n = 59) or "subzonal insemination" (SUZI n = 36) were performed in 88 IVF cycles obtaining a fertilization rate between 12.2% and 34.8% depending on the micromanipulation technique applied and a pregnancy rate of 19.2% per embryo transfer. So far ten ongoing pregnancies have occurred and five healthy children have been born. In certain cases of male factor infertility micromanipulation and special sperm preparation techniques therefore represent the only possibility of conceiving.

Use of lasers in assisted fertilization and hatching.

The erbium-yttrium-aluminium-garnet (Er:YAG) laser has been applied to micromanipulation in humans. It was used in the fertilization process for both subzonal insemination (SUZI) and for partial zona dissection (PZD). Laser-assisted micromanipulation achieved significantly higher fertilization rates (34.8%) when compared to mechanical SUZI (16.1%), but use of the laser did not improve the PZD results (laser 14.8% versus mechanical 14%). The Er:YAG laser was used to assist hatching. In the mouse it significantly improved the hatching rate (80 versus 29.3%) 110 h after administration of human chorionic gonadotrophin. This technique was applied in two different centres to patients with previous in vitro fertilization (IVF) failures. the implantation rate per embryo (14.4% laser-assisted hatching versus 6% control group) and the pregnancy rate per transfer (40 versus 16.2%) were improved.

[Assisted hatching and micro-fertilization].

Erbium-YAG laser has been applied for micromanipulation in human beings. In a first series of attempts it was used in the fertilization process for subzonal insemination (SUZI): laser assisted micromanipulation achieved significant higher fertilization rates (34.8%) when compared to mechanical SUZI (16.1%), and for partial zona dissection (PZD): in this case laser did not improve the results (laser 14.8% vs mechanical 14%). Erbium-YAG laser was used to assist hatching. In the mouse it significantly improved the hatching rate (80% vs 29.3%) 110 hours post hCG. This technique was applied in two different centres to patients with previous IVF failures. The implantation rate per embryo (14.4% laser assisted hatching vs 6% control group) and the pregnancy rate per transfer (40% vs 16.2%) were improved.

Results and complications of IVF therapy.

This review attempts to discuss the current efficacy of in-vitro fertilization (IVF) treatment, factors influencing IVF success rates, and complications of IVF therapy. The livebirth rate per oocyte retrieval and per embryo transfer (ET) have been among the most widely used indices of success. To determine true efficacy, success rates have to be related to patient characteristics, such as age and cause of infertility. The cumulative conception rate after specified periods of treatment provides the most useful estimate of success rate after IVF. Approaches to further improving IVF treatment in the future are discussed. Considering the ongoing scientific research in the field of assisted reproduction, it should be the challenge of the nineties to further improve IVF success rates per treatment cycle by 50-100%.

Laser surgery under sonographic control: preliminary experimental investigations.

The laser has gained increasing importance during recent years as a useful addition to the physician's therapeutic armoury. Reports on successful prostatectomy by ultrasound-guided laser coagulation in an animal model encouraged us to evaluate the possibility of using this technique for gynecological indications (for example, ectopic pregnancy, endometriosis, ovarian cysts, and other areas of medicine). We used two different laser systems. The devices consisted of a real-time abdominal scanner (4-5 MHz) or a transvaginal sector scanner (5-7.5 MHz) coupled to a neodymium : YAG laser with a 1064 nm wavelength or a holmium : YAG laser with 2120 nm wavelength. An experimental set-up using a water bath revealed clear visibility of the fiber tip and the laser irradiation in real-time ultrasound at 7.5 MHz. Subsequently, a needle (routinely used in transvaginal puncture of ectopic pregnancy) was placed in animal and human tissue specimens and the laser fiber was inserted. The tissue was exposed to both laser systems at different energy densities, pulse rates and pulse widths. The sonographic appearance was recorded and correlated with the macroscopic findings. Results indicate that the holmium : YAG laser at a pulse rate of 5 Hz and a power of 2 watts is effective in removing substantial amounts of tissue. Moreover, it offers immediate visualization and control of the damaged area by producing a strongly echogenic focus. The neodymium : YAG laser showed comparatively poorer sonographic visibility and a precarious thermal mode of action, which are important considerations influencing its applicability. The holmium : YAG laser system with its non-thermal mode of action seems promising for the further clinical application of ultrasound-guided laser surgery.

Primitive sex chromosomes in poeciliid fishes harbor simple repetitive DNA sequences.

The demonstration of the chromosomal mode of sex determination via genetic experiments as well as the absence of heteromorphic sex chromosomes affirm poeciliid fishes as a unique group among vertebrates that are endowed with the most primitive form of sex chromosomes. In many different taxa the evolutionary process involved in the differentiation of advanced sex chromosomes is outlined through sex specifically organized repetitive sequences. In this investigation hybridization of synthetic probes specific to genomic simple repeat motifs uncovers a sex-specific hybridization pattern in certain viviparous fishes of the family Poeciliidae. The hybridization pattern together with specific staining of the constitutive heterochromatin by C-banding reveals heterogamety in males (Poecilia reticulata) as well as in females (P. sphenops). In P. velifera, however, C-banding alone fails to unravel the heterogametic status. The female specific W-chromosome can be detected by simple repetitive sequence probes. Therefore, the principal significance of heterochromatization as a means of generating differentiated sex chromosomes is evident.

Agricultural work and male infertility.

Data from couples (n = 103) seeking artificial insemination with donor sperm because of poor sperm quality and a control group of couples treated by in-vitro fertilization due to female causes were analysed according to the occupations of the spouses. Comparison of occupational categories revealed a significant difference in the prevalence of agricultural occupation (11/103 vs. 1/103, p < 0.01) between the group with male factor infertility and the normospermic controls. These patients also reported more long-term exposure to several insecticides and other pesticides. These toxic chemicals probably have a detrimental effect on male fertility and therefore we suggest more caution in the way they are handled.
