Effect of tiapamil on perioperative cardiac arrhythmias and myocardial function.

Tiapamil was effectively used in 32 surgical patients in the treatment of intra- and postoperative cardiac arrhythmias such as atrial fibrillation, supraventricular paroxysmal tachycardia and extrasystoles and in ventricular ectopic beats. In 28 patients aged between 38 and 82 years, bolus doses of 0.3-1.5 mg/kg were used. Before administration, hypoxaemia, hypercarbia, metabolic acidosis and electrolyte imbalances were excluded. Small doses of 0.3-0.5 mg/kg were only effective in patients with tachycardic atrial fibrillation. In most other arrhythmias the effective dose was between 1 and 1.5 mg/kg. In 4 surgical patients aged between 56 and 82 years, the continuous infusion of 12-25 mg/h (in 2 patients preceded by a bolus injection of 1 mg/kg) was effective in abolishing arrhythmias. In 18 healthy subjects, systolic time intervals were measured continuously and noninvasively before, during and for 9 min after 1 mg/kg tiapamil or 0.13 mg/kg verapamil, given intravenously according to a randomization schedule. Heart rate increased and systolic blood pressure decreased slightly but significantly. A significant improvement in myocardial function, with a decrease in PEP and PEP LVET ratio, was found after both calcium antagonists. The effect of tiapamil on myocardial function lasted significantly longer than that of verapamil.

[The importance of continuous intra-arterial PO2 measurement for evaluation of PO2 changes with drugs, positioning and hyperoxygenation (author's transl)].

In 26 adult patients arterial pO2 was measured continuously with intra-arterial pO2 electrodes over 24 hr after cardiac surgery. The effects of nitroglycerin, Thalamonal and change of body position were investigated. Hyperoxygenation with 100% O2 was tested. The following results were obtained: 1. pO2 and drugs: i.v. nitroglycerin and Thalamonal lead to a decrease in pO2, which occurred with a delay after the blood pressure fall. - 2 pO2 and body position: In the majority of patients with mitral stenosis and in one patient with acute myocardial infarction we found increased pO2 values with 40 degrees head up tilt as compared to supine position. - 3. Hyperoxygenation: The peak values of pO2 after a F1O2 of 1.0 were reached between 6-18 min, with a gradual fall afterwards. The relationship of measured pO2 values, cardiac disease and surgical procedures is discussed.

[Cerebrospinal fluid pressure studies after the intravenous administration of the steroid narcotic, alphaxolone + alphadolone acetate (Althesin)].

The effect of alphaxalon + alphadolon-acetate on cerebrospinal fluid pressure (CSFP), mean arterial blood pressure (MPA), heart rate (BMP) and blood gases was investigated in 18 patients. Cerebral perfusion pressure (CPP) was calculated from the difference MAP minus CSFP. Alphaxalon + alphadolon-acetate lowered the normal CSFP and normalized ketamin induced increase of CSFP. Premedication with alphaxalon + alphadolon-acetate delayed the ketamin induced increase of CSFP, which returned to norm after a second dose of alphaxalon + alphadolon-acetate. This effect was seen despite elevation of pCO2 in all patients breathing spontaneously.

[Treatment of intra- and postoperative cardiac tachyarrhythmias with the new calciumantagonist Ro 11-1781 (author's transl)].

30 patients with cardiac tachyarrhythmias were treated intra- and postoperatively with the new calciumantagonist Ro 11-1781. Atrial fibrillation, paroxysmal supraventricular tachycardia, supraventricular and ventricular extrasystoles and atrial tachycardia were the main indications. A good antiarrhythmic effect was noticed, provided that a dose of not less than 1 mg/kg was given. The mean fall of blood pressure was 4%, the maximal fall 15%.

[Continuous mass-spectrometric measurement of CO2 while using a modified Mapleson D-(Bain-)system (author's transl)].

In 21 patients undergoing abdominal surgery a modified Mapleson D-(Bain-)system was used under controlled ventilation. An average endexpiratory CO2 of 40 mm Hg was measured at a flow of 70 ml/kg kontinuierlich registrierten endexspiratoritimes these values were stable. In some patients without evident metabolic disturbances we found the CO2 values at borderline or even pathologic. So an increased flow rate of 100 ml/kg is recommended.

[Sodiumnitroprusside in neurosurgery and intensive care of neurosurgical patients (author's transl)].

Sodium nitroprusside (Nipride) was used in 35 patients between 16 and 72 years for induced hypotension. The fast and dose-dependent effect on the smooth muscles of the vessels made it superior to other methods of lowering blood pressure. Sodium nitroprusside was used to achieve hypotension in 11 patients (group I) with an aneurysm of a cerebral vessel and in 16 patients (group II) with highly vascularized cerebral tumors. In 8 patients (group III) from the neurosurgical intensive care unit a hypertensive crisis was interrupted. 10 patients (group IV) without induced hypotension were used as controls to compare the action of our standardized method of anesthesia on the observed parameters of blood pressure lowering. The average dose of sodium nitroprusside was 2.74 mcg/kg/min. in group I, 1.77 mcg/kg/min in group II and 1.27 mcg/kg/min in group III. The initial blood pressure was lowered between 30--40% in the mean in all 3 groups. Careful monitoring of the intra-arterial blood pressure, the central venous pressure, the cerebral fluid pressure, the arterial blood gases and the function of the kidneys during and after induced hypotension with sodium nitroprusside made it possible to take advantage of the blood pressure lowering effect also in patients with increased intracranial pressure.

[The effect of etomidate on CSFP (author's transl)].

The effect of etomidate on cerebro-spinal fluid pressure (CSFP) was investigated in 25 patients. Mean arterial blood pressure (MAP), heart rate (BPM) and blood gases were measured additionally. Cerebral perfusion pressure (CPP) was calculated from the difference MAP minus CSFP. Etomidate lowered CSFP (p less than 0,01 paired Student t-test). Ketamine-induced increase of CSFP (p less than 0,01) was normalized by etomidate. Premedication with etomidate delayed ketamine-induced increase of CSFP, which dropped to normal after a second dose of etomidate. The effect in reducing CSFP of etomidate was seen despite elevation of pCO2(p less than 0,01) in all patients breathing spontaneously.

Digitalis-thiopentone effects on myocardial function. A study of systolic time intervals in man.

Systolic time intervals were measured in 10 patients with cancer of the esophagus who required two anaesthetic procedures, before and after an induction dose of thiopentone. The patients were digitalised between the first and the second procedures. Digitalis improved systolic time intervals and reduced the depressant effect of thiopentone on the heart as measured by systolic time intervals.
